[Human immunodeficiency virus infections (HIV-1 and HIV-2) in Dakar. Epidemiologic and clinical aspects].

The authors report on the results of an investigation carried out on 109 HIV seropositives ascertained by the Dakar Central Hospital between February 20, 1987 and May 31, 1988. These seropositives affected 44 patients with AIDS (0.78% of admitted patients) and 65 seropositive people of which 43 blood donors (1.35% of donors). Both viruses are present in about equal number: 50 HIV1 seropositives, 44 HIV2 seropositives, and 15 composite seropositives. Sex-ratio is 3,9 in favour of males; this figure is nearer to the one observed in Europe and North American than the one observed in Central Afric

Cutaneous vascular lesions and disseminated cat-scratch disease in patients with the acquired immunodeficiency syndrome (AIDS) and AIDS-related complex.

Cutaneous lesions develop frequently in patients infected with human immunodeficiency virus (HIV). We describe the clinical features of four patients with the acquired immunodeficiency syndrome (AIDS) or AIDS-related complex who developed angiomatous nodules involving skin and bone, 2 of whom were scratched by a cat. Some of these lesions were clinically indistinguishable from Kaposi sarcoma. When examined with Warthin-Starry staining and electron microscopy, these nodules were noted to contain numerous clumps of a bacterium. Immunoperoxidase staining with an antiserum raised against the cat-s

Biopsy pathology of the gastrointestinal tract in human immunodeficiency virus-associated disease: a 5 year experience in Zürich.

Patients with human immunodeficiency virus (HIV)-associated disease are subject to a wide variety of unusual opportunistic infections and to the development of malignant tumours. These complications frequently manifest themselves within the gastrointestinal tract and endoscopic biopsies may contribute to their diagnosis. The results of 63 such biopsies from 28 patients are reviewed in this study. The diagnoses made included: upper gastrointestinal tract candidiasis (n = 6); cytomegalovirus infection of large intestine (n = 2); cryptosporidiosis (n = 1); spirochaetosis (n = 2); Kaposi's sarcoma

Herpes simplex virus type-1 can reactivate transcription of latent human immunodeficiency virus.

The human immunodeficiency virus, HIV (formerly T-cell lymphotropic virus type III, HTLV-III or lymphadenopathy-associated virus, LAV) is the primary cause of AIDS (acquired immune deficiency syndrome). Patients with AIDS have profound immunosuppression as a result of almost complete absence of the OKT4+ cell population and are predisposed to a number of opportunistic infections as well as to certain malignant diseases such as Kaposi's sarcoma and B-cell tumours. The majority of the opportunistic infections observed in AIDS patients are from the herpesvirus group and these are frequently the c

Lymphomas associated with the acquired immune deficiency syndrome (AIDS): a study of 35 cases.

An increased incidence of lymphoid neoplasias is associated with the states of immune deficiency both congenital and acquired. Thirty-five cases of lymphoma in males at high risk for AIDS were diagnosed in one community hospital in New York City within the past 2 years. The mean age of these patients was 39.6 years; 34 were homosexual, and one was an intravenous drug abuser. There were four Hodgkin and 31 non-Hodgkin lymphomas of various histologic types but almost all of high-grade categories. The proportion of extranodal lymphomas, the involvement of the gastrointestinal tract, central nervo

Kaposi's sarcoma of the lung in AIDS: radiologic-pathologic analysis.

Kaposi's sarcoma in patients with AIDS involves the lung more often than it does in the African form of the neoplasm. This article describes the radiographic and histologic features in nine cases of pulmonary Kaposi's sarcoma, uncomplicated by infection, and reviews the radiographic descriptions of 22 similar cases from the literature. Pulmonary parenchymal disease in reported cases and in this series was usually diffuse. Three cases had localized disease. In one of these cases, the localized disease was segmental while in the other two cases, the disease involved a single entire lobe. Hilar a

AIDS and lymphadenopathy syndrome (LAS) patients display similar abnormal in vitro proliferation and differentiation of T-colony forming cells (T-CFC).

