Schmorl's nodes and vertebral fractures: A diagnostic dilemma.

A previously healthy 42-year-old man presented with sudden onset back pain whilst playing football. A T8 vertebral fragility fracture was diagnosed following X-ray and MRI thoracic spine, which demonstrated grade 1 compression. In addition, osteopenia was reported on dual-energy X-ray absorptiometry scan (DXA), which revealed a T-score of -1.8 in the lumbar spine. A follow-up MRI carried out after 18 months, reviewed by the metabolic bone clinic (MBC)-radiology multidisciplinary team, suggested features compatible with a ruptured Schmorl's node. The DXA was re-reviewed by an MBC specialist and

Relationship Between Lumbar Multifidus Morphometry and Pain/Disability in Individuals With Chronic Nonspecific Low Back Pain After Considering Demographics, Fear-Avoidance Beliefs, Insomnia, and Spinal Degenerative Changes.

BACKGROUND: Although individuals with chronic low back pain (CLBP) show increased fatty infiltration in the lumbar multifidus muscle (LMM), it remains unclear whether LMM changes are related to clinical outcomes (such as pain and disability) after considering confounders (spinal phenotypes, fear-avoidance beliefs [FABs] and insomnia). This study examined: (1) differences in confounders and LMM characteristics between individuals with and without CLBP; and (2) associations between confounders, LMM parameters, and clinical outcomes in the CLBP group alone. METHODS: Participants (CLBP = 70 and a

Treatment of Painful One Level Schmorl's Nodes Using Percutaneous Endoscopic Lumbar Discectomy: A Clinical Evaluation.

INTRODUCTION: Herniations of the intervertebral disc into the vertebral body, known as Schmorl's nodes (SNs), are frequently associated with degenerative disc disease (DDD). Painful SNs can contribute to discogenic lower back pain associated with DDD. Studies show that percutaneous endoscopic lumbar discectomy (PELD) yields favorable treatment outcomes for degenerative spinal diseases. This study retrospectively evaluated the efficacy of PELD in treating painful SNs. OBJECTIVE: To evaluate the clinical efficacy and safety of PELD in treating patients with painful SNs. METHODS: From February

Insights into the Pathophysiology of Scheuermann's Kyphosis: From Structural Deformities to Genetic Predisposition and Underlying Signalling Pathways.

Scheuermann's kyphosis (SK) is a rigid dorsal kyphosis of unclear pathophysiological origin. The aim of this review is to summarise current theories and both clinical and experimental findings regarding the underlying mechanisms of SK. Emerging evidence highlights the significant role of excessive mechanical loading as a major contributor to defective growth of the cartilaginous vertebral endplate. This is associated with the formation of Schmorl's nodes, disruption of the ring apophysis, and compromised intervertebral disc integrity-ultimately resulting in vertebral body wedging and thickenin

What can be observed in intervertebral cartilage endplate with aging? An animal model study of excessive axial mechanical loading.

INTRODUCTION: The cartilage endplate (CEP) plays a crucial role as both a mechanical barrier and nutrient channel for the intervertebral disc, but it is vulnerable to excessive axial loading. We modified the Ilizarov external fixator and applied it to the CEP of the rat tail to impose diurnal, controllable excess axial loading. The objective was to measure morphological changes in the CEP when subjected to loading during the aging process. METHODS: Two Kirschner wires were, respectively, inserted into the center of the eighth and ninth coccygeal vertebrae (Co8/9) of rat (n = 54) to apply axia

Massive bone destruction in a human proximal femur from ancient Nubia.

Massive bone destruction in a human right proximal femur is described and differentially discussed in this case study. The individual was an older adult female buried at the ancient Nubian site of Tombos (modern-day Sudan) dating to the early Napatan/Third Intermediate Period (c. 1069-750 BCE). The right femur displayed a pathologic fracture with extensive lytic destruction and resorption of the entire femoral neck, most of the femoral head, and trochanters. Macroscopic and radiographic analyses revealed cortical thinning of the proximal diaphysis with new bone formation enclosing the medullar

Experimentally dissociating the overuse mechanisms of endplate fracture lesions and Schmorl's node injuries using the porcine cervical spine model.

BACKGROUND: Compared to the documented overuse mechanisms of endplate fracture lesions, the cause of Schmorl's node injuries remains unknown, despite existing hypotheses. Therefore, this study aimed to examine and dissociate the overuse injury mechanisms of these spinal pathologies. METHODS: Forty-eight porcine cervical spinal units were included. Spinal units were randomly assigned to groups that differed by initial condition (control, sham, chemical fragility, structural void) and loading posture (flexed, neutral). Chemical fragility and structural void groups involved a verified 49% reduct
