During the past six years 60 patients have undergone anterior cervical microsurgical discectomy without exogenous intercorporal grafting using the operating microscope. This alternative method of treatment is a further refinement of previous techniques for the removal of cervical disc protrusions, which fulfils all the criteria of effective surgery. Discectomy provides ample exposure of the neural elements through the disc space. An open study with iohexol (Omnipaque) in cervical myelography has been performed in 20 patients to evaluate the properties and neurotoxicity of this new water soluble contrast medium. The image quality was good (30%) or excellent (70%) in all cases. The frequency of headache was 25%. No EEG changes were severe and the total frequency was 20%. We conclude that this study supports other reports of iohexol having a low neurotoxicity and is well suited for intrathecal use.

In this epidemiological study of acute prolapsed cervical intervertebral disc, we found that people in the fourth decade of life were affected somewhat more frequently than individuals in other age groups, and men with a prolapsed cervical disc outnumbered women by a ratio of 1.4 to one. The long-term results of surgical treatment of 102 patients for cervical myeloradiculopathy due to spondylosis or disc hernia are reported and discussed. The result was excellent or good in 63% of cases and progression of the disease was arrested in over 90%. Analogous to its other fields of application, CT of the spine and the spinal canal has also already achieved a permanent place among the diagnostic methods. Numerically, CT studies of the cervical, thoracic and lumbar spine for our patient population with a total of 460 examinations are in the ratio 2: 1: 4.5. The essential advantages are improved differential diagnosis for many processes, the exact breakdown of the degree of extraspinal and intraspinal involvement in the change, the increasing limitation of myelography and last but not least the possibility of reducing cost.

Ventral decompression was evaluated as a treatment for cervical disk protrusion in large- and giant-breed dogs. All of the dogs improved after ventral decompression. After a follow-up period averaging 16 months, half of the dogs were clinically normal and the other half were functional pets despite minor residual proprioceptive deficits. Ventral decompression alone appeared to be acceptable treatment for dogs not having other radiographic signs of caudal cervical spondylopathy.

Determinations of cerebrospinal fluid (CSF) albumin, IgG, albumin blood brain barrier (BBB) permeability and local IgG synthesis indexes in CNS were carried out on 85 patients with various neck, shoulder and upper extremity pain syndromes. CSF was obtained by lumbar puncture in 29, and by lateral neck puncture in 56 of the patients. The patients were classified into 3 different groups according to varying severity of degenerative changes, or cavitation verified by myelography. CSF protein patterns in these patients were compared with lumbar CSF findings in 18 patients with multiple sclerosis.

We report 25 cases of cervical disc lesion, treated with antero-lateral microsurgical discectomy without intercorporal graft. The posterior osteophyts were removed and the posterior longitudinal ligament lateral resected. In the control period of 6 months to 4 years we have in 84% of cases excellent/good results clinically and radiologically and concerning the working capacity. Comparing the results with 51 other cases operated on with the fusions technique after Cloward, there are no significant differences. We give a short description of the technique and review the literature.

Somatosensory evoked potentials (SEPs) were recorded in 16 healthy controls and 12 patients suffering symptoms typical of cervical spondylosis and/or herniated disc and showing abnormal findings both in clinical and myelographic examinations. Twenty four patients with spastic paraparesis due to cervical myelopathy were operated on by the anterior approach. This study evaluates the histological and histochemical changes and osteogenicity of Synthos (tricalcium phosphate ceramic) implanted in the cervical intervertebral space. The cervical vertebrae from C-3 to C-6 were exposed in 20 dogs, and discectomy was performed at the third and fifth spaces. Metrizamide seems to be a suitable contrast medium for ventriculography and myelography. This contrast medium is less toxic and far less epileptogenic and spasmogenic than the other water-soluble contrast media in current use. No serious complications were noted in the present series. Transient changes in EEG were recorded, mainly with cervical myelographies. However, it is too early to evaluate the long-term side effects.

Fifty-one patients with symptomatic cervical disc disease refractory to conservative management were allocated at random to one of two treatment groups. The standard anterior approach devised by Cloward was used for 25 patients, and radical discectomy and foraminotomy for the other 26.
