Wunderlich Syndrome in a Patient With Coexisting Giant Angiomyolipoma and Multicystic Clear Cell Renal Cell Carcinoma: A Case Report.

Angiomyolipoma (AML) is a benign mesenchymal renal neoplasm composed of dysmorphic blood vessels, smooth muscle cells, and adipose tissue. Although most AMLs are asymptomatic and incidentally detected, large tumors may present with spontaneous hemorrhage resulting in Wunderlich syndrome (WS), a rare urological emergency characterized by spontaneous nontraumatic renal bleeding. Renal cell carcinoma (RCC), on the other hand, is the most common malignant epithelial tumor of the kidney. The synchronous occurrence of AML and RCC within the same kidney is rare. We report the case of a 64-year-old wo

Transthoracic robot-assisted minimally invasive diaphragmatic rupture repair: a case report.

BACKGROUND: Nontraumatic diaphragmatic rupture is a rare condition with clinical presentations ranging from asymptomatic to severe respiratory or gastrointestinal symptoms. Owing to its nonspecific nature, it can be difficult to differentiate from other diaphragmatic disorders. CASE PRESENTATION: We present a case of a 35-year-old German woman with a right-sided nontraumatic diaphragmatic rupture, presenting with recurrent hypertensive crises and acute dyspnea. Although initially misdiagnosed, further clinical and radiological evaluations suggestive of diaphragmatic elevation led to the decis

ARC (Australian Rotator Cuff) trial: study protocol for a randomised placebo-controlled trial comparing rotator cuff repair to no repair during arthroscopic shoulder surgery for people with shoulder pain and non-acute rotator cuff tears.

BACKGROUND: Degenerative rotator cuff tears are common and are often treated with surgical repair. Randomised trials have not shown a clear advantage to surgery over non-surgical treatment, but there have been no published placebo-controlled trials investigating rotator cuff repair. This study aims to compare arthroscopic shoulder surgery with rotator cuff repair to surgery without rotator cuff repair (placebo) for improving shoulder pain and function in people with shoulder pain and full-thickness degenerative rotator cuff tears. METHODS: The study is a multicentre two-parallel arm, blinded,

Can radiologists and orthopaedic surgeons distinguish between traumatic and degenerative rotator cuff tears on MRI?

PURPOSE: MRI plays an important role in the evaluation of rotator cuff tears. Our study aimed to determine if specific MRI criteria can be used to distinguish between acute traumatic and pre-existing degenerative lesions. MATERIALS AND METHODS: The study included 40 MRIs or MR-arthrograms with rotator cuff tears. Twenty tears were traumatic and 20 were non-traumatic. Two radiologists specialized in musculoskeletal imaging, an experienced shoulder surgeon and a registrar trained in shoulder surgery assessed on each MRI the presence or absence of the following 11 parameters: tendon oedema, oede

Biceps lasso loop and self-locking tenodesis alongside repair of rotator cuff tears: randomised controlled trial study protocol (BLAST 1).

INTRODUCTION: Pathology of the long head of the biceps tendon (LHBT) frequently accompanies rotator cuff tears, with tenotomy and tenodesis often being used to address this pathology. While meta-analyses report comparable functional outcomes between these techniques, tenotomy is linked to higher rates of Popeye deformity, whereas tenodesis is more technically demanding and might involve extra material. A novel self-locking tenodesis technique aims to reduce deformity risk while being a simpler alternative to the conventional tenodesis procedure; however, comparative evidence is currently limit

Preliminary results of clinical, biochemical, and radiological investigation into the oxidative status in patients with rotator cuff tendinopathy.

We investigated oxidative status in patients with rotator cuff tendinopathy (RCT) and evaluated their relationship with radiological and clinical parameters. In this cross-section study, 88 patients with RCT (59 males and 29 females) and 86 healthy controls (66 males, 20 females) were enrolled. The sample consisted of nontraumatic patients who are suffering from shoulder pain because of rotator cuff disease, which was established by clinical tests and MRI scanning. Oxidative stress in patients with RCT was analyzed via the dynamic thiol/disulfide homeostasis (TDH). Thiol/disulfide homeostasis

Spontaneous longitudinal rupture of the thyroid cartilage: A management of the rare clinical case.

INTRODUCTION AND IMPORTANCE: Non-traumatic rupture or other injuries to the larynx are very rare disorder. According to the published series, there are only 15 cases reported with such kind of injury to the larynx. Despite the rarity of the non-traumatic larynx fracture, it is important to elucidate adequate management for the patients with such emergency. The aim of the study is to demonstrate the features of clinical manifestations, examination, and surgical treatment of a case of spontaneous longitudinal rupture of the thyroid cartilage. PRESENTATION OF CASE: A 54-year-old male patient pre

"Functional outcomes and MRI-based tendon healing after (antero-) superior rotator cuff repair among patients under 50 years: retrospective analysis of traumatic versus non-traumatic rotator cuff tears".

BACKGROUND: Rotator cuff tears among patients under 50 years either result from an adequate trauma or are considered non-traumatic due to work-related or athletic overuse. The impact of these different mechanisms on postoperative functional outcomes and tendon healing has not yet been fully understood. Therefore, it was the purpose of this study to investigate the influence of etiology of (antero-)superior rotator cuff tears on postoperative outcomes and the healing rates after arthroscopic rotator cuff repair in a young patient population. METHODS: Patients under 50 years who underwent arthr

No difference in clinical outcome after rotator cuff repair performed within or later than 3 months after trauma: a retrospective cohort study.

