Paraganglioma of the Lumbar Spine: A Case Report.

Paragangliomas are rare neuroendocrine tumors, with spinal involvement representing a small minority of cases. Lumbar spinal paragangliomas are especially uncommon and may present with nonspecific symptoms, often mimicking more common intradural tumors. We describe a 49-year-old female who presented with chronic low back and sacral pain, along with bilateral lower extremity symptoms. Lumbar MRI demonstrated an intrathecal, homogeneously enhancing mass at L3 causing canal stenosis. The patient underwent lumbar laminectomy with gross total resection of an intradural, extramedullary lesion. Histo

Real world evidence for glucosamine-chondroitin-calcium supplementation on joint pain.

BACKGROUND: Joint and bone diseases are major causes of morbidity worldwide. While glucosamine sulfate (GS) and chondroitin sulfate (CS) have demonstrated benefits in severe degenerative joint conditions, real-world evidence on the use patterns and reported outcomes of their combination with calcium (GCC) among individuals with mild-to-moderate joint discomfort remains limited. METHODS: This real-world study enrolled 9,090 adults who used GCC for ≥1 month and completed an online questionnaire; pre- and post-use symptom scores were analyzed among participants reporting joint symptoms or back p

Conservative Care for L3/L4 Spondylolisthesis With Severe Stenosis in the Setting of L5 Sacralization.

Lumbosacral transitional vertebrae may alter load transfer and are associated with degenerative changes at spinal levels cranial to the transitional segment. However, the relationship between complete L5 sacralization and pathology at L3-L4 remains uncertain. We report a 69-year-old woman with chronic low-back pain, lower-extremity symptoms, and walking intolerance in the setting of complete bilateral L5 sacralization, L3-L4 grade I anterolisthesis, severe central canal stenosis, and Modic type 1 endplate changes. Global and regional reference curves for pain across the lifespan in 6.1 million individuals in 118 countries.

Pain is the leading cause of disability worldwide, yet no harmonized self-reported reference framework exists to characterize how its burden is distributed across the lifespan and world regions. In this study, we harmonized individual-level self-reported pain data from 6,125,459 participants across 902 population-based data sources in 118 countries to establish global reference trajectories of pain. We implemented these trajectories in an open-access benchmarking platform for positioning external datasets against global pain norms. Pain prevalence ranged from roughly 2% for facial pain to 40%

Radiation-free real-time registration update for image-guided multilevel open posterior lumbar fusion: in vivo validation.

Background: Surgical navigation becomes inaccurate when spinal alignment changes, requiring repeat scan such as O-arm CT to restore accuracy, but this introduces additional radiation exposure, workflow disruption, and delay. We developed and confirmed ex vivo validation of a novel hand-held system (HHS) that can update a preoperative CT (pCT) using intraoperative stereovision (iSV), leveraging a level-wise registration framework to account for intervertebral motion. Methods: iSV guidance was compared against Medtronic Stealth Station guidance for lumbar pedicle screw placement in 6 live Yucata

Digital Interactive Media for Chronic Low Back Pain Management: A Feasibility Study.

Chronic pain is common and is associated with significant costs and suffering. Therapeutic hypnosis has shown promise as an effective chronic pain treatment, but access to this intervention is extremely limited. Digital applications that offer therapeutic hypnosis, if found to be effective, could address this access problem. The primary aim of the current paper is to describe the results of two studies (Study 1 = individuals with low back pain; Study 2 = individuals with mixed chronic pain conditions) conducted to evaluate the feasibility of the digital hypnosis interactive media application f

Infiltration between the popliteal artery and the capsule of the knee (IPACK) vs. selective tibial nerve block for analgesia after anterior cruciate ligament reconstruction: a randomised controlled trial.

INTRODUCTION: Infiltration between the popliteal artery and the capsule of the knee (IPACK) and the selective tibial nerve block both provide analgesia for the posterior aspect of the knee after anterior cruciate ligament reconstruction. This randomised controlled triple-blinded trial tested the hypothesis that IPACK provides superior analgesia to a selective tibial nerve block after anterior cruciate ligament reconstruction. METHODS: Eighty patients having anterior cruciate ligament reconstruction under general anaesthesia with a femoral triangle block were allocated randomly to receive eith

Are pain intensity measures interchangeable? agreement among ODI, SRS-22, and COMI in chronic low back pain with and without scoliosis.