T-cell colonies were generated from the peripheral blood and bone marrow of 61 patients with acquired immunodeficiency syndrome (AIDS), 54 patients with persistent lymphadenopathy syndrome (LAS), 14 clinically normal male homosexuals, and 17 healthy heterosexuals. Mononuclear cells were cultured in methylcellulose in the presence of IL2-containing conditioned medium. The number of T-cell forming cells (T-CFC) from healthy male homosexuals and AIDS and LAS patients was significantly (P less than 0.01) reduced compared to T-CFC from healthy heterosexuals. In AIDS patients, the low colony growth

Epidemiologic similarities leading to etiologic hypotheses.

Analogy is a useful means of generating etiologic hypotheses which can be tested by epidemiologic, clinical, or laboratory studies. We describe how the use of analogy has furthered the understanding of three disease entities--Hodgkin's disease (HD), testicular cancer (TC), and acquired immune deficiency syndrome (AIDS). HD is probably caused by a virus of low pathogenicity with clinical expression dependent on social factors. TC may have a viral etiology, although clinical disease may be triggered by other factors. AIDS is caused by a virus which may act as an initiator. AIDS-related Kaposi's

Pulmonary manifestations of Kaposi's sarcoma.

Pulmonary Kaposi's sarcoma is an unusual pre-mortem diagnosis in AIDS patients. During a four-year period, 318 patients with AIDS-associated Kaposi's sarcoma presented to New York University Medical Center; 19 patients were found to have pulmonary involvement. All 19 patients were homosexual men; 18 of these patients presented with cutaneous and/or visceral Kaposi's sarcoma. Seventy-nine percent of patients had previous or concurrent pulmonary opportunistic infections. Clinical presentation of pulmonary Kaposi's sarcoma was indistinguishable from opportunistic pneumonia with respect to symptom

Intestinal plasma cell alterations in acquired immunodeficiency syndrome.

Immunofluorescence studies of plasma cells were performed on small intestinal and colonic biopsies obtained from a series of patients with acquired immunodeficiency syndrome or the "AIDS related complex" plus homosexual male and heterosexual controls. The AIDS group was significantly depleted of IgA plasma cells compared to the control groups. In contrast, the numbers of IgM plasma cells were increased in the AIDS and homosexual control groups compared to the heterosexuals. Plasma cell fluorescence intensities for IgA and IgM were decreased in the AIDS patients, implying less cytoplasmic immun

Incidence and clinical significance of neutralizing antibodies in patients receiving recombinant interferon alfa-2a by intramuscular injection.

More than 1600 patients with neoplastic disorders have received recombinant human interferon alfa-2a (Roferon-A, Hoffmann-La Roche, Nutley, NJ) as part of ongoing or completed clinical trials. In this report, the efficacy of interferon alfa-2a therapy was compared with the incidence of antibodies to this interferon in 617 patients who received the drug by intramuscular administration. Antibody measurements were performed using a highly sensitive enzyme immunoassay, and an interferon antiviral neutralization bioassay. Partial or complete remission occurred in 28% (43 of 152) of the antibody-pos

Sera from HTLV-III/LAV antibody-positive individuals mediate antibody-dependent cellular cytotoxicity against HTLV-III/LAV-infected T cells.

The causative agent of the acquired immunodeficiency syndrome (AIDS) has been shown to be a human retrovirus called human T lymphotropic virus (HTLV)-III or lymphadenopathy-associated virus (LAV). The nature of the protective immune response against this virus is currently unknown. We report here results using an antibody-dependent cellular cytotoxicity (ADCC) assay which has been developed for measuring a specific immune response against HTLV-III/LAV. Forty-four sera were examined for their ability to mediate ADCC against HTLV-III/LAV-infected T cells. Sera from healthy HTLV-III/LAV seroposit

Combination chemotherapy of disseminated Kaposi's sarcoma in patients with the acquired immune deficiency syndrome.

