Efficacy and safety of low-temperature plasma ablation combined with collagenase in the treatment of lumbar disc herniation.

BACKGROUND: Lumbar disc herniation (LDH) is a common cause of radicular pain and disability. Evidence regarding the incremental effectiveness of combining plasma-based intradiscal decompression with collagenase chemonucleolysis remains limited. METHODS: This single-center retrospective cohort study included consecutive hospitalized adults with imaging-confirmed LDH treated between July 2021 and July 2025. Patients underwent low-temperature plasma ablation alone (control group, n = 108) or plasma ablation plus intradiscal collagenase (observation group, n = 116). The primary outcomes were long

Prospective feasibility study of diagnostic CT-based simulation-free palliative radiotherapy.

Conventional radiotherapy (RT) workflows require planning computed tomography (pCT) and treatment planning. However, these processes often take several days, leading to delays in RT initiation. Simulation-free radiotherapy (SFRT), which uses diagnostic CT (dCT) instead of pCT for treatment planning, may reduce the time from consultation-to-treatment delivery. However, the number of prospective studies conducted in Japan remains limited. This study prospectively evaluated the feasibility of SFRT in Japan. A single-center, single-arm prospective study (UMIN000054721) was performed in June 2024.

Individual and Family-Level Risk Factors of Daily Opioid Use in Adolescents Following Spinal Fusion Surgery.

OBJECTIVE: Exposure to prescribed opioids during adolescence increases the risk of future misuse. Opioid use following major musculoskeletal surgery varies substantially, yet predictors of daily use remain unclear in adolescents. This study examined associations between pre-existing individual and family factors and postoperative clinical risk factors with daily opioid use among adolescents undergoing spinal fusion surgery. METHODS: Data were from a prospective longitudinal cohort study in youth following spinal fusion surgery. Preoperative factors included age, sex, parent education, income,

Impact of pain severity on each EQ-5D-5L dimension of quality of life in patients with neuropathic pain associated with chronic low back or cervical pain: an ad hoc analysis.

OBJECTIVES: Clinical guidelines for patients with neuropathic pain (NeP) recommend setting achievable treatment goals relevant to their daily lives. We therefore evaluated the associations of the EuroQol 5-Dimensions 5-Levels (EQ-5D-5L) with reduced quality of life (QoL) in patients with chronic low back pain with NeP (CLBP-NeP) or chronic cervical pain with NeP (CCP-NeP) to help inform the setting of such treatment goals. METHODS: Secondary data were analyzed from two observational studies of patients with CLBP-NeP or CCP-NeP. The associations of each EQ-5D-5L dimension (mobility, self-care,

PTPRS mediates invasion of tetanus neurotoxin into the central nervous system.

Tetanus neurotoxin (TeNT) invades the central nervous system (CNS) via retrograde transport through motoneurons. Whether a host factor is needed to mediate TeNT trafficking has remained unknown. Here, using pull-down and mass spectrometry analysis, and by developing a luciferase-based in vivo transport assay, we identify receptor protein tyrosine phosphatase sigma (PTPRS; PTPσ) as the retrograde transport receptor for TeNT. Global or motoneuron-specific PTPRS knockout (KO) abolishes TeNT entry into the CNS in mouse models. PTPRS-TeNT interactions occur in endosomes, prevent toxin translocation

Consensus on the use of intrathecal gene therapy for spinal muscular atrophy.

BACKGROUND: Intrathecal (IT) delivery of gene therapy is an emerging strategy for Spinal Muscular Atrophy (SMA), aiming to enhance central nervous system targeting while potentially reducing systemic exposure. The new administration route of Onasemnogene abeparvovec is associated with a distinct safety profile, including dorsal root ganglia toxicity, and requires structured monitoring beyond existing intravenous guidelines, including planning for long-term multidisciplinary follow-up in pediatric and adult care as well as structured transition protocols. METHODS: We expanded the existing intr

Cholinergic inputs to motoneurons in the spinal cord augment focal muscle spasticity in rodents.

