Treatment variability among people assigned male at birth with repeat positive Mycoplasma genitalium tests: a case series.

BACKGROUND: Mycoplasma genitalium (MG) can cause nongonococcal urethritis (NGU) and symptomatic proctitis. Rising antimicrobial resistance and lack of clinically available molecular resistance assays make MG treatment challenging. This study adds systematic data on the effectiveness of alternative MG regimens. METHODS: Adult natal male patients who attended a public Sexual Health Clinic (SHC) and had ≥2 positive MG nucleic acid amplification tests within any 6-month period from January 2023-September 2025 were included. We classified cases as persistent infection or reinfection using clinical

Cross-omics mapping of the human HIV restrictome across clinical states, immune-cell compartments, and protein responses.

Host restriction factors can block HIV at multiple stages, but they are often studied as individual genes or within a single molecular layer. I performed an evidence-structured computational reanalysis of three independent public datasets to determine how the human HIV restrictome varies across clinical states, immune-cell compartments, and protein responses. The analysis used bulk monocyte RNA sequencing (GSE157198), PBMC single-cell/CITE-seq data (GSE256089), and primary CD4 T-cell proteomics (PXD012263) within a prespecified restrictome-immunometabolism framework. In GSE157198, ART-naive mo

Awareness, willingness to use and use of doxycycline post-exposure prophylaxis among gay, bisexual and other men who have sex with men in China: a national cross-sectional survey.

BACKGROUND: Bacterial sexually transmitted infections (STIs) disproportionately impact gay, bisexual and other men who have sex with men (GBMSM) in China. Doxycycline post-exposure prophylaxis (DoxyPEP) can prevent syphilis and chlamydia among GBMSM; however, it has not yet been officially endorsed in China. This study aims to investigate the perceptions and use of DoxyPEP among GBMSM in a nationwide sample. METHODS: We conducted a nationwide online cross-sectional survey in China between November 2024 and February 2025. Participants were recruited through community-based organizations and Ce

From social stigma to physical health: mediation effect of internalized stigma and mental health among people with HIV in rural China.

People with HIV (PWH) in rural China continue to experience multiple forms of stigma that threaten their overall wellbeing. This study explored how internalized stigma and mental health distress mediate the relationship between social stigma and physical health using baseline data from 800 PWH (42% women; mean age 54.8 years) from the Rise-Up Project in Guangxi, China. Social stigma was modeled as a latent construct composed of perceived and anticipated stigma, while mental health distress was represented by depressive, anxiety symptoms and perceived stress. Physical health included general he

Implementation Outcomes and Lessons From a Health-Department-Driven Quality-Based Financing Approach to HIV Prevention Programmes in New York City, United States: A Mixed Methods Analysis.

INTRODUCTION: In 2021, the New York City Health Department (NYCHD) became one of the first jurisdictions in the United States to adopt a quality-based financing (QBF) approach to HIV prevention programming, shifting away from the traditional fee-for-service model. NYCHD used a multi-level approach that defined quality through three programme-level implementation strategies (PLS): universal screening; holistic care provision; and sex-positive, "risk-free" HIV prevention conversations. By focusing their QBF programme on implementation strategies in addition to service delivery outcomes, NYCHD he

NIH Investment in HIV and Implementation Science.

INTRODUCTION: Research supported by the National Institutes of Health (NIH) has revolutionized HIV prevention and treatment, but not all people have benefited from these advancements. Implementation science seeks to bridge the gap between a discovery and when it is applied in the real world, by elucidating the methods that promote the adoption, uptake and integration of evidence-based strategies into routine care. Disparities in who benefits from the advancements have prompted calls to accelerate HIV implementation science. This paper provides a snapshot of research funded by the NIH at the in

Age-based outcomes of antiretroviral therapy for people living with HIV in current follow-up: a comparative retrospective cohort study, Ganzhou, China, 2001-2021.

BACKGROUND: The increasing population of elderly people living with HIV (PLHIV) presents new challenges for HIV management, while few studies have explored antiretroviral therapy (ART) effects in this population. This study aimed to compare ART outcomes across multiple physiological systems between young and elderly PLHIV. METHODS: A comparative retrospective cohort study was conducted among 2,178 young (18-49 years) and 1,495 elderly (≥50 years) PLHIV who received ART in Ganzhou, China, between 2001 and 2021. Temporal changes in multiple biomarkers related to virological, immunological, haem

A systemic, multistakeholder participatory approach to research, innovation and action agenda setting for more integrated and effective prevention of HIV and other STIs: the case of Catalonia, Spain.

INTRODUCTION: It is increasingly acknowledged that transformative change is needed to effectively address the complex and persistent health challenges facing society today. Realizing transformative change requires multistakeholder processes that co-define and implement research and innovation (R&I) priorities and actions that better respond to the complexity of these challenges in a contextualized manner. However, there is limited insight into the operationalization of participatory systems approaches for identifying the systemic elements of these complex challenges and for defining and implem

Do medication adherence trajectories predict clinical endpoints in a cohort with HIV and chronic comorbidities?

PURPOSE: With little known about medication adherence for non-HIV chronic conditions in people living with HIV (PLHIV), this research evaluated antiretroviral (ART) adherence and non-ART composite medication adherence patterns among PLHIV and non-HIV chronic conditions (i.e., type 2 diabetes, hypertension, and hyperlipidemia) and their association with corresponding treatment goals. METHODS: Group-based multi-trajectory modeling of ART and non-ART adherence was used to identify medication adherence trajectories (MATs) over a 37-month observation period (9/2018-9/2021) in 525 continuously-enro

Human papillomavirus detection in liquid-based cytology and cervical histology samples among women living with human immunodeficiency virus in Zambia.