Two clinical trials were conducted to assess the efficacy and safety of a combination chemotherapy regimen for the treatment of Kaposi's sarcoma in patients with the acquired immune deficiency syndrome (AIDS). Eighteen consecutive patients with disseminated Kaposi's sarcoma were treated with a six-drug regimen of doxorubicin (Adriamycin), vinblastine, bleomycin/actinomycin D, vincristine, dacarbazine (ABV/ADV). A brief partial or complete response was achieved in 13 patients. Most patients died of opportunistic infections. Eighteen consecutive patients with disseminated Kaposi's sarcoma were t

The role of interferon in the therapy of epidemic Kaposi's sarcoma.

Kaposi's sarcoma (KS) is the major neoplastic complication of the acquired immune deficiency syndrome (AIDS). Although most patients succumb to infectious complications of AIDS rather than as a direct consequence of the tumor, KS may, in some cases, pursue an aggressive clinical course resulting in considerable morbidity. A subset of KS patients, characterized by lack of systemic "B" symptoms (fever, weight loss, night sweats), absence of prior opportunistic infection, and relative preservation of immune function, appears to be most likely to benefit from interferon alpha treatment. A series o

Nutritional factors in epidemic Kaposi's sarcoma.

The role of nutritional factors in the management of acquired immunodeficiency syndrome-related, or epidemic, Kaposi's sarcoma (EKS) is complex, since there are known interactions between malnutrition, immunodeficiency, and cancer. Malnutrition is a well-established cause of immune aberrations, which are seen in deficiencies of both protein and energy, as well as specific nutrients, particularly trace metals. Conversely, malnutrition is a common result of both cancer and immunodeficiency. Cancer patients without an obviously immunological pathogenesis frequently have malnutrition and cachexia,

[Acquired immunodeficiency syndrome (AIDS). II. Clinical aspects with special reference to neuropsychiatric manifestations].

Opportunistic infections and neoplasias, which led to the identification and characterisation of AIDS, are the most common manifestations of this disease. The present review gives a comprehensive presentation of the current knowledge on the opportunistic infectious agents concerned and on the resulting clinical pictures with special emphasis on neuropsychiatric symptomatology. Prior to the development of the full syndrome of AIDS, a number of intermediate steps can be observed, the most important of which is persistent generalised lymphadenopathy (PGL). For all stages of the disease, problems

[Therapeutic use of alpha interferon in metastasizing malignant melanoma, disseminated Kaposi sarcoma and severe Behçet disease].

Three types of human interferons (IFNs) are differentiated: alpha-interferon (with about 20 subtypes), beta-interferon, and gamma-interferon. In addition to their antiviral properties, these IFN types exert numerous other biological effects, which are increasingly exploited for therapeutic purposes. From 1982 to 1985 long-term recombinant alpha 1-interferon (rIFN-alpha A) was administered systemically in 39 patients with various severe dermatoses. In this study we report our preliminary experiences with systemic administration of rIFN-alpha A in malignant melanoma (MM, 9 patients), in dissemin

Kaposi's sarcoma in AIDS. Multicentric angioneoplasia in early skin lesions.

Light-microscopic, immunohistochemical, and ultrastructural studies were performed on biopsy material from 15 young homosexual men with AIDS-associated mucocutaneous Kaposi's sarcoma; 19 Kaposi's sarcoma lesions in different developmental stages were investigated. These lesions showed multicentrically arising and proliferating vascular endothelia forming thick-walled and thin-walled capillaries and larger vessels, as well as spindle-shaped cells forming fascicles and bundles around them. Different amounts and organization of these two major cellular components were found in all stages of evolu

Histogenesis of Kaposi's sarcoma associated with AIDS: a histologic, immunohistochemical and enzyme histochemical study.

Twenty-one cases (25 biopsies including 9 frozen biopsies) of Kaposi's sarcoma associated with the acquired immune deficiency syndrome (AIDS) were examined immunohistochemically, lectin-histochemically, and enzyme histochemically to ascertain the histogenesis of the lesion. The Kaposi's sarcomas were histologically subtyped according to a modified Schmid's classification (granulation tissue-like-, angiosarcoma-like- and spindle cell type). In almost all lesions, many atypical vasoforming cells and at least some spindle cells without definite evidence of vasoformation by conventional microscopy
