Pyogenic Sacroiliitis in Older Adults Presenting With Massive Retroperitoneal Abscess: A Case Report.

Pyogenic sacroiliitis (PSI) is an uncommon form of septic arthritis and is frequently diagnosed late in older adults because initial symptoms are nonspecific and imaging findings may be misattributed to other sources of infection. We report two older patients with PSI complicated by massive retroperitoneal and gluteal abscesses, highlighting the importance of early recognition and, when indicated, timely surgical source control. Case 1 was an 83-year-old woman in whom a conspicuous retroperitoneal abscess and abnormal urinalysis led to suspicion of an intra-abdominal or urinary tract source, d

Cannabidiol (CBD) for Pain: A Narrative Review of Evidence to Date.

Cannabidiol (CBD), the principal non-intoxicating phytocannabinoid of Cannabis sativa, is widely marketed and self-administered for pain relief, yet its regulatory status as an analgesic remains unapproved. This review synthesizes preclinical mechanistic data, human pharmacokinetic data, and clinical trial evidence across major pain conditions in order to summarize the current evidence for, or against, efficacy and safety for pain. A synthesis of published information was conducted using PubMed/MEDLINE, PubMed Central, Cochrane Library, ScienceDirect, and other databases, integrating findings

Mpox outbreak associated with multiple sexual partnerships and HIV coinfection in Masaka District, Uganda, February-April 2025.

OBJECTIVES: Uganda experienced a substantial increase in mpox cases in early 2025, with notable clustering in central regions. The country reported approximately 4800 confirmed monkeypox (mpox) cases and 37 deaths, with the Masaka district contributing approximately 3% of the national burden. We investigated an outbreak in Masaka District to describe its epidemiology and identify associated risk factors. METHODS: We conducted an outbreak investigation with a descriptive epidemiological analysis and an unmatched case-control study. A suspected case was defined as onset of rash with fever and a

Differences in Clinical Presentation of Chronic Pelvic Pain Caused by Iliac Vein Obstruction Impacts Referral Pathway, Time to Diagnosis, and Post-stenting Pain Resolution.

OBJECTIVE: Iliac venous stenting improves chronic pelvic pain (CPP) caused by iliac vein obstruction (IVO). Signs and symptoms vary, but this pain is reproducible and easily identifiable. However, patients can experience significant delays in diagnosis and treatment. This study aimed to determine associations between the referral history, clinical presentation, pain characteristics, and stenting outcomes among women with CPP due to IVO. METHODS: This was a retrospective analysis of prospectively collected data of women who were referred to a vascular surgeon with signs and symptoms of IVO, in

Effects of Partner Distress and Controlling Responses on Back Pain in Daily Life.

Chronic back pain is embedded within close relationships, yet little is known about how partners' emotional and behavioral responses shape patients' daily pain experiences. This study examined whether partners' distress predicts their attempts to control (increase or decrease) the activity level of individuals with chronic back pain (ICBPs), and whether these responses are associated with worse subsequent pain outcomes the following day. One hundred forty-seven couples in which one partner had chronic back pain completed twice-daily electronic diary assessments for 30 days. The sample was olde

Association between frailty and multisite chronic pain in middle-aged and older adults: A population-based cross-sectional study with mediation analysis.

The corrected modified frailty index (CMFI), based on the accumulation of multidimensional health impairments, provides a more comprehensive reflection of an individual's overall frailty. Although chronic pain is associated with aging, its relationship with CMFI, in population-based cohorts remains unclear. This cross-sectional study included 5821 participants aged ≥45 years from the 2011 baseline wave of the China Health and Retirement Longitudinal Study. Chronic pain, including low back pain, multisite pain, and the number of pain sites, was assessed via self-reported questionnaires. Frailty

Diverging diagnostic trajectories of degenerative cervical myelopathy and cervical radiculopathy in South Korea: a nationwide time-series analysis of claims data, 2010-2024.

BACKGROUND: Degenerative cervical myelopathy (DCM) is the leading non-traumatic cause of spinal cord dysfunction in adults, and its burden is projected to grow as populations age. Cervical radiculopathy shares the same degenerative substrate but follows a separate diagnostic pathway. Long-term national trends in the two conditions, and their relation to surgical practice, are unquantified in an East Asian system with universal coverage. METHODS: This nationwide repeated cross-sectional ecological analysis used aggregate open data from the Korean Health Insurance Review and Assessment Service,

The Alkaline Trap: Severe Calcium-Alkali Syndrome Secondary to Chronic Calcium Carbonate Ingestion.

