Literature review of myasthenia gravis complicated by myocardial damage.

BACKGROUND: Myasthenia gravis (MG) is a chronic autoimmune disorder predominantly driven by pathogenic autoantibodies directed against postsynaptic acetylcholine receptors, impairing neuromuscular transmission. Although cardiovascular complications remain relatively uncommon among MG patients, their emergence markedly elevates the risk of fatal outcomes. This review synthesizes existing evidence to delineate the clinical manifestations of MG-associated myocardial damage, offering clinicians a comprehensive framework for early detection and evidence-based therapeutic intervention. METHODS: We

Targeting the ENO1 metabolic checkpoint via microenvironment-responsive nanotherapeutics promotes postinfarction cardiac repair.

Myocardial infarction (MI) triggers persistent inflammatory dysregulation, wherein macrophages undergo glycolytic reprogramming and adopt an inflammatory phenotype that impairs cardiac repair. However, the specific metabolic drivers underlying post-MI macrophage dysfunction remain unclear. Here, we identify enolase 1 (ENO1)-a glycolytic enzyme up-regulated in post-MI macrophages-as a critical regulator of macrophage metabolic fate and inflammatory function. To intervene, we engineered an acid-responsive nanocarrier (PECS) that externalizes phosphatidylserine within the acidic infarct microenvi

A metabolic-epigenetic switch governs multicellular cardiac repair following succinate dehydrogenase inhibition.

Cardiac regeneration is limited in the adult mammalian heart because of restricted cardiomyocyte proliferation and persistent fibrosis. We previously demonstrated that transient pharmacological inhibition of succinate dehydrogenase (SDH) with malonate promotes regeneration after myocardial infarction. Here, we integrate single-nucleus RNA sequencing and ATAC-seq to reveal multicellular transcriptional and epigenetic reprogramming underlying this response. Cell-specific Sdhb deletion dissected contributions of SDH inhibition; cardiomyocyte-specific Sdhb deletion transiently increased cardiomyoc

Roles of GDF-15 in Acute Heart Failure: from Mechanistic Insights to Clinical Applications.

Acute heart failure (AHF) is a clinical syndrome characterized by the rapid or gradual onset of symptoms and signs of heart failure that are severe enough to require urgent evaluation and treatment. It may represent de novo heart failure or acute decompensation of pre-existing chronic heart failure. Natriuretic peptides remain the cornerstone biomarkers for diagnosis and risk assessment, but predominantly reflect hemodynamic stress and incompletely capture inflammation, oxidative injury, cellular damage, and multiorgan dysfunction. Growth differentiation factor-15 (GDF-15), a stress-responsive

[Cell Death of Cardiomyocytes in Heart Failure].

Heart failure, a leading cause of death worldwide, is generally defined as a pathophysiological condition in which the heart is unable to eject the arterial blood required by all tissues in the body. Heart failure is broadly classified into acute heart failure and chronic heart failure. In clinical practice, acute heart failure is often considered synonymous with acute myocardial infarction. In acute heart failure, cardiac contractile dysfunction is induced due to necrosis of cardiomyocytes during ischemia/reperfusion. In contrast, in chronic (congestive) heart failure, cardiac contractile dys

Neutrophil-mediated myocardial ischemia-reperfusion injury: mechanisms and potential therapeutic targets.

Although reperfusion therapy improves the prognosis of acute myocardial infarction, myocardial ischemia-reperfusion injury continues to limit its clinical benefits, with neutrophil-driven inflammatory cascades representing a central mechanism of injury amplification. This article delineates the molecular event chain of neutrophils from recruitment to effector execution: activated neutrophils damage cardiomyocytes through three principal pathways-respiratory burst, degranulation, and neutrophil extracellular traps formation-thereby establishing a positive feedback loop that exacerbates inflamma

Self-Growing Conductive Hydrogels Establish Volumetric Biointerfaces for Cardiac Conduction Restoration.

Restoring three-dimensional electrical conduction in infarcted myocardium remains a critical challenge, as conventional conductive hydrogel patches largely remain surface-confined and prevent electrical coupling of residual cardiomyocytes within fibrotic scars. Here, we present a self-growing conductive volumetric interface (SCOVE) that transforms surface-confined biointerfaces into tissue-integrated, three-dimensional conductive networks. SCOVE is delivered as an injectable hydrogel precursor containing the tissue-permeable conductive monomer 3,4-ethylenedioxythiophene-acetic acid sodium salt

Severe Tetanus in an Older Adult Without an Identifiable Portal of Entry: A Case Report.

Tetanus remains a significant health concern, particularly in older adults with waning immunity. The diagnosis is typically clinical, based on characteristic features such as trismus, muscle rigidity, and stimulus-triggered spasms. However, diagnostic challenges arise when tetanus presents without an identifiable acute wound or with an occult portal of entry, as it may be misattributed to more common neurological conditions such as stroke or encephalitis. We report a 65-year-old Chinese male who presented with 15-day cough and sputum, followed by acute trismus and dysphagia. Initial evaluation

Multiparametric Cardiac Magnetic Resonance Characterization of the Noninfarcted Myocardium: Definition, Assessment, and Clinical Significance.

