A 64-year-old patient with stage IV non-small-cell lung carcinoma and several comorbidities, which include obesity and long-term smoking, was treated with N-allyl noroxymorphone eluting osteoinductive bone graft biomaterial. The patient had multilevel degenerative disk disease (DDD), which has a high rate of failure when osteoinductive bone grafts are not used. Infuse, the most widely administered osteoinductive bone graft, is contraindicated in the spine for patients with active tumor. As such, a novel drug eluting osteoinductive biomaterial was administered to this patient, for whom no other

BACKGROUND CONTEXT: Anterior Cervical Discectomy and Fusion (ACDF) is a surgical technique that can be used to treat several conditions of the cervical spine. Small sized studies have reported differences in radiological findings between cage only and cage with plate fusion techniques but no differences in clinical outcome, hence, larger studies are needed.
PURPOSE: The aim of this study was to compare the outcomes of ACDF between cage only and cage with plate fixation.
STUDY DESIGN: Retrospective study design on prospectively collected registry data.

Within the variable course of ankylosing spondylitis (AS), peripheral arthritis, enthesitis, and involvement of other organs can add to the burden of the disease. The primary complaints of patients with spondyloarthritis (SpA) are pain, stiffness, fatigue, and limitation in activities and social participation. Instruments currently available for the assessment of patients with SpA focus predominantly on specific aspects of health such as pain, disease activity, and physical function and measure specific concepts like physical function and health-related quality of life (HR-QoL). However, the o

BACKGROUND: Some studies suggest sedentary behavior is a risk factor for musculoskeletal disorders. This study aimed to investigate the potential causal association between leisure sedentary behavior (LSB) (including television (TV) viewing, computer use, and driving) and the incidence of sciatica, intervertebral disk degeneration (IVDD), low back pain (LBP), and cervical spondylosis (CS).
METHODS: We obtained the data of LSB, CS, IVDD, LBP, sciatica and proposed mediators from the gene-wide association studies (GWAS). The causal effects were examined by Inverse Variance Weighted (IVW) test,

BACKGROUND CONTEXT: Hybrid surgery (HS), which involves both anterior cervical discectomy and fusion (ACDF) and cervical disc replacement (ACDR), is increasingly used to treat multilevel cervical disc degenerative disease, yielding satisfactory clinical outcomes. Early fusion is critical after anterior cervical fusion surgeries, but there are no studies comparing the rate of early fusion of HS with that of ACDF.
PURPOSE: The purpose of this study was to compare the rate of early fusion (3-6 months postoperatively) of two-level HS with that of two-level ACDF surgery.
STUDY DESIGN: The fusion

BACKGROUND: Vertebral fractures in young populations are associated with intervertebral disc disorders later in life. However, damage to the annulus fibrosus has been observed in rapidly loaded spines even without the subsequent occurrence of a fracture. Therefore, it may not be the fracture event that compromises the disc, but rather the manner in which the disc is loaded. The purpose of this study was to quantify the mechanical properties of the annulus fibrosus following quasi-static compressive loading of the motion segment either to sub-fracture or fracture-inducing magnitude.
METHODS: P

PURPOSE: To evaluate the association between facet joints cross-sectional area asymmetry (FCAA) and cervical intervertebral disc herniation (CDH).
METHODS: Overall, we retrospectively recruited 390 consecutive patients with CDH who underwent surgical treatment at our institution and 50 normal participants. Clinical variables and radiological findings related to CDH were collected.
RESULTS: Patients with CDH were more likely to have a higher absolute value of the facet asymmetry factor (FAF) (p < .001), in which the FAF value of the left group was significantly higher than the other groups (

Compression of the spinal cord at the cervical level of the spinal column is the hallmark of the disorder known as cervical myelopathy. The aberrant reflexes, hyperreflexia, pathologic reflexes, clumsiness in the hands and fingers, and disturbance of the gait are caused by this compression. It usually starts slowly, increases gradually, and eventually results in a functional decline. For patients older than 55, the most common cause of spinal cord dysfunction is cervical spondylotic myelopathy (CSM). The traditional definition of the pathogenesis of CSM is multilevel spondylosis, where osteoph

