Impaired WNT signaling and the spine-Heterozygous WNT1 mutation causes severe age-related spinal pathology.

BACKGROUND: WNT signaling plays a major role in bone and cartilage metabolism. Impaired WNT/β-catenin signaling leads to early-onset osteoporosis, but specific features in bone and other tissues remain inadequately characterized. We have identified two large Finnish families with early-onset osteoporosis due to a heterozygous WNT1 mutation c.652T>G, p.C218G. This study evaluated the impact of impaired WNT/β-catenin signaling on spinal structures. METHODS: Altogether 18 WNT1 mutation-positive (age range 11-76years, median 49years) and 14 mutation-negative subjects (10-77years, median 43years)

Benign Bone Conditions That May Be FDG-avid and Mimic Malignancy.

Positron emission tomography with the radiotracer 18F-fluoro-2-deoxy-d-glucose (FDG) plays an important role in the evaluation of bone pathology. However, FDG is not a cancer-specific agent, and knowledge of the differential diagnosis of benign FDG-avid bone alterations that may resemble malignancy is important for correct patient management, including the avoidance of unnecessary additional invasive tests such as bone biopsy. This review summarizes and illustrates the spectrum of benign bone conditions that may be FDG-avid and mimic malignancy, including osteomyelitis, bone lesions due to ben

Analysis of risk factors causing short-term cement leakages and long-term complications after percutaneous kyphoplasty for osteoporotic vertebral compression fractures.

Background Percutaneous kyphoplasty (PKP) is a common treatment modality for painful osteoporotic vertebral compression fractures (OVCFs). Pre- and postoperative identification of risk factors for cement leakage and follow-up complications would therefore be helpful but has not been systematically investigated. Purpose To evaluate pre- and postoperative risk factors for the occurrence of short-term cement leakages and long-term complications after PKP for OVCFs. Material and Methods A total of 283 vertebrae with PKP in 239 patients were investigated. Possible risk factors causing cement leakag

Classification of Schmorl's nodes of the lumbar spine and association with disc degeneration: a large-scale population-based MRI study.

OBJECTIVE: Schmorl's nodes (SN) are highly associated with lumbar disc degeneration (DD). However, SN present with different morphologies/topographies that may be associated with varying degrees of DD. This study proposed a classification of SN to determine their morphological/topographical prevalence and association with the severity of DD. METHODS: Sagittal T2-weighted MRIs were assessed to identify SN and additional imaging findings from L1-S1 in 2,449 individuals. SN characteristics were classified by six criteria: disc level; endplate involvement; shape; size; location of endplate zone;

Prevalence and Clinical Relevance of Schmorl's Nodes on Magnetic Resonance Imaging in a Tertiary Hospital in Southern India.

INTRODUCTION: Schmorl's Nodes (SN), which appear as defects in superior and inferior endplates of vertebrae, are commonly seen around the thoracolumbar junction. They may be asymptomatic or symptomatic. Their prevalence varies with respect to age, gender, regions involved and other associated disc or vertebral findings. AIM: SN is quite a common finding on the Magnetic Resonance Imaging (MRI) of the spine. The purpose of the study was to evaluate the prevalence of SN in the patients who underwent MRI whole spine in the radiology department, ascertain its clinical relevance and to compare the

Measurement of fat content in vertebral marrow using a modified dixon sequence to differentiate benign from malignant processes.

PURPOSE: To determine whether fat-signal-fraction (FF) map using a modified Dixon sequence could help differentiate benign from malignant bone lesions. MATERIALS AND METHODS: Spine magnetic resonance images (MRIs) of 120 consecutive patients were studied by using a 3T MRI with standard T1 -weighted image (T1 WI) and modified-Dixon sequence for FF measurement. There were three groups: a control group (n = 51) with normal vertebrae; a benign group (n = 40) with focal red marrow deposition, Schmorl's nodes, benign compression fracture, or Modic type 1 endplate degeneration; a malignant group (n

18F-FDG PET/CT can differentiate vertebral metastases from Schmorl's nodes by distribution characteristics of the 18F-FDG.

BACKGROUND: Differentiation of vertebral metastases from Schmorl's nodes, especially with fluorine-18-fluorodeoxyglucose (18F-FDG) uptake, is very important for the appropriate management of these patients in case they have malignancy. We aimed to evaluate the value of 18F-FDG positron emission tomography/computed tomography (PET/CT) in differentiating vertebral metastases from Schmorl's nodes with 18F-FDG uptake. SUBJECTS AND METHODS: Fourteen patients with malignancy underwent 18F-FDG PET/CT, which showed abnormal 18F-FDG uptake in 19 Schmorl's nodes. Twenty one patients with vertebral meta

Schmorl Nodes Can Cause Increased 68Ga DOTATATE Activity on PET/CT, Mimicking Metastasis in Patients With Neuroendocrine Malignancy.

