STUDY DESIGN: A cross-sectional population-based magnetic resonance imaging study of Schmorl nodes (SN) in the lumbar spine.
OBJECTIVE: To determine the prevalence and potential determinants of SN, and their association with intervertebral disc degeneration.
SUMMARY OF BACKGROUND DATA: SN represent intravertebral disc herniation and are commonly seen in the spine. Their reported prevalence and determinants vary, and their association with disc degeneration remains uncertain. Data based on this large scale population-based study of intervertebral disc degeneration would provide important info

Schmorl's nodes (SNs) are common abnormalities in the human spine, which represent herniation of the nucleus pulposus of the intervertebral disc into the adjacent cartilaginous endplate of the vertebra. However, the principle mechanism of SNs is still not fully understood. And the relationship of SNs in the spine and their clinical significance as a source of low back pain in the general population remains unknown. It is therefore important to get better understanding of this. Here, we review the clinical and experiment evidence on inducing of the SNs and correlative back pain, and propose a p

INTRODUCTION: Degenerative disc disease (DDD) is a major health problem worldwide. Both Modic lesions and Schmorl's nodes are considered to correlate with DDD such as low back pain. Modic lesions are the changes of degenerative vertebral endplate and adjacent bone marrow observed on magnetic resonance imaging and are divided into three types. Modic type III lesions are thought to represent extensive subchondral bone sclerosis within the bone marrow of adjacent endplate. The pathological performance of Schmorl's nodes is cystic lesions around indistinct sclerotic margins and beneath the cartila

Although Schmorl's nodes (SNs) are a common phenomenon in the normal adult population, their prevalence is controversial and etiology still debatable. The objective was to establish the spatial distribution of SNs along the spine in order to reveal its pathophysiology. In this study, we examined 240 human skeleton spines (T4-L5) (from the Hamann-Todd Osteological Collection) for the presence and location of SNs. To determine the exact position of SNs, each vertebral body surface was divided into 13 zones and 3 areas (anterior, middle, posterior). Our results show that SNs appeared more frequen

OBJECTIVE: To investigate the type of MR imaging diagnosis that corresponded to spinal lesions detected on a bone scan according to the number of lesions in breast cancer patients.
MATERIALS AND METHODS: We retrospectively reviewed spinal MR images of 134 patients with breast cancer whose bone scans showed one or more spinal hot uptakes. The patients were grouped according to the number of spinal lesions detected on the bone scan: one or two lesions versus multiple (more than two) lesions. By using MR imaging, we determined the etiology of the spinal lesions in terms of being either benign or

BACKGROUND: Correlation between clinical features and magnetic resonance imaging (MRI) findings is essential in low-back-pain patients. Most previous studies have analyzed concordance in the interpretation of lumbar MRI among a few radiologists who worked together. This may have overestimated concordance.
PURPOSE: To evaluate intra- and interobserver agreement in the interpretation of lumbar MRI performed in an open 0.2 T system.
MATERIAL AND METHODS: Seven radiologists from two different geographic settings in Spain interpreted the lumbar MRIs of 50 subjects representative of the general Da

STUDY DESIGN: A descriptive study of the association between Schmorl nodes (SNs) and gender, ethnic origin, and age in a normal skeletal population.
OBJECTIVES: To gain reliable data on behavioral patterns of SNs in various human groups shedding light on its etiology.
SUMMARY OF BACKGROUND DATA: Opinions regarding SNs prevalence in human populations vary greatly (from 5% to 70%). This caveat greatly reduced our ability to recognize the etiology of the phenomenon and understand its clinical significance.

Lumbar Schmorl nodes usually remain asymptomatic. Painful nodes either heal spontaneously or respond to conservative therapy in most instances. Diagnosis and treatment may be difficult in patients presenting with chronic back pain. We present a 31-year-old man with a lumbar Schmorl node that was unrecognised for 10 years as the origin of his severe chronic back pain. Finally, MRI revealed a significant oedematous rim around a huge Schmorl node in the L4 vertebra. After conservative therapy failed the patient underwent a successful fluoronavigation-assisted, percutaneous vertebroplasty. In the

We report the development of acute Schmorl nodes at the L3-L4 intervertebral disc level after discography in a 36-year-old man. Although a few cases of acute Schmorl nodes have been reported in the literature, they have not been described because of discography. We surmise that the herniation of disc material through the vertebral endplates, with the ensuing formation of Schmorl nodes, should be regarded as a potential, but fortunately rare, complication of discography.

