A Case of Cervical Radiculopathy Presenting as Dystonic Tremor.

BACKGROUND: To our knowledge, this is the first case report of a cervical disc prolapse and foraminal stenosis presenting with dystonic tremor in addition to brachialgia. CASE DESCRIPTION: A 54-year-old, right-handed man presented with a dystonic tremor and aching throughout his right upper limb. Magnetic resonance imaging of the cervical spine revealed a disc osteophyte complex at C5/6 causing C6 foraminal impingement bilaterally in addition to a disc osteophyte complex at C6/C7 causing severe bilateral foraminal narrowing. Electromyography showed neurogenic changes consistent with chronic C

Feasibility of Posterior Cervical Foraminotomy for Adjacent Segmental Disease after Anterior Cervical Fusion.

OBJECTIVE: The aim of this study is to evaluate the feasibility of posterior cervical foraminotomy (PCF) for adjacent segmental disease (ASD) after anterior cervical fusion (ACF). As ACF is accepted as the standard treatment for cervical spondylosis, many studies have been conducted to evaluate the efficacy of various surgical techniques to overcome symptomatic ASD after the previous surgery. Herein, PCF was performed for the treatment of symptomatic ASD and the feasibility of the surgery was evaluated. METHODS: Forty nine patients who underwent PCF due to symptomatic ASD from August 2008 to

Long-term Radiological Evidence of Affected and Adjacent Segment Disease after Anterior Cervical Foraminotomy.

Anterior cervical foraminotomy (ACF) is a surgical procedure for cervical radiculopathy to avoid fusion and adjacent segment disease (ASD), but its long-term outcome has yet to be investigated. It is also unclear whether ACF enables preservation of range of motion (ROM) and decreases ASD compared with anterior cervical discectomy and fusion (ACDF). This study included nine patients who underwent ACF, and 12 who underwent ACDF and with follow-up period of at least 5 years (average follow-up: 8.7 years). Preoperative and postoperative radiological findings were investigated, comparing the change

Surgical strategy used in multilevel cervical disc replacement and cervical hybrid surgery: Four case reports.

BACKGROUND: Multilevel artificial cervical disc replacement and anterior hybrid surgery have been introduced as reliable treatments for multilevel cervical degenerative disc disease. Surgical techniques are important for resolving patients' symptoms and maintaining the normal functioning of cervical implants. However, the use of inappropriate surgical strategies could lead to complications such as implant migration and neurological deficit. In this paper, we summarize our surgical strategies used in multilevel cervical disc replacement and hybrid surgery into five major notes. CASE SUMMARY: W

Medical students vs general public awareness regarding disc prolapse in Jeddah.

BACKGROUND: Disc herniation is a condition where tearing occurs in the outer annular layer of the intervertebral pad leading the inner gel-like material to bulge ouwards the spinal cord, due to the high load of the vertebrae. This will produce back pain and symptoms in different sites according to the level of the herniation in the vertebral column. AIM: To compare the level of awareness regarding disc herniation among the general population and medical students in Jeddah. METHODOLOGY: A descriptive cross-sectional study was conducted through an electronic questionnaire to assess the level o

Cervical Disc Arthroplasty: Rationale and History.

Biomechanical studies have demonstrated that cervical fusion results in increased motion and intradiscal pressures at adjacent levels. Cervical disc arthroplasty (CDA) is an alternative treatment for cervical radiculopathy and myelopathy resulting from degenerative disc disease. By maintaining segmental motion, surgeons hope to avoid some of the primary drawbacks of anterior cervical discectomy and fusion (ACDF), such as pseudoarthrosis and adjacent segment disease. First introduced in the 1960s, CDA has evolved over the years with changes to implant geometry and materials. Early devices produ

Clinical Outcomes of Cervical Hybrid Reconstructions: A Prospective Study.

BACKGROUND: The study design was a prospective clinical cohort study. The aim of this study was to assess the patient-reported outcome measures (PROMs), patient satisfaction, as well as complication and reoperation rate of cervical hybrid procedures for symptomatic cervical multilevel degenerative disc disease (MLDDD). Cervical total disc replacement (CTDR) has been shown to be safe and effective for the treatment of degenerative pathologies. However, there is minimal PROMs data on the outcomes of combined CTDR and anterior cervical decompression and fusion procedures, commonly referred to as

Is Cervical Disc Replacement Valuable in 3-Level Hybrid Surgery Compared with 3-Level Anterior Cervical Discectomy and Fusion?

