Incidence of Kaposi's sarcoma and mycosis fungoides in the United States including Puerto Rico, 1973-81.

The incidence of Kaposi's sarcoma (KS) was examined with the use of data from the Surveillance, Epidemiology, and End Results (SEER) Program of the National Cancer Institute. KS is a manifestation of the recent epidemic of acquired immunodeficiency syndrome (AIDS) that has occurred particularly among homosexual men. The incidence of KS in 1973-79 was found to be higher (0.29 male and 0.07 female cases/100,000/yr) than is usually cited for the pre-AIDS KS incidence rates. Collectively, the 9 SEER registries in the United States showed only a slight increase in the incidence of KS between 1973-7

Acquired immune deficiency syndrome (AIDS). Medical challenge of the 80's.

Although the pathophysiology of acquired immune deficiency syndrome (AIDS) is not completely understood, we know a great deal about its epidemiology, risk factors, clinical manifestations, course, and immunologic features. Clinicians caring for high-risk individuals, particularly those in endemic urban areas, should remain alert for signs and symptoms of opportunistic infections or neoplasms associated with immunosuppression. It is clear that research on AIDS has increased exponentially; both physicians and patients should be encouraged by the fact that the apparent cause of this syndrome has

Herpesviruses, lymphocytes, and AIDS.

The acquired immune deficiency syndrome (AIDS) was first reported in the United States in mid-1981. Since that time, it has received widespread attention in the media and has produced an extremely sensitive public health situation. AIDS is a life-threatening condition that is characterized by an acquired defect(s) in the body's natural immunity to disease. Those at greatest risk for contracting the disease include male homosexuals, haemophiliacs, intravenous drug abusers, and children of mothers with AIDS. Recent results from laboratory investigations strongly support the concept that a virus

Molecular characterization of human T-cell leukemia (lymphotropic) virus type III in the acquired immune deficiency syndrome.

The human T-cell leukemia (lymphotropic) virus type III (HTLV-III) appears to be central to the causation of the acquired immune deficiency syndrome (AIDS). Two full-length integrated proviral DNA forms of HTLV-III have now been cloned and analyzed, and DNA sequences of the virus in cell lines and fresh tissues from patients with AIDS or AIDS-related complex (ARC) have been characterized. The results revealed that (i) HTLV-III is an exogenous human retrovirus, approximately 10 kilobases in length, that lacks nucleic acid sequences derived from normal human DNA; (ii) HTLV-III, unlike HTLV types

Acquired immune deficiency syndrome.

The acquired immune deficiency syndrome (AIDS) has been reported in more than 2800 persons in the United States (70 per cent of whom are homosexually active men). AIDS is manifested by impaired cell-mediated immunity that results in various opportunistic infections (especially pneumonia caused by Pneumocystis carinii) or malignant disease (especially disseminated Kaposi's sarcoma). The cause is unknown, and effective treatment is not yet available. AIDS apparently is not highly infectious for persons outside the acknowledged high-risk groups.

Lymphocytotoxic antibodies in the acquired immune deficiency syndrome (AIDS).

The acquired immune deficiency syndrome (AIDS) manifested by opportunistic infections or Kaposi's sarcoma is a newly recognized and often fatal disease. Three patients seen in Syracuse, New York, were noted to have lymphopenia and persistent serum lymphocytotoxic antibodies (LCTAs). In a double blinded study, 25 serum samples were coded and sent to us by the Centers for Disease Control Task Force on Kaposi's Sarcoma and Opportunistic Infections. Samples from 5 patients with Pneumocystis carinii pneumonia, 5 patients with Kaposi's sarcoma, 5 presumably healthy homosexual males, 5 presumably hea

Acquired immune deficiency syndrome (AIDS) trends in the United States, 1978-1982.

This report summarizes results of surveillance for the acquired immune deficiency syndrome (AIDS) in the United States by the Centers for Disease Control through the first quarter of 1983. Surveillance has been predominantly passive, supplemented by active follow-up of requests to the Centers for Disease Control for pentamidine isethionate for treatment of Pneumocystis carinii pneumonia. The 1,299 reported cases showed trends of increasing incidence among all risk groups: homosexual men (72 percent), intravenous drug abusers (17 percent), persons of Haitian origin (5 percent), persons with hem

The acquired immune deficiency syndrome: an international health problem of increasing importance.

The Acquired Immune Deficiency Syndrome (AIDS) is a new disease which first appeared in human populations about 1979. The disease is defined by the development of unusual types of cancer (e.g. Kaposi's sarcoma), or severe cellular immunodeficiency manifested by opportunistic infections (e.g. Pneumocystis carinii infection), or both. Although the etiology of AIDS is unknown, the epidemiologic evidence is consistent with an infectious agent transmitted by blood (e.g. transfusion, needle sharing) or sexual intercourse. Over three-quarters of the cases have been in homosexual or bisexual males and

Polymicrobial brain abscess in a homosexual man with Kaposi's sarcoma.

