Schmorl Nodes Mimicking Osteolytic Bone Metastases.

Bone is the third most common site of metastasis from upper tract urothelial carcinoma after radical nephroureterectomy. Although bone biopsy is the gold standard for the diagnosis of bone metastases, they can usually be diagnosed on the basis of imaging tests. We describe a case of upper tract urothelial carcinoma after radical nephroureterectomy presenting with a Schmorl node in the third lumbar vertebra, mimicking lytic bone metastasis. Differentiation of bone metastases from Schmorl nodes is essential for the appropriate management of patients with malignancy.

Phenotype profiling of Modic changes of the lumbar spine and its association with other MRI phenotypes: a large-scale population-based study.

BACKGROUND CONTEXT: Modic changes (MC) are associated with low back pain. They represent vertebral endplate and adjacent vertebral marrow changes on magnetic resonance imaging (MRI), classified into three types. Because of small sample sizes, patient cohorts, and limited phenotype assessment, the morphology and involvement of MC and their association with other spinal phenotypes remain speculative. PURPOSE: We addressed and proposed a phenotypic profiling of MC and their relationship with lumbar MRI phenotypes in a large-scale population-based study. STUDY DESIGN/SETTING: A cross-sectional s

Modic changes of the lumbar spine: prevalence, risk factors, and association with disc degeneration and low back pain in a large-scale population-based cohort.

BACKGROUND CONTEXT: Modic changes (MC) are bone marrow lesions on magnetic resonance imaging (MRI), suggestive of being associated with low back pain (LBP). Data on determinants of MC and their association with disc degeneration and other spinal phenotypes, as well as that of LBP, rely mostly on small-scale patient populations and remain controversial. PURPOSE: This study addressed the potential determinants of MC and their association with disc degeneration and LBP among Southern Chinese. STUDY DESIGN/SETTING: A cross-sectional, population-based study was carried out. PATIENT SAMPLE: This

Changes in vertebral strain energy correlate with increased presence of Schmorl's nodes in multi-level lumbar disk degeneration.

Patients with skipped-level disk degeneration (SLDD) were recently reported as having a higher prevalence of Schmorl's nodes than patients with contiguous multi-level disk degeneration (CMDD). Fourteen versions of a nonlinear finite element model of a lumbar spine, representing different patterns of single and multi-level disk degeneration, were simulated under physiological loading. Results show that vertebral strain energy is a possible predictor in the development of Schmorl's nodes. The analysis also shows evidence that the development of Schmorl's nodes may be highly dependent on the loca

Macroscopic anatomy of the vertebral endplate: quid significat?

Alterations of vertebral endplates have long been a subject of interest, but are of unclear clinical significance. The vertebral columns of a cohort-based sample of 850 individuals were therefore evaluated for vertebral endplate defects, noting adjacent vertebral size and shape variation, bone density and associated pathologies. Defects were found in the vertebral endplates of 458 individuals (Schmorl's nodes in 52.0% and linear defects in 10.6%). Vertebral centra osteophytes and diffuse idiopathic skeletal hyperostosis were more common; vertebral compression, less common in vertebrae with def

[Distribution of Schmorl's nodes in lumbar spine and their relationship with lumbar disc degeneration].

OBJECTIVE: To explore the distribution of Schmorl's nodes (SN) in normal adult lumbar spine and determine the association of lumbar disc degeneration and lumbar spine motion. METHODS: A total of 1 179 individuals underwent magnetic resonance imaging (MRI) scans. They had an age range of 15-85 years and were grouped by 10 years. The distribution of SNs along lumbar spine and their relationship with age and gender were examined by single factor analysis of variance chi-square test. Spine specialists performed lumbar disc degeneration evaluations through the Pfirrmann's classification system. Th

Differentiation of osteolytic metastases and Schmorl's nodes in cancer patients using dual-energy CT: advantage of spectral CT imaging.

PURPOSE: To assess the reliability of dual-energy CT (DECT) spectral imaging for the differentiation of bone metastases (BMs) from Schmorl's nodes (SNs) in the vertebrae of cancer patients. MATERIALS AND METHODS: In this retrospective study, 102 cancer patients who underwent DECT (GE spectral CT Discovery CT750 HD scanner) had 110 low density vertebral lesions. Each lesion was characterized as a BM or SN, based on the typical MRI or SPECT/PET-CT findings as well as size and number change in the 6 months follow-up. The means of 140 kVp polychromatic CT values, 40-140 keV monochromatic CT value

Spectrum of magnetic resonance imaging findings in congenital lumbar spinal stenosis.

AIM: To investigate whether congenital lumbar spinal stenosis (CLSS) is associated with a specific degenerative changes of the lumbar spine. METHODS: The lumbar spine magnetic resonance imaging studies of 52 subjects with CLSS and 48 control subjects were retrospectively evaluated. In each examination, the five lumbar levels were assessed for the presence or absence of circumferential or shallow annular bulges, annular tears, anterior or posterior disc herniations, epidural lipomatosis, Schmorl's nodes, spondylolisthesis, pars defects, and stress reactions of the posterior vertebral elements.

Unusual midsagittal defects in vertebrae from McKee Island, Alabama (1Ms32).

