Assessing the impact of high-risk committees in adult cervical deformity corrective surgery: a retrospective review of outcomes, complications, and frequency of procedures receiving preoperative multidisciplinary evaluation.

OBJECTIVE: The objective was to assess the frequency, outcomes, and impact of high-risk procedures receiving preoperative multidisciplinary review in adult cervical deformity (ACD) patients. METHODS: ACD patients were stratified by whether they underwent high-risk review (HRR). High-risk patients were defined as meeting ≥ 1 of the following criteria: anterior-posterior cervical deformity fusion ≥ 3 levels; planned 3-column osteotomy, vertebral column resection, and/or anterior column refinement; deformity correction with severe baseline neurological deficit; severe baseline myelopathy (modifi

A rare case report of neuro-brucellosis with concurrence of depression, visual impairment, bilateral sensorineural hearing loss, and paraplegia.

Neurobrucellosis is a rare manifestation of brucella infection, which would be life-threatening and result in multiple disabilities. Brucellosis commonly manifests with fever, arthralgia, and myalgia. Moreover, most patients with Neurobrucellosis present with significant lesions in the brain, spinal cord, or vertebral column; some cases show no lesions in their magnetic resonance imaging (MRI). The present case is a 32-year-old rural man with suspicious contact with animals at his work who was presented with ataxia, paraplegia, and urine-fecal incontinence without typical symptoms of brucellos

[How I manage … an infectious endocarditis complicated by neurological disorders].

Infectious endocarditis is considered as a potentially serious disease despite all advances in diagnosis and treatment. The left heart valves are the most affected, and embolic events, particularly mycotic aneurysms, perforations and abscesses, are their main but life-threatened complications. We report on a patient suffering from Enterococcus faecalis aortic endocarditis complicated by spondylodiscitis, moderate aortic insufficiency and cerebral hemorrhage due to the rupture of a mycotic aneurysm. We will emphasize the management of this mycotic aneurysm.

Are We Getting Better at Achieving Optimal Lumbar Segmental Sagittal Alignment in Complex Adult Spine Deformity Surgery?

STUDY DESIGN: Retrospective multicenter study. OBJECTIVE: To investigate how advances in spine realignment have impacted lumbar segmental alignment. SUMMARY OF BACKGROUND DATA: The understanding of spine alignment and adult spinal deformity (ASD) management continues to advance. It remains unknown how these advances have influenced lumbar segmental alignment changes in the setting of surgical correction. METHODS: Patients undergoing primary thoracolumbar fusion for ASD were stratified based on enrolment in two distinct multicenter registries; forming an 'Early' (2008-2017) and a 'Late cohor

Skull-femoral traction and vertebral osteotomies for severe rigid scoliosis with trunk imbalance.

This study aimed to explore skull-femoral traction and posterior vertebral column resection (PVCR) for the treatment of severe rigid scoliosis with trunk imbalance. The study also aimed to compare the procedure to the non-traction procedure with matched analysis. From January 2007 to December 2021, 59 patients (traction group) with severe rigid scoliosis and trunk imbalance underwent skull-femoral traction and PVCR. For comparison, another 1:1 matched group of 59 patients (non-traction group) was also retrospectively reviewed. These patients were treated with PVCR alone. The spinal function wa

Is surgery for multiple myeloma-related spinal involvement associated with higher blood loss compared to spinal metastases?

PURPOSE: Multiple myeloma (MM) is a hematologic malignancy often presenting with diffuse osteolysis of bone in the vertebral column. So far, little is known about the risk of perioperative blood loss in spinal surgery, as MM-related spinal lesions are often grouped with spinal metastases (SPMs). A more definitive understanding of bleeding risks for myeloma specifically will improve surgical planning and treatment. This study aims to assess the bleeding index (BI) and visually estimated blood loss (EBL) in MM patients versus SPM patients undergoing oncological spine surgery. METHODS: In this r

Construct-construct "rail technique" decreases screw strain during spinal deformity corrective maneuvers across a thoracic vertebral column resection: a cadaveric analysis.

PURPOSE: To biomechanically compare screw strains above and below a vertebral column resection (VCR) during segmental compression (SC) and cantilever bending (CB) performed via traditional methods and a novel, construct-to-construct accessory rod ("rail") technique. METHODS: Eight cadaveric torsos underwent a VCR with 250 kyphosis at T8 with pedicle screws implanted three levels above and below the VCR (T5-7; T9-11). Four screws (T6, T7, T9, T10) were instrumented with strain gauges to capture screw strains during SC and CB. Both deformity corrective maneuvers were performed over a traditiona

Staged short-term modified halo-pelvic traction and posterior spinal fusion versus posterior vertebral column resection for severe rigid kyphoscoliosis: a multicenter comparative study.

