The Linda Kizazi study: a comparison of morbidity and mortality from birth to 2 years between children who are HIV-unexposed and HIV-exposed, uninfected in the era of universal antiretroviral therapy.

BACKGROUND: Historically, children who are HIV-exposed, uninfected (CHEU) have been found to have greater morbidity and mortality than children who are HIV-unexposed, uninfected (CHUU). To assess whether this difference persists in the era of universal antiretroviral therapy (ART), we conducted a cohort study to compare the risk of acute diarrhoea, respiratory tract infections (RTI), malaria, hospitalisation, and all-cause mortality between Kenyan CHEU and CHUU from birth to 2 years. METHODS: From December 2018 to March 2020 at Mathare North Health Centre in Nairobi, we recruited pregnant wom

Unveiling the hidden health challenges: malaria, helminths, STIs and other pathologies among street children, adolescents and young adults in Cameroon.

BACKGROUND: The phenomenon of street children, adolescents and young adults (SCAYA) is a growing global challenge. In Africa, >10 million children reside in the streets, driven by poverty, abuse and family instability. In Cameroon, this phenomenon is particularly evident in major cities. Being on the street exposes SCAYA to unsanitary living conditions, sexual exploitation, drugs and health challenges. Despite significant health challenges faced by SCAYA, there are few studies on their health status. METHODS: A cross-sectional study was conducted in Yaoundé, Cameroon, from March to July 2022.

Causes of hospitalisation among people living with HIV worldwide, 2014-23: a systematic review and meta-analysis.

BACKGROUND: Despite improved access to antiretroviral therapy (ART), HIV-related morbidity and mortality remain high. A previous review (2007-14) found that AIDS-related illnesses were the leading causes of hospitalisations. We aimed to summarise the causes of hospitalisations among people living with HIV from 2014 to 2023. METHODS: For this meta-analysis we searched eight databases (Ovid Medline ALL, Ovid Embase Classic and Ovid Embase, Ovid Global Health, EBSCOhost CINAHL Complete, EBSCOhost Africa-Wide Information, Clarivate Analytics Web of Science Core Content, Clarivate Analytics Web of

Comparative genomics of the parasite Trichomonas vaginalis reveals genes involved in spillover from birds to humans.

Trichomonas vaginalis, the causative agent of the venereal disease trichomoniasis, infects men and women globally and is associated with serious outcomes during pregnancy, increased risk of HIV-1 infection, and cancers of the human reproductive tract. Species of trichomonad parasitize a range of hosts in addition to humans, including birds, livestock, and pets. Genetic analysis of trichomonads recovered from columbid birds has provided evidence that they undergo frequent host-switching, and that a spillover event from columbids likely gave rise to T. vaginalis in humans. Here we describe a com

The diagnostic value of two antigenic domains derived from the Trypanosoma cruzi Tc323 protein in chronic Chagas disease: a study in Brazil.

The diagnosis of chronic infection with the parasite Trypanosoma cruzi relies on detecting specific IgG antibodies using two or three (in the case of discordance) serological assays based on different principles. This diagnostic algorithm poses challenges in identifying patients with chronic Chagas disease (CCD), particularly in endemic areas where financial and human resources are scarce. The discovery of new antigens capable of achieving 100% sensitivity and specificity is a priority in this field. Previously, we introduced two recombinant domains, designated as rTcD3 and rTcD6 derived from

Quantitative PCR as a marker for preemptive therapy and its role in therapeutic control in Trypanosoma cruzi/HIV coinfection.

BACKGROUND: Trypanosoma cruzi and HIV coinfection can evolve with depression of cellular immunity and increased parasitemia. We applied quantitative PCR (qPCR) as a marker for preemptive antiparasitic treatment to avoid fatal Chagas disease reactivation and analyzed the outcome of treated cases. METHODOLOGY: This mixed cross-sectional and longitudinal study included 171 Chagas disease patients, 60 coinfected with HIV. Of these 60 patients, ten showed Chagas disease reactivation, confirmed by parasites identified in the blood, cerebrospinal fluid, or tissues, 12 exhibited high parasitemia with

Effects of Schistosoma haematobium infection and treatment on the systemic and mucosal immune phenotype, gene expression and microbiome: A systematic review.

BACKGROUND: Urogenital schistosomiasis caused by Schistosoma haematobium affects approximately 110 million people globally, with the majority of cases in low- and middle-income countries. Schistosome infections have been shown to impact the host immune system, gene expression, and microbiome composition. Studies have demonstrated variations in pathology between schistosome subspecies. In the case of S. haematobium, infection has been associated with HIV acquisition and bladder cancer. However, the underlying pathophysiology has been understudied compared to other schistosome species. This syst

Primary ciliary dyskinesia: Meeting the challenges of diagnosis in South Africa.

Primary ciliary dyskinesia (PCD) is an inherited ciliopathy that results in impaired mucous clearance and affects primarily the respiratory tract, causing upper airway disease, bronchial inflammation and bronchiectasis. The prevalence of PCD in low- and middle-income settings, including South Africa (SA), is unknown, largely owing to challenges with diagnosis, and identifying children or adults with PCD is challenging in a setting with a high prevalence of other infectious diseases, including lower respiratory tract infections and tuberculosis. No single test is diagnostic of PCD, and while so

Intermittent preventive treatment regimens for malaria in HIV-positive pregnant women.

BACKGROUND: Malaria and HIV infection overlap geographically in sub-Saharan Africa and share risk factors. HIV infection increases malaria's severity, especially in pregnant women. The World Health Organization (WHO) recommends intermittent preventive treatment in pregnancy (IPTp) with sulphadoxine-pyrimethamine (SP) for pregnant women living in areas of stable malaria transmission. However, HIV-positive women on daily cotrimoxazole prophylaxis (recommended for prevention of opportunistic infections in people with HIV) cannot receive SP due to adverse drug interactions, so malaria prevention i
