Lumbar Epidural Steroid Injection

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

Lumbar Disc Herniation: What's New in Surgical Treatment Hérnia de disco lombar: O que há de novo no tratamento cirúrgico

Lumbar disc herniation is one of the main causes of root pain and disability in the adult population, with a significant impact on the healthcare system and labor productivity. Initial management is predominantly conservative, but a portion of patients may evolve with refractory symptoms or progressive neurological deficit, requiring surgical intervention. Over the past few decades, minimally invasive techniques, such as endoscopic surgery, have revolutionized treatment with less tissue damage, accelerated recovery, and lower complication rates. This article reviews the evolution of surgical t

An Updated Systematic Review and Meta-Analysis of Fluoroscopic Caudal, Interlaminar, and Transforaminal Epidural Injections in Lumbar Disc Herniation.

BACKGROUND: Epidural injections are among the most frequently used interventions for management of low back and lower extremity pain. These injections are administered through caudal, interlaminar, and transforaminal approaches. Despite widespread use, controversy persists regarding effectiveness, appropriateness, medical necessity, and clinical indications. Although numerous studies have evaluated efficacy, there has been limited advancement in high-quality randomized evidence. In addition, multiple systematic reviews have repeatedly analyzed the same clinical trials, resulting in an evidence

Percutaneous Endoscopic Interlaminar Lumbar Discectomy for Treating Acute Cauda Equina Syndrome: A Case Series of 25 Patients.

BACKGROUND: Cauda equina syndrome (CES) is caused by compression of nerves in the lumbar spine below the conus medullaris. Symptoms include progressive weakness in the lower extremities, back pain with sciatica, saddle anesthesia, and urinary or bowel dysfunction. These symptoms vary in intensity and onset, and can sometimes be an emergency. The standard surgical procedure for CES typically involves a microscope-assisted interlaminar decompression with laminotomy and sequestrectomy, with or without partial, rather than complete, discectomy. However, endoscopic spine surgery was introduced as a

Clinical and Functional Outcomes of Bone Defect Reconstruction Using the Masquelet Technique: Experience with Rare Talus and Metatarsal Cases.

BACKGROUND: Segmental bone defects remain a major challenge in orthopedic surgery. The Masquelet's induced membrane technique (MIMT) is a two-stage reconstructive approach used for limb salvage in complex defects. This study evaluated radiographic healing and patient-reported outcomes after MIMT in a single tertiary center, including rare talus and metatarsal applications. METHODS: We retrospectively reviewed 14 patients (10 males, 4 females) treated with MIMT for post-traumatic or osteomyelitis-related lower-extremity bone defects (tibia, femur, talus, metatarsal) between 2010 and 2023. Inhibition of Retinal AdoRA2a Activity Attenuates Myopia Progression.

PURPOSE: Although 7-methylxanthine, a nonselective adenosine receptor (AdoRs) antagonist, suppresses myopia, the specific receptor subtype and target tissue involved remain unclear. This study aimed to investigate whether the AdoRA2a subtype plays a critical role in the regulation of myopia progression. METHODS: Monocular form deprivation myopia (FDM) was induced in mice to measure retinal concentrations of adenosine and its synthetic enzymes (CD39/CD73). Retina-specific AdoRA2a knockout (AdoRA2a-CKO) mice and their wild-type littermates were subjected to unilateral FDM or normal, unobstructe

Comparison between paravertebral injections of ozone and epidural injections of corticosteroids used in the treatment of lumbosacral pain: scoping review.

Paravertebral ozone injections have been used in the treatment of lumbosacral pain with or without radicular irradiation and have been reported to be effective in reducing pain when compared with placebo, physiotherapy programs or drug treatments; nevertheless, their effectiveness in comparison with minimally invasive procedures using corticosteroids is unknown. The main objective of this review was to explore the role of paravertebral ozone injections in the treatment of lumbosacral pain with radicular irradiation in comparison with minimally invasive procedures using corticosteroids. Long-acting dexamethasone viscous gel (SP-102) transforaminal injection for lumbosacral radicular pain.

