Current applications of transauricular vagus nerve stimulation in orthopedics: a narrative review of the literature.

OBJECTIVES: Transcutaneous auricular vagus nerve stimulation (taVNS) is a non-invasive neuromodulation technique with emerging applications in various medical fields. While its role has been explored in neurological, psychiatric, and inflammatory conditions, its potential in orthopedics has not been reviewed in depth. Our objective was to synthesize current evidence on the use of taVNS in orthopedic conditions. METHODS: A PubMed search was conducted for articles published between January 2010 and July 2025 using the terms "transcutaneous" AND "vagus nerve" AND "stimulation." Seventeen eligibl

Hemorrhagic lumbar synovial cyst mimicking a neoplastic process: illustrative case.

BACKGROUND: Synovial cysts with extensive hemorrhage, marked mass effect, and tumor-like appearance are rare and may mimic neoplastic processes. OBSERVATIONS: A 56-year-old man presented with progressive mechanical low back pain, neurogenic claudication, gait instability, and bilateral weakness. MRI revealed a 2.0-cm lesion arising from the right L3 facet joint, causing severe central canal and right foraminal stenosis with displacement of the cauda equina. Imaging features suggested pigmented villonodular synovitis. Intraoperatively, however, the lesion appeared fleshy, partially encapsulate

Ozone Therapy in the Treatment of Low Back Pain: A Systematic Review of Its Evolution, Clinical Application, and Efficacy.

Low back pain is a major cause of disability worldwide. Ozone therapy has emerged as a therapeutic alternative; however, its efficacy and safety remain controversial. A systematic review was conducted in accordance with PRISMA guidelines using PubMed, Scopus, Embase, Ovid, EBSCOhost, and Google Scholar (2013-2023). Randomized clinical trials, observational studies, systematic reviews, and meta-analyses were included. Study selection was performed using Rayyan, and methodological quality was assessed with STROBE, CONSORT, and QUADAS.A total of 28 studies were included. Although some reported cl

The utility of patient symptom questionnaires in CT of the lumbosacral spine.

BACKGROUND: Lower back pain and radiculopathy impose significant burdens on healthcare systems. Computed tomography (CT) scans are often used as an imaging modality for lower back symptomatology in the community. Accurately identifying relevant symptom sources using CT scans is often challenging due to the high incidence of incidental structural abnormalities. We examine the effectiveness of patient questionnaires in assisting reporting of CT scans of the lumbosacral spine. METHODS: In this retrospective audit, a questionnaire was provided to 250 consecutive patients undergoing lumbar spine C

Impact of pain severity on each EQ-5D-5L dimension of quality of life in patients with neuropathic pain associated with chronic low back or cervical pain: an ad hoc analysis.

OBJECTIVES: Clinical guidelines for patients with neuropathic pain (NeP) recommend setting achievable treatment goals relevant to their daily lives. We therefore evaluated the associations of the EuroQol 5-Dimensions 5-Levels (EQ-5D-5L) with reduced quality of life (QoL) in patients with chronic low back pain with NeP (CLBP-NeP) or chronic cervical pain with NeP (CCP-NeP) to help inform the setting of such treatment goals. METHODS: Secondary data were analyzed from two observational studies of patients with CLBP-NeP or CCP-NeP. The associations of each EQ-5D-5L dimension (mobility, self-care,

Severe Lumbar Spinal Canal Stenosis Due to Co-occurrence of Ossification of the Ligamentum Flavum and Posterior Longitudinal Ligament.

This report describes a rare and challenging case of severe lumbar spinal canal stenosis resulting from extensive, concurrent lumbar ossification of the posterior longitudinal ligament (OPLL) and ossification of the ligamentum flavum (OLF). A 44-year-old female patient who initially presented with numbness and gait instability due to thoracic spinal cord compression was successfully managed by thoracic decompression and fusion. However, she subsequently developed lower back pain, buttock pain, and numbness along both legs due to severe multilevel lumbar stenosis from L2 to L5. Surgical managem

The LL-TK index as a regional marker of sagittal balance and disability in adults with chronic low back pain.

PURPOSE: Population aging has increased both the prevalence and clinical impact of adult spinal deformity (ASD). Sagittal and regional parameters (e.g., SVA, PI-LL, LL-TK) guide assessment and treatment planning, but it remains unclear which indices are most closely associated with sagittal imbalance and functional disability. This study therefore evaluated the associations between regional and global spinopelvic parameters and clinical status in an adult outpatient cohort with chronic low back pain, and compared the ability of the LL-TK and PI-LL indices to discriminate concurrent sagittal im

Parity, menopausal timing, and spinopelvic mismatch: a moderated mediation analysis of low back pain in postmenopausal women.

PURPOSE: To evaluate the association of parity and menopausal timing with spinopelvic alignment in postmenopausal women and whether pelvic incidence-lumbar lordosis (PI-LL) difference statistically mediates the association between parity and low back pain. METHODS: This single-centre cross-sectional study included 239 postmenopausal women aged ≥ 45 years. Standardised standing lumbosacral radiographs were used to measure pelvic incidence, sacral slope, pelvic tilt, lumbar lordosis, and PI-LL difference. PI-LL mismatch was defined as ≥ 10°. Pain severity was assessed using a visual analogue sc

Effects of percutaneous kyphoplasty with concomitant postural reduction in the treatment of acute osteoporotic vertebral compression fractures.

OBJECTIVE: To investigate the effects of postural reduction during balloon expansion in percutaneous kyphoplasty (PKP) for patients with acute osteoporotic vertebral compression fractures (OVCF). METHODS: From January 2022 to December 2023, 75 patients with acute OVCF underwent PKP surgery at our institution and were followed up for at least 12 months. Patients were divided into Group A and Group B based on whether they underwent postural reduction during balloon expansion. Clinical evaluation included Oswestry Disability Index (ODI) and Visual Analog Scale (VAS) scores for back pain; Radiogr

Prevalence and predictors of silent vertebral compression fractures using DXA: a UK Biobank Imaging Study.

