Cryptosporidiosis in inborn errors of immunity: a single center cohort and review of the literature.

BACKGROUND: Severe intestinal or hepatobiliary cryptosporidiosis affects patients with inborn errors of immunity (IEI), particularly those with CD40 ligand or CD40 deficiency, major histocompatibility complex class II deficiency, interleukin-21 receptor deficiency, and DOCK8 deficiency. Susceptibility to Cryptosporidium reflects defects in lymphocyte development, activation, and effector function. METHODS: We conducted a retrospective chart review of IEI patients with cryptosporidiosis, focusing on 17 individuals with chronic infection who underwent hematopoietic cell transplant (HCT), to cha

Downregulation of HNF4α Mediates Microvillus Damage During Cryptosporidium parvum Infection.

Cryptosporidium is a significant zoonotic protozoan parasite that primarily infects intestinal epithelial cells, causing microvillus damage and other gastrointestinal malfunctions. However, the precise molecular mechanisms underlying this process have yet to be elucidated. In this study, IFN-γ knockout mice were infected with a virulent Cryptosporidium parvum IId isolate, and intestinal pathological changes and microvillus alterations were monitored throughout the infection. The results showed that C. parvum invasion induced both intestinal and microvillus damage in the host. Ileum tissues wer

CpCML subverts host PI3K/AKT-NF-κB signaling and autophagic flux via direct interaction with SUGT1 to facilitate Cryptosporidium parvum infection.

Cryptosporidium parvum is a water and foodborne apicomplexan pathogen that vigorously disrupts host signaling networks to enable intracellular colonization. However, the parasite-encoded effectors responsible for this modulation and the specific host signaling cascades they involve is still unexplored. In this study, we identify CpCML, a calmodulin-like protein encoded by C. parvum, as a perilous virulence effector that modulates host PI3K-NF-κB signaling and autophagic flux through direct interaction with the host protein SUGT1. Biochemical and cellular analyses reveal that CpCML binds SUGT1

Zoonotic Cryptosporidium and Giardia infections in pigeons (Columba spp.) at a wildlife hospital in France: Occurrence and molecular identification.

The feral rock pigeon (Columba livia domestica) and wood pigeon (Columba palumbus) are ubiquitous species found in cities worldwide. Its interaction with humans, domestic animals and wild birds makes them potential carriers of transmissible pathogens, including zoonotic parasites. The aim of this study was to identify protozoan parasites belonging to the genera Cryptosporidium spp. and Giardia spp. in these two pigeon species hosted at the Wildlife Animal Hospital of the Veterinary School of Alfort, in France. Fecal samples from 367 pigeons (Columba livia and Columba palumbus) including a wide

The role of noncoding RNAs in host-Cryptosporidium interactions.

Increasing evidence suggests that regulatory noncoding RNAs (ncRNAs) modulate a series of epithelial responses following Cryptosporidium invasion. During infection, ncRNAs are involved in the activation of intracellular signaling pathways, the production of antimicrobial molecules, the expression of cytokines/chemokines, the release of epithelial cell-derived exosomes, and the feedback regulation of immune homeostasis. In addition, Cryptosporidium may have developed strategies to modulate host ncRNA-mediated cellular function for immune evasion. These findings indicate that ncRNAs may be impor

Genomic heterogeneity of NAD(P)H dehydrogenase predisposes Cryptosporidium to clofazimine resistance.

The parasite Cryptosporidium is a leading cause of life-threatening diarrhoeal disease, and effective treatment is not available. Clofazimine, an antimicrobial used for treatment of leprosy and tuberculosis, was found to have potent anti-Cryptosporidium activity but it failed in a human trial. This was attributed to poor bioavailability. Here we observed differential clofazimine susceptibility among C. parvum parasite isolates, which we exploit to identify a single genomic locus encoding the type II NADH dehydrogenase (NDH2) in an unbiased genetic cross. Targeted genetic ablation of ndh2 resul

Cryptosporidium secreted proteins form a complex layered interface with the host cell.

