Persistent Post-traumatic Upper Limb Pain and Paraesthesia Following a Fall on an Outstretched Hand: A Case of Inconclusive Localisation.

Persistent upper limb pain following trauma can present a diagnostic challenge, particularly when clinical findings and investigations are discordant. We report the case of a 31-year-old female patient with persistent right upper limb and anterior chest wall pain associated with sensory disturbance in a multi-regional distribution, following a fall on an outstretched hand (FOOSH). Five years after the initial injury, the patient's symptoms, pain severity, functional limitations and neurological findings remained unchanged despite extensive conservative and interventional management, including

Spinal granulocytic sarcoma with both extradural and intradural involvement: a case report.

INTRODUCTION: Granulocytic sarcoma (GS) is a rare extramedullary accumulation of myeloid blasts. Cases of GS with isolated intradural presentation sparing the underlying bone are extremely rare. CASE DESCRIPTION: A 75-year-old male patient with a remote history of acute myeloid leukemia (AML) in 30-year remission presented with progressive lumbar radiculopathy. Magnetic Resonance Imaging (MRI) revealed intradural lesions spanning L2-S1, mimicking nerve sheath tumors. The patient underwent emergency laminectomy and resection of a vascular, grayish-green L2 mass adherent to the cauda equina. Im

Comparative study of biportal endoscopic and microscopic posterior cervical foraminotomy: clinical and radiological outcomes with minimum 2-year follow-up.

OBJECTIVE: Biportal endoscopic posterior cervical foraminotomy (BE-PCF) has gained popularity as a minimally invasive technique for the treatment of unilateral cervical radiculopathy. Although BE-PCF is expected to reduce tissue injury while providing effective neural decompression, long-term comparative data with minimally invasive microscopic PCF are limited. The aim of this study was to compare clinical outcomes, perioperative parameters, and tissue injury profiles between patients who underwent BE-PCF and those who underwent microscopic PCF with a minimum 2-year follow-up. METHODS: This r

Lumbar spinal brucellosis presenting as an atypical epidural abscess.

Brucellosis is a zoonotic infection that affects the spine, typically presenting as spondylodiscitis. Predominantly, epidural involvement without significant concurrent disc or vertebral involvement is rare and may closely mimic non-infectious pathologies. We report a man in his 50s presenting with low back pain and lower limb radiculopathy. Initial MRI demonstrated an epidural-predominant ventral epidural collection at L4-L5 associated with a posterior annular fissure, without significant vertebral or disc involvement, mimicking a sequestered disc fragment. Diffusion-weighted imaging showed p

Spinal leptomeningeal disease in patients with solid tumors: A single-center study on clinical presentation, diagnosis, treatment, and outcomes.

INTRODUCTION: Despite affecting 5-15% of metastatic cancer patients, leptomeningeal disease (LMD), and spinal LMD (sLMD) in particular, remain largely understudied. RESEARCH QUESTION: This study evaluates the clinical presentation, diagnosis, treatment, and outcomes of sLMD. MATERIAL AND METHODS: We retrospectively analyzed sLMD patients treated at a tertiary center (2011-2024) who underwent cranial and spinal imaging within 30 days of LMD diagnosis. Survival analysis was performed using Kaplan-Meier curves, with a multivariable Cox regression model being employed to identify independent pro

An Updated Systematic Review and Meta-Analysis of Fluoroscopic Caudal, Interlaminar, and Transforaminal Epidural Injections in Lumbar Disc Herniation.

BACKGROUND: Epidural injections are among the most frequently used interventions for management of low back and lower extremity pain. These injections are administered through caudal, interlaminar, and transforaminal approaches. Despite widespread use, controversy persists regarding effectiveness, appropriateness, medical necessity, and clinical indications. Although numerous studies have evaluated efficacy, there has been limited advancement in high-quality randomized evidence. In addition, multiple systematic reviews have repeatedly analyzed the same clinical trials, resulting in an evidence

Prediction of Therapeutic Outcome after Transforaminal Epidural Steroid Injection in Patients with Radiculopathy following Lumbar Spinal Stenosis Using Machine Learning.

BACKGROUND: Transforaminal epidural steroid injections are widely used to manage radicular pain caused by lumbar spinal stenosis; however, therapeutic outcomes vary among patients. Accurately predicting a transforaminal epidural steroid injection's response may help clinicians optimize treatment planning. OBJECTIVES: Our study aimed to develop and validate machine learning models, particularly a deep learning neural network, to predict therapeutic outcomes post transforaminal epidural steroid injections using both clinical and magnetic resonance imaging data. STUDY DESIGN: A study using mach

Influence of surgical approach on sexual dysfunction following single-level lumbar fusion in men: a propensity-matched analysis.

