Increased MR signal intensity due to cervical myelopathy. Analysis of 29 surgical cases.

The cases of 29 patients with cervical myelopathy, who had been treated by anterior spine fusion, were reviewed. The relationship between pre- and postoperative magnetic resonance (MR) images was investigated with special reference to increased signal intensity in the spinal cord on the T2-weighted images and the relevance of this finding to clinical conditions. Preoperatively, there were areas of increased signal intensity in 12 patients whereas there were no areas of increased signal intensity in the other 17. The lesions were not clearly demonstrated on T1-weighted images. The pre- and post

Herniated cervical disk presenting as ischemic chest pain.

Myocardial ischemia must be the first concern of every emergency physician in evaluating chest pain in the adult patient. Any suspicion of myocardial ischemia must be promptly evaluated and admitted. The American College of Emergency Physicians has recently published a standards document on the care of chest pain in the adult patient. The emergency physician must be familiar with this document. Once myocardial ischemia and other life-threatening causes are ruled out, one can consider that cervical disk disease may be the cause of chest pain. We present two cases of patients who presented to th

Disc herniation in cervical fracture subluxation.

Previous reports of computed tomographic scan with contrast myelography in cervical spinal cord injury have shown a rate of disc herniation of less than 5%. We hypothesized that injuries associated with forces adequate to cause bone or ligamentous injury in the region of the disc space could be associated with higher and more significant rates of disc herniation. Thirty-seven consecutive traumatic midcervical fracture subluxations were reviewed. Fracture subluxation was defined by fracture of the facet joints, pedicles, or vertebral bodies or more than 3.5 mm subluxation from C2-C3 to C7-T1. R

[Traumatic disk hernia of the cervical spine].

The authors present 26 cases of traumatic disc herniation in the cervical spine submitted to anterior discectomy. All of the patient presented neurologic deficit, being 18 classified as Frankel A, three as Frankel B, 2 Frankel C and 3 Frankel D. There were observed three deaths in the immediate post-operative period due to respiratory insufficiency in patients classified as Frankel A. The other 23 cases have a follow-up of 14 months (6-84 months), presenting improvement of the neurologic deficit in six patients classified as Frankel B, C and D and only one as Frankel A. The authors emphasize t

Esophageal perforation during surgery on the cervical spine.

Esophageal perforation during surgery for cervical disc herniation is a rare complication. Differences in the time of diagnosis of this complication in three patients--no delay and delays of 6 hours and 4 days, respectively--resulted in different symptoms and outcomes. Early detection of the perforation allows prompt treatment. In the early stage, primary suture and, if necessary, interposition of vital tissue are sufficient to complete healing. In later stages, only drainage procedures, sometimes with a diversion of the salivary leakage, are indicated. In the reported patients, the outcome wa

[Clinical analysis of ossified thoracic ligaments and thoracic disc hernia].

Thoracic lesions present several clinical problems, particularly in their diagnosis and treatment, compared with cervical or lumbar lesions. Since 1983, 18 cases of thoracic space lesions, excluding spinal tumors or trauma have been experienced: nine cases of ossification of yellow ligament (OYL), five of ossification of posterior longitudinal ligament (OPLL), and four of disc hernia (DH). In these 18 patients, problems of clinical manifestations, neuroradiological examination, and surgical approaches are analyzed and discussed. As clinical manifestations, there was a preponderant occurrence i

Compression of the cervical spine cord after reduction of fracture dislocations. Report of 2 cases.

Two cases of severe neurologic damage after open reduction of locked facet joint dislocations of the cervical spine are described. Both patients had progressive neurologic deterioration after the accident. In both cases, the cause of the neurologic injury was sequestrated disk material compressing the spinal cord. One patient, who was reoperated on as an emergency, recovered after removal of the sequestra, whereas the other patient, who was reoperated on later, remained tetraplegic.

[Neurosurgery in old age. I: Significance of the problem--herniated disk, spinal canal stenosis--craniocerebral injuries--chronic subdural hematoma--resorptive hydrocephalus].

The relative percentage of patients over 60 years who underwent operations in our clinic has greatly increased from 18% in 1874 to 27% in 1989. Postoperatively, difficulties may also result from additional medical problems, which may be cardiovascular, pulmonary, metabolic (especially diabetes mellitus), increased thromboembolism in origin as well as from the reduced tendency to neurological recovery in elderly patients. On the other hand, advances in diagnostics and surgical treatment often make it possible to help even these patients effectively today. The clinical pictures which are especia

Tears of the anulus fibrosus: assessment with Gd-DTPA-enhanced MR imaging.

T2-weighted images have been shown to be capable of defining anular tears in vitro as increased signal intensity within the normal low-signal-intensity anulus fibrosus. Since growth of granulation tissue into anular tears has been described as part of the healing process, it seemed likely that gadolinium-DTPA should enhance anular tears as it does scar tissue in other parts of the spine. We retrospectively reviewed spinal MR images from 30 previously unoperated patients and correlated areas of increased signal intensity within the anulus on T2-weighted images with areas of enhancement on T1-we

["Acro-erythro-cyanosis"--peculiar vasomotor symptoms due to cervical hernial myelopathy].