BACKGROUND: Matrix metalloproteinases (MMPs) are involved in the degradation of the extracellular matrix of the intervertebral disc. A SNP for guanine insertion/deletion (G/D), the -1607 promoter polymorphism, of the MMP1 gene was found significantly affecting promoter activity and corresponding transcription level. Hence it is a good candidate for genetic studies in DDD.
METHODS: Southern Chinese volunteers between 18 and 55 years were recruited from the population. DDD in the lumbar spine was defined by MRI using Schneiderman's classification. Genomic DNA was isolated from the leukocytes an

BACKGROUND: Nuclear replacement is an emerging surgical treatment for degenerative disc disease (DDD) and low back pain (LBP). While clinical experience is most extensive with the prosthetic disc nucleus PDN (Raymedica, Minneapolis, Minnesota), strict indications apply for the implantation of this device. The purpose of this study was to ascertain what percentage of patients treated surgically for degenerative disc disease with other surgical procedures would have been candidates for nuclear replacement implantation.
METHODS: The charts and films of 85 consecutive patients with failed conserv

Scheuermann's disease (SD) or vertebral osteochondrosis is the most frequent cause of non postural kyphosis and one of more frequent cause of adolescent's dorsalgia. The criteria for the diagnosis are: more than 5 degrees of wedging of at least three adjacent vertebrae at the apex of the kyphosis; a toracic kyphosis of more than 45 of Cobb's degree; Schmorl's nodes and endplates irregularities. In addition to classic SD, there are radiological alterations that remain asymptomatic for a long time to reveal in adult age: in that case it speaks of adult Scheuermann's disease (ASD). We considered

OBJECTIVE: Schmorl's nodes (SN) are common, but little is known of their relationship with degenerative change and back pain or genetic and environmental factors influencing their expression. We studied healthy female twin volunteers to determine the prevalence and clinical features associated with SN.
METHODS: Serial sagittal T1- and T2-weighted magnetic resonance images of the lower thoracic and lumbar spine were analyzed in 516 healthy female twins (150 monozygotic and 366 dizygotic). The images were scored for lumbar degenerative change. Presence of SN was noted at cranial and caudal vert

Prior imaging studies of scoliosis patients attempted to demonstrate a relationship between plain radiographic curve patterns and curve progression and pain, or used magnetic resonance imaging (MRI) to focus on spinal cord abnormalities. Pain in scoliosis patients may differ from nondeformity patients, yet may still be discogenic. The purpose of this study was to assess the possible relationship of degenerative disc findings on MRI to scoliosis patients' pain. This prospective study enrolled scoliosis and control patients, all of whom had assessment for back pain (visual analog scale) and disa

STUDY DESIGN: Observational cohort study.
OBJECTIVE: To evaluate by MRI whether a vertebral fracture during childhood is a risk factor for degeneration of adjacent discs.
SUMMARY OF BACKGROUND DATA: Several studies infer that trauma is a major cause of disc degeneration. Only 1 study has by magnetic resonance imaging (MRI) evaluated disc degeneration in children with a former thoracic or lumbar vertebral fracture. That study reported a 50% prevalence of degenerative disc changes 4 years after the fracture. However, due to the sparse literature, it is still unclear whether a vertebral fractur

STUDY DESIGN: A quasi 3-dimensional pathologic survey of tears in the L4-L5 disc.
OBJECTIVE: To seek accurate information on the pathogenesis and outcomes of tears to facilitate correlation with radiologic imaging and biomechanical testing; and to improve laboratory models for testing hypotheses of disc function and failure.
SUMMARY OF BACKGROUND DATA: Tears are evidence of structural failure involving the anulus. There are substantial differences in the structure and function of the anterior and posterior anulus and the nonlamellar "nucleus" is much smaller than generally conceptualized and

Several studies have been published on disc degeneration among young athletes in sports with great demands on the back, but few on competitive divers; however, there are no long-term follow-up studies. Twenty elite divers between 10 and 21 years of age, with the highest possible national ranking, were selected at random without knowledge of previous or present back injuries or symptoms for an MRI study of the thoraco-lumbar spine in a 5-year longitudinal study. The occurrence of MRI abnormalities and their correlation with back pain were evaluated. Eighty-nine percent of the divers had a histo

OBJECTIVE: To describe the patterns and note the evolution of edematous Schmorl's nodes.
MATERIALS AND METHODS: In 47 patients (M:F=26:21, 24-86 years, average 60), 84 Schmorls nodes with T2 hyperintensity with serial MR exams were evaluated. Interval between MR exams was 2-72 months (average 17). Two observers noted size, location, margins, internal and surrounding T1/T2 signal, adjacent disc herniation or bulge, concentric ring, underlying fracture, malignancy, infection, or prior disc surgery, and serial MR changes in these characteristics over time.
RESULTS: Node size averaged 7x9 mm. Mo

A Schmorl's node is the herniation of nucleus pulposus through the cartilaginous end plate of the vertebral disc into the body of the adjacent vertebra. Their formation is relatively common in people, and they may be symptomatic or asymptomatic. In contrast, there are few reported cases of Schmorl's nodes in non-human animals. This report describes a case of thoracic intervertebral disc degeneration, with Schmorl's node formation in a sheep.
