Scheuermann's juvenile kyphosis.

OBJECTIVE: Two cases are discussed to illustrate two different presentations, progressions and treatments of Scheuermann's juvenile kyphosis. CLINICAL FEATURES: In one case, a 13-yr-old boy suffered from a 2-yr history of lower back pain. Radiographs demonstrated irregularity of the upper lumbar vertebral endplates, associated with Schmorl's nodes. The second case is one of a 14-yr-old boy who was seen in an orthopedic outpatient clinic. Radiographs revealed wedging of the anterior border of T6, T7, and T8 vertebrae with a thoracic spine kyphotic deformity measuring 72 degrees. INTERVENTION

Radiologic interpretation of vacuum phenomena.

"Vacuum" phenomena may occur within peripheral joints or in the spine. These phenomena are related to the accumulation of gas, principally nitrogen, under physiologic or pathologic circumstances. Within the spine, vacuums may occur within the intervertebral discs and are useful indicators of intervertebral osteochondrosis , spondylosis deformans, Schmorl's nodes, and limbus vertebrae. In addition, intraosseous vacuum may occur within the vertebral bodies usually related to osteonecrosis. Under physiologic conditions, naturally occurring peripheral vacuums cannot exist in the presence of joint

Schmorl's nodes and low-back pain. Analysis of magnetic resonance imaging findings in symptomatic and asymptomatic individuals.

Magnetic resonance imaging (MRI) findings in cases with symptomatic and asymptomatic Schmorl's nodes have been analysed. In all symptomatic cases, the vertebral body marrow surrounding the Schmorl's node was seen as low signal intensity on T1-weighted images and as high signal intensity on T2-weighted images. It was confirmed by histological examination that the MRI findings indicated the presence of inflammation and oedema in the vertebral bone marrow. These MRI findings were not seen in asymptomatic individuals. Inflammatory changes in the vertebral body marrow induced by intraosseous fractu

Identification of an unknown corpse by means of computed tomography (CT) of the lumbar spine.

For the first time, a case is described in which an unknown corpse is identified by the comparison of antemortem and postmortem computed tomographic (CT) images. A posterolateral disc herniation at L5-S1 on the right side, Schmorl's nodes and a lucency in the ilium were found in identical locations in each case. Additionally there were characteristic morphological similarities in the vertebral bodies, spinous processes, transverse processes and neural arches. In CT identification, as in conventional radiographic identification, one must try to reproduce comparable scanning conditions and image

Schmorl's nodes on magnetic resonance imaging. Their incidence and clinical relevance.

Schmorl's nodes were observed in 76 (19%) of 400 patients with lumbar symptoms, and in 10 (9.4%) of 106 control patients. In the lumbar group, the highest incidence was 57%, in the second decade of life, and the lowest was 5%, in the sixth decade of life. Using MRI, among both groups, a total number of 218 Schmorl's nodes were observed at 170 disc levels; however, plain x-ray revealed only 73 (33%) of them. There were associated posterior disc herniations at 39 disc levels in 30 cases (39%), most of which were at the L4/5 level. The ratio of the cases showing only Schmorl's nodes to those with

Magnetic resonance imaging of the lumbar spine in people without back pain.

BACKGROUND: The relation between abnormalities in the lumbar spine and low back pain is controversial. We examined the prevalence of abnormal findings on magnetic resonance imaging (MRI) scans of the lumbar spine in people without back pain. METHODS: We performed MRI examinations on 98 asymptomatic people. The scans were read independently by two neuroradiologists who did not know the clinical status of the subjects. To reduce the possibility of bias in interpreting the studies, abnormal MRI scans from 27 people with back pain were mixed randomly with the scans from the asymptomatic people. W

Natural history of spinal cord infarction caused by nucleus pulposus embolism.

Nucleus pulposus embolism causing spinal cord infarction is exceptional. A 16-year-old girl was seen with sudden onset of interscapular pain and paraplegia from fatal ischemic transverse myelopathy due to arterial and venous occlusions by fibrocartilaginous embolism. In 32 cases of nucleus pulposus embolism, females predominated (69%) and age distribution was bimodal with peaks at 22 and 60 years (median, 38.5). Embolization was either arterial and venous (50%) or purely arterial (50%). Myelopathy predominated in cervical (69%) and lumbosacral (22%) segments. Schmorl's nodes, larger volume and

The thoracolumbar spine in young elite athletes. Current concepts on the effects of physical training.

Due to the increased interest in physical fitness and to the fact that athletes start their training at younger ages the risk for injuries to the growing individual has increased. The spine, as with the rest of the skeleton, is at greater risk of injury during growth, especially during the adolescent growth spurt. Back pain is more common among athletes participating in sports with high demands on the back than other athletes and nonathletes. Disc degeneration, defined as disc height reduction on conventional radiographs and reduced disc signal intensity on MRI, has been found in a higher freq

Reliability of interpretation of plain lumbar spine radiographs in benign, mechanical low-back pain.

A study was conducted to investigate the variability of interpretation of plain lumbar spine radiographs by rheumatologists in benign low-back pain (LBP). Intra- and interobserver agreement in classifying the presence of primary radiologic abnormalities according to pre-established criteria was assessed by the Kappa statistic in 115 anteroposterior and lateral radiograms. A significant variability of interpretation was observed for many findings often considered important in benign LBP. Particularly, low levels of agreement were observed for apophyseal joint abnormalities. Schmorl's nodes, spo

Magnetic resonance imaging of acute symptomatic Schmorl's node formation.