PURPOSE: Rotator cuff (RC) tear is one of the most common injuries of the shoulder. Patients with RC tears often report a trauma initiating shoulder pain and impaired function. The aim of this retrospective analysis of a prospectively registered cohort was to elucidate whether the time interval between the trauma and RC repair, using a cut off of 3 months, affects the functional outcome after 2 years. METHODS: In a single orthopedic unit, 819 consecutive patients were treated with rotator cuff repair during the period from 2010 to 2014 and 733 of the patients completed the Western Ontario Rot

Intramuscular hemorrhages in the pathway of an electric current through the body - two case reports.

Intramuscular hemorrhages at autopsy can have a variety of traumatic as well as non-traumatic causes, but their recognition in electrical deaths is almost a rarity. We report on two autopsy cases of electrical fatalities, the first relating to a portion of the right upper human extremity, consisting (only) of the forearm and hand, while the other case relates to a female child who died after a high voltage electrical shock. In both cases, layered dissection of the upper limb revealed fresh intramuscular hemorrhages in the skeletal muscles that could be topographically related to the path taken

Rotator Cuff-Related Shoulder Pain: Is It Time to Reframe the Advice, "You Need to Strengthen Your Shoulder"?

Progressive resistance exercise, in isolation or in combination with other noninvasive therapies such as therapeutic touch, is the first-line approach to managing nontraumatic rotator cuff-related shoulder pain (RCRSP). Resistance exercise may be effective for people with RCRSP secondary to improving mechanical features of the shoulder, including strength, kinematics, and muscle timing and activation. However, strength gains are often small and clinically unimportant when measured during clinical trials. In this Viewpoint, we argue that clinicians should (1) continue to prescribe resistance ex

Non-traumatic chronic shoulder pain is not associated with changes in rotator cuff interval tendon thickness.

OBJECTIVE: To determine whether the thickness of the rotator interval tendons is different when comparing both symptomatic and non-symptomatic sides in people with chronic shoulder pain, and to those free of pain. Furthermore, to calculate the level of association between the rotator interval tendon thicknesses and perceived shoulder pain-function. DESIGN: A cross-sectional, observational study. METHOD: The supraspinatus, subscapularis and biceps brachii tendon thickness of sixty two patients with chronic shoulder pain were determined from standardized ultrasonography measures performed on b

Protocol for a multi-centre pilot and feasibility randomised controlled trial with a nested qualitative study: rehabilitation following rotator cuff repair (the RaCeR study).

BACKGROUND: Shoulder pain is a highly prevalent complaint and disorders of the rotator cuff, including tears, are thought to be the most common cause. The number of operations repair the torn rotator cuff has risen significantly in recent years. While surgical techniques have progressed, becoming less invasive and more secure, rehabilitation programmes have remained largely like those initially developed when surgical techniques were less advanced and more invasive. Uncertainty remains in relation to the length of post-surgical immobilisation and the amount of early load permitted at the repai

Quadratus femoris tear as an unusual cause of hip pain: a case report.

PURPOSE: To show that quadratus femoris muscle partial or complete tear is an extremely rare if not underdiagnosed cause of hip pain. METHODS: We present the case of a 75-year-old Caucasian woman with spontaneous non-traumatic quadratus femoris tear. RESULTS: Computed Tomography showed quadratus femoris tear with occult underlying ischiofemoral impingement pathology. CONCLUSIONS: Quadratus femoris muscle is a hip external rotator and adductor and is situated along the posterior aspect of the hip joint and its proximity of anatomy with other external rotators leads to diagnostic pitfalls for

[Non-traumatic pathology of the shoulder: medical treatment].

Recently published meta-analyses emphasized weak methodology of clinical research evaluating medical treatment of non-traumatic shoulder pathology. Experts recommend medical treatment as the first step in the management of rotator cuff tendinopathies, the focus of this review. This step precedes surgical treatment of rotator cuff tear with the objective of recovering passive motion of the shoulder. It can be the unique therapeutic phase of rotator cuff tear or simple tendinopathy when clinical results match functional needs of the patient. Therefore, the physician prior treatment has to establ

[Reconstruction of the extensor pollicis longus tendon by transposition of the extensor indicis tendon].

Rupture of the extensor pollicis longus-tendon (EPL) is a frequent complication after distal radius fractures. Other traumatic and non-traumatic reasons for this tendon lesion are known, including a theory about a disorder in the blood supply to the tendon itself. We examined 40 patients after reconstruction of the EPL-tendon in a mean follow-up time of 30 months. All patients were clinically examined and a DASH questionnaire was answered by all patients. The method to reconstruct the EPL-tendon was the transposition of the extensor indicis-tendon. After the operations the thumb was put in a s

[Coraco-humeral space and rotator cuff tears].

PURPOSE OF THE STUDY: The anterior or subcoracoïd impingement is often mentioned but remains unprecise as far as clinical, radiological or even anatomical lesion are concerned. The purpose of our work was to study the different factors influencing the subcoracoïd space in case of cuff tear. METHODS: Our study was based on 206 shoulders operated for full-thickness rotator cuff tear. The SubCoracoïd Space (SPS), measured in millimeters on pre-operative arthro-CT-scan, was defined by the shortest distance between the coracoïd process and the humeral head.