PURPOSE: Pain intensity in chronic low back pain (CLBP) is often assessed using pain items embedded in multidimensional patient-reported outcome measures (PROMs), but differences in scale format and recall period may limit score interchangeability. The purpose of this study was to evaluate the association and agreement among three commonly used pain intensity items derived from ODI, SRS-22, and COMI in adults with CLBP, with and without scoliosis. METHODS: Secondary cross-sectional analysis of an observational cohort. From 1,092 eligible subjects (552 scoliosis-associated LBP; 540 non-scolios

Reduced lumbar myofascial mobility in chronic low back pain quantified using MR elastography-based slip interface imaging.

OBJECTIVES: Restricted sliding at lumbar myofascial interfaces may contribute to chronic low back pain with myofascial pain features (CLBP-MP). We aimed to evaluate magnetic resonance elastography (MRE)-based slip interface imaging (SII) to quantify lumbar intermuscular myofascial mobility in healthy volunteers (HV), assessing repeatability, and comparing CLBP-MP patients with matched HV. MATERIALS AND METHODS: MRE/SII was performed in 20 CLBP-MP patients and 41 HV. SII-derived maximal normalized displacement discontinuity (Dnorm) was analyzed at the quadratus lumborum-erector spinae (QL-ES)

Primary chondrosarcoma arising from the L5 transverse process: A rare cause of chronic low back pain.

BACKGROUND: Chronic low back pain is a highly prevalent condition; however, primary spinal malignancies, though rare, carry critical clinical implications. Chondrosarcoma is a slow-growing malignant bone tumor, with spinal involvement accounting for <10% of cases. Tumors originating from the posterior vertebral elements, particularly the transverse process, are exceedingly uncommon and frequently result in delayed diagnosis due to their indolent clinical course. CASE DESCRIPTION: We present a 51-year-old male with a 5-year history of intermittent low back pain who developed a progressively no

Smartwatch-derived digital biomarkers distinguish episodic pain phenotypes in chronic low back pain.

UNLABELLED: Chronic low back pain affects over half a billion people globally and is the leading cause of disability worldwide, yet objective tools for characterizing how individual patients experience pain over time remain lacking. Patients with chronic low back pain exhibit distinct pain trajectory patterns: some experience relatively stable pain while others experience episodic fluctuations with flare-ups. These trajectories have been found to be clinically meaningful but are currently captured only through subjective self-report. Consumer smartwatches offer an opportunity for passive, cont

Work-related movement and lumbar spine direction: a systematic review of lifting, posture, and pain risk.

Low back pain (LBP) is a prevalent musculoskeletal condition of occupational and socioeconomic importance. The main biomechanical exposures related to LBP among workers include repetitive lifting, non-neutral trunk postures, and long-duration static trunk positions. This systematic review aimed to evaluate the role of lumbar spine movement direction during work-related lifting and posture in relation to spinal loading and the risk of LBP. PubMed, Scopus, and Web of Science were searched for articles published in January 2015 - June 2025. Eligible studies included adult workers performing occup

Ultrasound-Guided Distal Thoracolumbar Fascia Hydrodissection at the Gluteal Junction: A Technical Report With Cadaveric Assessment of Compartmental Spread and Gross Neural Sparing.

Chronic low back pain with radiating lower-extremity symptoms in the absence of clear radiculopathy or major central canal stenosis remains a common diagnostic and therapeutic challenge. In patients with radiating low back pain without a convincing compressive correlate, posterior myofascial structures may be considered among several potential contributors; however, the present report evaluates the anatomical feasibility of a distal thoracolumbar fascia (TLF) hydrodissection approach rather than a clinical pain generator. We developed an ultrasound-guided hydrodissection technique targeting th

Thoracic Spontaneous Spinal Epidural Hematoma in a Young Adult: A Case Report.

Spontaneous spinal epidural hematoma (SSEH) is an uncommon condition that may present with acute back pain and neurological deficits. Prompt diagnosis and timely intervention are important to optimise neurological recovery. A 21-year-old female presented with a 5-day history of back pain associated with right lower limb weakness, gait instability, and bilateral foot numbness. Clinical examination demonstrated reduced muscle power (4/5) in the right lower limb and an ataxic gait. Magnetic resonance imaging (MRI) revealed a posterior epidural hematoma extending from T6 to T8 with significant spi
