OBJECTIVE: The presence of decremental responses following repetitive nerve stimulation (RNS) in amyotrophic lateral sclerosis (ALS) is well established. However, in spinal and bulbar muscular atrophy (SBMA), a rare X-linked recessive lower motor neuron disease, the incidence and distribution of decremental responses across different muscles have not been thoroughly investigated.
METHODS: Patients with SBMA were retrospectively identified in our database. RNS at a frequency of 3 Hz was performed on five muscles: the abductor pollicis brevis (APB), abductor digiti minimi (ADM), upper trapezius

BACKGROUND: Hirayama disease is a rare, self-limiting cervical myelopathy characterized by juvenile-onset distal upper-limb unilateral or asymmetrical weakness and wasting. Bilateral and nearly symmetrical involvement is rare and may mimic motor neuron disease. The diagnosis is typically established based on dynamic flexion magnetic resonance imaging (MRI).
CASE DESCRIPTION: A 23-year-old male presented with a 6-year history of progressive distal upper-limb weakness and atrophy that initially involved the right side and later the left. On examination, he demonstrated bilateral distal upper ex

BACKGROUND: Fibroblast growth factor 23 (FGF23)-related hypophosphatemic rickets/osteomalacia arises from excessive FGF23 activity, with X-linked hypophosphatemia (XLH) and tumor-induced osteomalacia (TIO) as the most common congenital and acquired forms, respectively. However, in a substantial subset of patients with acquired FGF23-related hypophosphatemic osteomalacia, phosphaturic mesenchymal tumors (PMTs) remain undetectable despite extensive imaging studies. A recent study identified autoantibodies against PHEX, the gene responsible for XLH, in 5 of 13 patients with acquired FGF23-related

Autosomal recessive hypophosphatemic rickets type 1 (ARHR1) is a rare, inherited cause of fibroblast growth factor 23 (FGF23)-mediated renal phosphate-wasting that results from homozygous inactivating pathogenic variants in the dentin matrix acidic phosphoprotein-1 (DMP1) gene. A variable clinical spectrum has been described in this condition, including features of rickets (in children), hyperostotic osteomalacia (in children and adults), bone pain, short stature, lower limb deformities, recurrent dental abscesses, and hearing defects. The purpose of this report is to describe the clinical evo

CONTEXT: Cutaneous skeletal hypophosphatemia syndrome (CSHS) is an ultrarare disorder defined by epidermal and/or melanocytic nevi, mosaic skeletal dysplasia, and FGF23-mediated hypophosphatemia. As in other FGF23-mediated hypophosphatemia conditions, individuals with CSHS have renal phosphate wasting and inappropriately normal or frankly low 1,25-dihydroxyvitamin D levels with resultant hypophosphatemia leading to rickets and osteomalacia. Conventional therapy for FGF23-mediated hypophosphatemia consists of multiple daily doses of oral phosphate and active vitamin D.

Background and Objectives: Chronic migraine (CM) remains a major cause of neurological disability, and a subset of patients remains highly treatment-resistant despite previous preventive and interventional treatments. Stellate ganglion block (SGB) has been proposed as a sympathetic neuromodulatory intervention, but evidence in this population remains limited. This study evaluated 6-month clinical outcomes and tolerability after a structured ultrasound-guided SGB protocol. Materials and Methods: This prospective, single-center, uncontrolled, single-group interventional study enrolled 42 adults

Ganglioneuroma (GN) is a rare benign tumor from neural crest tissue, composed of mature Schwann cells, ganglion cells, and nerve fibers. These tumors most often arise in the retroperitoneal region (52%) and posterior mediastinum (39%), while extra-adrenal lesions account for 51% of abdominal GN cases. This paper presents a 66-year-old female with paroxysmal atrial fibrillation found to have a left retroperitoneal mass on imaging. A 9.5-cm tumor was fully resected using robot-assisted laparoscopy. Pathology confirmed GN with ectopic inferior vena cava (IVC) invasion. This case highlights the ne

BACKGROUND: SYNE1 encodes nesprin-1, a nuclear envelope protein involved in cytoskeletal linkage, nuclear positioning, and neuromuscular integrity. Biallelic SYNE1 variants cause a broad spectrum ranging from cerebellar ataxia to arthrogryposis multiplex congenita and motor neuron disease-like phenotypes. Distinguishing these entities can be difficult when subtle distal contractures coexist with neurogenic electrophysiological findings.
CASE PRESENTATION: A 22-year-old woman born to consanguineous parents presented with bilateral intrinsic hand muscle wasting, impaired fine motor performance,

Iron is an essential trace element that participates in multiple physiological processes, including oxygen transport, electron transfer, DNA synthesis, and red blood cell production. Iron loss is particularly severe among athletes, so maintaining iron homeostasis is crucial for sports nutrition and health. Excess iron, iron deficiency, and ferroptosis can lead to muscle disorders and health issues, including sarcopenia, muscular atrophy, myocardial fibrosis, skeletal muscle injury, cardiovascular disease, and metabolic disorders. Maintaining iron homeostasis within physiological limits is esse

Type I spiral ganglion neurons, which play a major role in hearing by conveying electrical sound signals to the brain, are divided into electrophysiological and molecular subtypes. The differences in vulnerability among the subtypes reported in rodents are of pathophysiological importance for age-related hearing loss. However, the changes in spiral ganglion neurons in primates and humans have not been established. In this study, we investigated the age-related changes in the spiral ganglion neurons of the common marmoset, a primate model animal. Here, we show that the overall number of spiral

Fibroblast growth factor 23 (FGF23) was identified as an etiological factor for hypophosphatemic rickets/osteomalacia approximately a quarter-century ago. Since then, FGF23 has been shown to be a bone-derived hormone that regulates phosphate and vitamin D metabolism by binding to the α-KLOTHO-FGF receptor complex. Dysregulation of FGF23 activity results in hypophosphatemic and hyperphosphatemic disorders. Medical therapy targeting FGF23 is now available for hypophosphatemic diseases caused by FGF23 excess. Furthermore, elevated FGF23 levels have been associated with various adverse clinical ou

Allergic contact dermatitis (ACD) is a skin inflammation and immune disease, accompanied by persistent itching. These symptoms seriously affect the quality of life of patients. Trigeminal Ganglia (TG) play a key role in the facial sensory conduction and inflammatory regulation of ACD. It is reported that P2X4 receptor expressed in trigeminal ganglion neurons. Preliminary exploratory experiment showed the upregulated expression of P2X4 in TG during the pathophysiology of ACD. This study aims to investigate whether the P2X4 receptor in TG mediates the pathological changes of itching in ACD mouse

Chronic pruritus is a highly prevalent symptom that markedly impairs the quality of life in both humans and animals. Although pharmacological therapies, including Janus kinase (JAK) inhibitors, are widely used for itch management, long-term treatment is associated with adverse effects and increased patient burden. Therefore, the development of non-pharmacological antipruritic approaches is highly desired. In this study, we investigated the antipruritic effects of nitrogen gas-derived cold atmospheric plasma (N2-CAP) and explored its underlying mechanisms using behavioral and calcium imaging an

Cystinosis-associated metabolic bone disease (CMBD) is a severe complication in patients with infantile nephropathic cystinosis, which is a rare inherited lysosomal storage disease due to pathogenic variants in the CTNS gene that results in an accumulation of cystine in all organs. The kidneys are the organs primarily affected, resulting in renal Fanconi syndrome at around the age of 6-12 months. Without adequate treatment with the cystine-depleting agent cysteamine, this is followed by progressive chronic kidney disease (CKD) and kidney failure in the second decade of life. Patients usually p
