Muscle biopsy to end a diagnostic odyssey in a genetically complex case of spinal muscular atrophy type 3: the importance of "old school" in the next-generation sequencing era.

Spinal muscular atrophy (SMA) is an autosomal recessively inherited disease leading to severe and progressive muscle weakness due to loss of motor neurons in the spinal cord and the brainstem. Motor neuron degeneration is caused by the absence of a functional SMN1 gene copy, a gene located on 5q13.2, encoding the SMN ("survival of motor neuron") protein. Initially classified into five groups with varying severity (SMA type 0-4), SMA is now regarded as a progressive neurodegenerative disease with a continuum of clinical severity, which genetic and epigenetic modifiers can modulate. The highly h

Assessment of sarcopenia in patients with Takayasu arteritis: A cross-sectional observational study.

BACKGROUND: We aimed to evaluate muscle mass, muscle function, and sarcopenia prevalence in patients with Takayasu arteritis (TAK), and to explore associations with disease activity and vascular involvement. METHODS: A total of 103 TAK patients and 75 age- and sex-matched healthy controls (HC) were included. The muscle strength of the participants was assessed using handgrip and chair-stand tests, while muscle mass was evaluated through two separate measurement methods: bioelectrical impedance analysis (BIA) and ultrasonography (US). For US measurement, the quadriceps and the biceps muscle we

Muscle strength and HbA1c in geriatric patients with and without diabetes: a retrospective observational cohort study.

BACKGROUND/AIM: Handgrip strength (HGS) is a simple, reliable, and widely used biomarker of muscle strength and a key component in sarcopenia screening. Its potential role as an indicator of glycemic control has attracted increasing interest; however, the relationship between HGS and glycated hemoglobin (HbA1c) in older adults remains insufficiently understood, particularly in populations characterized by frailty and multimorbidity. This study aimed to explore the relationship between HGS and HbA1c levels in older adults with and without diabetes mellitus (DM) and to evaluate the potential of

Consensus-Defined Sarcopenic Obesity and a Related Dynapenic Obesity Phenotype in Relation to Early Cognitive Impairment.

BACKGROUND: Previous studies examining sarcopenic obesity in relation to cognitive function have utilized heterogeneous operational definitions. Some studies have evaluated low muscle strength combined with obesity, more appropriately described as dynapenic obesity, under the broader term sarcopenic obesity. We examined the associations of consensus-defined sarcopenic obesity and a related strength-based obesity phenotype with global and domain-specific cognitive function in older adults and evaluated the robustness of these associations across alternative adiposity definitions. METHODS: This

Comparative evaluation of the sarcopenia index and 5 related creatinine-cystatin C biomarkers in relation to all-cause mortality in older adults: A cohort study.

Several creatinine- and cystatin C-based biomarkers have been individually associated with mortality, but their comparative associations have rarely been evaluated head-to-head under a common analytic framework. We compared 6 related biomarkers in relation to all-cause mortality in older adults. In the China Health and Retirement Longitudinal Study 2015 to 2020 cohort, we studied 2987 men (428 deaths) and 2995 women (228 deaths) aged ≥60 years. Six related biomarkers (sarcopenia index [SI], predicted skeletal muscle index, total body muscle mass, creatinine-to-cystatin C ratio [CCR], eGFRcys(2

ROS- and iron ion-responsive gelatin modulates the pathological microenvironment to enhance myocardial infarction treatment.

Myocardial infarction (MI) generates a pathological microenvironment characterized by ischemia and hypoxia, and this increases mortality and morbidity worldwide. We fabricated p-hydroxyphenylpropionic acid-modified gelatin (GTN), a dual-responsive system capable of in situ gelation in the presence of hydrogen peroxide (H2O2) or iron ions. This feature enables GTN to cross-link in the infarcted myocardium, where excessive reactive oxygen species (ROS) and iron ions trigger ferroptosis. The successful modification of gelatin was verified, along with its time- and concentration-dependent responsi

Histopathological and biochemical effects of heteroatomic polycyclic aromatic hydrocarbons (Dibenzothiophene and 1-Nitropyrene) in the nerite snail Vitta zebra.

Sulfur- and nitrogen-derived polycyclic aromatic hydrocarbons (S- and N-PAHs) are a group of substances with little toxicological knowledge, but emerging in environmental samples. Thus, this study aimed to first evaluate the effects of exposure to dibenzothiophene (DBT) and 1-nitropyrene (1-NP) on antioxidant defense, oxidative damage, and histopathological markers in nerite snails (Vitta zebra). The animals were exposed to DBT and 1-NP at concentrations of 10, 50, and 100 μg/L and dissected to assess reduced glutathione (GSH) and the activity of glutamate-cysteine ligase (GCL), glutathione pe

A Mendelian randomization study reveals TNF-β as a potential protective factor against sarcopenia.

