Disease-specific body composition patterns in children with cerebral palsy and spinal muscular atrophy.

OBJECTIVES: Spinal muscular atrophy (SMA) and cerebral palsy (CP) are severe neurological conditions characterized by neurodegeneration and non-progressive brain injury, respectively, both profoundly affecting the nutritional status in children. This observational study aimed to comprehensively assess and compare anthropometry and body composition across groups of children with SMA, CP, and healthy controls (HCs). METHODS: We enrolled three age- and sex-matched groups of 40 children each: CP (Gross Motor Function Classification System IV-V), SMA (Types I-II), and HC. Anthropometry, skinfolds,

Deep brain stimulation and spinal cord stimulation for chronic disorders of consciousness: a decade of clinical evidence, mechanistic insights, and critical appraisal.

Deep brain stimulation (DBS) and spinal cord stimulation (SCS) have been investigated for over four decades as neuromodulatory treatments for chronic disorders of consciousness (DOC), including the vegetative state/unresponsive wakefulness syndrome (VS/UWS) and the minimally conscious state (MCS), for which no disease-modifying pharmacological therapy exists. This narrative review synthesizes literature published between 2016 and 2026 on the neurobiological rationale, clinical evidence, neurophysiological biomarkers, and safety of DBS and SCS for chronic DOC. The 'mesocircuit hypothesis', supp

CT-Based Clinical-Radiomics for Differentiating Talaromyces marneffei Spondylitis From Tuberculous Spondylitis Before Pathogen Confirmation: A Multicentre Diagnostic Study in an Endemic Region.

BACKGROUND: Talaromyces marneffei spondylitis (TMS) is an uncommon but clinically important form of talaromycosis in endemic regions. Where talaromycosis and tuberculosis overlap, TMS may be worked up as tuberculous spondylitis (TBS), delaying fungal investigations and antifungal treatment. OBJECTIVES: To assess whether pretreatment CT, routine clinical variables and radiomics features can help differentiate TMS from TBS at the initial assessment stage. PATIENTS/METHODS: This retrospective multicentre study included 263 patients meeting the prespecified composite reference standard (83 TMS a

Mitochondrial electron transport chain activity is not essential for the acute hypoxic response of spinal oxygen sensors.

Spinal sympathetic preganglionic neurons (SSPNs) are oxygen sensitive. These spinal oxygen sensors (SOS) increase sympathetic output when subjected to acute hypoxia, orchestrating cardiorespiratory responses to protect against the deleterious effects of oxygen deprivation. Most other oxygen-sensing systems rely on mitochondrial mechanisms to detect and signal decreases in oxygen. Here, we apply pharmacological blockers of the mitochondrial respiratory chain and a mitochondrial-targeted antioxidant to a rat perfused in situ spinal cord preparation to test the role of mitochondria in the SOS. We

Patient-reported propagated sensation along meridians during acupuncture for lumbar disc herniation: protocol for an exploratory prospective cohort study with multimodal neurophysiological and immune measurements.

BACKGROUND: Propagated sensation along meridians (PSM) refers to the radiation of needling sensation beyond the needling site along meridian pathways. In traditional acupuncture theory, PSM has long been considered an important factor associated with clinical efficacy, particularly in conditions like lumbar disc herniation where pain radiates along the lower limb. However, this phenomenon has not been well characterized by modern neurobiological methods. Whether patients who report PSM show distinct autonomic, sensory, or motor responses, and whether this relates to better outcomes, remains un

Clinical efficacy and sagittal alignment restoration of unilateral biportal endoscopic-assisted anterior cervical discectomy and fusion for cervical spondylosis: a retrospective comparative study.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) remains the established operative approach for managing cervical spondylosis (CS). However, traditional open ACDF involves considerable damage to the surrounding soft tissues and relatively slow postoperative recovery. The emergence of Unilateral Biportal Endoscopy (UBE)-assisted ACDF (UBE-ACDF) offers a novel approach that combines effective decompression with the advantages of a minimally invasive procedure. Nevertheless, clinical evidence comparing UBE- ACDF with traditional ACDF remains scarce, and its clinical efficacy requires fu

Systematic review and meta-analysis on comparing treatment approaches of cervical fractures in ankylosing spondylitis to determine improvement in outcomes.

