Selecting the optimal surgical treatment for multilevel cervical spondylotic myelopathy and radiculopathy significantly affects symptom improvement, postoperative prognosis, and quality of life. Proper patient selection and precise surgical execution are crucial for achieving successful outcomes, considering the favorable natural course of cervical radiculopathy. Several factors must be considered, including the number of affected segments, spinal alignment, kyphosis degree, stiffness, and surgeon expertise, when determining the surgical approach for cervical spondylotic myelopathy. STUDY DESIGN/SETTING: A retrospective cohort study.
OBJECTIVE: To compare long-term outcomes and complications of cervical disc replacement (CDR) and anterior cervical discectomy and fusion (ACDF) with cage-plate constructs (CPC) and stand-alone (SA) cages in treating degenerative cervical spondylosis.
SUMMARY OF BACKGROUND DATA: ACDF is commonly used for cervical radiculopathy but may increase adjacent segment degeneration (ASD). CDR has gained popularity by preserving motion and potentially reducing ASD, whereas SA cages offer a simpler alternative to CPC. Despite widespread adoption, furt

INTRODUCTION: Interlaminar epidural steroid injections (ILESIs) using a parasagittal approach are frequently employed to manage lumbar radicular pain. The degree of contralateral symptom relief achieved with parasagittal ILESIs remains underexplored. This study aims to evaluate the short-term effectiveness of parasagittal ILESIs in providing ipsilateral and contralateral symptom relief in patients with bilateral lumbar radiculopathy and unilateral contrast spread.
METHODS: A retrospective review was conducted on 12 patients with bilateral lumbar radicular symptoms who underwent parasagittal I

RATIONALE: Double crush syndrome (DCS) refers to multi-nerve entrapment based on the hypothesis that the distal nerve is likely to be more affected by pressure in the presence of a proximal lesion. DCS is difficult to diagnose, and thus far, few cases have been diagnosed from the perspective of DCS and reportedly treated with traditional Korean medicine.
PATIENT CONCERNS: The patient was a 50-year-old Korean man with DCS, hand edema, cervical radiculopathy, and upper limb paraesthesia.
DIAGNOSES: Based on the characteristic disc symptoms and symptoms extending beyond the dermatome and myotom

BACKGROUND CONTEXT: Endoscopic lumbar decompression is growing in popularity as an alternative to an open approach for management of lumbar radiculopathy. Although endoscopic procedures are inherently less invasive, emergency department (ED) visits may occur postoperatively. Although many quality improvement initiatives target readmissions, ED visits may be more common, be a marker of quality of care, affect patient satisfaction, and contribute to healthcare resource utilization and costs.
PURPOSE: To characterize the timing and risk factors of ED utilization following single-level endoscopic

Carpal tunnel syndrome (CTS) and cervical spondylosis (CS) are both common diseases, yet differentiation between the two is sometimes necessary. However, there are few evidence-based reviews on the differentiation of these conditions. This review examined the literature on the diagnosis of CTS and CS, focusing on how to distinguish between them. The analysis is divided into four categories: clinical symptoms, physical examination, diagnostic imaging, and electrodiagnosis. A total of 281 studies are reviewed, revealing a major issue: the inclusion criteria for defining each disease varies widel

Abdominal aortic aneurysms (AAAs) require complex surgical management. Currently, endovascular aneurysm repair (EVAR), including the placement of stent grafts and endovascular coils, offers early survival benefits over open surgery but carries its own distinct set of complications. We report the case of a 68-year-old man with a 35-mm saccular AAA who initially underwent stent graft placement and later required additional endovascular treatment for a type 2 endoleak. Intraoperatively, part of the coil mass appeared to deviate from the expected contour of the aneurysm. Postoperative CT imaging r

BACKGROUND: Lumbar fusion is the most common surgical intervention for chronic or severe low back pain. However, lumbar disk replacement (LDR) may be appropriate for certain patients. The objective of this study was to describe the postoperative management costs associated with both lumbar fusion and LDR in the 2-year period after surgery.
MATERIALS AND METHODS: An observational cohort study was conducted using the Merative MarketScan databases. Patients who underwent lumbar fusion or LDR between January 1, 2017, and December 31, 2017, were identified and included in the study. The primary ou

BACKGROUND: This is a retrospective study on endoscopic treatment of cervical spinal radiculopathy (CSR) conducted at a single academic institution. Conventional full-spine endoscopy is performed in a single portal which has certain limitations of high technical requirements, steep learning curve, and narrow indications. Although unilateral biportal endoscopy (UBE) technique has a gentle learning curve and is gradually applied to treat CSR, all procedures were performed in the uniportal working channel that potentially increase the surgical risk and time in the treatment of complex cases. It i

STUDY DESIGN: Retrospective cohort study.
OBJECTIVES: To evaluate the accuracy of ICD-10 codes in identifying isolated degenerative cervical radiculopathy versus concurrent myelopathy in surgical patients across an 11-hospital health care system (2016-2023).
SUMMARY OF BACKGROUND DATA: ICD-10 codes are commonly used for patient stratification in database studies, yet their accuracy in differentiating cervical radiculopathy from concurrent myelopathy remains poorly characterized. This distinction is clinically critical as treatment approaches and outcomes differ significantly between these co

Osteoporotic vertebral fractures are among the most common fractures associated with osteoporosis. Approximately 80% of cases heal by fusion in the natural process without the need for surgery, and if we include the so-called "incidental vertebral body fractures," which are discovered by chance during radiography, the frequency of prolongation of back pain due to pseudoarthrosis or delayed-onset neurological deficits is low. However, the prognosis of osteoporotic vertebral fractures is affected by the number of fractured vertebrae. In patients who become bedridden because of prolonged pain due

ABSTRACT: Single-point hand acupuncture with movement is a novel technique that can be used to provide immediate pain relief in the treatment of musculoskeletal disorders. It involves inserting just one needle along the second metacarpal bone, and then having the patient move through motions that were previously painful while retaining the needle. This study is a preliminary three-arm, single-blinded, randomized comparative trial looking at this technique compared with two control techniques.
ABSTRACT: Twenty-four patients were recruited in an outpatient musculoskeletal practice with neck, sh

This video demonstrates our surgical approach for an entirely tubular trans-spinous process bilateral lumbar discectomy. A 47-year-old man presented with 12 months of severe predominantly right L5 radiculopathy. Magnetic resonance imaging revealed a large diffuse disc bulge at L4/5 resulting in bilateral lateral recess and central stenosis. He underwent bilateral discectomies via a trans-spinous process tubular approach. Postoperatively, his pain resolved and was discharged home the following day. Clinical resolution was sustained at 6 weeks. An entirely tubular trans-spinous process technique

BACKGROUND: Thrombotic microangiopathy (TMA) is an acute syndrome characterized by microangiopathic hemolytic anemia, thrombocytopenia, and multi-organ dysfunction due to the microcirculation of platelet thrombi. Cancer-associated TMA is a rare and fatal complication, which often occurs during cancer remission. It is frequently misdiagnosed because of limited clinical awareness.
CASE SUMMARY: A middle-aged female patient presented to our clinic with a 15-days history of back pain, 15 months post-gastrectomy. Cancer-associated TMA was confirmed through bone marrow aspiration, biopsy, and imagi
