Efficacy and safety of garetosmab, an activin A-blocking antibody, in fibrodysplasia ossificans progressiva (OPTIMA): a randomised, double-blind, placebo-controlled, phase 3 trial.

BACKGROUND: Fibrodysplasia ossificans progressiva (FOP) is an ultrarare genetic disorder resulting in progressive heterotopic ossification of ligaments, tendons, and muscles, leading to disability and early mortality. FOP is caused by heterozygous gain-of-function mutations in the activin receptor type 1A, allowing this receptor to be aberrantly activated by activin A (ActA), which normally inhibits this receptor, thus driving heterotopic bone formation. In preclinical models and an earlier phase 2 trial (LUMINA-1) in FOP, an antibody blocking ActA (garetosmab) prevented heterotopic ossificati

Transforming growth factor-β3 in primate tissue induction: Heterotopic osteogenesis, periodontal cementogenesis and the limits of clinical translation; A personal historical perspective.

It has been demonstrated that naturally derived osteogenic proteins extracted and purified from allogeneic bone matrices, reconstituted and implanted in the rectus abdominis muscle of recipient non-human primates of the species Papio ursinus, induce bone as a recapitulation of embryonic development. There is the induction of cartilage anlagen, angiogenesis, and vascular invasion, chondrolysis with the induction of osteoblast differentiation and bone matrix deposition. Recombinant human osteogenic protein-1 (hOP-1) also induces large ossicles in the rectus abdominis muscle of Papio ursinus. Our

Myositis Ossificans of the Thenar Eminence mimicking a Fracture of an Accessory Trapezium: A Case Report.

INTRODUCTION: Septic arthritis of the hip and periprosthetic joint infection (PJI) are serious conditions that can cause joint destruction and severe functional impairment. In cases with advanced joint destruction or implant-related infection, two-stage reconstruction using an antibiotic-loaded cement spacer is widely performed. However, conventional polymethylmethacrylate (PMMA) spacers have limitations, including a short duration of antibiotic release and potential antibiotic inactivation caused by polymerization heat. A double-layered antibiotic-loaded cement spacer consisting of an antibio

Myositis Ossificans of the Soleus Muscle.

Myositis ossificans (MO) is a rare condition in which abnormal calcification and bone formation develop within a muscle. It appears most often in adolescents but can also occur in younger children. Although it usually affects a single muscle, it can sometimes involve different parts of the body. We report a case of post-traumatic MO. A 16-year-old girl presented with progressive left knee pain and swelling 15 days after minor trauma, with a visual analog scale (VAS) for pain of 8/10 and nocturnal exacerbation. Examination showed a soft, mobile, tender medial knee mass with reduced range of mot

Stromal Vascular Fraction in Bone Healing: A Systematic Review of Clinical Evidence and Potential Implications in Skull Base Reconstruction and Cranioplasty.

BACKGROUND: Bone healing following pathology or surgery is a complex, multiphase process involving coordinated cellular and molecular activities. Various stimulation strategies have been used to enhance bone repair. Recently, stromal vascular fraction (SVF)-a heterogeneous, adipose-derived cell mixture containing pericytes, smooth muscle cells, and adipose-derived stem cells with regenerative, immunomodulatory, and angiogenic properties-has been explored in clinical settings to promote accelerated bone regeneration. METHODS: Following the PRISMA guidelines, a systematic search was conducted i

Dissecting primary versus secondary effects of osteogenesis imperfecta on abnormal lung development and function.

Skeletal dysplasias, including osteogenesis imperfecta (OI), can lead to perinatal respiratory distress and failure with limited therapeutic options. OI is mostly caused by dominant mutations in either COL1A1 or COL1A2 genes and in addition to its dramatic impact on the skeleton, it affects other organs, including the lung with respiratory complications being a leading cause of mortality in patients with OI. In various mouse models of OI, we and others have shown impaired alveolar formation resulting in alveolar simplification, and functional changes in respiratory mechanics. However, assessin

Myostatin inhibition from preconception through lactation improves maternal muscle and skeletal health in the osteogenesis imperfecta murine mouse model.

Osteogenesis imperfecta (OI) is a rare type I collagenopathy characterized primarily by skeletal fragility. Despite extensive genetic and clinical understanding of OI, there is no cure and current treatment strategies are not compatible with pregnancy or lactation. Pregnancy and lactation are natural periods of maternal bone loss, as calcium is extracted from maternal bone to support offspring skeletal development, further challenging maternal musculoskeletal health in OI. We hypothesize targeting muscle-bone crosstalk through pharmacological myostatin inhibition in utero can improve maternal

Creation of intermuscular bone-free genetic mutants in grass carp and multiomics reveals molecular regulatory basis.

Grass carp (Ctenopharyngodon idellus) is an economically important fish, produced at the highest level among all freshwater aquaculture fish in the world, yet the significant number of intermuscular bones (IBs) present in the fillets severely limits its consumption and processing options. To address this limitation, we first investigated the developmental characteristics of IBs in grass carp. Our results revealed that the key period for IB ossification from myosepta occurred between 15 and 40 days post-hatching (dph). Through gene editing of runx2b, the key gene for IB development, we successf

Combined senolytics induce varied phenotypic and functional responses on senescent phenotypes of mesenchymal stromal cell populations.

Mesenchymal stem cells have emerged as a pivotal focus in regenerative medicine and therapeutic innovation due to their multipotent differentiation capacity and immunomodulatory properties. A major obstacle in maintaining human MSC potency and subsequently, the use of MSCs for cellular therapy is replicative senescence, or progressive aging. This obstacle may be alleviated or overcome through the use of senolytics; a class of drugs able to clear senescent cells while leaving non-senescent cells unharmed. Our study investigates the in vitro and in vivo functional effects of the combination of t

Asperosaponin VI ameliorates osteoporosis via skeletal muscle-derived FGF-2-mediated osteoblast differentiation.

