Posterior deformity correction and internal fixation for sagittal imbalance due to spontaneous fusion in lumbar spondylolisthesis: A case report.

RATIONALE: Lumbar spondylolisthesis (LS) with spontaneous fusion (SF) accompanied by sagittal imbalance (SI) poses a practical decision-making challenge in surgical planning. Here, we present a trauma-associated case of lumbar spondylolisthesis with SF and SI, and outline a reproducible, evidence-informed framework for surgical management. PATIENT CONCERNS: A 57-year-old patient developed low back pain after trauma. Lumbar computed tomography (CT) showed L4 anterolisthesis with bilateral pars interarticularis defects. Despite standardized conservative treatment, the back pain progressed, with

Repetitive Transcranial Magnetic Stimulation for Peripheral Neuropathic Pain: From Cortical Analgesia to Phenotype-Stratified Clinical Translation.

Peripheral neuropathic pain (PNP) arises from lesions or diseases of the somatosensory system and remains difficult to treat because peripheral nerve injury is coupled to spinal, thalamic, cortical and affective network changes. Repetitive transcranial magnetic stimulation (rTMS) can engage cortical pain-modulation circuits without implantation or systemic drug exposure, but the field needs a clearer account of which peripheral neuropathic phenotypes, targets and trial designs can convert transient neuromodulation into clinically durable benefit. This narrative, non-systematic review synthesiz

Defining Inactivity of Juvenile Spondyloarthropathies: Delphi Study Among Pediatric Rheumatology Academy (PeRA) From Türkiye.

INTRODUCTION: Juvenile spondyloarthropathies (JSpA) are a group of chronic inflammatory diseases that differ in their clinical features and course from adult spondyloarthropathies and other subtypes of juvenile idiopathic arthritis (JIA). Therefore, defining disease inactivity in JSpA requires specific criteria. This Delphi study aimed to establish a national consensus on its core clinical, laboratory, and radiological domains. METHODS: A total of 27 pediatric rheumatologists participated in the Delphi survey, conducted in two rounds. Participants were asked multiple-choice and Likert-type qu

Contemporary Outcomes of Thoracic Endovascular Aortic Repair in Patients with Syndromic Genetic Aortopathy: A Multi-Centre National Study.

BACKGROUND: Thoracic aortic aneurysms and dissection in individuals with syndromic genetic aortopathy (SGA) due to Marfan syndrome, Loeys-Dietz syndrome, and vascular Ehlers-Danlos syndrome are especially challenging. This is due to the impaired aortic architecture secondary to the genetic variants. Previous studies have analyzed the outcomes after open surgery in this population. However, there is little information on their outcomes after thoracic endovascular aortic repair (TEVAR). Recent studies have demonstrated high rates of subsequent secondary interventions in these patients treated wi

Low-Intensity Pulsed Ultrasound Promotes Spinal Cord Injury Recovery by Regulating Microglia-Mediated Neuroinflammation.

BACKGROUND: Spinal cord injury is a severe central nervous system disorder with high morbidity and mortality, whose pathological process is divided into primary and secondary injury. The regulation of neuroinflammation is a critical link in secondary injury, forming a positive feedback loop involving microglia and peripheral immune cells. Low-intensity pulsed ultrasound, a noninvasive physical therapy, exhibits significant potential in regulating inflammation and promoting microangiogenesis in orthopedics and nerve repair, providing new insights for intervening in the secondary pathological ca

Preliminary study assessing the long-term surgical outcomes of TBX6-associated congenital scoliosis (TACS) patients using the propensity score matching method: exploring the clinical implications of genetic discoveries in congenital scoliosis.

BACKGROUND: Compound inheritance of TBX6 accounts for approximately 10% of sporadic congenital scoliosis (CS) cases. Such cases are called TBX6-associated congenital scoliosis (TACS). TACS has been reported to have certain common clinical phenotypes. However, whether the surgical outcomes of TACS patients differ from those of other CS patients remains unclear. METHODS: We retrospectively searched for patients who were diagnosed with scoliosis. TACS was identified in genetic testing for CS. After propensity score matching, patients with TACS were matched with patients with NTACS according to s

DNA-guided transcription factor interactions extend human gene regulatory code.

In the same way that the mRNA-binding specificities of transfer RNAs define the genetic code, the DNA-binding specificities of transcription factors (TFs) form the molecular basis of the gene regulatory code1,2. The human gene regulatory code is much more complex than the genetic code, in particular because there are more than 1,600 TFs that commonly interact with each other. TF-TF interactions are required for specifying cell fate and executing cell-type-specific transcriptional programs. Despite this, the landscape of interactions between DNA-bound TFs is poorly defined. Here we map the bioc

Risk factors for residual neuropathic pain using specific screening tools in postoperative patients with ossification of the posterior longitudinal ligament of the cervical spine.

PURPOSE: Ageing, long illness duration, and poor preoperative Japanese Orthopaedic Association (JOA) score were reported to be risk factors for residual pain after cervical ossification of the posterior longitudinal ligament (cOPLL). In this study, we focused on residual neuropathic pain (NeP) and aimed to elucidate risk factors for residual NeP after cOPLL. METHODS: Total of 234 patients who underwent cOPLL surgery were included. NeP was evaluated using painDETECT (PDQ) and Spine painDETECT (SPDQ) questionnaires. Score of ≥ 13 / ≥ 0 was defined as NeP for PDQ/SPDQ. Patient backgrounds factor

Mobile device-based 3D scanning is superior to scoliometer in assessment of adolescent idiopathic scoliosis.

PURPOSE: Screening for adolescent idiopathic scoliosis (AIS) currently relies on clinical evaluations by trained practitioners, most commonly using a scoliometer. Modern structured light 3D scanning can generate high-quality 3D representations of surface anatomy using a mobile device. We hypothesized that a mobile-based 3D scanning system would provide accurate deformity assessments compared to a scoliometer. METHODS: Between August 2020 and June 2022, patients 10-18 years being evaluated for AIS in our clinic with a scoliosis radiograph obtained within 30 days of clinic evaluation and no his

[Challenges to Distinguish Idiopathic Normal Pressure Hydrocephalus from Progressive Supranuclear Palsy].

Idiopathic normal pressure hydrocephalus (iNPH) is a clinical condition characterized by symptoms of gait disturbance, cognitive dysfunction, and urinary disturbance. In contrast, progressive supranuclear palsy (PSP) is a progressive neurodegenerative disease characterized by supranuclear gaze palsy, akinetic rigidity, gait disturbance, and dementia. PSP manifests various clinical phenotypes that mimic other diseases and occasionally present iNPH-like presentations. Our previous publication showed that PSP develops iNPH-like magnetic resonance imaging (MRI) features more frequently than other
