T-cells specific for KSHV and HIV migrate to Kaposi sarcoma tumors and persist over time.

Kaposi sarcoma-associated herpesvirus (KSHV) is the etiologic agent of Kaposi sarcoma (KS), which causes significant morbidity and mortality worldwide, particularly in people living with HIV (PLWH) and in sub-Saharan Africa where KSHV seroprevalence is high. Postulating that T-cells specific for KSHV and HIV would be attracted to KS tumors, we performed transcriptional profiling and T-cell receptor (TCR) repertoire analysis of tumor biopsies from 144 Ugandan adults with KS, 106 of whom were also living with HIV. We show that CD8+ T-cells and M2-polarized macrophages are the most common immune

Stakeholder perspectives on barriers and facilitators to hypertension control in urban Haiti: a qualitative study to inform a community-based hypertension management intervention.

BACKGROUND: Uncontrolled hypertension is the leading modifiable risk factor for cardiovascular disease mortality and remains high in low-middle income countries like Haiti. Barriers and facilitators to achieving hypertension control in urban Haiti remain poorly understood. Elucidating these factors could lead to development of successful interventions. METHODS: We conducted semi-structured interviews with healthcare providers (10) and patients with hypertension (10) from the Haiti Cardiovascular Disease Cohort, using guides developed using the Consolidated Framework for Implementation Researc

Malignancies in people living with HIV: A 25-years observational study from a tertiary hospital in Italy.

BACKGROUND: HIV infection has been associated with an increased risk of cancer development and Kaposi's sarcoma, non-Hodgkin's lymphoma, and invasive cervical cancers have been a manifestation of AIDS. With the advent of antiretroviral therapy, a collateral appearance of non-AIDS defining cancers (NADC) has been observed in HIV positive patients. METHODS: From January 1997 to December 2022, we performed an observational cross-sectional study, involving HIV-infected outpatients with both AIDS-defining cancers (ADC) and NADC, followed up in a tertiary hospital in Italy. RESULTS: Overall, 153 c

Spectrum and trends of cancer among HIV patients in Southwestern Uganda.

BACKGROUND: Antiretroviral therapy (ART) restores cellular immunity, significantly reducing AIDS-related mortality and morbidity thus improving the quality of life among People living with HIV (PLHIV). Studies done in several countries show a decline in AIDS defining cancers (ADCs) with the introduction of ART however the increased longevity has led to the increase of Non-AIDS defining cancers (NADCs). The study was aimed at studying the changing spectrum and trends of cancer among Human Immunodeficiency Virus (HIV) patients in southwestern Uganda. METHODS: The study was a retrospective chart

Predictors of Clinical Outcomes Among People With Human Immunodeficiency Virus and Tuberculosis Symptoms After Rapid Treatment Initiation in Haiti.

BACKGROUND: Few studies have evaluated baseline predictors of clinical outcomes among people with human immunodeficiency virus (HIV) starting antiretroviral therapy (ART) in the modern era of rapid ART initiation. METHODS: We conducted a secondary analysis of a previously reported open-label randomized controlled trial of 2 rapid treatment initiation strategies for people with treatment-naive HIV and tuberculosis symptoms at a large urban clinic in Haiti. We used logistic regression models to assess associations between baseline characteristics and (1) retention in care at 48 weeks, (2) HIV v

Early initiation of fast-track care for persons living with HIV initiating dolutegravir-based regimens during a period of severe civil unrest in Port-au-Prince, Haiti: a pilot randomized trial.

INTRODUCTION: Differentiated service delivery (DSD) models have been widely implemented for patients in stable HIV care. However, DSD has rarely been offered to newly diagnosed patients. We assessed the effectiveness of early fast-track care during a period of severe civil unrest in Port-au-Prince, Haiti. METHODS: We conducted a pilot randomized trial among adults presenting with early HIV disease to determine whether early fast-track care (8-12 weeks after same-day HIV testing and antiretroviral therapy [ART] initiation) was associated with superior outcomes compared with standard (deferred

Kaposi's Sarcoma, Epidemiology and Clinical Characteristics in a Tertiary Hospital: A Cross-Sectional Study.

INTRODUCTION: Kaposi's sarcoma (KS) is a rare soft tissue tumor linked to human herpesvirus 8, a recognized oncogenic virus. Five distinct clinical presentations have been identified, with the epidemic type being the most prevalent and notably associated with human immunodeficiency virus (HIV). A delayed diagnosis significantly compromises patient prognosis and survival rates. METHODS: This article aimed to describe the epidemiological and clinical characteristics of KS cases diagnosed through histological examination between 2007 and 2023 in our dermatology department, after a comprehensive

Fifth subtype of Kaposi sarcoma in HIV-negative MSM: a retrospective single-arm cohort study from a tertiary care center in NYC from 2000 to 2022.

