Prognostic Value of Selective Nerve Root Blocks Prior to Pulsed Radiofrequency in the Treatment of Patients With Chronic Radicular Pain: A Systematic Review.

BACKGROUND/IMPORTANCE: Selective nerve root blocks (SNRBs) are frequently used in clinical algorithms for managing chronic radicular pain. However, their prognostic value in identifying patients likely to benefit from pulsed radiofrequency (PRF) treatment remains uncertain. OBJECTIVE: This systematic review evaluates whether a positive response to an SNRB predicts improved clinical outcomes following PRF in patients with chronic radicular pain. EVIDENCE REVIEW: A systematic search was conducted in PubMed, Embase, and Cochrane databases, along with reference lists of relevant articles. Eligib

Refractory Chronic Knee Pain After Total Knee Replacement Successfully Treated With L4 Dorsal Root Ganglion Pulsed Radiofrequency.

Persistent pain following total knee replacement (TKR) remains a significant clinical problem and may lead to long-term functional impairment despite technically successful surgery. The underlying mechanisms are often multifactorial and may include neuropathic and referred pain components. We report the case of a 71-year-old woman with severe bilateral knee pain persisting after bilateral TKR, refractory to pharmacologic therapy, physiotherapy, genicular nerve interventions, and peripheral nerve blocks. Ongoing neuropathic features and failure of peripheral treatments raised suspicion of a spi

Ultrashort Echo Time Magnetization Transfer MR Imaging of Lumbar Nerve Roots in Patients With Disc Herniation.

BACKGROUND: Lumbar disc herniation (LDH) causes compositional alterations within compressed nerve roots, resulting in low back pain (LBP). The ultrashort echo time magnetization transfer technique (UTE-MT) facilitates assessment of macromolecular changes in collagen- or myelin-rich tissues in nerve roots. PURPOSE: To assess lumbar nerve root composition in LDH using UTE-MT. STUDY TYPE: Prospective. POPULATION: One hundred and seventy-six participants (age range, 20-89; 72 females) with LDH. FIELD STRENGTH/SEQUENCE: 3T/UTE-MT, Carr-Purcell-Meiboom-Gill (CPMG). ASSESSMENT: UTE-MT ratio (UTE

Misdiagnosis of superior sulcus tumours: a scoping review.

BACKGROUND: Superior sulcus tumours (SSTs) are a rare but serious non-small cell lung cancer that requires aggressive multi-modal intervention. Existing literature suggests patient outcomes are adversely affected by diagnosis delay, often from misdiagnosis as a musculoskeletal condition. As such, the aims of this review were to identify the key clinical features of SSTs documented in the literature and differentiate them from commonly misdiagnosed musculoskeletal pathologies. METHODS: In accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines for scoping

Intense Coughing Following Caudal Epidural Steroid Injection for Lumbar and Lumbo-Sacral Disc Prolapse with Radiculopathy. A Case Report with Pathophysiological Insights.

Caudal epidural steroid injection (CESI) is a commonly performed intervention for chronic lumbar and lumbosacral radicular pain, generally considered safe with infrequent adverse effects. We report a rare case of sudden-onset, intense coughing immediately following ultrasound-guided CESI in a 32-year-old male with prolapsed intervertebral disks at the L4-L5 and L5-S1 levels. The episode lasted approximately four minutes and resolved spontaneously without neurological or hemodynamic compromise. Possible mechanisms include cephalad spread of the injectate, inadvertent intravascular injection or

Comparing CT-guided and fluoroscopic-guided interventions for chronic low back pain management: a randomized trial.

OBJECTIVE: Low back pain (LBP) is a leading cause of disability, with radicular symptoms often resistant to conservative treatments. While fluoroscopy and computed tomography (CT) play a pivotal role in procedural accuracy, direct comparisons of clinical outcomes remain limited. We compared the efficacy and safety of fluoroscopy-versus CT-guided interventions in the management of radicular LBP. MATERIALS AND METHODS: Adults with chronic LBP were prospectively randomized 1:1 to receive either fluoroscopy-guided or CT-guided interventions. Assessments were conducted at baseline, one week, one m

Impact of Cervical Radiculopathy on the Risk of Cubital Tunnel Syndrome Following Distal Humerus Fracture: A Retrospective Cohort Analysis of 165 Million Patients.

BACKGROUND: Cubital tunnel syndrome (CuTS) is a common peripheral neuropathy often exacerbated by trauma or anatomical disruption. Cervical radiculopathy has been proposed as a proximal risk factor via the double crush mechanism, but its role in post-traumatic CuTS following distal humerus fracture (DHF) remains underexplored. METHODS: A retrospective cohort study was conducted using the PearlDiver database (165 million patients). Patients with DHF were stratified by the presence of pre-existing cervical radiculopathy. Incidence rates of cubital tunnel syndrome diagnosis (CuTSD) and cubital t

Lumbar radiculopathy after left common iliac vein stent placement.

Left common iliac vein stent placement for symptomatic compression by the right common iliac artery, an entity known as May-Thurner syndrome, is a procedure performed with increasing frequency and good clinical outcomes. In this institutional review board-approved retrospective electronic medical record review, we describe three cases of left L5 lumbar radiculopathy immediately after left common iliac vein stent placement performed elsewhere. Additionally, we describe surgical and endovascular approaches for correction, the utility of multimodality diagnostic testing, and endovascular techniqu

Radiographic and clinical predictors of surgical outcomes following endoscopic decompression for radiculopathy in adult degenerative scoliosis: A multi-center retrospective study.

