INTRODUCTION/AIMS: The clinical significance of stereotyped post-F-wave discharges remains uncertain. We aimed to characterize F-wave-induced repetitive discharges (FIRDs) and their associated clinical and electrodiagnostic findings.
METHODS: We searched the Mayo Clinic electrodiagnostic database (January 1997-December 2025) using "F-wave" and "repetitive discharges" and reviewed retrieved waveforms. FIRDs were defined by repetitive discharges following F-wave activation, reproducible morphology within a train, and regular or semiregular firing.
RESULTS: The study included 20 patients with F

PURPOSE: Surgical management of mechanically driven neck pain without radiculopathy or myelopathy is a controversial topic, where comparative evidence to non-operative care is limited. This registry-based descriptive cohort study, without a nonoperative comparator, aimed to evaluate longitudinal outcomes after anterior cervical discectomy and fusion (ACDF) for mechanically driven pain presumed to derive from a dysfunctional cervical segment.
METHODS: This observational cohort included all patients identified in Swespine (2006-2024) who underwent ACDF for presumed segmental dysfunctional-relat

Opioids remain central to managing cancer-associated pain (CAP) but are often limited by constipation, nausea, sedation, and concerns about maladaptive use amid the ongoing opioid epidemic. Suzetrigine (Journavx), a first-in-class, non-opioid, selective NaV1.8 sodium channel inhibitor, was approved by the U.S. Food and Drug Administration in January 2025 for moderate-to-severe acute pain; its role in chronic pain syndromes such as CAP has not been established. This case series describes two patients with CAP of differing pathophysiology started on suzetrigine as an off-label adjuvant analgesic

BACKGROUND: Degenerative cervical radiculopathy (CR) causes neck and upper-extremity pain, sensory disturbance, and motor dysfunction resulting from nerve root compression or irritation by disc herniation, foraminal narrowing, or spondylotic changes. Ultrasound-guided acupotomy (UA), a minimally invasive acupuncture-based intervention, permits real-time visualization of target tissues and adjacent neurovascular structures, enabling controlled release of fibrotic or adhesive soft tissues that may contribute to nerve root compression. Evidence on its effectiveness, safety, and cost-effectiveness

Sequestered lumbar disc fragments are a well-recognized cause of radiculopathy; however, when disc material demonstrates contrast enhancement, it may closely resemble an intraspinal tumor on magnetic resonance imaging (MRI), leading to diagnostic uncertainty, particularly in patients with a history of malignancy. We present the case of a 75-year-old man with a history of sigmoid and prostate carcinomas who presented with persistent low back pain and right-sided radicular symptoms for two months. Neurological examination revealed no motor deficit, with preserved strength of 5/5, although interm

Carcinomatous meningitis, or leptomeningeal metastasis, is a severe neurological complication of solid and haematological malignancies caused by dissemination of tumour cells within the leptomeninges and cerebrospinal fluid pathways. Clinical presentation is heterogeneous, including headache, intracranial hypertension, cranial nerve palsies or radiculopathy, which frequently leads to diagnostic delay. Diagnosis relies on converging clinical, radiological, and cerebrospinal fluid findings, with gadolinium-enhanced magnetic resonance imaging of the brain and spine and cerebrospinal fluid cytolog

Progressive unilateral calf enlargement is more commonly attributed to vascular, inflammatory, neoplastic, or lymphatic disease than to chronic denervation. We report a 55-year-old female recreational runner with progressive right calf enlargement and long-standing mild low back pain. Lumbar MRI demonstrated multilevel disc herniations, including a right-sided L5-S1 extrusion with compression of the right S1 nerve root. During the initial phase of calf symptoms, calf MRI showed mild asymmetric enlargement of the right posterior compartment with edema-like signal on fluid-sensitive sequences. A

BACKGROUND: Degenerative cervical myelopathy (DCM) is the leading non-traumatic cause of spinal cord dysfunction in adults, and its burden is projected to grow as populations age. Cervical radiculopathy shares the same degenerative substrate but follows a separate diagnostic pathway. Long-term national trends in the two conditions, and their relation to surgical practice, are unquantified in an East Asian system with universal coverage.
METHODS: This nationwide repeated cross-sectional ecological analysis used aggregate open data from the Korean Health Insurance Review and Assessment Service,

