Foraminal [18F]FDG Uptake on PET/MRI Is Associated with Radiculopathy and Symptom Reduction after Image-Guided Nerve Root Block.

BACKGROUND AND PURPOSE: There is an imperfect correlation between morphologic MRI findings and radiating low back pain (LBP). Nerve irritation, visualized as glucose hypermetabolism on [18F]FDG-PET/MRI, has the potential to identify symptomatic segments. This study aimed to investigate the association of foraminal [18F]FDG uptake on PET/MRI, radiologic abnormalities, and patient outcomes. MATERIALS AND METHODS: Prospectively recruited patients with radiating LBP underwent [18F]FDG-PET/MRI of the lumbar spine in this observational study. Back pain and leg/buttock pain were assessed by using th

Sensorimotor cortical integration remains unchanged after dorsal root ganglion pulsed radiofrequency.

BACKGROUND: Dorsal root ganglion (DRG) pulsed radiofrequency (PRF) is a minimally invasive neuromodulation technique used for the management of chronic radicular pain. While its analgesic effects are well-documented, its impact on sensorimotor integration at the cortical level remains unclear. This study aimed to investigate whether DRG PRF modulates sensorimotor integration via the cholinergic system using the short-latency afferent inhibition (SAI) paradigm. METHODS: This prospective, 2-center study included 30 patients with chronic lumbosacral radicular pain who underwent DRG PRF at the L4

Minimally Invasive Biportal Endoscopic Spinal Cord Stimulation : Technical Report and Case Series.

OBJECTIVE: The insertion of a surgical paddle lead for spinal cord stimulation (SCS) is a cornerstone therapy for chronic refractory pain, with lower impedance and reduced battery usage than a percutaneous lead. However, the greater invasiveness of this procedure can cause complications. METHODS: This study introduces a novel SCS technique using the unilateral biportal endoscopy (UBE) approach, illustrated through intraoperative images and endoscopic videos. We retrospectively reviewed 14 patients who underwent SCS using the UBE technique. Clinical, surgical, and radiological data were collec

The Timing of Diskectomy as a Predictor of Outcomes in Patients With Lumbar Disk Herniation: A Cohort Study on Varying Durations of Preoperative Symptoms.

BACKGROUND AND OBJECTIVES: Currently, the treatment of lumbar disk herniation (LDH) consists of conservative treatment followed by surgery when symptoms persist; however, the optimal timing of surgery to achieve the best clinical outcome is not clear. To evaluate the impact of durations of preoperative low back pain, disability, radicular leg pain, motor, and sensory deficit on postoperative outcomes in patients with LDH who underwent microdiskectomy. METHODS: In our cohort study, we prospectively enrolled patients with different durations of preoperative LDH symptoms scheduled for microdiske

Analgesic efficacy of inhaled methoxyflurane during transforaminal epidural steroid injection for lumbar radiculopathy: a randomized, double-blind placebo-controlled trial.

STUDY DESIGN: Randomized, double-blind, placebo-controlled trial. PURPOSE: To evaluate the efficacy and safety of inhaled methoxyflurane for pain relief during transforaminal epidural steroid injection (TFESI) in patients with lumbar radiculopathy. OVERVIEW OF LITERATURE: TFESI is a common intervention for lumbar radiculopathy, but procedural pain remains a concern. Methoxyflurane is a rapid-onset, inhaled analgesic with a favorable safety profile. While its effectiveness in procedural pain relief has been demonstrated in other medical settings, its role in TFESI remains underexplored. METH

Cervical Spine Pathology Versus Upper Extremity Compressive Peripheral Neuropathy: Which Is It?

Cervical pathologies often mimic other peripheral compressive neuropathies in presentation, making their timely diagnosis challenging. Cervical pathologies such as radiculopathy and myelopathy may be present simultaneously along with other peripheral neuropathies. It is important to provide a practical approach to identify cervical pathologies and their treatments.

Prone Endoscopic Lateral Lumbar Interbody Fusion: Operative Technique and Functional Outcomes in 35 Patients.

BACKGROUND: Lateral lumbar interbody fusion is a widely used technique to address degenerative lumbar conditions but can be associated with injury to the psoas, lumbar plexus, and abdominal wall owing to retractor usage. We describe a minimally invasive endoscopic lateral lumbar interbody fusion (ELLIF) procedure that aims to reduce these complications by avoiding prolonged muscle retraction, preparing the disc space under direct endoscopic vision, and shortening the surgical time. METHODS: Between 2019 and 2024, 35 patients underwent ELLIF at a single center. Discectomy, endplate preparation

A New Perspective on Lumbar Disc Herniation Management Using Prone Knee Extension.