BACKGROUND AND PURPOSE: Cholinergic premotor inputs to motoneurons support high levels of motor output when required. However, the possible role of these inputs in pathologically elevated muscle tone has not been characterised. To address this, we examined the modulatory effect of spinal acetylcholine (ACh) on experimental spastic paralysis. EXPERIMENTAL APPROACH: Rats were injected with tetanus neurotoxin (TeNT, 33-100 pg, im) into gastrocnemius to induce sustained rigid hind-paw extension and resistance to passive ankle flexion. The effects of spinal ACh on experimental hypertonia were prob

What is the current state of evidence on passive exoskeletons designed to support the lumbar region? A systematic review and meta-analysis of physiological, biomechanical, and user-related outcomes.

INTRODUCTION: Work-related musculoskeletal disorders (WMSDs) are the most commonly reported health problem among workers in Europe. Manual handling of loads and awkward postures are cited as important risk factors for WMSDs. Exoskeletons, wearable devices that support and stabilize the locomotor system, are emerging as a lifting aid that could reduce muscular effort and potentially lessen the impact and the prevalence of WMSDs among manual workers. This systematic review and meta-analysis aims to summarize the current scientific evidence on passive exoskeletons designed to support the lower ba

Circulating angiogenic T cells as potential immune-vascular biomarkers associated with disease activity in axial spondyloarthritis.

OBJECTIVE: Axial spondyloarthritis (axSpA) is a chronic inflammatory autoimmune disease associated with persistent systemic inflammation and endothelial dysfunction. Angiogenic T cells (Tang cells), defined as CD3+CD31+CD184+ T cells, are specialized immune cells involved in vascular repair and endothelial homeostasis, but their clinical relevance in axSpA remains unclear. This study aimed to characterize circulating Tang cells in axSpA and evaluate their potential as immune-vascular biomarkers of disease activity. METHODS: Peripheral blood samples were obtained from patients with axSpA strat

FAP adipogenesis drives early spinal muscle loss in dysferlinopathy and reveals a therapeutic target.

Dysferlinopathy (Limb Girdle Muscular Dystrophy R2/LGMD2B) causes progressive weakness of limb-girdle muscles. Although gait abnormalities are common, the contribution of spinal muscles to disease symptoms remains poorly understood. Using cross-sectional MRI in patients and longitudinal analyses in a dysferlinopathic mouse model, we identify early-onset, progressive adipogenic degeneration of spinal muscles as a previously unrecognized hallmark of dysferlinopathy. Multiple spinal muscles, including the iliocostalis, longissimus, and spinalis, are affected, with degeneration preceding pathology

[Carpal Tunnel Syndrome and Lumbar Spinal Stenosis: When to Suspect Amyloidosis and How to Proceed with the Diagnostic Work-up].

Transthyretin (TTR) amyloidosis, particularly the wild-type form (ATTRwt), is an age-related systemic disease characterized by the deposition of misfolded TTR in tendons and ligaments. Patients therefore frequently present initially to orthopaedic or hand surgeons with carpal tunnel syndrome (CTS) or lumbar spinal stenosis (LSS) rather than to cardiologists or neurologists. These musculoskeletal manifestations may precede ATTR cardiomyopathy (ATTR-CM) by several years to more than a decade, serving as "red flags" that provide an opportunity for early diagnosis, particularly now that disease-mo

[Cervical Lesions or Peripheral Nerve Disorders? How Far Can Neurological Examination Localize the Lesion?].

Accurate lesion localization is a fundamental component of neurological diagnosis. This review describes how neurological examination can be used to differentiate cervical spinal cord lesions, cervical radiculopathy, brachial plexopathy, and peripheral nerve disorders in patients presenting with upper-limb sensory and motor symptoms. Particular attention is given to the distribution of sensory impairment, tendon reflex abnormalities, and patterns of muscle weakness. The characteristic clinical features of cervical spondylotic myelopathy, cervical radiculopathy, brachial plexus disorders, and e

[Decision-Making for Cervical Spine Lesions: An Orthopedic Surgeon's Perspective].

In patients presenting with numbness, sensory disturbances, or motor deficits in the upper extremities, it is important to differentiate between cervical spinal cord or nerve root disorders and peripheral neuropathies. This article discusses the key points for distinguishing C8 spinal cord segment and nerve root disorders from peripheral neuropathies, particularly ulnar neuropathy and posterior interosseous nerve palsy. A comprehensive evaluation of the patterns of muscle weakness, distribution of sensory disturbances, and electrophysiological findings is essential, with careful consideration
