What is the evidence for abdominal and pelvic floor muscle training to treat diastasis recti abdominis postpartum? An updated systematic review and meta-analysis of randomized clinical trials and trial sequential analysis.

INTRODUCTION: This systematic review and meta-analysis seeks to evaluate the effectiveness of abdominal and pelvic floor muscle (PFM) exercise programs in the treatment of postpartum diastasis recti abdominis (DRA). Specifically, we aim to update the current evidence by reassessing the impact of exercise-based interventions on inter-recti distance (IRD). METHODS: A comprehensive online search was conducted using MEDLINE/PubMed, Cochrane Library, EMBASE, Web of Science, and LILACS, from inception until January 2025 without any filter applied. Our study followed the Preferred Reporting Items for

Inter-rectus distance and its relation to pelvic floor function in primiparous women in the remote postpartum period: A cross-sectional study.

Postpartum diastasis recti abdominis (DRA), characterized by increased inter-rectus distance (IRD), is common and linked to functional impairments, but its association with pelvic floor dysfunction (PFD) remains uncertain. This study investigated whether IRD correlates with pelvic floor muscle (PFM) function and examined the association between DRA and PFD in primiparous women during the remote postpartum period. This cross-sectional study included primiparous women 45 days to 12 months postpartum. The IRD and PFM function were assessed using standardized ultrasound protocols, with additional

[The innovative FRANCIS method-A case report : Combination of abdominoplasty and sublay repair with preservation of the umbilicus in postpartum rectus diastasis and umbilical hernia].

INTRODUCTION: Postpartum rectus diastasis and abdominal wall insufficiency frequently cause functional and aesthetic complaints, including impaired core stability, pain, postural disorders and psychosocial burden. Concomitant umbilical hernias often require surgical treatment. Although current guidelines recommend minimally invasive procedures with mesh reinforcement, many patients remain dissatisfied with the cosmetic outcome. Therefore, treatment requires a combination of functional reconstruction and aesthetic restoration. Preservation of the umbilical viability represents a particular surg

Risk factors and interaction analysis of severe diastasis recti abdominis within 6 months postpartum.

OBJECTIVES: Severe diastasis recti abdominis (DRA) in the postpartum period may lead to long‑term functional impairment in women. However, the independent effects and interactions of perinatal risk factors during the early postpartum recovery window have not been fully elucidated. This study aims to analyze the risk factors and interaction effects associated with severe postpartum DRA and to identify characteristics of high‑risk populations. METHODS: This retrospective cohort study included postpartum women who underwent postpartum rehabilitation treatment at Hengyang Maternal and Child Healt

[Rectus diastasis surgery : indications and new approaches].

Rectus abdominis diastasis is defined as a widening of the linea alba exceeding 2 cm. The European Hernia Society (EHS) provides a classification to guide its management. This article details the abdominal wall surgeon's perspective, particularly for symptomatic patients without major skin excess. Following a well-conducted physiotherapy, the rise of minimally invasive extraperitoneal techniques, particularly robotic approaches, offers an alternative to abdominoplasty in selected cases. By avoiding large subcutaneous dissections, these posterior approaches drastically reduce morbidity. The goa

Rectus Diastasis With Midline Hernias: Abdominal Wall Reconstruction With Autologous Tissue.

Surgical intervention is recommended for patients presenting with ventral hernias accompanied by rectus diastasis. The priority goals of reconstruction are to restore normal abdominal wall anatomy and to minimize the risk of recurrence. Despite the availability of various surgical techniques, recurrence remains a significant concern for surgeons. This report presents the authors' experience with abdominal wall reconstruction using autologous tissue. This surgical technique is indicated for patients presenting with a midline hernia with severe rectus diastasis (defined by an inter-rectus distan

When Does Rectus Diastasis Worsen the Most? A Pregnancy-Order Analysis of Midline Fascial Failure.

ABSTARACT: BACKGROUND: Diastasis of the rectus abdominis muscles (DRA) is common after pregnancy, yet little is known about its sequential progression across multiple gestations. While multiparity is a recognized risk factor for greater diastasis severity, it remains unclear whether midline widening increases linearly with each pregnancy or follows a nonlinear biomechanical pattern with specific points of maximal fascial failure. OBJECTIVES: To evaluate how maximal inter-rectus distance (IRDmax) changes after each pregnancy in women who have completed childbearing, and to determine whether di

Rectus diastasis is not a single entity: phenotype-specific controversies in surgical management.

PURPOSE: Rectus diastasis (RD) is increasingly treated by general, abdominal wall and plastic surgeons, but controversies persist because clinically distinct phenotypes are often analysed together. This critical narrative review re-examines surgical management of RD through a phenotype-specific framework. METHODS: PubMed, MEDLINE, Embase and Cochrane Library were searched for English-language studies, systematic reviews, meta-analyses, consensus statements and guidelines addressing adult RD management. Evidence was interpreted by phenotype, anatomical severity, associated hernia status, opera

Minimally Invasive Repair of Rectus Diastasis (MILA) with Preservation of the Umbilical Ligament: A Technical Description.

Diastasis rectus abdominis (DRA) is a frequent condition of the anterior abdominal wall, with functional and aesthetic impact, whose surgical treatment has progressively evolved to less invasive approaches. Among these, MILA (Minimally Invasive Lipoabdominoplasty) stands out for allowing the correction of diastasis endoscopically in the supra-aponeurotic plane. The present article details the MILA technique with umbilical ligament preservation, emphasizing its technical and anatomical aspects, as well as the selection criteria and the fundamental surgical steps for its reproducibility. This is

Long-term psychosocial benefits after combined abdominoplasty and breast surgery in postpartum women: A prospective 12-month study.

BACKGROUND: The combined surgical correction of abdominal wall laxity and breast ptosis-commonly referred to as the "mommy makeover"-has gained widespread popularity among postpartum women. Although esthetic benefits are well recognized, the procedure's long-term psychosocial impact remains insufficiently characterized. We prospectively evaluated the effects of combined body contouring surgery on health-related quality of life and psychological well-being in postpartum patients. METHODS: A prospective observational study was conducted at the Campus Bio-Medico University Hospital (Rome) betwee

A multitask deep learning framework for the automated quantification of abdominal tissue morphology on ultrasound and the therapeutic monitoring of diastasis recti abdominis.

BACKGROUND: Diastasis recti abdominis (DRA) requires accurate ultrasound-based quantification of abdominal tissue morphology, but manual measurement is operator-dependent and inefficient for longitudinal monitoring. This study developed and evaluated a multitask deep learning model for automated abdominal ultrasound analysis and the monitoring of magnetic stimulation therapy. METHODS: A retrospective dataset of 2,750 abdominal ultrasound images from 632 women was used to train a ResNet-50-encoded U-Net with dual branches for tissue segmentation (linea alba, rectus abdominis, and subcutaneous

Modified SCOLA Repair for Large Incisional Hernia With Rectus Diastasis.

BACKGROUND: Postoperative incisional ventral hernias (POIH) can sometimes be avoided and are challenging to repair. If the underlying rectus diastasis (RD) is left untreated, the attenuated linea alba predisposes to hernia formation and recurrence. Guidelines recommend concomitant repair of hernia and RD using mesh, but data on endoscopic subcutaneous onlay techniques for large or complex POIH defects remain scarce. This study reports long‑term outcomes of a modified subcutaneous onlay endoscopic approach (SCOLA) combined with bilateral rectus fascia release (RFR), triple‑action seroma prevent