BACKGROUND: Sexual dysfunction represents a meaningful yet understudied complication of spine surgery, with substantial implications for quality of life. Differences in surgical exposure between anterior and posterior approaches may influence these outcomes through variation in anatomic disruption, neural risk, and perioperative course. While existing studies have reported select postoperative sexual complications in broad patient populations, comparative data specifically assessing dysfunction by operative approach among men undergoing single-level lumbar fusion remain sparse. This study comp

Clinicopathologic Features of Metaplastic Breast Carcinoma: Experience from A Cancer Research Centre in India.

Rapidly growing global demand for nutrient-dense foods is currently testing planetary boundaries, while global food systems are failing to alleviate a persistent burden of malnutrition in human populations. Naked clams (Teredinidae) have recently emerged as a promising candidate to address these deficiencies, offering high-nutrient, low-environmental impact "blue food" production. While previous research demonstrated the viability of maintaining adult naked clams in modular systems, full life-cycle culture and quantified growth performance remained unexplored. This study evaluates the capacity

Plate Augmentation in The Prevention of 3D-Printed Tritanium Cage Subsidence Following Anterior Cervical Discectomy and Fusion: A Retrospective Cohort Study.

BACKGROUND: Interbody cage subsidence following anterior cervical discectomy and fusion (ACDF) surgery can be associated with increased pain, segmental kyphosis, accelerated adjacent segment disease, and decreased rates of fusion. This study investigated the incidence of cage subsidence following the use of cages using Tritanium technology as cage-only (CO) or cage-plate (CP) constructs in patients undergoing ACDF. METHODS: This was a retrospective multicentre cohort study. Data was collected from hospital records and radiographs. All patients undergoing ACDF for cervical myelopathy or radicu

PCK2-Mediated PQBP1 Lactylation Promotes Asthmatic Inflammation through PRMT5 Inhibition.

Based on their immunodominant nature and ability to induce appropriate immune responses in the host, several antigens of Mycobacterium tuberculosis have shown promise of protection. However, most of the candidate vaccines developed by employing various strategies have afforded protection that is at best comparable with bacillus Calmette-Guérin (BCG) in animal models. Due to the inherent ability of BCG to prime cellular responses in the host, it has become an attractive vehicle for development of a vaccine against intracellular infections. In this study, we have cloned the genes of three immuno

Dissecting pseudoaneurysm in a spinal cord AVM treated by flow diversion: Technical note.

We report the case of a 29-year-old woman presenting with severe back pain and S1-radiculopathy. Digital subtraction angiography revealed a spinal cord arteriovenous malformation complicated by acute feeder dissection and pseudoaneurysm (PA) formation causing mass effect. After conservative management with limited angiographic and clinical improvement, selective treatment of the PA by flow diversion (FD) led to occlusion and shrinkage with the resolution of clinical symptoms. FD may be a feasible treatment option for spinal artery aneurysms in selected patients with favorable anatomy. The need

Percutaneous treatment of lumbar synovial cysts with bleomycin: A case report.

Lumbar synovial cysts (LSC) that protrude into the spinal canal can cause lower back pain, neurogenic claudication, and radiculopathy. Often diagnosed in the elderly population (typically ∼60 years of age) with a slight preponderance for females, their underlying etiology is thought to be due to degeneration of the adjacent facet joint, with the most common location at the level of L4-L5. Treatment of LSC can be conservative (with NSAIDs and physical therapy), percutaneous (with rupture), or surgically (with decompression with or without fusion). Percutaneous treatment of LSC involves rupturin

Postpanniculectomy Improvement in Low Back Pain: A Case Report.

While panniculectomy is traditionally considered a cosmetic surgery, there is growing interest in the potential role of panniculectomy for the treatment of low back pain. We present a case of a 69-year-old man with a history of over 10 years of significant low back pain with radiculopathy rated as 7 out of 10 and resistant to conservative measures as well as multiple spinal decompression surgeries. He had undergone bariatric surgery 14 years prior and maintained a stable body mass index. He presented with a large pannus desiring panniculectomy for aesthetic purposes. He underwent an uncomplica

Comparative Study on Clinical Outcomes of Posterior Endoscopic Cervical Foraminotomy under Local Anesthesia with Conscious Sedation and General Anesthesia.

OBJECTIVE: Posterior endoscopic cervical foraminotomy (PECF) is a minimally invasive surgical technique for treating cervical radiculopathy. Traditionally, PECF is performed under general anesthesia in the prone position, but concerns over anesthesia-related complications have led to the exploration of local anesthesia in the lateral decubitus position as an alternative. This study aims to compare the clinical outcomes, safety, and efficacy of PECF performed under local anesthesia in the lateral decubitus position versus general anesthesia in the prone position. METHODS: We conducted a retros
