TeleBACK clinical trial: comparing the effectiveness of telerehabilitation and in-clinic physical therapy for patients with chronic low back pain - protocol for a multicentre, pragmatic, individually randomised group treatment trial.

INTRODUCTION: Chronic low back pain (LBP) is a leading cause of disability and healthcare spending in the USA. While physical therapy (PT) has been shown to improve disability and pain as well as decrease the cost of care associated with LBP, only a small portion (<15%) of eligible patients with LBP ever attend PT. These low utilisation rates are likely influenced by logistical barriers to attending visits (eg, transportation, missed work time). Patients with chronic pain may also struggle to attend frequent clinic visits because of their symptoms. Telerehabilitation, which expanded substantia

Effectiveness, Safety, and Cost-Effectiveness of Ultrasound-Guided Acupotomy for Degenerative Cervical Radiculopathy: A Protocol for a Randomized Controlled Trial.

BACKGROUND: Degenerative cervical radiculopathy (CR) causes neck and upper-extremity pain, sensory disturbance, and motor dysfunction resulting from nerve root compression or irritation by disc herniation, foraminal narrowing, or spondylotic changes. Ultrasound-guided acupotomy (UA), a minimally invasive acupuncture-based intervention, permits real-time visualization of target tissues and adjacent neurovascular structures, enabling controlled release of fibrotic or adhesive soft tissues that may contribute to nerve root compression. Evidence on its effectiveness, safety, and cost-effectiveness

Selection and justification of performance-related measures in exercise trials for people with chronic low back pain: A scoping review and content analysis.

BACKGROUND: Chronic low back pain (CLBP) is the leading cause of years lived with disability worldwide. Although exercise is a first-line, cost-effective treatment for CLBP, no single exercise modality has proven superior, suggesting uncertainty around the mechanisms linking exercise to clinical outcomes. AIM: To identify the performance-related measures used to assess physical and physiological characteristics (including strength, endurance, mobility, stability, motor control, and muscle morphology) in exercise trials for chronic low back pain, and to examine the rationales and proposed mech

Modic changes, chronic low back pain, and lifestyle and mental health factors: a cross-sectional analysis of a cohort study.

PURPOSE: Modic changes (MC) are vertebral endplate and adjacent bone marrow MRI findings that have been associated with chronic low back pain (CLBP), although their clinical relevance remains debated. This study examined whether MC presence and extent are associated with CLBP status, pain intensity, and disability, and whether lifestyle or mental health factors moderate the MC-pain relationship. METHODS: This exploratory cross-sectional sub-study included 239 adults: 135 with CLBP and 104 controls without. Lumbar MRI scans were evaluated for MC presence, type, craniocaudal extent of vertebral

Effects of transcranial direct current stimulation on patient-reported outcomes in chronic low back pain: a systematic review and three-level meta-analysis.

BACKGROUND: Growing evidence suggests that central nervous system alterations may contribute to the persistence of chronic low back pain (CLBP). Transcranial direct current stimulation (tDCS) can effectively modulate cortical excitability and holds considerable potential for the rehabilitation of CLBP. However, evidence regarding its effects remains controversial. PURPOSE: This study aimed to systematically evaluate the effects of tDCS on pain, disability, and psychosocial outcomes in individuals with CLBP. METHODS: Five electronic databases were searched from their inception to January 20,

Moderate exosuit use reduces the incidence of back discomfort in logistic warehouse workers: a randomised controlled trial with an exploratory dose-stratified analysis.

Evidence on whether exosuits reduce workplace injuries is mixed. We studied whether exosuits can reduce back discomfort among warehouse workers. Ninety-seven participants were randomised to a control or exosuit group; for exploratory analysis, exosuit participants were stratified as moderate or low users. Surveys at baseline and four follow-ups assessed workplace discomfort, perceived back effort, usability, and adverse events. Intention-to-treat analysis showed that exosuit users had no difference in back discomfort intensity and incidence than the control group. However, exosuit users did ha

Ambivalent acceptance of active exoskeletons in long-term care: between perceived task-specific support and practical constraints.

