Reconceptualizing Sacroiliac Joint Mechanics: From Independent Joints to a Unified Functional Unit.

BACKGROUND: Pain localized to the area around the "Dimple of Venus" is a common clinical presentation in patients with low back pain (LBP). This region represents the most peripheral dorsal portion of the sacroiliac joint (SIJ). CLINICAL QUESTION: Although the SIJ is recognized as a source of unilateral LBP, its deep anatomical location and complex morphology make diagnosis difficult. Consequently, current clinical SIJ tests often yield inconsistent results, leading to skepticism regarding the joint's clinical relevance. This doubt is largely rooted in a historically accepted biomechanical mod

Clinical characteristics of persistent ısotretinoin-associated ınflammatory back pain: a retrospective cohort study.

OBJECTIVE: Isotretinoin is commonly used to treat severe acne vulgaris, and reports of inflammatory back pain (IBP) during therapy have grown over the past decade. Previous studies have reported improvement in musculoskeletal symptoms following isotretinoin discontinuation; however, persistent symptoms have also been described. This study aimed to describe the clinical and demographic characteristics of isotretinoin-associated IBP according to symptom persistence after isotretinoin discontinuation. METHODS: This retrospective cohort study included 100 consecutive adults with new-onset IBP ful

A Proteomic Glance at the Bovine Intervertebral Disc.

Trauma- or age-inflicted intervertebral disc (IVD) degeneration (IVDD) is a leading cause of chronic and severe low back pain. Raising interest in regenerative, cell-based therapies depends on a solid understanding of cell identities and their natural habitat, most importantly the extracellular matrix (ECM) composition. We and others previously proposed IVD biomarkers to characterize IVD cells. To complement our transcriptome data, we also investigated the proteome of the annulus fibrosus (AF) and nucleus pulposus (NP) bovine coccygeal tissue via nano liquid chromatography electrospray ionizat

Beyond antifracture efficacy: Osteoanabolic versus antiresorptive therapy for chronic pain after osteoporotic vertebral fractures. A systematic review and meta-analysis.

Osteoporotic vertebral fractures (OVFs) associated pain can last long, reducing health-related quality of life. Concurrent risk of new fractures warrants osteoporosis therapies, i.e., antiresorptive [bisphosphonates and the anti-RANKL monoclonal antibody (mAb) denosumab] or osteoanabolic [teriparatide, abaloparatide, and the anti-sclerostin mAb romosozumab] agents, the latter demonstrating greater antifracture efficacy, but unclear effect on post-fracture pain. The present systematic review and meta-analysis explore whether osteoanabolics exert better pain control compared to antiresorptives i

Deep Spinal Infection Caused by Corynebacterium simulans: A Case Report.

Corynebacterium simulans is a commensal coryneform bacterium that commonly colonizes human skin and mucosal surfaces. Owing to limitations in conventional microbiological identification, it is often regarded as a contaminant, and clinically significant deep-seated infections caused by this organism are rarely reported. Here, we describe a case of lumbar infectious spondylitis caused by C. simulans in a 62-year-old male farmer who presented with a 2-month history of persistent low back pain accompanied by pain in both lower limbs. The patient had several potential predisposing factors including

Outcomes after anterior cervical discectomy and fusion for presumed segmental dysfunction-related neck pain: a Swedish nationwide registry study.

PURPOSE: Surgical management of mechanically driven neck pain without radiculopathy or myelopathy is a controversial topic, where comparative evidence to non-operative care is limited. This registry-based descriptive cohort study, without a nonoperative comparator, aimed to evaluate longitudinal outcomes after anterior cervical discectomy and fusion (ACDF) for mechanically driven pain presumed to derive from a dysfunctional cervical segment. METHODS: This observational cohort included all patients identified in Swespine (2006-2024) who underwent ACDF for presumed segmental dysfunctional-relat

The Diagnostic Funnel: A Multi-Stage Diagnostic Selection Protocol for Minimally Invasive Sacroiliac Joint Fusion.

