Socio-Demographic Inequalities in the Burden of Low Back Pain Across Development Strata: Sex-Specific, Age-Standardized Trends in Prevalence, Incidence and Disability from the Global Burden of Disease Study 2023, 2003-2023.

BACKGROUND: Low back pain (LBP) is a leading contributor to non-fatal disability and rehabilitation needs worldwide. Monitoring trends across sociodemographic strata is important for understanding whether the disability burden is changing equitably across health systems. PURPOSE: To examine temporal trends and socio-demographic inequalities in the burden of LBP across Socio-demographic Index (SDI) groups from 2003 to 2023 using Global Burden of Disease Study 2023 estimates. METHODS: GBD 2023 estimates for LBP prevalence, incidence and years lived with disability (YLDs) were analysed across H

Cationized gelatin nanosphere-mediated release-controlled RNA interference targeting mTORC1 protects against intervertebral disc degeneration.

Intervertebral disc degeneration is a major cause of chronic low back pain, but effective disease-modifying therapies remain unavailable. Sustained and localized RNA interference (RNAi) offers a promising therapeutic approach; however, its application to the disc is limited by poor local retention and delivery-related toxicity. Here, we developed a release-controlled RNAi platform using biodegradable cationized gelatin nanospheres (cGNS) to deliver small interfering RNA (siRNA) targeting Rptor, which encodes RPTOR, a key component of the mechanistic target of rapamycin complex 1 (mTORC1). cGNS

Applications of Traditional and Digital Anthropometric Measures in Orthopaedics and Rehabilitation: A Systematic Review.

Anthropometry offers noninvasive measurements of body size, proportions, and composition that can inform musculoskeletal assessment and rehabilitation. Traditional anthropometric measures continue to be widely used owing to their accessibility, whereas digital technologies provide further approaches for objective body and movement assessment. However, evidence on the applications and measurement properties of traditional and digital anthropometric measures across orthopaedic and rehabilitation settings remains inconsistent. This systematic review assessed the measurement methods, clinical appl

Postvoid Residual Urine as a Clue to the Diagnosis of Acute Bacterial Prostatitis: A Case Report.

Acute bacterial prostatitis (ABP) typically presents with fever and urinary symptoms; however, its diagnosis can be challenging in patients who lack characteristic findings such as perineal pain or prostatic tenderness. Although postvoid residual (PVR) urine may be associated with voiding dysfunction in ABP, its potential role as a diagnostic clue has received little attention. A 44-year-old man presented with a 2-day history of fever ranging from 38 to 39°C, irritative urinary symptoms, back pain, and difficulty initiating urination. At presentation, the patient was afebrile and had no perine

Tuberculous spinal epidural abscess: a cautionary case report of permanent paraplegia due to delayed diagnosis and management.

BACKGROUND: Spinal epidural abscess caused by tuberculosis is a rare but serious complication of spinal tuberculosis. Its atypical presentation predisposes to diagnostic delay and a poor prognosis. This case not only documents the clinical and radiologic evolution of tuberculous spinal epidural abscess but also suggests that delays in diagnosis and treatment may jointly contribute to serious neurological outcomes. CASE PRESENTATION: A 39-year-old man initially underwent acupotomy for recurrent low back pain with recent exacerbation. Subsequent imaging revealed a paraspinal abscess within the

Neuromuscular effects of warm needle acupuncture versus mild moxibustion in knee osteoarthritis: An active-controlled randomized trial.

BackgroundKnee osteoarthritis (KOA) is associated with neuromuscular dysfunction and impaired quadriceps activation. This study compared the short-term neuromuscular effects of warm needle acupuncture (WNA) and mild moxibustion (MM).MethodsIn this single-center, assessor- and statistician-blinded randomized controlled trial, 60 patients with KOA were randomized 1:1 to WNA or MM for 4 weeks. Thirty healthy participants provided an exploratory physiological reference at baseline. Co-primary outcomes were isokinetic quadriceps peak torque (PT) and average power (AP). Secondary outcomes included V

Reprogramming NOX4-associated ferro-aging with an iron-chelating senescent-cell-targeted therapy for intervertebral disc degeneration.

