Evolving drivers of preexposure prophylaxis discontinuation among US MSM, 2018-2024.

OBJECTIVE: To examine temporal trends in reasons for stopping preexposure prophylaxis (PrEP) among MSM from 2018 to 2024, including how reasons co-occur and vary by demographics. DESIGN: Pooled repeated cross-sectional analysis of seven annual waves of the American Men's Internet Survey (AMIS), a national behavioral survey of US cisgender MSM (N = 54 612). METHODS: Among 3074 noncurrent PrEP users who reported reasons for stopping, we collapsed checklist items into six thematic categories and estimated adjusted prevalence by survey year using multivariable logistic regression with g-computat

Hepatitis B Seroprevalence and Associated Factors Among Inpatients from 2014 to 2023: A Single-Centre Retrospective Study in a Malaysian Tertiary Hospital.

BACKGROUND: Hepatitis B virus (HBV) remains a global health burden, causing significant health problems, including morbidity and mortality. The World Health Organization (WHO) aims to reduce new infections by 90% and deaths by 65% by 2030, with a focus on the Western Pacific and Southeast Asia. In Malaysia, HBV incidence rose from 2.26 per 100,000 in 2010 to 12.65 per 100,000 in 2015, classifying the country as having an intermediate burden with a seroprevalence of 1.5% to 9.8%. The major transmission routes include perinatal transmission, blood exposure, sexual contact, and needle sharing. Th

Genetic characterization of M. tuberculosis isolates from participants in a Phase 2b randomized trial evaluating the M72/AS01E tuberculosis candidate vaccine.

BACKGROUND: The efficacy and safety of the M72/AS01E tuberculosis (TB) candidate vaccine were evaluated in the phase 2b randomized, placebo-controlled trial NCT01755598. In the trial, participants with Mycobacterium tuberculosis sensitization by a positive interferon-gamma release assay received either M72/AS01E or placebo (randomized 1:1) and were followed for 3 years. In this descriptive study, we genetically characterized M. tuberculosis isolates from participants who developed pulmonary TB during the trial to assess variability among isolates and evaluate the possibility of preferential pr

Immune control of HIV viraemia in early paediatric infection is associated with induction of HIV-specific CD8+ T-cell activity following multiple treatment interruptions.

BACKGROUND: Virus-specific CD8+ T-cells play an important part in HIV cure/remission in adults, yet their role in paediatric immune control is limited by tolerogenic early-life immunity. Very-early ART initiation, while effective in restricting viral reservoir size, also prevents antigenic exposure and, thereby, the induction of HIV-specific CD8+ T-cell responses. Analytical treatment interruption (ATI) is an established tool in cure/remission studies for assessing time to viral rebound and viral setpoint off ART yet the impact of ATI on HIV-specific immune responses and plasma viral load (pVL

Circulating markers of organ damage associate with COPD in people with HIV: an exploratory proteomic analysis.

BACKGROUND: People with HIV (PWH) develop chronic obstructive pulmonary disease (COPD) more frequently and at younger ages than the general population, yet the circulating proteomic correlates of COPD in PWH remain poorly characterized. METHODS: We applied a targeted 92-plex organ damage proteomics panel (Olink Organ Damage) to compare serum protein profiles between 38 PWH with established COPD and 49 PWH without COPD, enrolled in a prospective observational cohort. RESULTS: Differential abundance analysis adjusted for age, sex, BMI, and current smoking status identified several proteins (NU

Barriers to Treatment Resulting in Multiple Coexisting Infections and Hospice in a Young Person With Acquired Immunodeficiency Syndrome (AIDS).

Barriers to treatment in people with human immunodeficiency virus (HIV) are often multifactorial and can lead to devastating outcomes. The authors present the case of a young woman with HIV who eventually developed acquired immunodeficiency syndrome (AIDS) attributed to barriers to continuous treatment, including difficulty attending appointments, obstacles to obtaining medication, untreated depression, lack of a support system, and other unmet social needs. She unfortunately developed multiple complications related to her AIDS diagnosis, including numerous hospital admissions and several seve

APOL1 kidney disease: a critical narrative review of molecular mechanisms, clinical heterogeneity, and the emerging therapeutic landscape.

BACKGROUND: The G1 and G2 variants of the APOL1 gene represent significant genetic risk factors for APOL1 kidney disease and contribute substantially to the excess burden of renal disease observed in individuals of African ancestry. Importantly, both variants exhibit incomplete penetrance, with only approximately 15-20% of high-risk genotype carriers ultimately developing overt nephropathy. OBJECTIVE: To provide a critically appraised, clinically oriented narrative synthesis of APOL1 kidney disease that (i) assigns an explicit certainty rating to each major mechanistic and clinical claim, (ii

Sepsis in the Setting of Bilateral Submandibular Abscesses: A Case Report.