The intraspongious herniation of intervertebral disk material into the endplate of a vertebral body was first described by Schmorl in 1927. Any process which weakens the cartilaginous endplate or the subchondral cancellous bone may predispose to the development of Schmorl's nodes. These include Scheuermann's disease, infection, metabolic disorders, neoplastic disorders, and degenerative disease. In the young individual, however, trauma may precipitate an acute focal and symptomatic endplate herniation by the well-hydrated and delineated nucleus pulposus. Magnetic resonance imaging (MRI) is the

Back pain and radiologic changes in the thoraco-lumbar spine of athletes.

Back pain and radiological changes of the thoraco-lumbar spine were investigated in 142 top athletes, representing wrestling, gymnastics, soccer and tennis (age range 14-25 years). All groups of athletes reported back pain at high frequencies (50-85%). Male gymnasts had significantly increased incidence and severity of back pain as compared to the rest of the athletes. Radiological abnormalities occurred in 36-55% of the athletes. Reduced disc height, Schmorl's nodes and change of configuration of vertebral bodies correlated with back pain (P less than 0.05, P less than 0.01 and P less than 0.

Rheumatic diseases in Neolithic and Medieval populations of western Switzerland.

An investigation of three groups from ancient populations (Neolithic, Early Middle Ages, Middle Ages) was performed on 273 adult skeletons. Despite unequal preservation of the remains, a study of a series of large joints and spinal segments permitted some conclusions: rheumatoid arthritis, ankylosing spondylitis, and osteoarthrosis of large joints (hip, knee, shoulder) were not found. The main findings were: osteoarthrosis in spinal zygapophyseal joints (particularly at cervical level); intervertebral osteochondrosis (particularly at the cervical and lumbar levels); Schmorl's nodes (particular

Biochemical and structural properties of the cartilage end-plate and its relation to the intervertebral disc.

The cartilaginous end-plate is thought to play an important role in the nutrition of the intervertebral disc, and hence may be of significance in the etiology of back pain. The present study describes the biochemical and histologic properties of the end-plate and adjacent tissues in the young human adult lumbar spine. Thus, a datum is established in which to demonstrate variation with location and relationships between properties of different tissues. Results of the chemical analyses show a change in composition through the end-plate with that at the outer annulus and nearer the bone, having h

Pathogenesis of discovertebral and manubriosternal joint abnormalities in rheumatoid arthritis: a cadaveric study.

The precise pathogenesis of discovertebral lesions in rheumatoid arthritis has long been debated with the emergence of 2 conflicting theories: discovertebral extension of synovial inflammation from a neighboring articulation (Luschka, costovertebral joint) and repetitive discal trauma with Schmorl's nodes. In order to address this controversy, we performed a radiographic-pathologic correlation of a rheumatoid cadaver and compared the alterations seen in the cervical and lumbar discovertebral junction; we further included an evaluation of the manubriosternal joint, another cartilaginous articul

End-plate lesions of the lumbar spine.

It has been suggested that the pattern of development and attrition of blood vessels and notochord remnants leaves congenitally weak points in the cartilage disc plates. There is strong presumptive evidence that nuclear prolapse can take place through these weak spots in the cartilage plates to form Schmorl's nodes. It has been postulated that the development of Schmorl's nodes may also lead to anomalies of the vasculature in the end-plates, which may lead to further end-plate lesions. In the present study, we have examined lumbar intervertebral discs and vertebrae from eight specimens with Sc

Anterior extension of the thoracic vertebral bodies in Scheuermann's kyphosis. An anatomic study.

Changes consistent with Scheuermann's kyphosis were noted in 103 specimens (7.4%) of a sample of 1,384 thoracic spines in the Hamann-Todd collection of human skeletons. In 94% of the affected vertebrae, a distinct anterior elongation of the vertebral centrum was present. This anterior extension was composed of mature cancellous bone and was morphologically and roentgenographically different from marginal osteophyte formation. It was not present in any vertebrae of a control group of 50 unaffected spines. Associated findings included vertebral wedging and Schmorl's nodes.

Computed tomographic features of renal osteodystrophy.

The spectrum of musculoskeletal abnormalities seen on routine computed tomographic (CT) examinations of five patients with renal osteodystrophy are described. CT findings included multiple brown tumors, osteitis fibrosa cystica, abnormal sacroiliac joints, periarticular tumoral calcifications, prominent Schmorl's nodes, and slipped capital femoral epiphyses. In this small group of patients, the musculoskeletal abnormalities ranged from the subtle to the dramatic. Although CT examination is rarely used as a screening test for renal osteodystrophy, it is important to be familiar with its many ap

Disc degeneration in Scheuermann disease.

Comparison of the radiographic signs of Scheuermann disease and the corresponding disc degeneration on thoracolumbar magnetic resonance (MR) images was made in 21 young patients. Marginal sclerosis, Schmorl nodes and narrowed disc spaces, but not irregular or wedge-shaped end-plates, were significantly associated with disc degeneration. Fifty-five percent of the discs in the patients with Scheuermann disease were abnormal on MRI, compared with 10% in asymptomatic controls. Our study confirms that thoracolumbar disc degeneration is enhanced in 20-year-old patients with low back pain who have ra

Schmorl's nodes. Correlation of X-ray and histological findings in postmortem specimens.

Histological examinations were conducted on specimens of 368 intervertebral discs (T11/12-L5/S1), using X-ray photography and discography. Specimens were obtained from 61 individuals (36 males and 25 females) whose ages ranged from 25 to 85 years. Fifty-four Schmorl's nodes were found in 28 of the patients (19 males and 9 females), and in 43 discs. In the peripheral regions of Schmorl's nodes, where the vertebral bodies were in contact with the node, growth of cartilaginous cells was seen in many cases. Thickened bone trabeculae were also seen in three nodes accompanied by syncytia. These thre