Sarcopenia, defined by the loss of muscle mass and strength, significantly impacts the elderly and is linked to chronic inflammation. The exact causal relationship between circulating inflammatory factors and sarcopenia is not yet clear. We employed a 2-sample Mendelian randomization analysis using 91 circulating inflammatory factors as exposures and 4 sarcopenia traits as outcomes to explore potential causality. Our analysis confirmed a significant causal association between circulating inflammatory factors and sarcopenia. Specifically, tumor necrosis factor-beta was significantly associated

Prodh2-Mediated Mitochondrial Stress Drives TNF-α-Induced Myoblast Dysfunction and Sarcopenia in COPD.

Sarcopenia in chronic obstructive pulmonary disease (COPD) is driven by elevated tumor necrosis factor-alpha (TNF-α), yet the underlying mechanism remains unclear. Here, we identify the mitochondrial metabolic enzyme Prodh2 as a pivotal mediator. Prodh2 was significantly upregulated in sarcopenic COPD patients and a corresponding mouse model. Mechanistically, TNF-α induced Prodh2 transcription in myoblasts via a p53-dependent pathway. This upregulation triggered excessive mitochondrial reactive oxygen species, leading to the release of mitochondrial DNA (mtDNA). Cytosolic mtDNA subsequently ac

Literature review of myasthenia gravis complicated by myocardial damage.

BACKGROUND: Myasthenia gravis (MG) is a chronic autoimmune disorder predominantly driven by pathogenic autoantibodies directed against postsynaptic acetylcholine receptors, impairing neuromuscular transmission. Although cardiovascular complications remain relatively uncommon among MG patients, their emergence markedly elevates the risk of fatal outcomes. This review synthesizes existing evidence to delineate the clinical manifestations of MG-associated myocardial damage, offering clinicians a comprehensive framework for early detection and evidence-based therapeutic intervention. METHODS: We

Association between sarcopenia and inflammation in non-geriatric patients with chronic kidney disease: a comparison of hemodialysis and non-hemodialysis patients.

BACKGROUND: Sarcopenia is an increasingly recognized complication of chronic kidney disease (CKD), contributing to impaired physical function and higher mortality. While most evidence derives from elderly populations, its frequency and inflammatory correlates in younger adults remain underexplored. This study aimed to investigate the prevalence of sarcopenia and its association with inflammatory markers in a non-geriatric CKD cohort, including both hemodialysis and non-dialysis patients. METHODS: This cross-sectional observational study included 128 non-geriatric CKD patients (18-65 years), 6

Current concepts review: Low-vs high-tone neuromuscular spinal deformities: An evolving distinction with clinical implications.

BACKGROUND: Neuromuscular scoliosis (NMS) was historically classified into two groups as neuropathic and myopathic, which offers limited benefit beyond describing the underlying pathology. More recently, NMS is being classified according to changes in muscle tone, which more accurately characterizes presenting features, and also helps emphasize the critical differences in management. CLINICAL PRESENTATION: High-tone NMS, with cerebral palsy (CP) as a typical example, is characterized by spasticity, rigidity, and often reduced flexibility. Low-tone NMS, as in spinal muscular atrophy (SMA) and

Prehabilitation and Patient Optimization for Cystectomy.

Prehabilitation is often defined as the process of enhancing or optimizing a patients' functional capacity preoperatively to improve tolerance of perioperative physiologic stressors. This is accomplished beginning with a standardized risk assessment using laboratory values, imaging, and validated questionnaires. Following this risk assessment, patients are referred to a team of multidisciplinary providers with a goal of optimizing nutritional status, exercise capacity, cessation of smoking, and enabling psychiatric preparedness for the stressors of radical cystectomy. Herein, we provide a crit

Targeting the ENO1 metabolic checkpoint via microenvironment-responsive nanotherapeutics promotes postinfarction cardiac repair.

Myocardial infarction (MI) triggers persistent inflammatory dysregulation, wherein macrophages undergo glycolytic reprogramming and adopt an inflammatory phenotype that impairs cardiac repair. However, the specific metabolic drivers underlying post-MI macrophage dysfunction remain unclear. Here, we identify enolase 1 (ENO1)-a glycolytic enzyme up-regulated in post-MI macrophages-as a critical regulator of macrophage metabolic fate and inflammatory function. To intervene, we engineered an acid-responsive nanocarrier (PECS) that externalizes phosphatidylserine within the acidic infarct microenvi

Excellent response to high-dose Riboflavin in Brown-Vialetto-Van Laere syndrome: A case report.

RATIONALE: Brown-Vialetto-Van Laere syndrome, currently recognized within the spectrum of riboflavin transporter deficiency type 3 (RTD3), is a rare inherited neurometabolic disorder associated with pathogenic variants in SLC52A3. It typically presents with progressive sensorineural hearing loss, pontobulbar dysfunction, peripheral neuropathy, and respiratory involvement. Diagnosis may be delayed because routine metabolic and neuromuscular investigations can be unrevealing despite a characteristic clinical phenotype. PATIENT CONCERNS: A young Asian man presented with several years of progress
