Disease-related and psychosocial factors associated with difficult-to-manage and treatment-refractory axial spondyloarthritis: insights from the Greek AxSpA Registry.

Patients with axial spondyloarthritis (AxSpA) with difficult-to-manage (D2M) or treatment-refractory (TR) disease represent an unmet clinical need, but real-world data remain limited. To determine the frequency, demographic, clinical, and psychosocial characteristics of D2M and TR AxSpA patients in a real-world setting, we analyzed data from the Greek AxSpA Registry. D2M disease was defined as Ankylosing Spondylitis Disease Activity Score [ASDAS] ≥ 2.1 despite failure of ≥ 2 biologic (b) or targeted synthetic (ts) disease-modifying antirheumatic drugs (DMARDs) with different mechanisms of acti

Exploring the influence of sagittal imbalance on spinal mobility, function and quality of life in axial spondyloarthritis: a novel comprehensive compensatory mechanism of adaptation.

OBJECTIVES: To investigate whether sagittal imbalance, defined by sagittal vertical axis (SVA) using EOS® imaging, is associated with spinal mobility, function, and quality of life in patients with axial spondyloarthritis (axSpA). METHODS: Patients with axSpA were cross-sectionally assessed for sagittal imbalance (SVA ≥ 50 mm). Spinal mobility (BASMI), function (BASFI) and quality of life (ASQoL) were compared between patients with and without sagittal imbalance. Multivariable analyses examined the associations between SVA and the above-mentioned disease outcomes, adjusted for confounders. Me

Increase in comorbidities with age among patients with psoriatic arthritis: a multicenter observational study.

BACKGROUND: Psoriatic arthritis (PsA) significantly contributes to increased morbidity, reduced life expectancy, and higher healthcare costs due to the burden of comorbidities. This study assessed the prevalence of comorbidities in PsA patients in India and explored the influence of age and disease duration on these comorbidities. METHODS: The prospective, multicenter observational study was conducted across seven centers in India, utilizing data from the Indian Rheumatology Association. Data were collected using expert-validated proformas, with comorbidities classified according to the ICD-1

Complications of Venous Thromboembolism Chemoprophylaxis in Lumbar Laminectomy With and Without Fusion.

BACKGROUND: The benefit of chemoprophylaxis (CPX) agents in preventing venous thromboembolism must be weighed against potential risks. Current literature regarding the efficacy of CPX after laminectomies with or without fusion is limited, with no clear consensus to inform guidelines. OBJECTIVE: This study evaluated the association between CPX and surgical complications after lumbar laminectomy with and without fusion. STUDY DESIGN: Retrospective study of patients at a single large academic institution. METHODS: The medical records of patients who underwent lumbar laminectomies with or witho

Minimally invasive debridement and drainage using intraoperative CT-Guide in multilevel spondylodiscitis: a long-term follow-up study.

BACKGROUND: Spondylodiscitis is an unusual infectious disease, which usually originates as a pathogenic infection of intervertebral discs and then spreads to neighboring vertebral bodies. The objective of this study is to evaluate percutaneous debridement and drainage using intraoperative CT-Guide in multilevel spondylodiscitis. METHODS: From January 2002 to May 2017, 23 patients with multilevel spondylodiscitis were treated with minimally invasive debridement and drainage procedures in our department. The clinical manifestations, evolution, and minimally invasive debridement and drainage tre

Chiropractic case reports: a review and bibliometric analysis.

OBJECTIVE: To determine publication trends, gaps, and predictors of citation of chiropractic case reports (CRs). METHODS: A bibliometric review was conducted by searching PubMed, Index to Chiropractic Literature (ICL), and Google Scholar to identify PubMed-indexed CRs, which were screened according to selection criteria. Case reports were categorized by International Classification of Disease (ICD-10) code, patient age, topic describing case management or adverse effects of care, focus being spinal or non-spinal, journal type, integrative authorship, title metrics, and citation metrics. Pathways of healthcare utilisation in patients with suspected adolescent idiopathic scoliosis: a cross-sectional study.

BACKGROUND: School screening programs for adolescent idiopathic scoliosis (AIS) have been discontinued in Canada and elsewhere because they were not considered cost-effective. In communities lacking such programs, we expect a significant variety of healthcare pathways and timeframes for patient referrals to orthopaedics. The objectives of this study were: 1) to characterise the healthcare pathways of young children with suspected AIS in a population without school screening; and 2) to investigate the relationships between these healthcare pathways and the appropriateness of referrals to specia

Prevalence of diagnosis and direct treatment costs of back disorders in 644,773 children and youths in Germany.

BACKGROUND: Many authors have reported about the high prevalence rates of self-reported back pain in children. Nevertheless, little is known about the diagnosis of back disorders--regardless of whether the diagnosis is associated with back pain or not. Therefore, the aim of this study was to analyse the prevalence rates and costs of diagnosis of back disorders in childhood and youth. METHODS: We conducted a secondary data analysis of a large, population based German data set (2,300,980 insurants of statutory health insurance funds) which allowed for identification of prevalence rates of diagn

[The management of cases of "conditional" work fitness due to spinal pathologies in health personnel].

On the basis of the trend in the literature, a process was identified, that uses the results of assessment of exposure to manual handling of patients, in order to practically manage, according to defined priorities, the transfer to other jobs of a large number of cases (about 8-10% of the workforce) that were potentially unfit due to back disorders. Aside from the procedures for clinical classification and grading of the severity of the disorders, which have been dealt with elsewhere, general indications are given for the association of cases of disease with assistance to hospitalised patients

Molecular epidemiology and pathogenesis of some equine herpesvirus type 1 (equine abortion virus) and type 4 (equine rhinopneumonitis virus) isolates.

Representative strains of EHV isolated from an aborted foetus and from a horse with rhinopneumonitis in New Zealand had restriction endonuclease DNA fingerprints typical of those usually associated with these syndromes elsewhere and now designated EHV1 and 4 respectively. EHV1 was isolated from the brain and spinal cord of a 4-year-old gelding that died of myeloencephalitis. A mare on the same farm, at about the same time as the gelding developed myeloencephalitis, aborted and EHV1 was isolated from the tissues of the aborted foetus. Restriction endonuclease DNA fingerprints of the viruses iso
