Beyond Sarcopenia: A Systematic Review and Meta-Analysis on Muscle Outcomes After Bariatric Surgery.

BACKGROUND: Metabolic and bariatric surgery (MBS) produces substantial and sustained weight loss, but concerns remain about skeletal muscle loss and sarcopenia. METHODS: We conducted a systematic review of longitudinal and cross-sectional studies evaluating changes in muscle quantity, strength, and physical performance following MBS without interventions to preserve muscle. Searches were performed in PubMed, EMBASE, CINAHL, Web of Science, and Cochrane Central, and the review was registered with PROSPERO (CRD42022354153). Study quality was assessed using the Evidence Project Risk of Bias Tool

Systematic Review and Meta-Analysis of Herbal and Traditional Medicine-Derived Interventions for Sarcopenia and Exploration of Core Herbs.

INTRODUCTION: Sarcopenia is a medical condition predominantly associated with the aging process. Currently, the most effective approach for sarcopenia involves a combination of physical activity and nutritional supplementation. However, older individuals may face challenges engaging in physical activity due to inconvenience or limited access to consistent nutritional resources. Previous studies suggest that herbal and traditional medicine-derived interventions can enhance muscle strength in sarcopenia patients; however, its effectiveness remains unclear. This study aimed to evaluate the effica

Inhibition of Ornithine Decarboxylase 1 Mitigates Denervation-Induced Muscle Atrophy by Suppressing Proteolysis and Preserving Muscle Stem Cell Homeostasis.

Neural innervation is of vital importance for muscle mass and function, and denervation induces progressive muscle atrophy, which lacks effective treatments. Polyamine metabolism is reported to be innervation responsive and involved in denervation-induced muscle atrophy, yet the effects of polyamine in denervated muscle atrophy remain unknown. In this study, using a sciatic nerve transection model, we observed progressive increases in putrescine and spermidine, alongside induction of polyamine metabolism enzymes, coincident with the early rapid atrophy phase post denervation. Inspiratory Muscle Strength and Cardiorespiratory Fitness in Sarcopenic and Dynapenic Older Adults: A Cross-Sectional Study.

BACKGROUND AND PURPOSE: While sarcopenia and dynapenia, common in older adult, are known to affect general motor function, their specific effects on inspiratory muscle strength and cardiorespiratory fitness have not been adequately investigated. Therefore, the aim of our study was to compare inspiratory muscle strength and cardiovascular fitness levels in sarcopenic, dynapenic, and older adults without sarcopenia or dynapenia. METHODS: The study included 75 community-dwelling older adults aged 65-85. Individuals were divided into 3 groups: dynapenic, sarcopenic, and control groups (older adul

Relationship between polypharmacy, sarcopenia and drug classes in hospitalized older patients.

BACKGROUND: Sarcopenia and polypharmacy are common geriatric syndromes associated with adverse outcomes in older adults. Although previous studies have suggested a potential relationship between these conditions, the independent nature of this association remains unclear, particularly among hospitalized populations. Certain medication classes may also exert muscle-protective effects that could influence this relationship. PURPOSE: To examine whether polypharmacy is independently associated with sarcopenia in hospitalized older adults and to evaluate the relationship between specific medicatio

Exogenous salicylic acid promotes the accumulation of volatile sulfur compounds and anti-muscle atrophy activity in garlic sprouts.

BACKGROUND: Despite the wide use of garlic as a spice and medicinal plant, limited information is available on garlic sprouts (GSs), which are rich in bioactive sulfur compounds. Sprouting is an effective process to improve nutritional quality and functional properties by activating metabolic pathways and enhancing secondary metabolite biosynthesis. In addition, elicitation has been applied to stimulate the accumulation of bioactive compounds in sprouts. Among various elicitors, salicylic acid (SA) is a key plant signaling molecule involved in stress responses and secondary metabolism. However

Prognostic value of sarcopenia markers in geriatric patients presenting to the emergency department: a prospective study.

BACKGROUND: Early risk stratification in geriatric patients presenting to the emergency department remains challenging. Sarcopenia has emerged as a potential marker of reduced physiological reserve and adverse clinical outcomes. This study aimed to evaluate the prognostic value of quadriceps muscle thickness and the paraspinal muscle index for predicting short-term mortality in geriatric emergency department patients and to examine their association with established frailty scales. METHODS: In this single-center prospective observational study, consecutive patients aged ≥ 65 years presenting

Progression of sarcopenia among patients with pancreatic ductal adenocarcinoma receiving neoadjuvant therapy and the impact of pancreatic enzyme replacement therapy.

BACKGROUND: Neoadjuvant therapy (NAT) is increasingly used among patients with pancreatic ductal adenocarcinoma (PDAC). Patients with PDAC commonly present with sarcopenia, which may progress during the period of NAT and subsequently impact treatment outcomes. However, data assessing trends in sarcopenia are currently lacking. As such, this audit sought to quantify changes in body composition during NAT, and to assess whether these correlated with patient outcomes or pancreatic enzyme replacement therapy (PERT) usage. METHODS: Computed tomography (CT) scans performed pre- and post-NAT were an

A comparative evaluation of the GINI index and the modified Glasgow prognostic score in predicting postoperative sarcopenia in rectal cancer patients undergoing NACRT.

OBJECTIVE: Postoperative sarcopenia following neoadjuvant chemoradiotherapy for locally advanced rectal cancer negatively affects recovery and oncological outcomes. Both the Global Immune Nutrition and Inflammation Index (GINI) and the modified Glasgow Prognostic Score (mGPS) are derived from routine laboratory data. There are no studies comparing the performance of these scores and indices in predicting sarcopenia in patients with CT-confirmed sarcopenia. The aim of this study is to determine which of these two indices is a more specific predictor of postoperative sarcopenia in patients who u

Cross-Sectional Differences in BMI, Sarcopenia Risk, and Quality of Life During the First Postoperative Year Following Sleeve Gastrectomy: An Exploratory Study.

