BACKGROUND: Because 3-dimensional computed tomography and magnetic resonance imaging analysis of the spinal architecture is done with the patient in the supine position, stereoradiography may be more clinically relevant for the measurement of the relative displacements of the cervical vertebrae in vivo in the upright position. The innovative EOS stereoradiography system was used for measuring the relative angular displacements of the cervical vertebrae in a limited population to determine its feasibility. The precision and accuracy of the method were investigated.
METHODS: In 9 patients with

STUDY DESIGN: In vitro biomechanics, randomized control trial.
OBJECTIVE: The objectives of this study were 2-fold: first, to determine the effect of exposure to axial vibration on the initiation and progression of disc herniation; second, to determine the effect of vibration exposure and the presence of disc damage on the mechanical properties of individual lamella from the annulus.
SUMMARY OF BACKGROUND DATA: Vibration exposure has been linked to a higher reporting of low back pain and disc herniation via epidemiological studies. However, these studies are unable to determine causal relati

OBJECT: Extension teardrop fracture of the axis is an extremely rare cervical spinal injury. The classic clinical features, which have mainly been described by radiologists rather than spine surgeons, include its occurrence in elderly patients with osteoporosis, an association with minimal or no prevertebral soft-tissue swelling, and an absence of associated neurological deficit. However, recent case studies indicate notable exceptions to these clinical features, although few studies have investigated osteoporosis in these patients. The purpose of the present study was to clarify the clinical

INTRODUCTION: Anterior cervical discectomy and fusion (ACDF) is an accepted technique for the treatment of symptomatic disc disease at the cervical spine. In this study the clinical outcome of 67 patients who underwent ACDF due to cervical disc herniation are presented. Based on the results of this study and those of other working groups the pros and cons of ACDF were analysed and discussed.
METHODS: 67 patients with cervical disc herniation underwent ACDF of a single level. Clinical and/or radiological examination was performed immediately preoperatively, one day after surgery and 7 months p

BACKGROUND: Polymethylmethacrylate (PMMA) assisted ventral discectomy has been criticized for high rates of graft migration and pseudarthrosis when compared with various other fusion procedures for the treatment of cervical degenerative disc disease (DDD), therefore rendering it not the preferred choice of treatment today. Recently however spine surgery has been developing towards preservation rather than restriction of motion, indicating that fusion might not be necessary for clinical success. This study presents a long term comparison of clinical and radiological data from patients with pseu

PURPOSE: Patient-orientated outcome questionnaires are essential to evaluate treatment success. To compare different treatments, hospitals, and surgeons, standardised questionnaires are required. The present study examined the validity and responsiveness of the Core Outcome Measurement Index for neck pain (COMI-neck), a short, multidimensional outcome instrument.
METHODS: Questionnaires were completed by patients with degenerative problems of the cervical spine undergoing cervical disc arthroplasty before (N = 89) and 3 months after (N = 75) surgery. The questionnaires comprised the EuroQol-F

BACKGROUND: Anterior cervical discoidectomy with or without fusion is a well established surgical remedy for cervical prolapsed intervertebral disc (PIVD) disease. If fusion is done by an iliac bone graft then internal fixation is commonly used to keep the graft in position. This study was conducted to determine the efficacy and tolerability of shape memory alloys, especially NiTi (nickel titanium) clips in the stabilization of grafts following anterior cervical discoidectomy.
METHODS: 133 NiTi clips were applied in 119 patients between January 2002 and December 2008. The patients age ranged

The authors performed a systematic review and meta-analysis to evaluate whether there is a beneficial clinical effect of total disk replacement compared with anterior cervical diskectomy and fusion for the treatment of single-level symptomatic cervical disk disease. A comprehensive literature search of multiple databases, including PubMed (1966-2011), Cochrane Controlled Trials Register (CENTRAL; issue 1, 2011), and Embase (1984-2011), was conducted to identify studies that met the inclusion criteria. Methodologic quality was assessed and relevant data were retrieved, and if appropriate, meta-

BACKGROUND: Motor palsy is a serious complication that can result from cervical surgery. We introduced cases of motor palsy after posterior cervical foraminotomy (PCF) and consider cervical anatomy.
METHODS: Between January 2007 and August 2010, 133 PCFs were performed on 106 consecutive patients with radiculopathy caused by foraminal stenosis or posterolateral disc herniation.
RESULTS: Three of 133 (2.3%) levels that underwent PCF developed a motor palsy. Two cases involved the C5 nerve root, and one case involved the C6 nerve root. The cause of the C5 palsy may have been excessive retracti

The purpose of this study was to analyze the pathogenic mechanisms, clinical presentation, and surgical treatment of cervical disc herniation without external trauma. Between 2004 and 2008, 9 patients with cervical disc herniation and no antecedent history of trauma were diagnosed with cervical disc herniation and underwent surgical decompression. Pathogenic mechanisms, clinical presentation, surgical treatment, and prognosis were analyzed retrospectively. In 6 patients, herniation resulted from excessive neck motion rather than from external trauma. An injury from this source is termed an end

BACKGROUND: Anterior decompression and fusion (ADF) has conventionally been used, with stable outcomes, for cervical myelopathy caused by soft disc herniation. However, complications related to bone grafting and recurrence of myelopathy due to adjacent segment degeneration are its drawbacks. The efficacy of laminoplasty as an alternative has been sporadically reported, but no prospective study has been conducted to verify it. The purpose of this study was to determine whether laminoplasty is comparable for this condition.
METHODS: Patients with cervical myelopathy caused by soft disc herniati

STUDY DESIGN: Retrospective study of patients enrolled in a prospective randomized Food and Drug Administration trial with single level cervical disc replacement (CDR) with the ProDisc-C (Synthes, Paoli, PA).
OBJECTIVE: Evaluate the segmental range of motion (ROM) in the cervical spine pre- and postoperative after CDR.
SUMMARY OF BACKGROUND INFORMATION: Each cervical level is believed to have its own biomechanical characteristics, ultimately leading to different sagittal and lateral ROM. Our understanding of the factors that influence motion after CDR continues to change and expand.

BACKGROUND CONTEXT: The use of motion-preserving spinal implants versus conventional arthrodesis instrumentation systems, which stabilize operative segments, necessitates improved understanding of their effect on spinal kinematics and the biomechanically optimal method for surgical reconstruction.
PURPOSE: The primary objective of this study was to measure operative- and adjacent-level kinematics after single- and two-level cervical arthroplasty and compare them with those after anterior cervical arthrodesis. A secondary objective was to locate the centers of intervertebral rotation at the op