A progressive visual field defect and fever occurred in a 39-year-old homosexual man with acquired immunodeficiency syndrome and Kaposi's sarcoma. Computerized tomography of the brain documented a left parieto-occipital abscess. A specimen obtained at the time of surgical excision of the lesion revealed organisms with morphologic features of Toxoplasma gondii and Candida. Culture of this material grew Staphylococcus epidermidis. Appropriate therapy for this polymicrobial infection resulted in resolution of the fever and clinical improvement. The visual deficit remained unchanged despite antimi

[The first case of AIDS in the Tyrol].

This case report deals with a description of the first case of AIDS in Tirol. Our patient was an intravenous drug abuser and presented with both opportunistic infections (candidiasis, interstitial pneumonia due to Pneumocystis carinii) and with Kaposi's sarcoma. He died within 4 weeks as a result of staphylococcal septicaemia refractory to intensive chemotherapy.

Pharyngeal obstruction by Kaposi's sarcoma in a homosexual male with acquired immune deficiency syndrome.

A homosexual man with AIDS developed multifocal KS with involvement of the palate, larynx, trachea, and esophagus. Symptoms included dysphagia and gagging with resultant inanition. Short-course local radiation therapy effectively resolved the mucosal KS lesions in the treated areas. Other otolaryngologic manifestations included herpes stomatitis and oral candidiasis.

Hypothesis: the pathogenesis of AIDS. Activation of the T- and B-cell cascades.

The hypothesis is presented that a human T-cell leukemia virus (HTLV) or a related agent produces a lytic response of T cells manifested by the acquired immune deficiency syndrome (AIDS) and a proliferative response represented by the adult leukemia/lymphoma (ATL) syndromes. The sequence or cascade of T-cell events following loss of T4 helper cells in AIDS includes reactivation of Epstein-Barr virus and a B-cell cascade of cytomegalovirus, resulting in Kaposi sarcoma in genetically susceptible persons, and of other intracellular agents (CNS viruses, M. avium intracellulari, T. gondii); opportu

Ophthalmic involvement in acquired immunodeficiency syndrome.

Forty patients with acquired immunodeficiency syndrome (AIDS) were examined for ocular abnormalities. Twenty of these patients died and the eyes were obtained for culture and histopathologic examination. These patients have multiple opportunistic infections and neoplasms as the result of a severe depression of cellular immunity. Fifty percent of all patients with AIDS and 75% of the autopsy group have ocular signs attributable to AIDS. Ocular findings were confined to four major categories: cytomegalovirus retinitis (10 patients), retinal cotton wool spots (11 patients), conjunctival Kaposi's

Thoracic manifestations of the acquired immune deficiency syndrome.

The acquired immune deficiency syndrome is characterized by the development of multiple recurrent opportunistic infections or unusual neoplasms in individuals with no prior history of immune suppression. This report summarizes the thoracic diseases encountered in such patients before after death and the role of diagnostic techniques currently used in the evaluation of thoracic disease in 15 patients with this syndrome. Efficacy of treatment was determined by correlation with postmortem findings in all patients. Pulmonary disease was present in all 15 patients and necessitated 23 transbronchial

Humoral immune responses in healthy heterosexual, homosexual and vasectomized men and in homosexual men with the acquired immune deficiency syndrome.

In homosexual men, the acquired immune deficiency syndrome (AIDS) is associated with sexual promiscuity and the appearance of circulating immune complexes (CICs) and antibodies to spermatozoa which crossreact with lymphoid cells. A comparative study was initiated to determine whether similar humoral responses existed in 38 heterosexual men lacking sperm antibodies, 13 heterosexuals with sperm antibodies, 42 heterosexual vasectomized men, 22 healthy homosexual men, 26 homosexuals with lymphadenopathy and 16 with AIDS or Kaposi's sarcoma (KS). Sperm antibodies were detected in 12% of the vasecto

Chemotherapy in advanced Kaposi's sarcoma. Implications for current cases in homosexual men.

Kaposi's sarcoma has recently appeared in an aggressive form in young, previously healthy, homosexual men. The disease in these patients corresponds most closely to disseminated Kaposi's sarcoma common in areas of Africa. This and the underlying acquired immune deficiency in the current patients in the United States affect the choice of appropriate therapy. Because Kaposi's sarcoma in these patients is rapidly progressive, it often requires aggressive systemic therapy, but this can be difficult given the patients' susceptibility to opportunistic infections. Reports from Africa suggest that Kap

Cytologic findings in homosexual males with acquired immunodeficiency.

During a three-year period (1979 to 1982), 248 cytology specimens were obtained from 38 homosexual males who were admitted for treatment and had a variety of nonspecific symptoms, such as general malaise and weight loss. Twenty patients had Kaposi's sarcoma. Exfoliative and aspiration/brush specimens were prepared mainly from cerebrospinal fluids and tracheobronchial secretions. One hundred twenty-four specimens were of diagnostic value, exhibiting cytologic abnormalities: acute inflammation (11 specimens), marked cellular atypia (39 specimens), increased number of mature and immature lymphore
