PURPOSE OF REVIEW: Occipital neuralgia, cervicogenic headache, and migraine are disorders that are commonly accompanied by neck pain. Although these disorders may present with similar features, coexist in the same patient, and have variable degrees of involvement of the C2 and C3 nerve roots, it is important to distinguish between these three entities, as the presumed diagnosis can lead to different diagnostic and therapeutic modalities.
RECENT FINDINGS: Occipital neuralgia in particular is underdiagnosed, occipital nerve blocks are underutilized, and technical aspects that can influence the

Objective: Although Global Postural Re-education (GPR) is widely used for musculoskeletal conditions, its specific benefits for this population remain unclear due to inconsistent findings across studies. This systematic review aims to analyze the effects of GPR on pain intensity, functionality, and range of motion (ROM) in individuals with chronic non-specific neck pain. Methods: Computerized search was performed in the Cochrane CENTRAL, Lilacs, EBSCO, PEDro, Pubmed, RCAAP and Scielo databases using the keyword combination ("Global Postural Rehabilitation" OR "Global Postural Reeducation" OR "

Background and Objectives: Questionnaires are patient-reported outcome measures that require a validation process to assess their reliability and replicability. Over time, questionnaires have not only focused on a single health condition, such as neck pain, but also expanded their assessment spectrum to other areas in order to gather additional and relevant information from the patient. The main objective of this study was to conduct a systematic review of the different structural and psychometric characteristics of neck pain questionnaires. Materials and Methods: A systematic review was condu

OBJECTIVE: Chronic musculoskeletal pain negatively impacts gait performance, but the impact of neck pain on gait remains unclear. This study aimed to assess measures of gait (speed and variability) in individuals with chronic neck pain (CNP) compared to non-CNP controls across various walking conditions.
METHODS: Sixty participants with CNP and 41 controls were recruited. Pain, disability, general health, and gait were measured. Gait outcomes included gait speed, stride time variability (STV), and a multi-component gait variability index (GVI) across four different walking conditions: quiet w

Identifying pain generators for cervical spine patients can be challenging, particularly in those with prior surgery. Single-photon emission computed tomography (SPECT/CT) may help localize potential areas of pain generation through increased radionuclide uptake. We conducted a single-institution retrospective cohort study evaluating the concordance between MRI and SPECT/CT findings and assessing outcomes after surgery targeting spinal levels with increased uptake. Demographic, clinical, and imaging data were extracted. Degenerative changes (spondylosis, facet arthropathy, stenosis, disc disea

BACKGROUND: Previous research has identified a need for relevant evidence-based clinical guidelines in primary care physiotherapy. However, an essential first step is to identify the most prevalent diagnoses among patients seeking physiotherapy. Thus, this study aims to (I) identify the most prevalent musculoskeletal diagnoses and, (II) describe physiotherapy utilization and patient characteristics among patients consulting physiotherapists in Swedish primary care.
METHODS: This was a register-based study including all physiotherapy visits in primary care in Region Skåne, southern Sweden duri

We report a case of an 82-year-old patient with crowned dens syndrome, who presented with symptoms mimicking acute meningitis. The patient exhibited severe acute cervicalgia, fever, arthritis of the wrists and hands, along with elevated inflammatory parameters. Acute meningitis was ruled out with a normal cerebrospinal fluid study. A computed tomography scan of the cervical spine revealed calcifications around the odontoid process, confirming the diagnosis of crowned dens syndrome (CDS). The patient showed a favorable response to treatment with prednisone. CDS is an uncommon presentation of ca

BACKGROUND: In Part I of this series we identified spinal cord injury without fracture or dislocation (SCIwoFD) as having become the most common type of spinal cord injury at our level I trauma center. The purpose of this paper was to examine two databases, one local and one national, to determine whether this population could be discovered using traditional search techniques.
METHODS: We analyzed ICD-10 codes applied by our institution to SCIwoFD patients identified in Part I for accuracy and appropriateness. Concurrently, we queried the National Inpatient Sample (NIS) database between 2015

BACKGROUND: Calcium pyrophosphate dihydrate (CPPD) deposition disease at the craniocervical junction (CCJ) typically presents with a retro-odontoid pseudotumor. Here, the authors report a case of CPPD-induced basilar impression, causing vertebral artery (VA) dissection and hemorrhage.
OBSERVATIONS: A 65-year-old male presented with worsening chronic cervicalgia, occipital headaches, and unstable tandem gait. CT revealed basilar impression with severe atlantoaxial erosion and instability, absent on imaging 8 months prior. MRI showed high-grade CCJ stenosis without myelopathy, due to CPPD. Befo

Syringomyelia is an abnormal collection of cerebrospinal fluid in the spinal cord. This fluid-filled cavity can elongate or expand over time, causing paresthesia, paralysis, or pain. We review the case of a 68-year-old woman with a thoracic syrinx from T5 to T11. The patient has a long-standing history of cervicalgia, lumbar radiculopathy, nerve root impingement, and degenerative disc disease. After years of chronic pain, the patient presented with worsening right-sided back pain that radiated to the umbilicus, which was initially treated with conservative pharmacotherapy. Due to inadequate pa

UNLABELLED: Clevidipine (Cleviprex), a late-generation dihydropyridine calcium channel antagonist available as a lipid emulsion for intravenous infusion, is approved in the US for the reduction of blood pressure (BP) when oral therapy is not feasible or desirable. Intravenous clevidipine is effective in the treatment of both acute preoperative and postoperative hypertension in adult cardiac surgery patients, and with a rapid onset and short duration of action the drug can be easily titrated for predictable BP control. Moreover, in terms of controlling acutely elevated BP in this patient popula

INTRODUCTION: Many of the injury mechanisms that cause mild traumatic brain injury (mTBI) also create forces commonly associated with whiplash, resulting in cervical pain injury. The prevalence of associated neck pain with mTBI is not well established. There is a strong indication that injury to the cervical spine may aggravate, cause, and/or impact recovery of symptoms and impairments associated with the concussive event and its primary effect on the brain. The purpose of this study is to help identify the prevalence of ensuing cervical pain within 90 days of a previously documented mTBI and

BACKGROUND: Deep neck flexor muscle atrophy and increased superficial neck muscle activation are associated with disability and pain intensity in individuals with neck pain. There is a lack of evidence to support direct assessment of deep neck flexor muscles in a non-invasive way during exercise performance to help determine the effectiveness of different neck strengthening exercises.
OBJECTIVE: Compare longus colli (LC) and sternocleidomastoid (SCM) activation between individuals with and without neck pain using real time ultrasound (RTUS) during a series of craniocervical exercises.