BACKGROUND: Cervical degenerative disc disease is common in clinical practice. Surgery is among the most effective treatments for patients with cervical radiculopathy, myelopathy, or stenosis. Which treatment is the best option for multilevel cervical degenerative disc disease is controversial. Hybrid surgery, combined cervical disc replacement and anterior cervical discectomy and fusion, may be an alternative treatment for multilevel cervical degenerative disc disease. METHODS: Sixty-four patients with 3-level hybrid surgery and 37 patients with 3-level anterior cervical discectomy and fusio

Two-level cervical disc arthroplasty in patients with Klippel-Feil syndrome: A case report and review of the literature.

BACKGROUND: Klippel-Feil syndrome (KFS) is defined by multiple abnormal segments of the cervical spine with congenital synostosis of two or more cervical vertebrae. KFS patients who demonstrate progressive symptomatic instability and/or neurologic sequelae are traditionally managed with operative decompression and arthrodesis. CASE DESCRIPTION: A 44-year-old female with chronic neck pain and radiculopathy and a C7-T1 KFS presented with adjacent segment degenerative disc disease at the C5-6 and C6-7 levels. She was successfully managed with a two-level cervical disc arthroplasty (CDA). CONCLU

Case Report (Precis): Patient with MR-Documented Large Lateral Cervical Disc Misdiagnosed as Neurodegenerative Disease.

BACKGROUND: Patients who present to neurologists with cervical radiculopathy typically undergo initial MR scans. If reports show "abnormal" findings they, and other physicians, should review the studies with the interpreting radiologists/neuroradiologists. When patients' neurological deficits progress, neurologists should review their electromyographic (EMG) findings (i.e. especially if documenting neurodegenerative disease), the initial "abnormal" MR scans/reports (i.e. review with radiologists/neuroradiologists), and obtain spinal surgical consultations to rule out "surgical" disease. The Necessity of Upper Extremity Neurologic Examination while Evaluating Breast Pain.

INTRODUCTION: Breast pain is one of the most frequent complaints and occurs in 45-69% of all women. Cervical disc herniation is a common cause of cervical radiculopathy with an incidence rate of 18.6 per 100,000, and it should be considered as a cause of breast pain. OBJECTIVE: To identify the extent to which upper extremity neurologic findings and cervical root disorders accompany breast pain. METHODS: We prospectively collected clinical features of patients with breast pain. An upper extremity neurologic examination was performed in patients and controls. Patients who had neurologic findin

The Effect of Percutaneous Nucleoplasty vs Anterior Discectomy in Patients with Cervical Radicular Pain due to a Single-Level Contained Soft-Disc Herniation: A Randomized Controlled Trial.

BACKGROUND: Cervical radicular pain (CRP) is a common problem in the adult population. When conservative treatment fails and the severe pain persist, surgical treatment is considered. However, surgery is associated with some serious risks. To reduce these risks, new minimally invasive techniques have been developed, such as percutaneous nucleoplasty. Several studies have shown that percutaneous nucleoplasty is a safe and effective technique for the treatment of CRP, but until now no randomized controlled trials have been conducted that compare percutaneous cervical nucleoplasty (PCN) to anteri

Surgical Strategies to Prevent Adjacent Segment Disease in the Cervical Spine.

The most popular approach to treating symptomatic cervical disk disease is anterior cervical discectomy and fusion. Although this procedure has significant long-term clinical success, it is associated with progressive adjacent segment degeneration with an annual incidence of ∼3%. Total disk arthroplasty was designed as an alternative to fusion that could preserve segmental motion at the operative level and potentially delay or prevent adjacent-level breakdown. The etiology of adjacent segment pathology (ASP) is multifactorial, and it is likely that most cases of ASP are unavoidable. When attem

Artificial disc replacement versus fusion in patients with cervical degenerative disc disease and radiculopathy: a randomized controlled trial with 5-year outcomes.

In BriefIn this study the authors compare cervical arthroplasty with fusion surgery in a randomized controlled trial using patient-reported outcome measures and MRI after 5 years of follow-up. Because the main purpose of arthroplasties is to prevent adjacent-segment pathology, it is important to investigate if that is actually realized in practice.
