Suspected immune-mediated inflammatory myositis with rhabdomyolysis following yellow fever vaccination: a case report.

BACKGROUND: The yellow fever vaccine is a live-attenuated vaccine with an excellent safety profile; however, rare adverse events have been reported. Musculoskeletal complications, particularly inflammatory myositis with rhabdomyolysis, are exceedingly uncommon and not well characterized in the literature. CASE PRESENTATION: We report a case of a 49-year-old man who developed acute febrile illness, severe myalgia, and progressive proximal muscle weakness 1 week after receiving his first yellow fever vaccination. An extensive diagnostic evaluation was performed. The patient presented with high-

A rare case of ultrastructurally proven toxoplasmic myositis masquerading as necrotizing myopathy with emphasis on pathobiology.

Toxoplasmosis is a zoonotic infection caused by the obligate protozoan Toxoplasma gondii. In an immunocompetent human host, the parasite forms bradyzoites (tissue cysts), mainly in skeletal muscle, eye, brain, or cardiac muscle, and remains latent. Once cysts are established, type 1 cytokine-based immunity controls the conversion to tachyzoites from bradyzoites. We propose that short-term immunosuppression can only cause a transient defect in this cycle of conversion control. The temporary lapse in conversion control releases few tachyzoites, and the host immunity in response gives rise to cyt

Sex Hormone-Binding Globulin Limits Glucocorticoid-Induced Muscle Wasting by Preserving Estrogen-Dependent Repression of Catabolic Signaling.

AIM: Chronic glucocorticoid (GC) therapy induces severe metabolic complications, notably secondary sarcopenia driven by excessive glucocorticoid receptor (GR)-mediated catabolism. Although sex steroids protect skeletal muscle metabolism, the role of sex hormone-binding globulin (SHBG), the primary determinant of sex steroid bioavailability, in modulating local endocrine crosstalk under GC stress remains unknown. METHODS: We evaluated the clinical correlation between serum SHBG and endocrine factors in a rheumatology cohort frequently exposed to GC. To establish causality and underlying mechan

Diagnostic Utility of USP6 RNA Chromogenic In Situ Hybridization in the Differential Diagnosis of Nodular Fasciitis and Aneurysmal Bone Cyst.

Recurrent USP6 gene rearrangements underlie a subset of mesenchymal neoplasms, including nodular fasciitis and aneurysmal bone cyst. These rearrangements place USP6 under the regulatory control of another gene's constitutively active promoter, resulting in USP6 overexpression. Recently, it has been demonstrated in a limited number of cases that detection of USP6 overexpression by RNA chromogenic in situ hybridization (CISH) can serve as a surrogate marker for USP6-rearranged tumors. To further assess the diagnostic utility of USP6 RNA CISH, we evaluated its application in 1210 bone and soft ti

Nontraumatic Myositis Ossificans Adjacent to the Femoral Nerve: A Ubiquitin-Specific Peptidase 6 (USP6)-Rearranged Lesion Mimicking Malignancy.

Myositis ossificans (MO) is a benign, self-limiting lesion characterized by heterotopic ossification that can mimic malignancy both radiographically and histologically. Non-traumatic MO is particularly challenging to diagnose, especially when arising adjacent to major neurovascular structures. Recent studies have identified ubiquitin-specific peptidase 6 (USP6) gene rearrangements in a subset of MO, linking it to a family of transient fibroblastic/myofibroblastic neoplasms. We present a 21-year-old female who presented with progressive right lower extremity pain and weakness without a history

Correlation Between Paravertebral Muscles Degeneration and Spinopelvic Parameters in Patients With Osteoporotic Vertebral Compression Fractures.

BACKGROUND Osteoporotic vertebral compression fractures (OVCFs) are correlated with thoracolumbar sagittal imbalance associated with paravertebral muscles (PVM) atrophy and fatty infiltration (FI). This study investigated PVM degeneration and its correlation with spinopelvic parameters in patients with OVCFs. MATERIAL AND METHODS This retrospective case-control study enrolled 50 patients with OVCFs and 50 controls with lumbar degeneration, frequency-matched for age (±5 years). Complete sex matching was not achieved. Primary comparisons were adjusted for age, sex, BMI, and lumbar bone mineral d

Exploring the Therapeutic Efficacy and Molecular Mechanisms of Fu-zheng-yi-qi Recipe in Ameliorating Cancer Cachexia-associated Muscle Atrophy.

INTRODUCTION: Cancer Cachexia (CC) is a systemic wasting syndrome characterized by persistent skeletal muscle depletion. This study aims to systematically evaluate the therapeutic effect of the Fu-zheng-yi-qi recipe (FZYQR) against CC and to explore the potential molecular mechanisms underlying its ameliorative effects on muscle atrophy. METHODS: The components of FZYQR were characterized using UPLC-Class Plus/VION IMS QTof. Pharmacological network analysis was performed to predict the therapeutic targets of FZYQR. An in vivo tumor cachexia model was established by subcutaneously implanting C

Interlaboratory comparison of immunoprecipitation-based testing for myositis-specific autoantibodies.

INTRODUCTION: Myositis-specific autoantibodies (MSAs) are clinically important biomarkers that help classify idiopathic inflammatory myopathies and guide clinical management, but reliable detection remains challenging. Immunoprecipitation (IP) is widely regarded as a reference method for MSA detection in specialist laboratories. However, IP comprises a range of related techniques rather than a single standardised assay, and variability between expert laboratories using different IP-based workflows is not well characterised. METHODS: Serum samples previously submitted to the Bath Diagnostic Se

Imaging of acute sports injuries of the shoulder girdle.