OBJECTIVES: The Canadian healthcare system faces a critical access block, with neurosurgical wait times reaching a median of 49.9 weeks from referral to treatment. This study evaluates the impact of the Rapid Access in Neurosurgery (RAIN) Clinic, a rapid access ambulatory model on reducing specialist wait times and diverting non-emergent cases from overburdened emergency departments (EDs).
METHODS: A prospective analysis was conducted on 2,979 patients referred to the RAIN Clinic at the University Health Network between April 2024 and December 2025. Key metrics included the referral-to-consul

BACKGROUND: Cervical radiculopathy may occasionally present with atypical pain patterns extending beyond the classical cervical and upper-limb distribution, potentially leading to diagnostic uncertainty. The thoracodorsal nerve originates predominantly from the C6-C8 roots and innervates the latissimus dorsi muscle, providing a possible anatomical basis for referred pain involving the posterior thoracolumbar region. This case describes an unusual presentation of cervical radiculopathy manifesting predominantly as severe thoracolumbar pain and its management using a combined interventional appr

BACKGROUND: Lumbar synovial cysts are benign, fluid-filled lesions arising from the facet joints, often associated with degenerative changes and segmental instability. These cysts can cause neurogenic claudication and radiculopathy, significantly limiting mobility and quality of life. While traditional treatments include conservative management, injections, aspiration, and open surgery, each has notable limitations. The Minimally Invasive Lumbar Decompression (MILD®) procedure, originally designed for lumbar stenosis, may offer a less invasive alternative for synovial cyst resection, especiall

BACKGROUND: Epidural steroid injections (ESIs) are commonly used for chronic low back pain but can rarely lead to serious infectious complications. Reactivation of latent tuberculous spondylodiscitis after ESI is uncommon. Our case highlights diagnostic challenges of underlying spinal infection prior to interventional pain procedures.
CASE REPORT: A 43-year-old woman presented with low back and radicular pain. There were no constitutional symptoms, and baseline laboratory findings were unremarkable. Lumbar magnetic resonance imaging (MRI) showed Modic-type degenerative changes and nerve root

BACKGROUND: Lower back pain and radiculopathy impose significant burdens on healthcare systems. Computed tomography (CT) scans are often used as an imaging modality for lower back symptomatology in the community. Accurately identifying relevant symptom sources using CT scans is often challenging due to the high incidence of incidental structural abnormalities. We examine the effectiveness of patient questionnaires in assisting reporting of CT scans of the lumbosacral spine.
METHODS: In this retrospective audit, a questionnaire was provided to 250 consecutive patients undergoing lumbar spine C

Segmental zoster paresis (SZP) is an uncommon motor complication of herpes zoster (HZ) characterized by focal, asymmetric weakness in a myotomal distribution. Motor weakness usually follows the cutaneous eruption, and pre-eruptive weakness can mimic cervical radiculopathy, brachial plexopathy, neuralgic amyotrophy, or other causes of acute focal limb weakness. A 57-year-old woman developed right shoulder pain and, five days later, became unable to abduct her right arm. A vesicular eruption appeared on the lateral aspect of the right upper arm three days after the onset of weakness, and HZ was

Symptomatic immobile spondylolisthesis with severe circumferential stenosis consisting of foraminal, lateral recess, and central canal narrowing secondary to disc height loss and facet arthropathy typically may require direct decompression followed by fixation given the need for complete facetectomies to adequately release exiting nerve roots. Recent evidence suggests that standalone grafts may offer a favorable risk profile for indirect decompression in cases with appropriate spinal alignment and normal bone density. A 54-year-old female presented with a one-year history of neurogenic claudic

Accurate lesion localization is a fundamental component of neurological diagnosis. This review describes how neurological examination can be used to differentiate cervical spinal cord lesions, cervical radiculopathy, brachial plexopathy, and peripheral nerve disorders in patients presenting with upper-limb sensory and motor symptoms. Particular attention is given to the distribution of sensory impairment, tendon reflex abnormalities, and patterns of muscle weakness. The characteristic clinical features of cervical spondylotic myelopathy, cervical radiculopathy, brachial plexus disorders, and e