Background Minimally invasive sacroiliac joint fusion (MI SIJF) is an established intervention for chronic sacroiliac joint dysfunction, yet outcome variability across the literature suggests that surgical success may heavily depend on preoperative selection accuracy. Traditional diagnostic pathways relying on transient anesthetic blocks are susceptible to false-positives and variable responder rates. The purpose of this study is to evaluate the clinical effectiveness of a rigorous, multi-stage selection protocol, termed the "diagnostic funnel," which utilizes a therapeutic durability threshol

Suzetrigine (Journavx) as a Non-Opioid Adjuvant for Cancer-Associated Pain: A Case Series.

Opioids remain central to managing cancer-associated pain (CAP) but are often limited by constipation, nausea, sedation, and concerns about maladaptive use amid the ongoing opioid epidemic. Suzetrigine (Journavx), a first-in-class, non-opioid, selective NaV1.8 sodium channel inhibitor, was approved by the U.S. Food and Drug Administration in January 2025 for moderate-to-severe acute pain; its role in chronic pain syndromes such as CAP has not been established. This case series describes two patients with CAP of differing pathophysiology started on suzetrigine as an off-label adjuvant analgesic

[Arachnoid web presenting as tetraparesis].

Arachnoid webs are rare intradural lesions which disrupt cerebrospinal fluid flow and may cause spinal cord compression. This is a case report of a previously healthy 19-year-old woman who presented with acute back pain and rapidly progressive tetraparesis. MRI revealed cervical spinal cord ischaemia and an arachnoid web at the thoracic level. Surgical removal led to partial neurological recovery. Early recognition is essential, as timely surgery may prevent irreversible injury.

Falciform Ligament Inflammation Complicated by Umbilical Vein Thrombophlebitis, Left Portal Vein Thrombosis, and Multiple Hepatic Abscesses: A Case Report.

Falciform ligament pathology is a rare cause of acute upper abdominal pain and may resemble more common pancreatic and hepatobiliary disorders. A woman in her 50s presented with sudden severe epigastric pain radiating to her back and marked hyperamylasaemia of 1,747 U/L. This combination fulfilled clinical and biochemical diagnostic criteria for acute pancreatitis, although computed tomography (CT) demonstrated no pancreatic inflammation and instead showed severe inflammatory change centred on the falciform ligament, with associated focal fat infarction, left portal vein thrombosis, and gas lo

Percussive Vibration Therapy as an Adjunct to Conventional Physiotherapy in Chronic Non-Specific Low Back Pain: A Randomized Controlled Trial.

Background/Objectives: Hand-held percussive vibration therapy (PVT) is widely used in musculoskeletal rehabilitation, yet its incremental benefit beyond an active physiotherapy program for chronic non-specific low back pain (CNLBP) is unclear. We investigated whether PVT added to conventional physiotherapy (CPT) improves disability, sleep quality, and pain. Methods: This single-center, parallel-group, assessor-blinded randomized trial enrolled 50 adults aged 18-65 years with CNLBP and baseline visual analogue scale (VAS) pain ≥4/10. Participants were allocated equally to PVT + CPT or CPT alone

Efficacy of traditional chinese exercises on chronic low back pain: A systematic review and network meta-analysis.

BACKGROUND: Chronic low back pain (CLBP) is a global health problem. Traditional Chinese exercises (TCEs) may relieve pain and improve physical function, but evidence to guide TCE selection remains limited. This study compared the efficacy of different TCEs on pain and functional outcomes in adults with CLBP. METHODS: Seven databases and grey-literature sources were searched for randomized controlled trials (RCTs) of TCEs for CLBP from inception to August 2, 2026. Study quality and evidence certainty were assessed using risk-of-bias tools, the NIH Quality Assessment Tool, and the CINeMA frame