BACKGROUND: Skeletal muscle functions as a dynamic endocrine organ, secreting myokines that contribute to systemic physiological homeostasis. Despite the established osteoprotective effects of Asperosaponin VI (AVI), it remains unknown whether these effects are mediated via the modulation of muscle-derived factors that subsequently regulate bone metabolism. PURPOSE: The present study aims to investigate whether AVI alleviates ovariectomy (OVX)-induced bone loss by modulating muscle-derived signals that exert regulatory effects on osteogenesis. METHODS: Female C57BL/6 OVX mouse models and the

Liquiritigenin ameliorates circadian rhythm-mediated osteoblast differentiation through Rorα and Nr1d1 signaling pathways.

Circadian rhythm disorders can destroy the balance of bone metabolism and hence closely related to bone metabolic diseases. To further explore the relationship between circadian rhythm and bone metabolism, the sleep deprivation model of zebrafish induced by continuous light and the MC3T3-EI cell model induced by Nr1d1-specific agonist STL1267 (STL1267) were used to study the intervention mechanism of liquiritigenin on bone metabolism imbalance mediated by circadian rhythm. C4/5 Facetectomy Is Associated With Improved Recovery of C5 Palsy in Severe Foraminal Stenosis After Posterior Cervical Decompression and Fusion.

INTRODUCTION: C5 palsy is a disabling complication after posterior cervical decompression and fusion (PCDF). Whether C4/5 facetectomy reduces the incidence or improves the outcomes of C5 palsy remains uncertain. Therefore, this study aimed to (1) test the association of C4/5 facetectomy with the incidence and recovery of C5 palsy and (2) identify radiological risk factors for C5 palsy. METHODS: A single-center retrospective cohort study was conducted between April 2013 and January 2024. Adults with degenerative cervical myelopathy, including ossification of the posterior longitudinal ligament

Precision RNAi for Fibrodysplasia Ossificans Progressiva: a combinatorial, unimolecular, allele selective approach.

Fibrodysplasia ossificans progressiva (FOP) is a rare genetic disorder caused by a dominant mutation in the ACVR1 gene (R206H, 97% of cases), leading to debilitating heterotopic ossification (HO) characterized by abnormal bone growth triggered by inflammatory flare-ups. Here, we report the development of disease-modifying, allele-selective small interfering RNA (siRNA) targeting ACVR1 R206H . Allele selectivity is essential as wildtype ACVR1 is crucial for many functions including skeletal homeostasis and development. When conjugated to docosanoic acid (DCA), administration of the fully modifi

Multifocal Skeletal Muscle Mineralisation Following Severe Trauma and Prolonged Critical Illness: A Case Report.

Soft tissue mineralisation is an uncommon complication of severe trauma and critical illness and may occur in association with metabolic derangements, tissue injury and prolonged immobilisation. We present a case of a 26-year-old male patient who developed extensive skeletal muscle calcification following multiple stab wounds, hypovolemic shock, and surgical interventions, including limb amputation and bowel resections. Initial imaging showed no calcification; however, six weeks post-injury, CT demonstrated extensive amorphous calcification in the abdominal wall, chest, iliopsoas, and gluteus

Hypophosphatasia: Results of a Country-Wide Selective Screening Program Using NGS Technology as a First-Tier Test.

Hypophosphatasia (HPP) is a rare hereditary metabolic disorder caused by nucleotide variants (NVs) in the ALPL gene, leading to a deficiency of tissue-nonspecific alkaline phosphatase (TNSALP). HPP has a variable clinical presentation (such as impaired mineralization of bones and teeth, respiratory dysfunctions, muscle weakness, muscle and bone pain, growth deficiency, nephrocalcinosis, seizures, encephalopathy, intracranial hypertension, and intellectual disability) overlapping with other disorders which often delays a correct diagnosis. In this study, we present the results of a two-center (

Revisiting Myositis Ossificans: A Comprehensive Stage-by-Stage Imaging Review.

Myositis ossificans (MO) is a benign, self-limiting heterotopic ossification process that typically develops within soft tissues following trauma, although non-traumatic forms have also been described. Despite its benign nature, MO frequently represents a diagnostic challenge, particularly in its early stages when imaging findings may mimic aggressive soft-tissue tumors, leading to unnecessary biopsies or surgical interventions. This narrative review provides an updated overview of the classification, pathophysiology, and imaging features of myositis ossificans, with a specific focus on the ti

Avulsion Fracture of the Lesser Trochanter and the Use of Conservative Treatment.

Avulsion fractures of the lesser trochanter of the femur are extremely rare injuries in the pediatric population, most commonly resulting from physical activity. The typical mechanism involves a sudden contraction of the iliopsoas muscle, which inserts on the lesser trochanter. We present a rare case of such an injury in a pediatric patient, emphasizing the importance of appropriate imaging and the effectiveness of conservative management in achieving full recovery. An 11-year-old boy presented to the emergency department with perineal trauma sustained after losing balance while riding a bicyc

A bioorthogonal nanoconcentrisome system for epigenetic and immunometabolic reprogramming of tissue regeneration in aged female rats.

Stem cell-based therapies show promise for tissue regeneration but are limited by therapeutic instability, particularly in aged tissues. Here, multi-omics profiling of regenerating bone reveals marked depletion of S-adenosylmethionine (SAM), indicating epigenetic dysregulation associated with impaired repair. To address this, we develop a spatially compartmentalized, fusogenic nanovesicle system (SMNV), termed a "nanoconcentrisome," composed of SAM-loaded fusogenic liposomes encapsulating mesenchymal stem cell-derived nanovesicles (MNV). This dual-compartment architecture facilitates cytosolic
