Imaging of Spinal Cord Infections.

Primary infectious myelitis is a rare but critical diagnosis with potential for severe morbidity and permanent neurologic deficit if not detected and treated early. This article introduces an image-guided framework that organizes spinal cord infections into 5 distinct transverse magnetic resonance imaging patterns: extramedullary, centromedullary, eccentric tract-specific, frontal horn, and irregular or mass-like. This article recognizes the substantial variability in imaging manifestations of various infections in the spinal cord, and the proposed pattern-based approach is not intended to ser

Imaging of Acquired Toxic and Metabolic Disorders of the Spinal Cord.

Metabolic and toxic myelopathies constitute a clinically significant subset of spinal cord disorders, often presenting with overlapping neurologic and imaging features. They primarily result from nutritional deficiencies or toxic exposures such as nitrous oxide, radiation, drugs, and systemic conditions. Patients typically exhibit subacute or progressive myelopathy with dorsal and lateral column involvement. MR imaging is the imaging modality of choice, often demonstrating characteristic longitudinal and symmetric signal abnormalities. Diagnosis requires integration of clinical findings, labor

[The role of NLRP3 inflammasome in the pathogenesis of ankylosing spondylitis research progress].

Ankylosing Spondylitis(AS) is a chronic inflammatory disease primarily affecting the spine and sacroiliac joints, with a higher prevalence in young males. Without timely diagnosis and treatment, it can progress to spinal ankylosis, joint function loss, and disability. The exact pathogenesis remains incompletely understood, leaving some patients unresponsive to current treatments and facing poor outcomes. In recent years, the role of NOD-like receptor family pyrin domain containing 3 (NLRP3) inflammasome in the pathogenesis of AS has attracted much attention. Upon activation, the NLRP3 inflamma

Altered hippocampal subfield functional connectivity and its association with back pain in axial spondyloarthritis: a multimodal neuroimaging study.

To investigate hippocampal subfield volumes and resting-state functional connectivity (rsFC) in patients with axial spondyloarthritis (axSpA), and to assess their clinical relevance. In this cross-sectional study, 39 patients with axSpA and 41 healthy controls (HCs) underwent 3-T structural T1-weighted and resting-state functional MRI. Hippocampal subfields were segmented using FreeSurfer, and seed-based rsFC maps were computed for 12 subfields per hemisphere. Group differences were assessed with general linear models (FDR-corrected) for volumes and permutation-based family-wise error correcti

ZNF865 (BLST) a Novel Regulator of DNA Damage and Cell Senescence in Back Pain.

Senescence has been shown to contribute to the progression of aging-related diseases including degenerative disc disease (DDD). However, the mechanisms regulating senescence in the intervertebral disc (IVD) and other tissues/diseases remain poorly understood. Recently, ZNF865 (BLST) was identified as a previously uncharacterized zinc finger protein that regulates a wide array of genes related to protein processing, cell senescence, and DNA damage repair. Here, we show that ZNF865 expression decreases with age and pathology in human and mouse IVD samples. Utilizing CRISPR-guided gene modulation

BI-TfR1 CapX rapidly transits the blood-brain barrier for efficient, low-dose gene delivery throughout the CNS.

Safe, efficient gene delivery throughout the CNS remains a central obstacle to treating genetic diseases of the brain and spinal cord. We describe BI-TfR1 CapX, a human transferrin receptor (TfR1)-binding AAV capsid that, after a low intravenous dose in adult humanized TFRC mice, transduced more than 80% of cortical and spinal neurons and ∼60% of cortical astrocytes, with reduced distribution and expression in several peripheral tissues relative to AAV9. Unlike other engineered BBB-crossing capsids, CapX bound the CNS vasculature within minutes and a large fraction of its genomes reached paren

Limb-on-a-Chip: An All-Hydrogel Platform for Scalable and Reproducible Engineering of Functional Neuromuscular Tissues.

Diseases or injuries that impact neuromuscular tissues severely impact human health, motivating the development of in vitro models of the motor control system. Current platforms either have limited reproducibility or require complex fabrication protocols that preclude high-resolution imaging and scalable functional analysis. We have developed a one-step method for fabricating an all-hydrogel Limb-on-a-Chip that addresses key challenges of current systems by enabling reproducible and scalable manufacturing of neuromuscular tissues compartmentalized into "spinal cord" and "limb" chambers. Primary care and idiopathic scoliosis: an evaluation of family physicians' roles.

UNLABELLED: Idiopathic scoliosis (IS) can progress silently during growth. Early primary care detection prevents curve progression and reduces treatment burden. This study aimed to assess the role of family physicians in screening, diagnosis, counseling, and follow-up of adolescents with IS, and to evaluate primary care attributes using the Primary Care Assessment Survey (PCAS). This cross-sectional study included 150 patients with IS followed at a university orthopedics clinic in Türkiye. Data were collected via structured telephone interviews using a custom 13-item questionnaire and the vali

EARLY-ALS: a multicentre study on prodromal amyotrophic lateral sclerosis.

BACKGROUND: Neurodegenerative diseases often feature a prolonged phase of subtle changes preceding diagnosis. In amyotrophic lateral sclerosis (ALS), defining this prodromal period is critical for identifying early disease features and intervention windows. METHODS: In this case-control study, 475 patients with ALS and 285 controls from 20 centres in Germany and Switzerland completed a digital questionnaire on prodromal symptoms and health-related factors. Logistic regression included subgroup analyses by sex and clinical strata. FINDINGS: Patients with ALS reported a higher burden of prodro
