Dysregulation of YTHDF2 promotes the over expression of interferon beta in anti-MDA5 antibody-positive dermatomyositis.

BACKGROUND: Dysregulation of N6-methyladenosine (m6A) has been implicated in the pathophysiology of various autoimmune diseases. However, its role in dermatomyositis (DM), particularly in cases associated with anti-MDA5 antibodies, remains unclear. This study aimed to elucidate the potential involvement of m6A modifications of mRNAs in the pathogenesis of anti-MDA5+DM. METHODS: We assessed mRNA m6A methylation levels in peripheral blood mononuclear cells (PBMCs) from anti-MDA5+DM patients and healthy controls (HC) using LC-MS/MS assay. Quantitative reverse transcription-polymerase chain react

Kikuchi-Fujimoto Disease as the Initial Presentation of Dermatomyositis.

BACKGROUND: Kikuchi-Fujimoto Disease (KD) is a rare necrotizing lymphadenitis that is often self-limiting but occasionally associated with autoimmune conditions. Dermatomyositis is an autoimmune inflammatory myopathy that typically presents with proximal muscle weakness and a characteristic skin rash. In this report, we present a unique case of KD, the first documented association worldwide as the initial manifestation of dermatomyositis, highlighting the rarity and novelty of this case. CASE PRESENTATION: A 30-year-old Saudi female presented with a persistent fever, cervical lymphadenopathy,

Anti-signal recognition particle immune-mediated necrotising myopathy in an octogenarian.

We report a woman in her early 80s who presented with rapidly progressive proximal muscle weakness and functional decline and was diagnosed with anti-signal recognition particle (SRP) immune-mediated necrotising myopathy. Given the recognised aggressive phenotype of anti-SRP-associated disease, she received standard induction and early combination immunosuppressive therapy. Despite appropriate treatment, recovery was limited, with persistent severe weakness and progressive dysphagia, and she died at home 3 months after discharge. This case highlights that in very frail older adults with severe

Characteristics, treatments and outcomes of patients with dermatomyositis using real-world data.

OBJECTIVE: Studies of dermatomyositis (DM) are frequently limited to single-centre cohorts. We used two large nationally representative US cohorts to conduct a descriptive epidemiological study of the characteristics, treatments and outcomes of patients with incident DM. METHODS: This retrospective study identified two DM inception cohorts using (1) commercial claims and (2) electronic health record (EHR) data from the Excellence Network in Rheumatology to Innovate Care and High-impact research (ENRICH), a community rheumatology practice-based research network. Patient characteristics, treatm

Association between physical activity and all-cause mortality in rheumatoid arthritis: evidence from NHANES and UK Biobank cohort studies.

IMPORTANCE: Rheumatoid arthritis (RA), a chronic autoimmune disease linked to higher mortality, benefits from physical activity (PA), which has been shown to reduce disease activity and improve physical function. However, PA’s association with all-cause mortality in RA patients remains unclear. OBJECTIVE: To investigate the relationship between various levels of PA and all-cause mortality among patients with RA. DESIGN, SETTING, AND PARTICIPANTS: This dual-cohort observational study used data from the National Health and Nutrition Examination Survey (NHANES) from 2007 to 2018 and the UK Bioban

The Role of Imaging Techniques in the Evaluation of Extraglandular Manifestations in Patients with Sjögren's Syndrome.

Sjögren's syndrome is a chronic autoimmune disease marked by lymphocytic infiltration of the exocrine glands and the development of sicca symptoms, yet some patients also develop extraglandular involvement. Imaging has become relevant for describing these systemic features and supporting clinical assessment. This review discusses the roles of ultrasonography, elastography, computed tomography, and magnetic resonance imaging in evaluating multisystem disease associated with Sjögren's syndrome. Ultrasonography and elastography help assess muscular involvement by showing changes in echogenicity a

The Impact of the COVID-19 Pandemic on the Clinical Course of Influenza in Hospitalised Children in the Years 2017-2025.

BACKGROUND: The COVID-19 pandemic substantially altered the epidemiology of respiratory infections. Its impact on the clinical course of influenza in hospitalised children remains insufficiently characterised. OBJECTIVES: We aimed to compare the clinical course, complications, and selected laboratory parameters of influenza in children before, during, and after the COVID-19 pandemic. METHODS: This single-centre retrospective study included 553 children hospitalised with laboratory-confirmed influenza between September 2017 and August 2025. Patients were divided into three groups: pre-pandemi

Immune Checkpoint Inhibitor-Associated Myocarditis: Risk, Diagnosis, and Clinical Impact.