Two cases of cervical myelopathy, which exhibited peculiar vasomotor symptoms ("acro-erythro-cyanosis") on distal regions of the four limbs are reported. Continuous reddening, swelling and skin temperature increase were observed on both hands and feet in case 1, a-44-year-old man, and on both hands in case 2, a-47-year-old man. Cold stimulation resulted in cyanosis and decrease of skin temperature on the affected regions paroxysmally. The condition of skin at room temperature may be caused by arteriectasia of arterioles due to hypotonia of vasomotor fibers, and this was similar to erythromelal

Operations for the unstable cervical spine in rheumatoid arthritis. Sixteen cases of subaxial subluxation.

Sixteen patients with seropositive rheumatoid arthritis were operated on for subaxial subluxations. Four of the patients had slight, but progressive, tetraparesis, and 5 had severe or total tetraparesis; they were operated on 1-4 months after the first signs. Seven patients were treated for severe neck and shoulder pain. Nine patients had subluxation at the C3-4 level, the most common site, and 3 patients also had an atlantoaxial subluxation. Patients with cord compression were treated with posterior laminectomies and fusions that relieved the tetraparesis. Two patients died during the early p

A radiographic and histologic study of the topographic relations in the temporomandibular joint region: implications for a nerve entrapment mechanism.

A radiographic and histologic investigation was performed in 18 temporomandibular joint (TMJ) autopsy specimens. Disc position was determined arthrotomographically. The pathway of the nerve branches in the vicinity of the joint was reconstructed from serial sagittal or frontal histologic sections. The relationship between the joint components and the different nerves running in the vicinity of the joint was studied. The results revealed the existence of topographical prerequisites for mechanical influence upon the nerve branches passing in the TMJ region. In two joints, both with a displaced d

MRI in cervical disk herniation.

Eleven operated herniated disks in 10 patients were evaluated preoperatively with plain films, myelography and magnetic resonance imaging. Plain X-ray was a valuable supplement to MRI for studying the bony changes. Myelography showed 7 of 11 herniated disks while MRI gave correct diagnosis in all. It is concluded that MRI can replace myelography and computerized tomography in the preoperative evaluation of cervical herniated disk. The other examinations may be supplementary in some cases.

Flexion teardrop fracture of the cervical spine: radiographic characteristics.

Teardrop fracture of the cervical spine is a confusing and loosely used term, often referring to any fracture with a triangular fragment in the involved body. The flexion teardrop fracture is a specific entity that should not be confused with other types of injury with a teardrop fragment. In a radiographic analysis of 45 patients with flexion teardrop fracture, the most characteristic feature was posterior displacement of the upper column of the divided cervical spine, observed in 78% of the cases. Other radiographic characteristics included backward displacement of the posterior fragment of

MR imaging of the pars interarticularis.

MR imaging of the lumbar spine has become a useful method for the noninvasive evaluation of low back pain. However, bone abnormalities are more difficult to detect than soft-tissue lesions, such as herniated disk. We reviewed 14 MR images of the lumbar spine in adults with spondylolisthesis. These were correlated with CT scans and plain films in all cases. From the CT scans and plain films we found that seven patients had spondylolysis and that seven had other causes for their spondylolisthesis. It was our opinion that the MR images suggested an abnormality of the pars interarticularis in all

Radiological features of dialysis amyloid spondyloarthropathy.

Nine patients undergoing regular dialytic treatment (RDT) for more than 60 months (mean 125 +/- 33 months) showed clinical and radiological evidence of non-infective destructive spondyloarthropathy (DSA). The cervical spine was the skeletal segment most affected (100% of cases). Three patients were found also to be suffering from discal and bone alterations of the dorsal column, and in two other patients the vertebral bodies L4-L5 were changed. Typical radiological pictures showed a narrowing of intervertebral spaces with the destruction or sclerosis of adjacent subchondral bones, erosions of

Nonunion of the clavicle and thoracic outlet syndrome.

Fifteen adult patients with clavicular nonunions were evaluated over a 10-year period. Fourteen of these patients were treated initially with a "figure-of-eight" clavicle strap. Seven presented with narrowing of the costoclavicular space, and symptoms of intermittent brachial plexus impingement or thoracic outlet syndrome. Two of these resulted from resection of the mid-portion of the clavicle with subsequent impingement from hypertrophy of the lateral stump. Most commonly, hypertrophic nonunion of the clavicle caused the thoracic outlet syndrome, frequently many months after the initial injur

Biomechanical evaluation of cervical spinal stabilization methods in a human cadaveric model.

The authors have previously reported in vitro testing of various posterior and anterior constructs (sublaminar, Rogers', and Bohlman's triple-wire wiring; AO hook plate fixation; and Caspar anterior plate fixation) in a bovine model with multiaxial biomechanical testing. This study was undertaken to evaluate the above constructs and other constructs in human cadaveric spines. Six subaxial human cervical spine specimens were biomechanically tested at the C5-C6 motion segment both intact and with a simulated distractive-flexion Stage 3 injury created at the C5-C6 level with complete disruption o