Skeletal remains were excavated at a village site on McKee Island, Alabama in 1937, which were stored and later analyzed at the University of Alabama. During analysis, unusual vertebral defects were observed among the historic-era remains of two mature adults: one male, one female. In each case, the inferior endplates of two thoracic vertebral bodies exhibited midsagittal defects that resemble incomplete fractures with sclerotic reaction. These defects are described here as midsagittal vertebral infractions (MVI). Alternative diagnoses are proposed based on various causal mechanisms, including

Comparison between two types of "Scheuermann disease-like people": thoracolumbar disc herniation patients and healthy volunteers with radiological signs of Scheuermann's disease.

BACKGROUND: Scheuermann's disease (SD) is a spinal disorder and includes both a classic form and an atypical form. Interestingly, its existence among the general population as well as the disc disease patients is common. One of our previous studies showed that about 18% of the hospital staff members meet the SD criteria. On the other hand, another study has demonstrated that 95.2% of the symptomatic thoracolumbar disc herniation (STLDH) patients meet the SD criteria, which suggests that STLDH is very likely a special form of SD. The purpose of this study was to discriminate the factors contrib

Magnetic resonance imaging of the cervical, thoracic, and lumbar spine in children: spinal incidental findings in pediatric patients.

Study Design Retrospective case series. Objective To determine the rate of spinal incidental findings on magnetic resonance imaging (MRI) of the cervical, thoracic, and lumbar spine in the pediatric population. Methods We reviewed MRI imaging of the neuraxial spine in patients less than 18 years of age and documented abnormal spinal findings. We then reviewed the charts of these patients to determine the reason for ordering the study. Those who presented with pain were considered symptomatic. Those who had no presenting complaint were considered asymptomatic. The data were analyzed to break do

Symptomatic giant Schmorl's node treated by a decompression procedure.

Schmorl's nodes (SN) are common lesions that represent disc material prolapse through the vertebral endplate, into the vertebral body with generally limited clinical significance. Rarely they have been related to the onset of back pain, usually self-limited. Recently giant variants of SN have been described, with very discrete clinical and imaging expression. Knowledge of the typical imaging appearance in both CT-scan and MRI may preclude the use of unnecessary diagnostic and treatment procedures. The authors present a case of symptomatic giant SN resembling these rare variants by its size and

The epidemiology of Schmorl's nodes and their correlation to radiographic degeneration in 4,151 subjects.

PURPOSE: Schmorl's nodes (SNs) are commonly seen in vertebral imaging of the normal adult population referred for different reasons and are duly noted by the radiologist. However, little is known about their etiology: either SNs are perceived as largely inert developmental or congenital herniations of disc tissue into weak areas of the vertebral end-plates, or they are perceived as a common pathological pathway of different adverse and general factors such as malignancy, trauma, infection, osteoporosis, Paget's disease and so forth. A commonly accepted morphological definition of what precisel

Degenerative alterations of the spine in an Early Mediaeval population from Mannheim-Seckenheim, Germany.

Palaeopathological and palaeoepidemiological analyses of human skeletal remains are some of the most important bases for the reconstruction of life of past populations. The assessment of frequency and degree of pathological alterations contributes to conclusions of a population's health status, labour conditions, and environmental influences. Degenerative diseases of the spine are among the most common lesions observed in archaeological human remains. The large number of excavated Early Mediaeval cemeteries in Germany enables the comparison of contemporary populations increasing the reliabilit

Schmorl's nodes: current pathophysiological, diagnostic, and therapeutic paradigms.

Schmorl's nodes were first described by the pathologist Christian Schmorl in 1927 as a herniation of the nucleus pulposus through the cartilaginous and bony endplate into the vertebral body. Although such lesions present most commonly as incidental findings in asymptomatic patients (or in patients with back or radicular pain due to other etiology), there have been several reports emphasizing the deleterious effects of the inflammatory response and endplate changes elicited by the herniation of for such reasons, Schmorl's nodes have been occasionally implicated in the etiology of chronic axial

Acute schmorl node in dorsal spine: an unusual cause of a sudden onset of severe back pain in a young female.

Schmorl nodes represent displacement of intervertebral disc tissue into the vertebral body and have been considered as an asymptomatic incidental radiological finding on plain radiographs, computed tomography and magnetic resonance imaging (MRI). Although uncommon, acute symptomatic Schmorl nodes causing severe back pain do occur. We report here an unusual case of acute painful Schmorl node in a young healthy woman, with no previous trauma, presenting with a sudden significant localized back pain within hours accompanied by characteristic findings on a MRI scan. We reviewed all reports of symp

Patterns of lumbar disc degeneration are different in degenerative disc disease and disc prolapse magnetic resonance imaging analysis of 224 patients.

BACKGROUND CONTEXT: Existing research on lumbar disc degeneration has remained inconclusive regarding its etiology, pathogenesis, symptomatology, prevention, and management. Degenerative disc disease (DDD) and disc prolapse (DP) are common diseases affecting the lumbar discs. Although they manifest clinically differently, existing studies on disc degeneration have included patients with both these features, leading to wide variations in observations. The possible relationship or disaffect between DDD and DP is not fully evaluated. PURPOSE: To analyze the patterns of lumbar disc degeneration i

Schmorl's nodes.

INTRODUCTION: First described in 1927, a Schmorl's node (SN) is the herniation of nucleus pulposus (NP) through the cartilaginous and bony end plate into the body of the adjacent vertebra. SNs are common findings on imaging, and although most SNs are asymptomatic, some have been shown to become painful lesions. In this manuscript, we review the literature regarding the epidemiology, clinical presentation, pathogenesis, imaging, and management of SNs.