BACKGROUND CONTEXT: Surgical correction of severe rigid kyphoscoliosis remains technically challenging and is associated with high complication rates. Posterior vertebral column resection (pVCR) is often required for satisfactory correction but entails substantial surgical risks. Halo-pelvic traction (HPT) has been proposed as a safer alternative that may reduce the need for high-grade osteotomy by partially correcting the deformity preoperatively. PURPOSE: To compare the clinical outcomes of HPT combined with posterior spinal fusion (HPT+PSF) versus pVCR for severe rigid kyphoscoliosis, focu

Breed-specific variations in canine spinal cord anatomy: conus medullaris and dural sac termination and filum terminale internum length.

BACKGROUND: Limited research exists on breed differences in conus medullaris termination (CMT), dural sac termination (DST), and filum terminale internum length (FTIL). Dramatic vertebral column variations exist among breeds, suggesting possible differences in caudal spinal cord structures. This retrospective magnetic resonance imaging (MRI) study aimed to investigate breed-related differences with three cohorts of dogs. Cohort 1 consisted of 117 dogs without spinal cord compression (SCC) to identify demographic influences. Cohort 2 included 37 Corgis with and without SCC to assess if SCC affe

Incidence, Causes, and Timing of Unplanned Reoperations Following Three-Column Osteotomy for Pediatric and Adult Spinal Deformities: A Long-Term Single Center Study.

This study provides one of the most extensive long-term follow-up analyses of unplanned revisions following 3CO for spinal deformities, with a mean follow-up duration of 9.8 years, which has been underexplored in prior research. The findings provide critical data for preoperative discussions with patients and their families regarding the risks of unplanned reoperation. Additionally, the study highlights the need for long-term surveillance and proactive strategies to mitigate revision risks, particularly in patients undergoing multilevel 3CO.

Application value of Ultrasound-Guided cervical nerve root block test before percutaneous nucleoplasty in the treatment of patients with cervical chest pain: A retrospective study.

PURPOSE: Cervical chest pain (CCP) is an atypical symptom of cervical spine disease that often overlaps with other chest-related diseases but a type of noncardiogenic chest pain. The obvious relief of CCP after selective cervical nerve root block (SCNRB) near intervertebral disc herniation under ultrasound guidance should be considered to help identify the potential pathological source. The purpose of this study was to explore the application value of a positive ultrasound-guided SCNRB test before percutaneous cervical nucleoplasty (PCN) in the treatment of patients with CCP. METHODS: A retros

What Factors Were Associated With Implant Failure After Posterior Vertebral Column Resection for Severe Thoracolumbar Pott Deformity?

BACKGROUND: Posterior vertebral column resection (PVCR) is associated with improved spinal alignment and function in patients with severe thoracolumbar Pott deformity. However, the loads it places on spinal implants can result in those implants breaking or loosening, which can result in instability, pain, and unplanned reoperations. Identifying the controllable factors associated with implants breaking or loosening may be beneficial in the development of more effective surgical strategies. QUESTIONS/PURPOSES: What (1) radiographic and (2) clinical factors were associated with implant failure

Oncostatin M enhances the lengthening of sensory nerves and skin hypersensitivity.

BACKGROUND: Oncostatin M (OSM) is a cytokine that mediates inflammatory processes and is overexpressed in skin lesions of atopic dermatitis (AD). By amplifying neural responses to chemicals such as histamine, OSM increases sensitivity to pruritus. However, the morphological effects of OSM on peripheral sensory nerves and their subsequent impact on pruritus remain unclear. This study investigated OSM-induced peripheral nerve elongation, which may contribute to skin hypersensitivity. METHODS: We assessed neurite outgrowth using primary mouse dorsal root ganglion (DRG) cells treated with OSM, IL

Localisation and outcome of presumed thoracolumbar vertebral stress fracture in 35 UK Thoroughbred racehorses using nuclear scintigraphy.

BACKGROUND: Stress fracture is an important cause of mortality and morbidity in racing Thoroughbreds. Nuclear scintigraphy is the imaging modality of choice for investigating unlocalised stress-related bone remodelling in horses. Stress fractures of the caudal lumbar vertebral column have been previously described in both racing Quarter horses and racing Thoroughbreds post-mortem. There is a lack of literature describing the imaging appearance of vertebral stress fractures in racehorses ante-mortem. OBJECTIVES: To describe: (1) ante-mortem scintigraphic appearance of presumed thoracolumbar ve

Imperatorin activates TRPV1/GRPR pathway to induce scratching behaviors in mice.

Chronic itch is a prevalent and distressing symptom associated with several diseases, and significantly affects patients' quality of life. Investigating the mechanisms underlying itch and identifying novel anti-pruritic drugs require the development of innovative preclinical animal models. In this study, an acute animal model of itch was established using an intradermal (i.d.) injection of imperatorin (IMP) into the nape of the neck or cheek of male and female mice. The i d. IMP injection in the nape elicited a robust, dose-dependent scratching behavior that was independent of histamine. IMP i