BACKGROUND & OBJECTIVE: Anterior, posterior and combine surgical approach for treatment of cervical fractures in ankylosing spondylitis is debatable in current existing literature. There are recognized advantages and disadvantages associated with anterior, posterior, and combined surgical approaches to the cervical spine when treating fractures in patients with ankylosing spondylitis (AS). Despite growing surgical experience, the optimal approach with respect to neurological recovery, complication rates, and mortality remains unclear, and the choice is largely guided by surgeon expertise and p

Targeting neuronal FKBP5 attenuates mechanical allodynia by suppressing NF-κB-mediated neuroinflammation in an NTG-induced chronic migraine mouse model.

BACKGROUND: Chronic migraine (CM) is a debilitating neurological disorder characterized by persistent trigeminovascular sensitization and neuroinflammation. While FK506-binding protein 51 (FKBP5) modulates spinal neuroinflammation in chronic pain, its specific role in trigeminovascular nociceptive processing and CM pathogenesis remains largely unknown. METHODS: We used a nitroglycerin (NTG)-induced CM mouse model. Following RNA-sequencing (RNA-seq) of total RNA isolated from trigeminal nucleus caudalis (TNC) tissue lysates, target expression was validated via reverse transcription quantitativ

Continuous mechanical compression-induced intervertebral disc degeneration and pain in the caudal spine in rats.

BACKGROUND: Chronic low back pain imposes substantial social and economic burdens due to its rising prevalence and associated disability. Intervertebral disc degeneration is the primary driver of discogenic back pain, though the mechanistic links between degenerative changes and persistent pain remain incompletely defined. Aberrant mechanical loading is recognised as a key trigger of disc degeneration. This work built a caudal spine compression rat model to explore how the P2Y2 receptor mediates load-induced disc degeneration and associated pain. METHODS: Fifty-six eight-week-old male laborat

Aging-enhanced accumulation of fibroblasts excludes oligodendrocytes in demyelinated lesions.

Fibroblast dysregulation contributes to pathological fibrosis and aberrant repair. Emerging evidence suggest that fibroblasts accumulate in lesions following central nervous system injury, but whether and how they influence oligodendrocyte repair responses, including in aging, is uncertain. Here we report that fibroblasts accumulate in the parenchyma of spinal cord white matter lesions of 6 - 10 week old young mice after lysolecithin-induced demyelination. This was first observed through immunofluorescence microscopy that employed several markers attributed to fibroblasts, including platelet-d

Prospects of pathogen whole-genome sequencing in outbreak tracing and antimicrobial resistance surveillance of spinal infections.

Spinal infections are a serious category of deep-seated infectious diseases that pose significant threats to human health. They are caused by a broad and complex spectrum of pathogens, including bacteria, fungi, and rare opportunistic organisms. Traditional diagnostic methods often face limitations in timeliness, accuracy, and source-tracing capability. Whole-genome sequencing (WGS), with its high resolution and throughput, provides comprehensive genetic information from pathogens, opening new avenues for precise diagnosis, outbreak tracing, and antimicrobial resistance monitoring and genomic

Zinc Hexacyanoferrate Nanoenzymes Attenuate Neuroinflammation After Spinal Cord Injury by Modulating Mitochondrial Function and Microglial Polarization.

BACKGROUND: Secondary pathology after acute spinal cord injury (SCI) is driven by pathological iron accumulation, reactive oxygen species (ROS) overproduction, and mitochondrial failure, which together sustain chronic neuroinflammation. Methylprednisolone (MP), the most widely used ROS-scavenging agent for acute SCI, provides limited clinical benefit and carries considerable side effects. To address this, we engineered zinc hexacyanoferrate nanoenzymes (ZnHPB) that chelate excess ferrous ions while displaying intrinsic SOD- and CAT-like activities, attenuating oxidative injury at its source ra

WHEEL-LEARN: Evaluating the usability, feasibility, and proof of concept of an internet-based healthy lifestyle behavior change intervention for people with spinal cord injury.

BACKGROUND: Individuals with spinal cord injury (SCI) are less physically active compared to the general population and face a higher risk of cardiometabolic diseases, some of which are modifiable through physical activity (PA). Personal and environmental barriers prevent people with SCI from meeting PA recommendations. WHEEL-LEARN, a 4-week virtual behavior change program, was developed to reduce PA barriers. OBJECTIVE: To evaluate the usability, acceptability, feasibility, and proof of concept of WHEEL-LEARN in people with SCI. METHODS: A pre-/postintervention study was conducted using mix