Submandibular abscesses, a type of polymicrobial deep neck infection (DNI), are an otolaryngologic emergency due to the risk of airway compromise and septic shock. Unilateral submandibular abscesses are common and are often the result of odontogenic infections, while comorbidities that weaken the immune system, such as diabetes mellitus or human immunodeficiency virus (HIV), can exacerbate the infection. Bilateral submandibular abscesses are rare with limited cases reported in the literature, and are often associated with severe or systemic infections. This case report highlights the importanc

Multi-pathogen serosurveillance reveals correlated vaccination performance, waning tetanus immunity, and diphtheria boosting in Zambian children.

BACKGROUND: Vaccination coverage estimates and case-based surveillance have limitations in evaluating immunization programs. Serosurveillance offers a complementary approach by directly measuring population immunity. We assessed whether serologic analyses across multiple antigens (i.e., measles, diphtheria, tetanus) could provide additional insights into vaccination program performance. METHODS: We conducted a matched case-control study among children aged 2- to 10-years-old (n = 1286) in Zambia using specimens from the 2016 ZAMPHIA survey. Using previously generated data on measles serostatu

Case Report: A rare case of Pneumocystis jirovecii infection with left hydropneumothorax following immunotherapy for stage IVB clear cell renal cell carcinoma.

BACKGROUND: Pneumocystis jirovecii pneumonia (PJP) is an opportunistic infection that predominantly affects immunocompromised individuals, most commonly HIV-infected patients with significantly reduced CD4+lymphocyte counts, and is associated with high clinical mortality. Currently, there are few reports of pneumothorax secondary to PJP, and most cases occur in HIV-infected populations. However, PJP complicated by hydropneumothorax in cancer patients receiving immunotherapy is exceedingly rare, with limited reports in the literature. To our knowledge, this article reports a rare clinical case

Low Trauma Fractures in People With HIV: Longitudinal Time Trends in the Swiss HIV Cohort Study, 2009-2022.

BACKGROUND: People with human immunodeficiency virus (HIV) are at increased risk of low trauma fractures (LTFs). Published data on LTF incidence trends over time have not been uniform. This study sought to analyze LTF time trends in the Swiss HIV Cohort Study (SHCS) over the time period 2009-2022. METHODS: Fractures are prospectively captured in the SHCS. Since 2008, using a standardized form, each fracture and its low trauma nature was validated by the treating HIV physician and the main investigators. Applying negative binomial regression, we estimated the LTF incidence rate ratio per calen

Virus variant quantification with Orthanq.

BACKGROUND: Existing tools for virus variant identification can pinpoint the most abundant virus variant in a sequencing sample. However, patients can be infected by more than one variant of the same virus species or strain, for example by multiple variants of SARS-CoV-2. This leads to the more complicated problem of virus variant quantification from samples containing virus mixtures. RESULTS: We report on improvements of Orthanq, our generic tool for haplotype quantification, and show how it can be applied to perform uncertainty aware quantification of virus variants. We evaluate this ability

Spatial and bulk transcriptomics reveal distinct molecular signatures in Kaposi sarcoma with and without other KSHV-associated diseases.

BACKGROUND: Kaposi sarcoma (KS) is an angioproliferative tumor caused by Kaposi sarcoma herpesvirus (KSHV) that occurs in people with HIV. Concurrent KSHV-associated diseases (KAD), including multicentric Castleman disease, primary effusion lymphoma, and KSHV-associated inflammatory cytokine syndrome may modify KS biology and impact clinical outcomes. Transcriptomic profiling of archival KS tissue enables investigation of molecular heterogeneity associated with these overlapping disease states. METHODS: Archival formalin-fixed paraffin-embedded (FFPE) KS skin biopsies from 42 patients with HIV

Re-entry of donors deferred for false-reactive screening tests in Canada: 10 years post implementation.

BACKGROUND AND OBJECTIVES: Canadian Blood Services implemented a re-entry programme in 2014. Donors deferred because of false-reactive or indeterminate screening tests can provide a specimen for re-entry testing 6 months after their initial test, and resume donating if they test negative for all infectious markers. We evaluated the impact of the programme on donor return and retention, and assessed factors associated with reactive results upon re-entry testing. MATERIALS AND METHODS: Using extracted data from our donor database, we followed up donors who tested false-reactive for hepatitis B
