Comparison of lumbar paraspinal muscle size and density between members of the US Framingham Heart Study and Tsimane forager-horticulturalists of Bolivia.

Paraspinal muscles support and move the spine, and reduced size and density of these muscles is associated with increased risk of vertebral fracture and back pain. We test the hypothesis that subsistence-oriented forager-horticulturalists (Bolivian Tsimane) have larger and denser paraspinal muscles, and smaller age-related differences in paraspinal muscle size and density, than more sedentary post-industrialized suburbanites (Framingham Heart Study [FHS]). We measured cross-sectional area (CSA) and density of trunk extensors and psoas major at L3 from computed tomography scans of 411 FHS and 3

Multilevel Lumbar Epidural Angiolipoma: A Case Report.

Spinal angiolipomas are rare benign tumors composed of mature adipose tissue and vascular elements, most commonly located in the thoracic epidural space. Lumbar involvement is considerably less frequent, and variable proportions of adipose and vascular tissue may result in atypical magnetic resonance imaging (MRI) characteristics and diagnostic uncertainty. We report the case of a 36-year-old woman with an approximately one-year history of persistent low back pain associated with subjective bilateral lower-extremity weakness despite a preserved neurological examination. An initial non-contrast

The utility of patient symptom questionnaires in CT of the lumbosacral spine.

BACKGROUND: Lower back pain and radiculopathy impose significant burdens on healthcare systems. Computed tomography (CT) scans are often used as an imaging modality for lower back symptomatology in the community. Accurately identifying relevant symptom sources using CT scans is often challenging due to the high incidence of incidental structural abnormalities. We examine the effectiveness of patient questionnaires in assisting reporting of CT scans of the lumbosacral spine. METHODS: In this retrospective audit, a questionnaire was provided to 250 consecutive patients undergoing lumbar spine C

Prevalence and predictors of silent vertebral compression fractures using DXA: a UK Biobank Imaging Study.

OBJECTIVES: Silent vertebral fractures are common but under-recognised. We aimed to estimate the prevalence in an asymptomatic population and to identify demographic and clinical predictors using the UK Biobank data. MATERIALS AND METHODS: DXA spine images from 2446 asymptomatic UK Biobank participants aged 40-79 years were analysed retrospectively. Vertebral fractures were defined as moderate or severe deformities (≥ 25% vertebral height loss). Fractures were identified using an artificial-intelligence-assisted six-point morphometric algorithm with manual correction and radiologist confirmat

Robot-assisted buccal graft uretero-pyeloplasty for pediatric recurrent UPJO: a multi-center case-series.

Redo pyeloplasty is technically more complex due to more significant scar tissue and risk for poor ureteral blood supply affecting outcomes. Use of buccal graft uretero-pyeloplasty was originally reported in the pediatric population back in 2017 as an alternative for complex reoperations. We hypothesize that in highly selected group of pediatric patients undergoing redo pyeloplasty, a buccal graft uretero-pyeloplasty would demonstrate efficacy. A total of 6 institutions identified patients undergoing a robot-assisted buccal uretero-pyeloplasty from 2014 to 2025. Demographic, pre-operative, int

How Do Mindfulness-Based and Cognitive Behavioral Therapies Help Adults With Opioid-Treated Chronic Low Back Pain? A Qualitative Study.

BACKGROUND: Chronic low back pain (CLBP) is common and often refractory to existing treatments, necessitating long-term opioid therapy, which has substantial risks. Mindfulness-based (MBT) and cognitive behavioral (CBT) therapies are safe and can be effective for CLBP. AIMS: To better understand how MBT and CBT may help improve opioid-treated CLBP: METHODS: This qualitative study involved a subgroup of participants with opioid-treated CLBP who completed a 12-month randomized clinical trial comparing the effects of MBT versus CBT. Participants who reported improvements at 12 months, compared t

Multiple Neurological Complications and Multidrug-Resistant Bacterial Infections During Prolonged Intensive Care for Undocumented Meningoencephalitis in Côte d'Ivoire: A Case Report.

Acute meningoencephalitis requiring intensive care may be complicated by severe neurological injury and nosocomial infections. Hydrocephalus is well recognized, whereas spinal cord ischemia, central diabetes insipidus, and flaccid tetraparesis are rarely reported. Their coexistence, together with successive infections involving multidrug-resistant organisms, presents substantial diagnostic and therapeutic challenges. A previously healthy 32-year-old man presented with neck and back pain following a road traffic accident, subsequently developing cerebellar signs, acute confusion, and meningeal

The power of words: the role of clinician communication and language in the management of women with chronic pelvic pain through a narrative review.

BACKGROUND AND OBJECTIVE: Chronic pelvic pain (CPP) is a common symptom amongst women and poses many challenges including physical, psychological, functional and economic. Clinician language is a key component of communication and has the ability to influence the patient experience and their engagement with healthcare. The importance of language has been reviewed in relation to chronic pain generally and in specific conditions like back pain and migraine, for its role in reducing stigmatisation and facilitating seeking care. This review discusses the current landscape of qualitative data perta

ZNF865 (BLST) a Novel Regulator of DNA Damage and Cell Senescence in Back Pain.

Senescence has been shown to contribute to the progression of aging-related diseases including degenerative disc disease (DDD). However, the mechanisms regulating senescence in the intervertebral disc (IVD) and other tissues/diseases remain poorly understood. Recently, ZNF865 (BLST) was identified as a previously uncharacterized zinc finger protein that regulates a wide array of genes related to protein processing, cell senescence, and DNA damage repair. Here, we show that ZNF865 expression decreases with age and pathology in human and mouse IVD samples. Utilizing CRISPR-guided gene modulation

Paraganglioma of the Lumbar Spine: A Case Report.

Paragangliomas are rare neuroendocrine tumors, with spinal involvement representing a small minority of cases. Lumbar spinal paragangliomas are especially uncommon and may present with nonspecific symptoms, often mimicking more common intradural tumors. We describe a 49-year-old female who presented with chronic low back and sacral pain, along with bilateral lower extremity symptoms. Lumbar MRI demonstrated an intrathecal, homogeneously enhancing mass at L3 causing canal stenosis. The patient underwent lumbar laminectomy with gross total resection of an intradural, extramedullary lesion. Histo

Real world evidence for glucosamine-chondroitin-calcium supplementation on joint pain.

BACKGROUND: Joint and bone diseases are major causes of morbidity worldwide. While glucosamine sulfate (GS) and chondroitin sulfate (CS) have demonstrated benefits in severe degenerative joint conditions, real-world evidence on the use patterns and reported outcomes of their combination with calcium (GCC) among individuals with mild-to-moderate joint discomfort remains limited. METHODS: This real-world study enrolled 9,090 adults who used GCC for ≥1 month and completed an online questionnaire; pre- and post-use symptom scores were analyzed among participants reporting joint symptoms or back p