BACKGROUND: Kaposi sarcoma (KS) is a vascular tumor caused by human herpesvirus 8, also known as Kaposi sarcoma herpesvirus. There are 4 distinct subtypes: classic, endemic, iatrogenic, and epidemic (HIV-associated). A fifth subtype is increasingly recognized: non-epidemic KS in men who have sex with men (MSM) who are HIV-negative. Our primary objective was to characterize non-epidemic KS to identify associated risk factors, presentation, treatment course, and prognosis of these patients. PATIENTS AND METHODS: This retrospective cohort included all patients evaluated at Memorial Sloan Ketteri

Kaposi's sarcoma herpesvirus seroprevalence among blood donors in Uganda.

BACKGROUND: Kaposi's sarcoma herpesvirus (KSHV) causes a life-long infection that can progress to several types of KSHV-associated diseases. There is evidence for transfusion transmission of KSHV. In endemic regions, such as sub-Saharan African, KSHV seroprevalence is >40%. However, previous studies of blood donors utilized immunoassays that detect KSHV-associated disease-specific antigens, which may underestimate the true burden of KSHV in a healthy population. STUDY DESIGN AND METHODS: We utilized samples from an on-going transfusion transmitted infection clinical trial to estimate the sero

Artificial Intelligence-based Diagnosis of Kaposi Sarcoma using Photographs in Dark-skinned Patients.

IMPORTANCE: Advanced-stage disease at the time of diagnosis, with resultant high mortality, is among the most urgent issues for HIV-related Kaposi sarcoma (KS) in sub-Saharan Africa. Lack of access to skilled clinical personnel and histopathologic technology in the region contribute to diagnostic delays and advanced stage at diagnosis. Accordingly, new paradigms for KS diagnosis are needed. OBJECTIVE: To evaluate the accuracy of artificial intelligence (AI)-based interpretation of digital surface images of skin lesions to diagnose KS among dark-skinned patients in Uganda. DESIGN: Cross-secti

Calycosin inhibits lytic replication of Kaposi's sarcoma-associated herpesvirus by downregulating early growth response 1.

BACKGROUND: Kaposi's sarcoma-associated herpesvirus (KSHV) is linked to several diseases, including primary effusion lymphoma, multicentric Castleman's disease, and KSHV inflammatory cytokine syndrome. Current treatment options for KSHV-associated diseases are sometimes ineffective, and antiviral drugs are still lacking. Calycosin (CA), an O-methylated isoflavone found in Astragalus membranaceus, has previously demonstrated strong activity against coxsackievirus B3 (CVB3) and human immunodeficiency virus (HIV), but its effect against KSHV has not been previously reported. METHODS: Viral lytic

The association of immunosuppression and HIV control with Kaposi sarcoma among patients on antiretroviral therapy.

OBJECTIVE: Access to antiretroviral therapy (ART) has resulted in a decline in Kaposi sarcoma incidence among people with HIV (PWH). However, Kaposi sarcoma is still occurring among PWH receiving ART, and it is important to understand the degree to which risk of Kaposi sarcoma is impacted by response to ART. METHODS: We examined the changing epidemiology of Kaposi sarcoma among more than 20, 000 PWH receiving HIV care between 1996 and 2016 in the Center for AIDS Research Network of Integrated Clinical Systems (CNICS). We evaluated the association of Kaposi sarcoma with CD4 + T-cell count and

Survival After Diagnosis of HIV-Associated Kaposi Sarcoma Among Adults in East Africa in the "Treat-All" Era.

BACKGROUND: HIV-associated Kaposi sarcoma (KS) remains among the most common malignancies in sub-Saharan Africa in the "Treat All" era. Survival after KS diagnosis has historically been poor in Africa, but knowledge about whether survival has changed in the contemporary era has been limited by lack of community-representative surveillance and monitoring systems. METHODS: We identified people living with HIV with a new diagnosis of KS made between 2016 and 2019 at primary care facilities in Kenya and Uganda using rapid case ascertainment. In a cohort study, they were subsequently followed for

Assessment of γ-herpesvirus infection dynamics in non-hospitalised people living with HIV during the COVID-19 pandemic in South Africa.

Epstein-Barr virus (EBV) and Kaposi's sarcoma-associated herpesvirus (KSHV) are oncogenic human γ-herpesviruses highly prevalent in Sub-Saharan Africa. Both establish latent early-life infections, with intermittent lytic reactivations, often triggered by viral co-infections, particularly in immunocompromised individuals. In this retrospective observational cross-sectional sub-study, we leveraged a cohort of 407 non-hospitalised people living with HIV (PLWH) attending antiretroviral therapy services in South Africa during the COVID-19 pandemic, with previously reported increased reactivation of