INTRODUCTION: Sleeve gastrectomy (SG) effectively reduces body mass index (BMI); however, rapid postoperative weight loss may result in skeletal muscle loss and transiently impair health-related quality of life (QoL). Concurrent data on these three outcomes across the first postoperative year remain limited, particularly in the southern United States. METHODS: This exploratory cross-sectional study enrolled 55 patients (aged 30-50 years; preoperative BMI 30-45 kg/m²) who underwent SG at healthcare facilities across Texas and were stratified by postoperative interval: 3-6, 6-9, and 9-12 months

Learning muscle-health trajectories in ageing: Towards AI-guided organ chips.

Age-related decline in muscle health is commonly defined using cut-offs for strength, muscle quantity or quality, and physical performance. These measures are essential for diagnosis, but prevention and recovery require an understanding of how muscle responds to stress over time. A central question is when recovery remains possible and why older adults with similar clinical measurements follow different trajectories. Clinical cohorts, animal studies and static cell models each address part of this problem. None readily combines human-tissue relevance with controlled perturbation, repeated func

Prevalence of Sarcopenia in Korean Adults Based on the Asian Working Group for Sarcopenia 2025 Criteria: Analysis of the 2024 Korea National Health and Nutrition Examination Survey.

BACKGROUND: The Asian Working Group for Sarcopenia (AWGS) updated its diagnostic guideline in 2025 to incorporate body mass index (BMI)-adjusted cut-off values for muscle mass and extended diagnostic criteria to middle-aged adults (50-64 years). Using nationally representative data from the 2024 Korea National Health and Nutrition Examination Survey (KNHANES), we examined how the new AWGS 2025 criteria influence the prevalence of sarcopenia among Korean adults and characterized age- and sex-specific patterns. METHODS: The study population comprised 2,366 adults aged ≥ 50 years (876 men and 1,

Putative Butyrate-Producing Gut Bacteria Profile and Muscle Health in Older Post-Stroke Patients.

AIM: Muscle health is a clinically relevant dimension of vulnerability in older post-stroke patients, but its gut microbial correlates remain unclear. We examined whether a taxonomy-based putative butyrate-producing gut bacteria profile was associated with admission muscle health. METHODS: This registry-based cross-sectional study included patients aged 65 years or older with 16S rRNA gut microbiota data and no systemic antibiotic exposure within 4 weeks before stool sampling. A six-genera profile was constructed from genus-level relative abundance using centered log-ratio transformation, sta

Consensus on the use of intrathecal gene therapy for spinal muscular atrophy.

BACKGROUND: Intrathecal (IT) delivery of gene therapy is an emerging strategy for Spinal Muscular Atrophy (SMA), aiming to enhance central nervous system targeting while potentially reducing systemic exposure. The new administration route of Onasemnogene abeparvovec is associated with a distinct safety profile, including dorsal root ganglia toxicity, and requires structured monitoring beyond existing intravenous guidelines, including planning for long-term multidisciplinary follow-up in pediatric and adult care as well as structured transition protocols. METHODS: We expanded the existing intr

[The gut-muscle axis and the role of Roseburia inulinivorans in age-related muscle loss: a narrative review].

Age-related muscle loss (sarcopenia) represents one of the most significant health burdens in a globally ageing population. The relationship between the gut microbiome and skeletal muscle, referred to as the gut-muscle axis, is the subject of increasing research, yet the identification of specific responsible microbial species and mechanisms has remained limited. This review aims to summarize the current evidence on the gut-muscle axis, with particular focus on the association between the intestinal bacterium Roseburia inulinivorans and muscle performance reported in a 2026 Gut publication, an

Differential alternative splicing in skeletal muscle after 14 days immobilization-induced atrophy in young men.

Skeletal muscle disuse during injury decreases muscle mass and function. Previous studies have determined differential gene expression associated with atrophy, but changes in alternative splicing (AS) in response to unloading is poorly defined. This study aimed to identify global changes in differential alternative splicing (DAS) in skeletal muscle following unloading. Twenty-one healthy males (26.0 ± 5.7 years; 81.2 ± 11.6 kg) underwent 14 days unilateral knee-brace immobilization. Participants underwent strength and muscle mass assessment before and after the immobilization and vastus latera

Suspected immune-mediated inflammatory myositis with rhabdomyolysis following yellow fever vaccination: a case report.

BACKGROUND: The yellow fever vaccine is a live-attenuated vaccine with an excellent safety profile; however, rare adverse events have been reported. Musculoskeletal complications, particularly inflammatory myositis with rhabdomyolysis, are exceedingly uncommon and not well characterized in the literature. CASE PRESENTATION: We report a case of a 49-year-old man who developed acute febrile illness, severe myalgia, and progressive proximal muscle weakness 1 week after receiving his first yellow fever vaccination. An extensive diagnostic evaluation was performed. The patient presented with high-

A rare case of ultrastructurally proven toxoplasmic myositis masquerading as necrotizing myopathy with emphasis on pathobiology.

Toxoplasmosis is a zoonotic infection caused by the obligate protozoan Toxoplasma gondii. In an immunocompetent human host, the parasite forms bradyzoites (tissue cysts), mainly in skeletal muscle, eye, brain, or cardiac muscle, and remains latent. Once cysts are established, type 1 cytokine-based immunity controls the conversion to tachyzoites from bradyzoites. We propose that short-term immunosuppression can only cause a transient defect in this cycle of conversion control. The temporary lapse in conversion control releases few tachyzoites, and the host immunity in response gives rise to cyt