Acute myocardial infarction (AMI) remains a major cause of cardiovascular illness and death. Advanced imaging techniques, particularly cardiac magnetic resonance (CMR), have become essential in assessing tissue alterations within the infarcted zone. The CMR published reports focus primarily on evaluating infarct size and the peri-infarct region in the context of AMI. Recent evidence highlights that the noninfarcted myocardium (NIM) surrounding the infarct sustains functional, structural, and inflammatory changes due to ischemic injury. CMR can detect significant changes in the NIM, such as inf

Exercise modalities associated with reduced functional disability in nonspecific neck pain: a network meta-analysis and exploratory dose-response analysis.

OBJECTIVE: At present, the incidence of acute myocardial infarction is increasing year by year, and it has become one of the diseases with the highest mortality rate in humans. Myocardial ischemia-reperfusion injury (MIRI) is a major problem in the treatment of myocardial infarction, but clinically there is no effective way to treat MIRI. This study used Cystatin C (Cys C) to treat cardiomyocytes and rats to investigate the effect of Cys C on MIRI. MATERIALS AND METHODS: We used H2O2 to induce rat cardiomyocytes (H9c2 cells) injury and stimulated the cells with Cys C. Cell counting kit 8 (CCK

Multimodal assessment of exceptional responders refines subtyping-directed precision treatment in HR+/HER2- breast cancer.

BACKGROUNDWe previously defined 4 hormone receptor+/human epidermal growth factor receptor 2- (HR+/HER2-) subtypes: SNF1 canonical luminal, SNF2 immunogenic, SNF3 proliferative, and SNF4 RTK driven. The FASCINATE-N proof-of-concept trial demonstrated that subtyping-directed precision treatment improves pathologic complete response in locally advanced HR+/HER2- breast cancer, yet efficacy remains suboptimal.METHODSWe integrated transcriptomic, genomic, digital pathology, and clinical data from this trial (n = 230) and focused on exceptional responders to explore opportunities for refining treat

Isn't it ironic? Functional iron deficiency at the core of Parkinson's disease pathobiology.

Myocardial infarction is a cardiovascular disease with high mortality. Human umbilical cord mesenchymal stem cells (hUC-MSCs) with strong self-renewal capacity and multipotency, provide the possibility of replacing injured cardiomyocytes. hUC-MSCs were cultured on polyacrylamide hydrogels with stiffnesses corresponding to Young's modulus of 13-16kPa and 62-68kPa which mimic the stiffnesses of healthy heart tissue and fibrotic myocardium. The expression of early myocardial markers Nkx2.5, GATA4, Mesp1 and the mature myocardial markers cTnT, cTnI, α-actin were detected by RT-PCR and Western Blot

Dual-Target Cardiac Regeneration: Controllable Nfyb/Nr3c1 Cointervention Spurs Cardiomyocyte Cytokinesis After Myocardial Infarction.

BACKGROUND: Myocardial infarction can cause a massive loss of functional cardiomyocytes, yet effective strategies to stimulate cardiac regeneration remain lacking. A key barrier to adult cardiomyocyte proliferation appears to be cytokinesis inhibition. This study aimed to determine whether combining proliferation stimulators with the removal of cytokinesis-inhibitory constraints could unlock regenerative potential after myocardial infarction. METHODS: Transcriptomic profile from 5 regeneration models, including Aurkb-tdTomato cytokinesis reporter mice and Myh6-MerCreMer;mosaic analysis with d

Frailty exacerbates left atrial remodeling and dysfunction through activation of AIM2 and NLR inflammasome-dependent cardiomyocytes pyroptosis after myocardial infarction in aged mice.

Myocardial infarction (MI) is a major driver of left atrial dysfunction and structural remodeling, substantially increasing the risks of heart failure and stroke. Frailty, a common geriatric syndrome, has been linked to adverse cardiovascular outcomes, but its mechanistic contribution remains poorly defined. This study examined the impact of frailty on left atrial remodeling after MI in aged mice and explored the underlying mechanisms. Frailty was modeled in 24-month-old IL-10 knockout mice, and MI was induced by permanent ligation of the left anterior descending coronary artery. Cardiac struc

Ubiquitin-specific protease 11 and YY1 mediate myocardial ischemia-reperfusion injury in myocardial infarction by regulating the ubiquitination and transcription of voltage-dependent anion channel 1.

BACKGROUND: Myocardial Ischemia-Reperfusion Injury (MIRI) is a key pathological link in the treatment of myocardial infarction (MI), leading to further necrosis of cardiomyocytes and deterioration of cardiac function. Abnormal expression and function of voltage-dependent anion channel 1 (VDAC1) has been confirmed to be closely related to MIRI. This study aims to explore the effects and mechanisms of VDAC1 in MI development. METHODS: By establishing an in vitro cardiomyocyte hypoxia-reoxygenation (H/R) model and an in vivo rat model, and combined with flow cytometry, fluorescent probe assay, W

Plicamycin protects against myocardial ischemia-reperfusion injury by activating the CREB-BDNF axis in mice.

Plicamycin (PLI) is a small-molecule aureolic acid antibiotic that has been approved for cancer treatment since the 1970s. However, its interaction with cardiovascular biological systems during myocardial ischemia-reperfusion injury (MIRI) remains largely unknown. This study examines its cardioprotective effects and the mechanisms associated with cAMP response element-binding protein (CREB) and brain-derived neurotrophic factor (BDNF) in a murine MIRI model. PLI, administered intraperitoneally to mice at 150 μg/kg/day for seven days before cardiac ischemia-reperfusion, attenuated the rise in m