Calcium-alkali syndrome is an increasingly recognised cause of parathyroid hormone (PTH)-independent hypercalcaemia resulting from excessive calcium and absorbable alkali ingestion, commonly through calcium-containing antacids or supplements. Its clinical presentation may mimic neurological or malignant disease, potentially leading to extensive investigation and delayed diagnosis. A 64-year-old woman presented following a fall with paraparesis and lower back pain. Investigations demonstrated severe hypercalcaemia (adjusted calcium 4.83 mmol/L), suppressed PTH of 1.4 pmol/L, metabolic alkalosis

Social Support Modifies the Pain-Disability Relationship in Degenerative Spine Disease: Evidence From the PROGRES Study.

BACKGROUND: Low back pain from degenerative spine disease is the main contributor to years lived with disability globally. Although social support is a multifaceted construct with both health-promoting and potentially deleterious effects on chronic pain, whether perceived social support modifies the pain-disability relationship in lumbar degenerative spine disease remains unclear. We hypothesised that social support level would modify the pain-disability association, with distinct predictor patterns of disability emerging for low- versus high-support patients. METHODS: In this cross-sectional

Third lumbar transverse process syndrome with acute abdominal pain as the main clinical manifestation: A case report.

RATIONALE: Third lumbar transverse process syndrome is an uncommon musculoskeletal cause of referred abdominal pain. Because acute abdominal pain is appropriately evaluated first for visceral, vascular, urinary, and surgical emergencies, a spinal or paraspinal pain generator may be overlooked. PATIENT CONCERNS: A 66-year-old man presented with persistent left lower-quadrant abdominal pain radiating to the left lower back. Baseline pain intensity was 7/10 on a VAS, and the pain was aggravated by postural change, standing, and walking. DIAGNOSES: Serial laboratory tests, endoscopy, ultrasonogr

Intraosseous Lidocaine Trigger Point Injection for Vertebrogenic Pain: From Molecular Principles to Clinical Applications: A Narrative Review.

Intraosseous lidocaine trigger point injection (LTPI) has been utilized for an extended period in the management of chronic vertebrogenic pain; yet, its molecular mechanisms and clinical efficacy require critical appraisal. This narrative review highlights the milestones of osteoreception and elucidates the molecular mechanisms driving intraosseous receptor irritation and its pivotal role in chronic pain development. Pathological vertebral bone remodeling triggers a receptor activator of nuclear factor-kappa B ligand/osteoprotegerin (RANKL/OPG) imbalance, accelerating hyperactive osteoclastoge

Navigating Cognitive Biases and Surgical Decision-Making: Unraveling the Causes of Persistent Spinal Pain Syndrome After Spine Surgery.

OBJECTIVE: Persistent spinal pain syndrome (PSPS) affects up to 40% of patients after spine surgery, yet the interplay between technical factors, diagnostic accuracy, and cognitive biases remains poorly characterized. This study examines contributors to PSPS in a single-institution cohort, emphasizing preventable causes and surgeon cognitive biases in decision-making. METHODS: A retrospective review of 482 PSPS patients (2009-2023) at a tertiary spine center evaluated clinical data, procedures, and complications. A blinded multidisciplinary team classified etiologies using standardized criter

Anterior Lumbar Interbody Fusion Versus Total Disc Replacement with a Viscoelastic Prosthesis for Single-Level Lumbar Degenerative Disc Disease in Young Adults: A Comparative Retrospective Cohort Study.

Background/Objectives: The optimal surgical approach for patients with lumbar degenerative disc disease (DDD) remains a subject of ongoing debate, particularly when comparing anterior lumbar interbody fusion (ALIF) and total disc replacement (TDR). The primary objective of this study was to compare short-term outcomes between these two surgical approaches in terms of clinical efficacy, patient satisfaction, and safety profile. The secondary objective was to evaluate patient-specific variables associated with postoperative improvement in order to identify potential factors that may guide a pers

Endoscopic Decompression for Lateral Recess Stenosis Due to Vertebral Compression Fracture: A Case Report.

STUDY DESIGN: Case presentation. OBJECTIVE: The interlaminar endoscopic lateral recess decompression (IE-LRD) represents a novel surgical alternative to consider in carefully selected cases, particularly when patients do not respond to conservative treatment. BACKGROUND: Lumbar spinal stenosis (LSS) constitutes one of the most prevalent spinal conditions in the elderly population around the world. A myriad of causes have been elucidated, and although infrequent, vertebral compression fractures (VCF) may also cause LSS. Conservative treatment might be the first option; however, patients often

Friend or Foe: The Use of Cannabis as a Potential Analgesic in Postoperative Lumbar Spine Surgery.

Cannabis is a plant-based product which has been documented in medicinal usage dating back nearly 5000 years ago in 400 AD. With time, cannabis would come to become the most grown, cultivated, trafficked, and abused illicit drug worldwide. As a result, cannabis became a schedule 1 substance having no accepted medical use. In 1996, however, cannabis emerged in the medicinal scene when California became the first state to allow legal access to cannabis for medicinal purposes under the Compassionate Use Act. As of November 8, 2023, 24 states, two territories, and the District of Columbia all enac