INTRODUCTION: Lumbar disc herniation (LDH) is a frequent cause of low back pain and radiculopathy, often resulting in diminished functional capacity and a lower quality of life. Nonsurgical interventions are frequently sought to manage symptoms and enhance spinal stability. This case report explores a novel application of the prone knee extension (PKE) exercise as part of a comprehensive physiotherapy regimen aimed at addressing pain, mobility limitations, and functional impairments in a patient with LDH. CASE PRESENTATION: A 34-year-old male with a history of LDH and radiating lower limb sym

Spinal Cord Stimulation as a Potential Therapeutic Modality for Managing Concurrent Chronic Low Back and Abdominal Pain Complicated by Device Migration: A Case Report.

A 48-year-old man with chronic pancreatitis-related chronic abdominal pain (CAP) and concurrent chronic low back pain (LBP) with radiculopathy had inadequate relief from injectable and opioid therapies. A spinal cord stimulation (SCS) trial with dual leads spanning T4-T6 produced significant CAP relief, leading to permanent implantation at T5, after which he reported improvement in both CAP and LBP. Several months later, analgesia abruptly failed due to caudal lead migration to T7 with high impedance at multiple contacts and unsuccessful reprogramming, prompting referral for surgical revision

Transforaminal Epidural Steroid Injection Technique: Fluoroscopy-Guided Minimally Invasive Injection for Lower Limb Radiculopathy-Twelve Golden Steps.

Radicular back pain is defined as low back pain radiating into the lower extremities in a dermatomal pattern caused by compression or irritation of the nerve root. Fluoroscopy is an excellent tool for the management of lower limb radiculopathies that are secondary to discogenic pathologies not responding to the conservative trial. Transforaminal epidural steroid injection technique is a minimally invasive intervention for radiculopathies, which can easily be done in a day care setting, and it should be considered before surgery; however, precision and expertise is required.

Cervical Radiculopathy and Neuromusculoskeletal Mimics in the Athlete.

Cervical radiculopathy presents with symptoms involving the head, neck, and upper limb that can significantly impact athletic performance. While the presence of radiating pain and/or paresthesias may increase the clinical suspicion for a cervical radiculopathy, a variety of alternative neurogenic and musculoskeletal disorders can mimic cervical radicular patterns. Familiarity with key differentiating variables in the history and physical examination is essential for early and accurate diagnosis. Evaluation may require the use of imaging, electrodiagnostic testing, and/or diagnostic injections.

Predictive value of muscle tone loss and perfusion index in the success of supraclavicular brachial plexus block: A pilot study.

BACKGROUND: Currently, no standardized tool accurately predicts the success of supraclavicular brachial plexus block (SBPB) in the shortest time possible. This study aimed to evaluate the early predictive value of muscle tone loss and perfusion index in predicting SBPB success. This is the first study to report the use of this device for objective assessment of nerve block success. METHODS: A prospective observational pilot study was conducted at "Dr. Ignacio Morones Prieto" Central Hospital, Mexico, involving 33 patients (ASA 1, 2, 3) requiring surgery on the distal third of the upper limb w

Factors Associated With Failure of Symptomatic Relief After Single-Level Posterior Cervical Foraminotomy for Unilateral Radiculopathy.

BACKGROUND AND OBJECTIVES: Posterior cervical foraminotomy (PCF) is a motion-sparing treatment for cervical radiculopathy with rates of symptomatic pain relief similar to anterior cervical diskectomy in patients with persistent symptoms despite nonsurgical management. Although the operation is effective in well-selected patients, some who undergo PCF do not show significant symptomatic improvement, and objective radiographic parameters to identify these patients would assist with operative planning. We sought to determine the radiographic, demographic, and operative factors that are associated

Management of Pseudomeningoceles after Surgery or Trauma.

Pseudomeningocele, a post-operative or post-traumatic cerebrospinal fluid (CSF) collection from a dural defect, is an underrecognized complication following spinal or cranial surgery. Incidence varies from 0.07 to 2% in lumbar and up to 23% in posterior fossa procedures. While many resolve spontaneously, symptomatic cases can cause headaches, radiculopathy, or infection, necessitating stepwise management. Diagnosis relies on magnetic resonance imaging findings of fluid continuity with the subarachnoid space. Asymptomatic, small pseudomeningoceles are managed conservatively through bed rest, ab