OBJECTIVES: Silent vertebral fractures are common but under-recognised. We aimed to estimate the prevalence in an asymptomatic population and to identify demographic and clinical predictors using the UK Biobank data. MATERIALS AND METHODS: DXA spine images from 2446 asymptomatic UK Biobank participants aged 40-79 years were analysed retrospectively. Vertebral fractures were defined as moderate or severe deformities (≥ 25% vertebral height loss). Fractures were identified using an artificial-intelligence-assisted six-point morphometric algorithm with manual correction and radiologist confirmat

Assessment and characterization of back pain and biopsychosocial influences in lower limb amputation: scoping review.

Back pain affects over half of individuals with lower limb amputation, yet research findings on associated factors remain fragmented. In view of the evolution of pain science in the general population, this fragmentation likely stems from significant variability in the identification, characterization, and classification of pain, as well as inconsistent assessment of associated biopsychosocial factors. This scoping review aims to examine how back pain is currently identified, assessed, classified, and characterized in people with lower-limb amputation, to determine the gaps and inconsistencies

Reimagining UBE: Midline Biportal Endoscopic Approach with "Under the Cave" Decompression Technique for Lumbar Canal Stenosis-A Technical Note and Clinical Pearls.

The purpose of this study was to describe the technique for midline biportal endoscopic decompression (MBED) for lumbar canal stenosis by a new "under the cave" decompression technique: a paradigm shift in biportal endoscopic decompression. Laminectomy with/without fusion and facet-preserving microforaminotomy has been performed as conventional surgical treatments for lumbar canal stenosis (LCS). Recently, endoscopic spinal surgery has been introduced as a minimally invasive therapeutic modality of LCS by several authors. Here, we describe the surgical steps and technical pearls of MBED with "

Robot-assisted buccal graft uretero-pyeloplasty for pediatric recurrent UPJO: a multi-center case-series.

Redo pyeloplasty is technically more complex due to more significant scar tissue and risk for poor ureteral blood supply affecting outcomes. Use of buccal graft uretero-pyeloplasty was originally reported in the pediatric population back in 2017 as an alternative for complex reoperations. We hypothesize that in highly selected group of pediatric patients undergoing redo pyeloplasty, a buccal graft uretero-pyeloplasty would demonstrate efficacy. A total of 6 institutions identified patients undergoing a robot-assisted buccal uretero-pyeloplasty from 2014 to 2025. Demographic, pre-operative, int

Emotional imagery alters symptom perception but not postural or respiratory behavior in individuals with chronic non-specific low back pain.

Chronic non-specific low back pain (CNSLBP) often coincides with alterations in postural behavior and respiration which contribute to pain. Increasing evidence suggests that emotions are closely related to postural behavior and respiration. Thereby, emotions can affect symptom perception processes in CNSLBP. This quasi-experimental study examined how emotions affect symptom perception, postural behavior, and respiration in individuals with (n= 40) and without (n= 40) CNSLBP using script-driven emotional imagery. Participants imagined three auditory scripts describing states of relaxation, host

How Do Mindfulness-Based and Cognitive Behavioral Therapies Help Adults With Opioid-Treated Chronic Low Back Pain? A Qualitative Study.

BACKGROUND: Chronic low back pain (CLBP) is common and often refractory to existing treatments, necessitating long-term opioid therapy, which has substantial risks. Mindfulness-based (MBT) and cognitive behavioral (CBT) therapies are safe and can be effective for CLBP. AIMS: To better understand how MBT and CBT may help improve opioid-treated CLBP: METHODS: This qualitative study involved a subgroup of participants with opioid-treated CLBP who completed a 12-month randomized clinical trial comparing the effects of MBT versus CBT. Participants who reported improvements at 12 months, compared t

Stable disease after multimodal treatment of SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum: a case report.

We report a clinically rare SMARCA4-deficient undifferentiated tumor concurrently involving the stomach and mediastinum. A 54-year-old man presented with a 1-month history of left upper abdominal pain radiating to the anterior chest and back. Contrast-enhanced computed tomography of the chest and abdomen showed thickening and abnormal enhancement of the gastric wall along the greater curvature, multiple enlarged lymph nodes along the lesser curvature, and a posterior inferior mediastinal mass. Biopsies of both the gastric body lesion and the mediastinal mass revealed a poorly differentiated/un

Navigation-Assisted Minimally Invasive Direct Pars Repair for Lumbar Spondylolysis.

UNLABELLED: Spondylolysis is a common cause of mechanical low back pain in adolescent athletes. While nonoperative management is frequently successful, a subset of patients fail conservative treatment and require surgical intervention. Direct repair of the pars defect offers a motion-preserving alternative to fusion in appropriately selected patients, but traditional open and fluoroscopy-guided minimally invasive techniques are limited by soft-tissue morbidity and variability in screw placement accuracy, and detailed technical descriptions of navigation-assisted approaches remain limited in th

Spontaneous Spinal Cord Infarction Presenting as Anterior Spinal Artery Syndrome: A Case Report.

Spinal cord infarction is a rare cause of acute myelopathy and may be difficult to recognize when it occurs outside a traumatic or perioperative setting. We report a 68-year-old man with hypertension, type 2 diabetes mellitus, dyslipidemia, and active smoking who presented with sudden severe low back pain followed within one hour by rapidly progressive bilateral lower-limb weakness and sensory loss. Neurologic examination showed severe paraparesis, absent lower-limb reflexes, impaired pain sensation below T10, and relative preservation of proprioception. Aortic computed tomography angiography