Intervertebral disc degeneration (IVDD) has been a major contributor to chronic low back pain, yet effective disease-modifying therapies remain limited. Here, we identified a NOX4-associated ferro-aging program linking iron overload, oxidative stress, lipid peroxidation, and cellular senescence in degenerative NPCs. Clinical imaging, transcriptomic analyses, human disc specimens, iron-overloaded NPCs, and naturally aged rats consistently revealed ferro-aging-associated changes during IVDD. Mechanistically, gain- and loss-of-function, genetic rescue, and temporal analyses positioned NOX4 as an

A Case of Acute Spontaneous Spinal Epidural Hematoma Caused by Repeated Bleeding Thoracic Radiculomeningeal Artery.

Spontaneous spinal epidural hematoma (SSEH) presents with back pain followed by myelopathy. Venous plexus disruption is common, and arterial bleeding is extremely rare. Here, we report a case of acute spontaneous spinal epidural hematoma that showed spot sign on computed tomographic angiography and was treated. A 48-year-old woman with no medical history experienced unprovoked back pain and visited a local hospital, where she was placed under observation. Numbness gradually developed in both legs, and the patient visited our emergency room 5 hours after the onset of symptoms. Magnetic resonanc

Public perception of osteopathic manipulative medicine as an effective treatment for musculoskeletal disorders.

CONTEXT: Musculoskeletal disorders (MSDs) are among the most prevalent causes of disability globally, contributing significantly to diminished quality of life and rising healthcare costs. While pharmacological treatments and physical therapy remain standard, alternative modalities such as osteopathic manipulative medicine (OMM) have emerged as promising nonpharmacologic options. However, public awareness of OMM remains limited, and misconceptions persist, particularly regarding its distinction from chiropractic care. This study aimed to assess public perception of OMM and its effectiveness in

Efficacy and safety of bone marrow aspirate and bone marrow aspirate concentrate in orthopedic conditions: a systematic review of randomized controlled trials.

INTRODUCTION: Bone marrow aspirate (BMA) and bone marrow aspirate concentrate (BMAC) have emerged as autologous orthobiologic strategies aimed at modulating inflammation and promoting tissue repair in orthopedic conditions. However, evidence from randomized controlled trials (RCTs) remains heterogeneous, and their clinical role relative to established treatments is still debated. This systematic review aimed to evaluate the efficacy, structural effects, and safety of BMA and BMAC in adults with orthopedic conditions based exclusively on RCT evidence. METHODS: A systematic review was conducted

Pyogenic Sacroiliitis in Older Adults Presenting With Massive Retroperitoneal Abscess: A Case Report.

Pyogenic sacroiliitis (PSI) is an uncommon form of septic arthritis and is frequently diagnosed late in older adults because initial symptoms are nonspecific and imaging findings may be misattributed to other sources of infection. We report two older patients with PSI complicated by massive retroperitoneal and gluteal abscesses, highlighting the importance of early recognition and, when indicated, timely surgical source control. Case 1 was an 83-year-old woman in whom a conspicuous retroperitoneal abscess and abnormal urinalysis led to suspicion of an intra-abdominal or urinary tract source, d

Mpox outbreak associated with multiple sexual partnerships and HIV coinfection in Masaka District, Uganda, February-April 2025.

OBJECTIVES: Uganda experienced a substantial increase in mpox cases in early 2025, with notable clustering in central regions. The country reported approximately 4800 confirmed monkeypox (mpox) cases and 37 deaths, with the Masaka district contributing approximately 3% of the national burden. We investigated an outbreak in Masaka District to describe its epidemiology and identify associated risk factors. METHODS: We conducted an outbreak investigation with a descriptive epidemiological analysis and an unmatched case-control study. A suspected case was defined as onset of rash with fever and a

Differences in Clinical Presentation of Chronic Pelvic Pain Caused by Iliac Vein Obstruction Impacts Referral Pathway, Time to Diagnosis, and Post-stenting Pain Resolution.