INTRODUCTION: Endoscopic transforaminal decompression is a minimally invasive approach for treating radiculopathy in adult degenerative scoliosis (ADS) secondary to foraminal stenosis. Previous research supports the feasibility and safety of this technique, but the correlation of preoperative measures with postoperative outcomes remains unclear. RESEARCH QUESTION: This study aims to assess the efficacy of endoscopic decompression for ADS-related radiculopathy and identify predictive factors for postoperative pain and functionality to optimize surgical decision-making. MATERIALS AND METHODS:

(Cost-)Effectiveness of personalised multimodal physiotherapy compared to surgery in patients with painful cervical radiculopathy. Protocol for a randomised non-inferiority trial (The MOVE-IT study).

BACKGROUND: Painful cervical radiculopathy can lead to substantial and long-lasting limitations in activities and participation. Surgery is considered when conservative treatment fails to deliver relevant improvements or when neurological signs are severe. However, personalised multimodal physiotherapy may offer non-inferior outcomes to surgery, with possibly fewer adverse events and lower costs. Further research is needed to assess the (cost-)effectiveness of personalised multimodal physiotherapy compared to surgery. METHODS: This randomised non-inferiority study compares personalised multim

A Comparative Analysis of Ultrasound versus Fluoroscopy-Guided Transforaminal Endoscopic Lumbar Discectomy: Protocol for a Randomized Clinical Trial Targeting Lumbar Disc Herniation.

Lumbar disc herniation (LDH) is a prevalent cause of chronic disability that often necessitates surgical intervention when conservative treatments fail. This study aims to compare the clinical efficacy and safety of ultrasound-guided transforaminal endoscopic lumbar discectomy (UG-TELD) against fluoroscopy-guided transforaminal endoscopic lumbar discectomy (FG-TELD) in a single-center, parallel, block-randomized trial involving 80 adult patients aged 18 to 75 years with MRI-confirmed radiculopathy unresponsive to at least six weeks of conservative management. Participants will be randomly assi

Tarlov cysts and sexual dysfunction: A multidisciplinary approach to evaluation and surgical treatment.

BACKGROUND: Persistent Genital Arousal Disorder/Genito-Pelvic Dysesthesia (PGAD/GPD) involves chronic, unrelenting arousal and dysesthesia of the genito-pelvic region. In 2012, Komisaruk and Lee identified a high incidence of sacral Tarlov cysts in patients with PGAD/GPD, prompting development of a multidisciplinary diagnostic and treatment algorithm. This study reviews surgical outcomes for patients with PGAD/GPD in whom a Tarlov cyst was identified as the symptomatic driver. METHODS: A retrospective review of prospectively collected data was conducted on patients with PGAD/GPD who underwent

Improvement in spinopelvic parameters after laminectomy and complete factectomies above prior fusion facilitated by posterior arthroplasty: illustrative case.

BACKGROUND: Patients with adjacent segment degeneration after previous lumbar fusion often present with flat back posture. However, it is sometimes unclear if the flat back results from stenosis above the fusion or from structural spinal deformity. The authors illustrate the case of a patient who refused further fusion and asked for decompression only coupled with posterior arthroplasty. They demonstrate his change in sagittal alignment despite absence of a three-column osteotomy. OBSERVATIONS: A 72-year-old man with a previous lumbar fusion presented with 1 year of progressive neurogenic cla

A rare symptomatic L5-S1 anterior retroperitoneal lumbar disc herniation causing foot drop and mimicking a peripheral nerve sheath tumor: illustrative case.

BACKGROUND: Symptomatic anterior retroperitoneal lumbar disc herniations (ARLDHs) are exceedingly uncommon and pose a significant diagnostic challenge. The challenge arises when the disc herniation directly compresses the peripheral nerve and mimics a peripheral nerve sheath tumor (PNST). This case report highlights the difficulty in accurate radiological differentiation between a PNST and ARLDH, despite sophisticated neuroimaging techniques. OBSERVATIONS: The authors present the case of a 70-year-old male with symptomatic right-sided L5 radiculopathy and foot drop. Initial contrast MRI sugge

Bertolotti joint-related cyst: illustrative case.

BACKGROUND: Bertolotti syndrome is an underrecognized cause of chronic low back pain. Back pain generated from Bertolotti syndrome is believed to result from altered weight dissipation mechanics and/or instability of the pseudoarticulation, while local nerve root compression may result in radiculopathy. Here, the authors describe the findings of a patient with treatment-refractory low back pain found to have a cystic lesion arising from a Bertolotti joint causing L5 nerve root compression. To their knowledge, this has not been previously described in the literature. OBSERVATIONS: Despite earl

Spontaneous Resorption of Lumbar Disc Herniation: A Narrative Review of Pathophysiology, Predictive Factors, and Clinical Decision-Making.

Lumbar intervertebral disc herniation is a common cause of low back and radicular leg pain, traditionally managed with a combination of conservative therapies and, when indicated, surgical discectomy. An intriguing phenomenon observed in many patients is the spontaneous resorption of herniated disc material over time, often correlating with significant symptom improvement. This article is presented as a narrative review synthesizing experimental, imaging, and clinical literature relevant to spontaneous disc resorption and its implications for clinical decision-making. This paper provides a com