Cryptosporidium parvum, one of the leading causes of diarrheal death in children, remodels its infection site through secreted effector proteins. Several of them accumulate at the host-parasite interface, forming a complex structure whose function and importance for infection remain poorly understood. Here, we localized and functionally characterized the putative dense granule protein DG8. We confirmed the protein to be in the dense granules, forming a ring structure once secreted at the host-parasite interface. Deletion of DG8 showed reduced infection in mice in vivo and early defect in vitro

Mechanistic insights into restoration of gut barrier and microbiota homeostasis by allicin and Qingchang Huashi formula in Cryptosporidium parvum-infected immunosuppressed murine model.

Nitazoxanide (NTZ), the only FDA-approved anti-Cryptosporidium drug, shows inconsistent efficacy in immunocompromised patients. This study employed HCT-8 cell-based cytotoxicity/parasite inhibition assays and an immunosuppressed murine model to explore the anti-C. parvum efficacy and mechanisms of combined allicin and Qingchang Huashi Formula (QHF). In vitro, WST-1-based cytotoxicity screening preceded inoculation of 4 × 104 C. parvum oocysts to assess parasitic inhibition. In vivo, sixty immunosuppressed ICR mice were divided into control (C), model-infected (M), paromomycin-treated (P), alli

Gold-containing compounds target an apicomplexan Thioredoxin reductase: Disclosing reactivity through structural and functional characterization.

Cryptosporidium spp. cause cryptosporidiosis, a severe diarrheal disease, particularly in young children and immunocompromised individuals. Treatment options are limited, and the only approved drug, nitazoxanide, is poorly effective in the most susceptible populations. Cryptosporidium relies exclusively on the thioredoxin reductase/thioredoxin (TrxR/Trx) system for redox homeostasis as Cryptosporidium lacks a glutathione reductase gene. Consistent with this vulnerability, TrxR ablation blocks parasite proliferation and sexual development, identifying TrxR as an essential drug target. C. parvum

Updated mapping of Plasmodium falciparum kelch13 gene polymorphisms in Madagascar, 2024.

OBJECTIVES: Artemisinin-resistant Plasmodium falciparum has emerged in several East African countries neighbouring Madagascar. Despite the island's substantial malaria burden, recent data on artemisinin partial resistance are limited, raising concerns about the potential emergence of resistant parasites. This study provides an updated overview of the prevalence and diversity of P. falciparum Kelch13 (pfkelch13) polymorphisms in Madagascar. METHODS: During a nationally representative, cross-sectional survey conducted between January and May 2024, dried blood samples were collected from 4850 fe

Randomized clinical trial evaluating triazolopyridazine SLU-2633 as a treatment for gastric cryptosporidiosis in eastern indigo snakes (Drymarchon couperi).

Gastric cryptosporidiosis caused by Cryptosporidium serpentis has negatively impacted the conservation efforts for the imperiled eastern indigo snake (EIS, Drymarchon couperi). Multiple treatments, including paromomycin, nitazoxanide-azithromycin-rifabutin, clofazimine, curcumin, hydrogen peroxide gavage, and triazolopyridazine MMV665917 have proved ineffective in eliminating the parasite. In a structure-activity relationship study, a 10-fold more potent analog of MMV665917 (SLU-2633) was discovered. Additionally, greater potency of SLU-2633 improved the therapeutic index of this compound. Whe

Quantitative sugar-flotation in a cover-slide system for quantifying Cryptosporidium parvum oocysts in feces: optimizing specific gravity, floating time, and counting strategies.

Cryptosporidium parvum is a zoonotic protozoan parasite that causes cryptosporidiosis, a disease characterized by intestinal tissue damage in animals and humans. The oocysts of Cryptosporidium parvum, which are the infective stage of this parasite in feces, are extremely small (4-6 μm in diameter), making microscopic detection and quantification difficult. In the present study, we developed a quantitative sugar-flotation in a cover-slide system (QUSIC), which is a novel, simple, sensitive, and cost-effective method for quantifying C. parvum oocysts in feces. Fecal samples from non-infected mic