OBJECTIVE: Iliac venous stenting improves chronic pelvic pain (CPP) caused by iliac vein obstruction (IVO). Signs and symptoms vary, but this pain is reproducible and easily identifiable. However, patients can experience significant delays in diagnosis and treatment. This study aimed to determine associations between the referral history, clinical presentation, pain characteristics, and stenting outcomes among women with CPP due to IVO. METHODS: This was a retrospective analysis of prospectively collected data of women who were referred to a vascular surgeon with signs and symptoms of IVO, in

Effects of Partner Distress and Controlling Responses on Back Pain in Daily Life.

Chronic back pain is embedded within close relationships, yet little is known about how partners' emotional and behavioral responses shape patients' daily pain experiences. This study examined whether partners' distress predicts their attempts to control (increase or decrease) the activity level of individuals with chronic back pain (ICBPs), and whether these responses are associated with worse subsequent pain outcomes the following day. One hundred forty-seven couples in which one partner had chronic back pain completed twice-daily electronic diary assessments for 30 days. The sample was olde

Association between frailty and multisite chronic pain in middle-aged and older adults: A population-based cross-sectional study with mediation analysis.

The corrected modified frailty index (CMFI), based on the accumulation of multidimensional health impairments, provides a more comprehensive reflection of an individual's overall frailty. Although chronic pain is associated with aging, its relationship with CMFI, in population-based cohorts remains unclear. This cross-sectional study included 5821 participants aged ≥45 years from the 2011 baseline wave of the China Health and Retirement Longitudinal Study. Chronic pain, including low back pain, multisite pain, and the number of pain sites, was assessed via self-reported questionnaires. Frailty

The Alkaline Trap: Severe Calcium-Alkali Syndrome Secondary to Chronic Calcium Carbonate Ingestion.

Calcium-alkali syndrome is an increasingly recognised cause of parathyroid hormone (PTH)-independent hypercalcaemia resulting from excessive calcium and absorbable alkali ingestion, commonly through calcium-containing antacids or supplements. Its clinical presentation may mimic neurological or malignant disease, potentially leading to extensive investigation and delayed diagnosis. A 64-year-old woman presented following a fall with paraparesis and lower back pain. Investigations demonstrated severe hypercalcaemia (adjusted calcium 4.83 mmol/L), suppressed PTH of 1.4 pmol/L, metabolic alkalosis

Social Support Modifies the Pain-Disability Relationship in Degenerative Spine Disease: Evidence From the PROGRES Study.

BACKGROUND: Low back pain from degenerative spine disease is the main contributor to years lived with disability globally. Although social support is a multifaceted construct with both health-promoting and potentially deleterious effects on chronic pain, whether perceived social support modifies the pain-disability relationship in lumbar degenerative spine disease remains unclear. We hypothesised that social support level would modify the pain-disability association, with distinct predictor patterns of disability emerging for low- versus high-support patients. METHODS: In this cross-sectional

Third lumbar transverse process syndrome with acute abdominal pain as the main clinical manifestation: A case report.

RATIONALE: Third lumbar transverse process syndrome is an uncommon musculoskeletal cause of referred abdominal pain. Because acute abdominal pain is appropriately evaluated first for visceral, vascular, urinary, and surgical emergencies, a spinal or paraspinal pain generator may be overlooked. PATIENT CONCERNS: A 66-year-old man presented with persistent left lower-quadrant abdominal pain radiating to the left lower back. Baseline pain intensity was 7/10 on a VAS, and the pain was aggravated by postural change, standing, and walking. DIAGNOSES: Serial laboratory tests, endoscopy, ultrasonogr
