BACKGROUND: Trichomonas vaginalis, which affects at least 170 million individuals globally, may increase the risk of transmission of HIV and predispose pregnant women to premature rupture of membranes and early labour.
OBJECTIVE: To more clearly define the epidemiology of trichomoniasis and to develop a mathematical model of disease transmission dynamics in order to explore various treatment strategies.
DESIGN: A deterministic model of trichomoniasis was constructed. Parameter values were set to fit the model to known endemic prevalence levels of Trichomonas vaginalis. Two treatment interven

The ability of infectious disease agents to cross the species barrier has long been recognised for many zoonotic diseases. New viral zoonotic diseases, such as acquired immune deficiency syndrome (AIDS), caused by human immunodeficiency viruses 1 or 2, emerged in the 1980s and 1990s, and have become established in the human population. Influenza virus continues to find new ways to move from avian species into humans. The filoviruses and the newer paramyxoviruses, Hendra and Nipah, highlight the increasing proclivity of some animal viral agents to infect human populations with devastating resul

Patients with active diarrhea caused by infection with Cryptosporidium parvum can potentially contaminate the environment, which could serve as a risk for transmission to other patients in a hospital setting. A retrospective cohort study was performed to quantify the risk of nosocomial roommate-to-roommate transmission of Cryptosporidium and to evaluate the need for isolation of Cryptosporidium-infected patients. Thirty-seven human immunodeficiency virus (HIV)-infected roommates of 21 index patients with Cryptosporidium were identified between 1994 and 1996. Each exposed roommate (median CD4 c

According to data by H. Zhabilov serum fractions in the zone of alpha 1, alpha 2 and beta-globulins from healthy people form a characteristic precipitation curve in terms of structure and density with the TNP (thymus nuclear protein) and CNP (carcinoma nuclear protein) as antigens when tested in two-dimensional agarose-gel electrophoresis. The observed immune phenomenon has not so far, to the best of our knowledge, been reported in the scientific literature and has profound implications for fundamental and applied immunology. The object of the present study was to confirm Zhabilov's data and s

AIMS: A retrospective study was carried out to determine the histopathological alterations of the gastric mucosa associated with the presence of Cryptosporidium parasites in the stomach.
METHODS AND RESULTS: Gastric biopsies from eight patients with AIDS and cryptosporidiosis were reviewed. Most individuals were males (7/8, mean age 34.7 years) intravenous drug users (6/8), with very low peripheral blood CD4 lymphocyte counts (< 150/mm3; mean 56/mm3). Endoscopic examination showed no significant mucosal abnormalities in half of the individuals; inflammation was observed in the other four indi

BACKGROUND: Patients with AIDS frequently show secondary involvement of the brain by different infectious agents, and Chagas' disease is now recognized as a potential opportunistic infection. To our knowledge, pseudotumoral chagasic meningoencephalitis has not been previously reported as the first manifestation of AIDS.
METHODS AND RESULTS: A 30-year-old Argentinian man without any risk factor for HIV infection was admitted to the hospital with an acute onset of drowsiness. A computed tomography scan showed a hypodense parietal tumor-like lesion. Open brain biopsy revealed hemorrhagic necrosi

OBJECTIVE: Potential transmission of cryptosporidiosis through drinking water supplies has been highly publicized; however, it is unknown whether this reporting has increased patient awareness or reduced other risk behaviors for exposure to this organism, such as high-risk sexual behavior.
METHODS: Consecutive patients presenting for initial evaluation to the Gastroenterology AIDS Clinic completed a questionnaire that assessed knowledge about cryptosporidiosis, perceived risk of infectious diarrhea, drinking water sources, and high-risk sexual behavior.
RESULTS: Fifty-one patients completed

BACKGROUND: CC chemokine receptor 5 (CCR5) is a cell entry cofactor for macrophage-tropic isolates of human immunodeficiency virus-1 (HIV-1). Recently, an inactive CCR5 allele (designated here as CCR5-2) was identified that confers resistance to HIV-1 infection in homozygotes and slows the rate of progression to AIDS in heterozygotes. The reports conflict on the effect of heterozygous CCR5-2 on HIV-1 susceptibility, and race and risk levels have not yet been fully analyzed. Here we report our independent identification of CCR5-2 and test its effects on HIV-1 pathogenesis in individuals with co

BACKGROUND: Cryptosporidiosis, an intestinal parasitic infection, has gained considerable media attention since a 1993 waterborne outbreak in Milwaukee, Wis, in which more than 400,000 persons became ill. However, the incidence of and risk factors for human cryptosporidiosis in the general US population are unknown. It has been suggested, but not documented, that physicians are generally unaware of the need to specifically request testing for this organism.
OBJECTIVE: To assess physician awareness of cryptosporidiosis and knowledge of laboratory testing for Cryptosporidium oocysts.

A HIV seropositive patient with chronic diarrheic syndrome, in which oocysts of Isospora belli were detected in feces by the modified technique of Ziehl-Neelsen was studied. Other coproparasitological techniques were applied and cultures were applied made with negative results. Any other infectious origin was excluded from the picture. The patient was treated with 2 tablets of Co-trimoxazole (trimethoprin 80 mg, sulphamethoxazole 400 mg) every 6 hours for ten days, with referral of the symptoms.

Leukocytoconcentration is an easy, fast and inexpensive technique for the diagnosis of leishmaniasis from peripheral blood. The technique involves concentration of blood parasites on a small surface of a microscope slide while the red blood cells are removed by lysis. The results, compared with those of other methods (examination of cultures of blood samples and bone marrow smears), were very good and accurate. All but one of our cases of leishmaniasis were patients with HIV co-infection. Leukocytoconcentration facilitates follow-up of cases and fast detection of any relapse.

The control of malaria in pregnant African women, one of several child survival strategies applied through antenatal care, has been particularly challenging. Prevention and control recommendations for typical areas of high Plasmodium falciparum transmission have promoted the use of antimalarial chemoprophylaxis to prevent placental infection. Persistently low program coverage coupled with diminishing intervention effectiveness have forced a re-evaluation of the relative importance of malaria in pregnancy. The Mangochi Malaria Research Project (MMRP), a prospective evaluation of malaria prevent

An increased incidence of visceral leishmaniasis in patients infected with the human immunodeficiency virus (HIV) is observed in areas in which both infectious diseases are endemic. Intensive worldwide traveling has also resulted recently in an increasing number of leishmanial and HIV coinfections in nonendemic areas. We describe the clinical, light microscopic, and ultrastructural features of two cases of imported, HIV-related, visceral leishmaniasis involving the alimentary tract, including the esophagus, the stomach, the duodenum, the ileum, the colon, and the rectum. We also discuss the di

To evaluate whether the detection of Toxoplasma gondii DNA in CSF could contribute to the differential diagnosis of AIDS-related focal brain lesions, CSF samples from 88 HIV-infected patients (56 with focal brain lesions and 32 without) were tested prospectively by a nested PCR for the B1 gene of T. gondii. The assay had a detection limit of 10 trophozoite equivalents. Six of 18 patients with toxoplasmic encephalitis, but none of the 70 patients with other disorders, were PCR-positive (33.3% sensitivity and 100% specificity). Considering only those patients with cerebral toxoplasmosis from who