Schmorl node (SN) is the herniation of the nucleus pulposus through the cartilaginous and bony endplate into the adjacent vertebral body. It is documented that SNs produce areas of moderately increased F-FDG uptake. We present a case of a patient with history of neuroendocrine tumor, who underwent Ga DOTATATE PET/CT for follow-up, showing increased focal vertebral uptake suggestive of bone metastasis. CT revealed typical findings of an SN. The presented case indicates that SNs should be considered when encountering focally increased skeletal uptake in Ga DOTATATE PET/CT studies, which can mimi

Does the correlation between Schmorl's nodes and vertebral morphology extend into the lumbar spine?

Schmorl's nodes are depressions on vertebrae due to herniation of the nucleus pulposus of the intervertebral disc into the vertebral body. This study provides an extension of our previous study which analyzed the shape of the lower thoracic spine and found that vertebral morphology was associated with the presence of Schmorl's nodes. Ninety adult individuals from the late Medieval site of Fishergate House, York, and the Post-Medieval site of Coach Lane, North Shields, Tyne and Wear, England, were analysed using 2D geometric morphometrics to identify possible relationships between vertebral mor

A semiautomatic method to identify vertebral end plate lesions (Schmorl's nodes).

BACKGROUND CONTEXT: There are differences in the definitions of end plate lesions (EPLs), often referred to as Schmorl's nodes, that may, to some extent, account for the large range of reported prevalence (3.8%-76%). PURPOSE: The purpose of this study was to develop a technique to measure the size, prevalence, and location of EPLs in a consistent manner. STUDY DESIGN/SETTING: This study proposed a method using a detection algorithm that was applied to five adolescent females (average age, 15.1 [range, 13.0-19.2] years) with idiopathic scoliosis (average major Cobb angle, 60° [range, 55°-67°]

The ancestral shape hypothesis: an evolutionary explanation for the occurrence of intervertebral disc herniation in humans.

BACKGROUND: Recent studies suggest there is a relationship between intervertebral disc herniation and vertebral shape. The nature of this relationship is unclear, however. Humans are more commonly afflicted with spinal disease than are non-human primates and one suggested explanation for this is the stress placed on the spine by bipedalism. With this in mind, we carried out a study of human, chimpanzee, and orangutan vertebrae to examine the links between vertebral shape, locomotion, and Schmorl's nodes, which are bony indicators of vertical intervertebral disc herniation. We tested the hypoth

Complete Separation of the Vertebral Body Associated with a Schmorl's Node Accompanying Severe Osteoporosis.

A Schmorl's node is defined as a simple endplate intravertebral herniation resulting from trauma or idiopathic causes. Although Schmorl's nodes have been considered clinically insignificant, they might indicate an active symptomatic process or cause serious complications. In this study, we report an interesting case of complete separation of a vertebral body caused by an untreated Schmorl's node accompanying severe osteoporosis. To our knowledge, this is the first clinical report in the published literature to evaluate the complete separation of a vertebral body associated with a Schmorl's nod

MRI evaluation of lumbar disc degenerative disease.

INTRODUCTION: Lower back pain secondary to degenerative disc disease is a condition that affects young to middle-aged persons with peak incidence at approximately 40 y. MRI is the standard imaging modality for detecting disc pathology due to its advantage of lack of radiation, multiplanar imaging capability, excellent spinal soft-tissue contrast and precise localization of intervertebral discs changes. AIMS AND OBJECTIVE: To evaluate the characterization, extent, and changes associated with the degenerative lumbar disc disease by Magnetic Resonance Imaging. STUDY DESIGN: Cross-sectional and

Distribution of Schmorl nodes in the lumbar spine and their relationship with lumbar disk degeneration and range of motion.

STUDY DESIGN: A kinematic magnetic resonance imaging study. OBJECTIVE: To investigate the distribution of Schmorl nodes (SNs) in the lumbar spine in healthy adults, and determine the association with lumbar disk degeneration and lumbar spine motion. SUMMARY OF BACKGROUND DATA: SNs have been associated with several pathologies of the lumbar spine, although it has been demonstrated that they also occur in the healthy adult population without a clearly identified cause. A thorough understanding of SN distribution may help reveal reasons for their formation. How disk degeneration and lumbar spin

Common normal variants of pediatric vertebral development that mimic fractures: a pictorial review from a national longitudinal bone health study.

Children with glucocorticoid-treated illnesses are at risk for osteoporotic vertebral fractures, and growing awareness of this has led to increased monitoring for these fractures. However scant literature describes developmental changes in vertebral morphology that can mimic fractures. The goal of this paper is to aid in distinguishing between normal variants and fractures. We illustrate differences using lateral spine radiographs obtained annually from children recruited to the Canada-wide STeroid-Associated Osteoporosis in the Pediatric Population (STOPP) observational study, in which 400 ch