Acute injuries of the shoulder girdle commonly result from sports activities and lead to imaging studies. Radiologists who interpret these studies should be familiar with traumatic mechanisms and the imaging manifestations of glenohumeral dislocation, acromioclavicular sprain, rotator cuff tear, labral tear, osseous contusion or fracture, and myotendinous strain. Imaging findings are crucial for making accurate diagnoses, grading the severity of injuries, guiding treatment plans, and managing the athletes' expectations for rehabilitation and return to sport. In this review article, we address

Long-Term Determinants of Lean Mass in Ageing Adults: A 17-Year Longitudinal Cohort Study With Repeated Measures.

OBJECTIVE: Age-related loss of lean mass is a hallmark of sarcopenia and a contributor to functional decline and morbidity in ageing populations. This study aimed to identify long-term determinants of lean mass during ageing. METHODS: Data from 3033 participants (ages 20-96 years; 49% women) in the Geelong Osteoporosis Study were analysed using linear mixed effect models with random intercepts. Appendicular lean mass (ALM) was derived from arm and leg lean mass measured using dual-energy X-ray absorptiometry. Time-varying covariates were included; missing data were imputed using predictive me

Highly potent, brain-penetrant EP2 receptor antagonists for the treatment of permanent focal cerebral ischaemia in male and female mice.

BACKGROUND AND PURPOSE: Cerebral ischaemia is a leading cause of death in older adults and imposes a substantial global economic burden. Recent studies indicate that inhibiting the Gαs-coupled EP2 receptor reduces ischaemic brain injury by modulating neuroinflammation. However, these findings derive from the transient middle cerebral artery occlusion (tMCAO) model, which represents only a small subset of large-vessel occlusion (LVO) cases with successful recanalisation. EXPERIMENTAL APPROACH: We characterised the pharmacokinetic parameters of selective EP2 antagonists CJ-18, CJ-19 and CJ-21 t

Concomitant Abdominoplasty and Repair of Midline Abdominal Wall Defects.

BACKGROUND: Diastasis recti and midline abdominal wall hernias are frequently encountered in patients undergoing abdominoplasty, particularly postpartum women and individuals following significant weight loss. The associated musculoaponeurotic laxity results in functional abdominal wall weakness and contour deformities. METHODS: We describe a surgical technique combining abdominoplasty with midline fascial repair, performed without mesh reinforcement. The procedure utilizes a modified overlapping Mayo double-suturing method to restore midline strength and contour. This paper presents a 39-yea

Relationship Between Sarcopenia and Intensity-Specific Physical Activity in Older Adults Living in a Rural Area: An Isotemporal Substitution Analysis from the NEIGE Study.

BACKGROUND AND PURPOSE: This study examined the relation between sarcopenia and the replacement of sedentary behavior (SB) with physical activity (PA) of varying intensities using the isotemporal substitution (IS) model. We explored effective intensities and time substitution patterns that may contribute to the prevention of sarcopenia through increased PA and reduced SB. METHODS: A total of 527 community-dwelling adults aged 65-84 years in Tokamachi city, Niigata, wore accelerometers for 7 consecutive days to measure time spent in SB, light PA (LPA), and moderate-to-vigorous PA (MVPA). BMI-adjusted low muscle mass under AWGS 2025 identifies functionally vulnerable patients missed by height-adjusted index in severe knee osteoarthritis.

BackgroundIn severe knee osteoarthritis (KOA) patients, muscle mass is often evaluated using appendicular skeletal muscle mass adjusted for height (ASMI). However, with higher body mass index (BMI), muscle mass may appear normal relative to height but remain insufficient for body size and mechanical demands. The Asian Working Group for Sarcopenia (AWGS) 2025 criteria newly include BMI-adjusted appendicular skeletal muscle mass (ASM/BMI). This study examined discordance between ASMI- and ASM/BMI-based low muscle mass classifications in severe KOA patients undergoing total knee arthroplasty (TKA

Responsiveness and clinical correlates of continuous turns-amplitude analysis measures in myositis: A prospective longitudinal study.

OBJECTIVE: Turns-amplitude analysis (TAA) is a rapid quantitative electromyographic technique used in neuromuscular disorders, but its categorical interpretation (normal, myopathic, neurogenic) limits the granularity needed for longitudinal monitoring. We investigated whether continuous TAA-derived metrics could reflect longitudinal clinical change in idiopathic inflammatory myopathies (IIM). METHODS: In this prospective single-center study, 30 adults with IIM underwent TAA of biceps brachii, quadriceps, and iliopsoas at baseline, 6, and 12 months. From each cloud, we extracted descriptors of

Patients know best: DESTINIES Patient Panel appraisal of a novel, risk-stratified approach to COVID-19 disease surveillance in the adult immunosuppressed.

The immunosuppressed are poorly represented in disease surveillance and pharmacovigilance dataflows, proving especially consequential during the COVID-19 pandemic. This mixed methods study recruited a panel of immunosuppressed patients (DESTINIES Patient Panel) to quality assure a new digital phenotype for COVID-19 risk in the adult immunosuppressed (DESTINIES phenotype) prior to its implementation in real-world surveillance. Applying their lived experiences, the DESTINIES Patient Panel met to critique the strategy and structure of the provisional phenotype before voting on its adoption in UK

Sex-specific skeletal muscle fiber atrophy and reduced macrophage inflammation in a rodent transverse aortic constriction model.

Heart failure is associated with systemic skeletal muscle remodeling, where sex-specific differences suggest differential peripheral involvement in disease progression. Skeletal muscle macrophages regulate angiogenesis and tissue remodeling and may contribute to early alterations in muscle structure and function during cardiac pressure overload. We tested whether chronic pressure-overload induces sex-specific skeletal muscle remodeling and alters macrophage abundance and paracrine signaling in male and female Sprague Dawley rats that underwent transverse aortic constriction (TAC) or sham surge