OBJECTIVES: Cervicovaginal human papillomavirus (HPV) prevalence is high among women living with human immunodeficiency virus (WLHIV), but not all HPV genotypes detected in liquid-based cytology (LBC) may be present in cervical tissue. We explored HPV genotype concordance in paired LBC and histology samples among WLHIV in Zambia. METHODS: WLHIV aged 18-65 years who were enrolled in a cervical screening test accuracy study were invited for follow-up after 30-36 months. LBC samples and two or more cervical biopsies were collected and tested for 28 HPV genotypes (Anyplex II/Allplex HPV28, Seegen

Factors perceived as relevant to starting antimicrobial treatment in patients with non-tuberculous mycobacterial lung disease in the Italian scenario: a repeated cross-sectional survey after three years of educational activities.

BACKGROUND: Non-tuberculous mycobacterial pulmonary disease (NTM-PD) is increasingly recognised, yet initiating antimycobacterial therapy remains challenging. Following a three-year period of national educational activity in Italy, we re-assessed which factors Italian clinicians perceive as relevant to the decision to start treatment. METHODS: We conducted a repeated cross-sectional web-based survey. The second wave was administered between November 2023 and February 2024 to members of the Italian SITA and IRENE networks, using the instrument administered in 2020. Participants rated the relev

[Update on anal dysplasia and anal cancer].

Anal dysplasia and anal cancers are predominantly caused by high-risk types of the human papillomavirus. Lesions are mostly asymptomatic and disproportionately affect high-risk populations such as people living with HIV, women with a history of vulvar (pre)cancer, and long-term immunosuppressed individuals. For these risk groups, the incidence of anal cancer is up to 100 times higher, which is why screening is recommended in the S2k guideline completed in 2024. However, it should be noted that two-thirds of anal cancers occur in older women without HIV, for whom scalable screening strategies d

Implementing Sustainable Mobile Health Technology to Optimize a Smoking Cessation Program for Lao People With HIV (Project I-STOP): Protocol for a Hybrid Type-2 Pragmatic Effectiveness-Implementation Study.

BACKGROUND: Tobacco use remains the leading modifiable risk factor for causing cancer worldwide, particularly among people with HIV. In Laos, 61%-80% of male people with HIV and 3%-10% of female people with HIV smoke cigarettes. They currently have no theoretically and empirically based smoking cessation support. Our team developed a scalable and affordable mHealth (mobile health)-based automated treatment program to support Lao and Cambodian smokers to quit smoking. We also pioneered the Ask-Advise-Connect approach to identify patients who smoke and to connect them to treatment. OBJECTIVE: T

Natural Language Processing Analysis of WhatsApp Messages to Identify Unrecognized HIV Prevention Needs Among Young Women in Kenya.

INTRODUCTION: Identifying HIV prevention needs is challenging. Common approaches (e.g. public health campaigns, HIV risk scores) have limited sensitivity. Natural language processing techniques, such as topic modelling and computational linguistics, can extract meaning and subjective information from text. This study used these techniques with WhatsApp messages to identify unrecognized HIV prevention needs among young women in Kenya. METHODS: Prior work established key ethical protections (e.g. privacy, confidentiality). Between July and December 2024, women (18-24 years) who used WhatsApp we

Psychological adjustment, HIV knowledge, and antiretroviral treatment adherence in newly diagnosed HIV patients: a cross-sectional study from an Albanian infectious diseases clinic.

BACKGROUND: The first year after an HIV diagnosis is a period of intense psychological vulnerability, with elevated rates of depression, anxiety, and adjustment problems documented in multiple settings. However, data from Albania and Western Balkan contexts are scarce. OBJECTIVE: To assess psychological adjustment, coping responses, HIV knowledge, and antiretroviral treatment (ART) adherence among adults diagnosed with HIV within the previous 12 months in Tirana, Albania. METHODS: A cross-sectional study was conducted with 120 patients registered at the ambulatory HIV/AIDS clinic at the Infe

Preliminary Development and Assessment of a Multiplex Assay for Human Papillomavirus Antibody Detection Using Commercial L1 Proteins.

BACKGROUND: Human papillomavirus (HPV)-associated cancers are preventable with vaccination. Serologic assays monitor antibody responses, but their complexity and cost pose limitations when evaluating population level immunity. We developed an L1-based assay for the detection of multiple type-specific HPV antibodies. METHODS: The Luminex-based assay used recombinant L1 proteins from seven HPVs (HPV-6/11/16/18/35/45/58) for the detection of type-specific immunoglobulin (Ig)G and IgA. Cutoffs were established using plasma from infants and children. The assay was optimized and qualified using a s

Positive Syphilis Serology With Concurrent Disseminated Talaromycosis marneffei in a Patient With HIV: A Case Report.

BACKGROUND Talaromyces marneffei (TM) infection is an opportunistic infection that typically affects individuals with compromised immune systems. TM is an invasive fungal infection associated with high mortality in patients with advanced human immunodeficiency virus (HIV) disease. Syphilis, caused by Treponema pallidum, is a re-emerging infectious disease with diverse clinical manifestations. While primary syphilis often goes unnoticed, secondary syphilis is characterized by systemic symptoms and variable skin lesions, earning it the epithet "the great imitator." In areas where TM is non-endem

Avoiding the cost of complacency: barriers and solutions to ending the HIV epidemic in Europe.

INTRODUCTION: Despite significant advances in antiretroviral therapy and prevention strategies, Europe remains off track to meet the UNAIDS 2030 targets of ending HIV as a public health threat. This study explores the implications of failing to meet these targets by quantifying the associated costs and identifying pathways to get back on track. METHODS: We adopted a mixed-methods approach across eight European countries. Targeted literature reviews estimated annual and lifetime HIV costs, identified barriers and solutions, and assessed evidence on long-acting injectables (LAIs). Epidemiologic