BackgroundAnterior cervical discectomy and fusion (ACDF) for the surgical treatment of cervical degenerative disease often includes an intervertebral cage, which restores disc height and lordosis while promoting fusion . Cage materials include titanium (TTN) or polyetheretherketone (PEEK). Controversy in material selection stems from higher fusion rates with TNN, despite a higher subsidence rate, while PEEK cages demonstrate superior preservation of interspace height. Combining the advantages of both materials, TTN-coated PEEK (TCPEEK) cages were developed, featuring a PEEK core with similar s

BACKGROUND CONTEXT: In the treatment of cervical radiculopathy due to a herniated disc, potential surgical treatments include: anterior cervical discectomy (ACD), ACD and fusion using a cage (ACDF), and anterior cervical disc arthroplasty (ACDA). Previous publications yielded comparable clinical and radiological outcome data for the various implants, but research on their comparative costutility has been inconclusive.
PURPOSE: To evaluate the cost utility of ACD, ACDF, and ACDA.
STUDY DESIGN: Cost-utility analysis.

BACKGROUND: Chronic neck or back pain is a common clinical problem. The most likely cause is degenerative change, whereas other causes are relatively rare. There is increasing evidence on using hybrid single-photon emission computed tomography (SPECT) to identify the pain generator in spine degeneration. This systematic review explores the diagnostic and therapeutic evidence on chronic neck or back pain examined by SPECT.
METHODS: This review is reported in accordance with the PRISMA guidelines. In October 2022, we searched the following sources: MEDLINE, Embase, CINAHL, SCOPUS, and 3 other s

BACKGROUND: Cervical fusion surgery is associated with adjacent-level degeneration, but surgical and technical factors are difficult to dissociate from the mechanical effects of the fusion itself.
OBJECTIVE: To determine the effect of fusion on adjacent-level degeneration in unoperated patients using a cohort of patients with congenitally fused cervical vertebrae.
METHODS: We identified 96 patients with incidental single-level cervical congenital fusion on computed tomography imaging. We compared these patients to an age-matched control cohort of 80 patients without congenital fusion. We qua

BACKGROUND: Schmorl's node (SN) corresponds to nucleus pulposus herniation into the vertebral spongy bone with thickened trabeculae around the formed node. We hypothesize that a pathway may exist that: osteoporosis → weakened endplate → SN development ↔ endplate fracture of an osteoporotic vertebra.
METHODS: For osteoporotic fractures in men (MrOS) and in women (MsOS) Hong Kong studies, at 14-year follow-up, thoracic spine magnetic resonance imaging (MRI) was sampled in 270 males (mean: 82.9±3.7 years) and 150 females (mean: 81.5±4.3 years). SN and Modic change were assessed as existed or not

OBJECTIVE: Cervical disc replacement (CDR) is an alternative treatment to anterior cervical discectomy and fusion (ACDF), which is the current gold standard, for degenerative cervical diseases such as cervical spondylotic myelopathy and cervical radiculopathy. CDR has several theoretical benefits over ACDF, including preservation of motion, earlier return to unrestricted activity, and potentially a lower risk of adjacent-segment disease. Recent literature has reported positive clinical results for CDR, but few studies have investigated the long-term risk of revision surgery of CDR versus ACDF.

BACKGROUND AND OBJECTIVES: Anterior cervical discectomy and fusion (ACDF), which is widely used in the treatment of cervical spondylosis, may cause complications such as adjacent segment degeneration (ASD). However, such studies on complications are limited, and quantitative evidence is still lacking. To evaluate the clinical value of cervical discometry combined with adjacent intraoperative intradiscal pressure in cervical vertebral surgery through clinical explorations.
MATERIALS AND METHODS: In this retrospective study, 100 patients who were treated with anterior decompression, reconstruct

Background and Purpose: Evidence has emerged for an association between degenerative disc disease (DDD) and multiple sclerosis (MS). The purpose of the current study is to determine the presence and extent of cervical DDD in young patients (age <35) with MS, an age cohort that is less well studied for these changes. Methods: Retrospective chart review of consecutive patients aged <35 referred from the local MS clinic who were MRI scanned between May 2005 and November 2014. 80 patients (51 female and 29 male) with MS of any type ranging between 16 and 32 years of age (average 26) were included.