BACKGROUND: Lower back pain (LBP) is prevalent among nursing staff in long-term care facilities. This condition has been linked to repetitive resident handling, sustained non-neutral postures, and cumulative physical workloads. Despite growing interest in exoskeletons as ergonomic and digital assistive technologies (DAT), evidence regarding the implementation of active exoskeletons in real-world nursing practice remains limited. OBJECTIVE: The objective of this exploratory mixed-methods feasibility study was to evaluate the implementation of the HAL® Lumbar Type active exoskeleton in long-ter

Tissue-specific peripheral sensory remodeling in chronic low back pain: established and emerging nociceptive niches.

Chronic low back pain arises from heterogeneous peripheral and central mechanisms, yet the sensory biology of individual lumbar tissues is rarely considered within a unified framework. This Mini Review compares peripheral sensory remodeling and sensitization across four candidate nociceptive niches: the intervertebral disc, vertebral endplate, thoracolumbar fascia, and paraspinal muscle. Disc pathology is characterized by nociceptive ingrowth, endplate pathology by osteochondral sensory remodeling and afferent sensitization, and thoracolumbar fascia by inflammation-associated remodeling of a p

Four-Year Longitudinal Phenotyping of Post-Acute Sequelae of SARS-CoV-2 Infection (PASC): A Single-Centre Cohort Study.

Background: Although an increasing number of studies have reported 2- and 3-year outcomes of post-acute sequelae of SARS-CoV-2 infection (PASC), the long-term symptom patterns of and their clinical phenotypes beyond four years remains unknown. We aimed to characterize 48-month clinical outcomes and symptom profiles using hierarchical cluster analysis. Methods: We conducted a longitudinal cohort study of 79 patients with persistent PASC enrolled between March 2021 and March 2022. Of these, 77 patients completed the 48-month follow-up. Symptoms were assessed at 11 and 48 months post-infection. H

Association between the dietary index for gut microbiota and fracture prevalence among women aged 50 years and older: a NHANES-based study with a preliminary exploratory pilot analysis.

INTRODUCTION: Gut microbiota has been implicated in bone metabolism, but whether a diet supporting gut microbial health is associated with fracture prevalence in older women remains unclear. This study aimed to examine the association between the dietary index for gut microbiota (DI-GM) and fracture prevalence in women aged 50 years and older. METHODS: We analyzed cross-sectional data from five NHANES cycles, including 6,318 women aged 50 years and older (mean age 63.8 ± 9.6 years). DI-GM scores were derived from 24-h dietary recalls based on 13 dietary components. Self-reported hip, wrist, o

Compact Navigation-Assisted Transpedicular Approach Using Unilateral Biportal Endoscopy for Caudally Migrated Lumbar Disc Herniation.

INTRODUCTION: Severely migrated lumbar disc herniations are challenging to access using conventional interlaminar or transforaminal approaches, especially in revision cases. A transpedicular route provides direct access but raises concerns regarding accuracy, safety, and radiation exposure when fluoroscopy is required. In this Technical Note, we describe a transpedicular unilateral biportal endoscopic (UBE) approach assisted by a compact navigation system for a caudally migrated disc herniation, highlighting its feasibility and potential advantages. TECHNICAL NOTE: A 73-year-old man with a hi

Severe Osteoarthritis and Avascular Necrosis of the Hips in a 37-Year-Old Man: Case Report.

BACKGROUND: Hip osteoarthritis has been considered a condition commonly encountered in the elderly, however, it is predicted to be increasing in prevalence in the population below age 50. Avascular necrosis (AVN) is usually caused by a compromise in blood flow, usually occurring in patients with risk factors. CASE REPORT: A 37-year-old man presented with left-sided hip pain. Magnetic resonance imaging revealed severe left hip osteoarthritis, as well as bilateral hip AVN, although the patient had no known risk factors. CONCLUSIONS: Historically, differential diagnoses for hip pain in younger

Sequestered Lumbar Disc Herniation Mimicking a Spinal Tumor: A Case Report.

Sequestered lumbar disc fragments are a well-recognized cause of radiculopathy; however, when disc material demonstrates contrast enhancement, it may closely resemble an intraspinal tumor on magnetic resonance imaging (MRI), leading to diagnostic uncertainty, particularly in patients with a history of malignancy. We present the case of a 75-year-old man with a history of sigmoid and prostate carcinomas who presented with persistent low back pain and right-sided radicular symptoms for two months. Neurological examination revealed no motor deficit, with preserved strength of 5/5, although interm