Lumbosacral radicular pain (LRP) is a condition characterized by debilitating pain and functional impairment. Epidural steroid injections (ESIs) are frequently used to relieve symptoms of LRP; there are currently no FDA-approved corticosteroid products specifically indicated for this condition. Moreover, existing products carry safety warnings against epidural administration, which have been linked to the off-label use of formulations. SP-102 (10 mg dexamethasone in a 2 ml viscous gel) is a proposed product that is formulated to provide rapid onset of action while prolonging the duration of ef

Epidural Transforaminal Infraneural Infiltration Using Corticosteroids in Lumbar Radiculopathy.

BACKGROUND: A lumbar disc herniation is the primary cause of sciatica, affecting 1% to 5% of the population annually. Epidural corticosteroid injections, whether via the transforaminal or interlaminar route, are an option for patients with refractory sciatica pain. The transforaminal route has the advantage of directly addressing the site of pathology in the anterior epidural space, between the disc herniation and the nerve root. PURPOSE: To evaluate the efficacy of transforaminal infraneural epidural injections using corticosteroids in the treatment of radiculopathy secondary to acute lumbar

A new hernia blocking system prevents lumbar disc herniation recurrence and disc degeneration: 2 years results of a multicentric clinical investigation.

INTRODUCTION: Recurrent lumbar disc herniation (rLDH) is a common complication of limited discectomy for patients with large annular defects. This manuscript presents the first long-term experience with a new hernia blocking system (HBS) that aims to reduce rLDH. RESEARCH QUESTION: Whether a new HBS, implanted after a limited discectomy, prevents rLDH. MATERIAL AND METHODS: Thirty patients with a postero-lateral disc herniation between L4 and S1 and large annular defects, who underwent a limited discectomy and were treated with a new HBS (DISC care, NEOS Surgery S.L.), were included in the s

Spinal manipulative therapy for adults with chronic low back pain.

RATIONALE: Many therapies exist for the treatment of chronic low back pain (LBP), including spinal manipulative therapy (SMT), which is a worldwide, extensively practised intervention. The effectiveness of SMT for chronic LBP is not without dispute. This Cochrane review is an update of a Cochrane systematic review published in 2011. OBJECTIVES: To evaluate the benefits and harms of SMT compared to (1) sham SMT/placebo intervention, (2) no treatment, and (3) other conservative interventions in people with chronic LBP (18+ years old). SEARCH METHODS: We searched the Cochrane Central Register o

Non-Specific Low Back Pain in South Australian Public Hospital Emergency Departments: A Retrospective Observational Study.

INTRODUCTION: Low back pain (LBP) is a leading cause of disability and a common reason for emergency department (ED) presentation. Although clinical guidelines recommend community-based treatment for non-specific LBP, a range of factors drive people to seek hospital-based care in Australia. This study aimed to describe the patient flow, hospital resource utilisation, and economic impact of LBP-related presentations to South Australian public hospital EDs to identify clinical and financial levers to support innovation. METHODS: This retrospective observational study utilised data from public h

Recurrent Symptoms in Lumbar Disc Herniation with Concomitant Schwannoma: The Role of Tumor Microenvironment and Anti-Inflammatory Therapy.

INTRODUCTION: Lumbar intervertebral disc herniation commonly causes back pain and radiculopathy, but its coexistence with paravertebral tumors is rare. Nerve sheath tumors are the most frequent, yet their simultaneous occurrence with disc herniation producing similar symptoms is uncommon. Such cases are diagnostically and therapeutically challenging, as overlapping features may obscure the pathology and delay treatment. CASE REPORT: This is a rare case of a 33-year-old male presenting with low back pain and bilateral radiculopathy, left greater than right, for 1 month with positive straight l