Background: Immune checkpoint inhibitors (ICIs), such as anti-programmed death (PD)-1 and anti-cytotoxic T-lymphocyte-associated protein (CTLA)-4 agents, have revolutionized oncology but are associated with immune-related adverse events (irAEs). Among these, ICI-associated myocarditis (ICI-M) is a rare but life-threatening complication, with mortality rates ranging from 27% to 50%. Objective: This narrative review summarizes the pathogenesis, epidemiology, clinical presentation, diagnostic methods, and management strategies for ICI-induced myocarditis, specifically highlighting emerging biomar

Evaluation of a screening algorithm to detect systemic sclerosis-related myopathy.

OBJECTIVES: Prevalence of muscle involvement in systemic sclerosis (SSc) ranges from 6% to 96%. If inflammatory myopathy (IM) is present, early diagnosis enables timely treatment; however, there is no standardized approach to detection. We evaluated a screening algorithm for SSc-related muscle involvement. METHODS: Consecutive SSc patients in the Australian Scleroderma Cohort Study (ASCS) were assessed annually for clinical features of myopathy [proximal weakness and elevated creatine kinase (CK)] during routine care, with further investigations performed according to physician judgement. Anesthetic and airway management for inguinal hernia repair in a pediatric patient with fibrodysplasia ossificans progressiva: a case report.

BACKGROUND: Fibrodysplasia ossificans progressiva is a rare genetic disorder characterized by the progressive heterotopic ossification of soft connective tissues, ultimately leading to cumulative disability and ankylosis. Airway management during general anesthesia in patients with fibrodysplasia ossificans progressiva is particularly challenging because of anatomical abnormalities and the risk of triggering flare-ups due to minor trauma. However, reports on the anesthetic management of pediatric patients remain limited. CASE PRESENTATION: We present the case of a 10-year-old Japanese girl wi

Clinical Characteristics and Evolution of Interstitial Lung Disease in Subtypes of Idiopathic Inflammatory Myositis With Prevalent Lung Manifestation: A Retrospective Analysis.

OBJECTIVE: Interstitial lung disease (ILD) is common in idiopathic inflammatory myositis (IIM), particularly in antisynthetase syndrome (ASyS), antimelanoma differentiation-associated protein 5 (anti-MDA5) syndrome, and scleromyositis. ILD can progress despite resolution of extrapulmonary symptoms, termed postmyopathic progressive pulmonary fibrosis (PmPPF). We outlined ILD trajectories in these subgroups of IIM, focusing on PmPPF. METHODS: A retrospective review of patients with IIM (ASyS, anti-MDA5+ dermatomyositis [DM], scleromyositis) from a British Columbia cohort (2019-2024) assessed de

Thrombotic microangiopathy in a patient with anti-signal recognition particle antibody-positive immune-mediated necrotising myopathy: a case report.

Thrombotic microangiopathy (TMA) is a rare but life-threatening complication in idiopathic inflammatory myopathies. Although TMA has been increasingly recognised in anti-melanoma differentiation-associated gene 5 antibody-positive dermatomyositis, it remains exceptionally uncommon in anti-signal recognition particle (SRP) antibody-positive immune-mediated necrotising myopathy (IMNM). We describe a 62-year-old woman with anti-SRP antibody-positive IMNM who developed rapidly progressive TMA shortly after the initiation of immunosuppressive therapy, including tacrolimus. At presentation, she exhi

Phospho-MLKL Upregulation in Immune-Mediated Necrotizing Myopathy: Implications for Disease Pathophysiology and Diagnosis.

Immune-mediated necrotizing myopathy (IMNM) is a subtype of idiopathic inflammatory myopathies (IIMs) characterized by severe muscle weakness and refractory response to immunotherapy. Recent studies proposed a role for necroptosis in IMNM-associated myonecrosis. This study aims to investigate the potential diagnostic value of phospho-mixed lineage kinase domain-like protein (pMLKL), the executive molecule of necroptosis, for IMNM. Sero-positive and -negative IMNM patients and various myopathies with myonecrosis diagnosed in Huashan Hospital during October 2020 to October 2022 were enrolled in

Lighting up the muscles: 18F-FDG PET/CT in rare disseminated tuberculous myositis.

Tuberculous pyomyositis is a rare and often under-recognized extrapulmonary manifestation of tuberculosis, presenting with non-specific symptoms such as fever and abscess, that may delay diagnosis. We report a unique case of disseminated tuberculous pyomyositis in a 56-year-old male with underlying chronic inflammatory arthritis. The diagnosis was confirmed using CBNAAT, line probe assay (LPA), and histopathology. Currently MRI is the standard imaging modality in use for imaging pyomyositis. 18F-FDG PET/CT can play a crucial role in detecting widespread metabolically active lesions involving m

Variant Resolution Through RNA Testing and Affected Tissue Analysis in the Neurofibromatoses: A Case Series.

OBJECTIVES: This series reports 3 unrelated individuals with features of neurofibromatosis type 1 (NF1) and/or schwannomatosis (SWN) in whom variants of uncertain significance (VUSs) were resolved through RNA testing and tissue analysis, highlighting the clinical impact of these tests. METHODS: Each patient's genetic testing identified a VUS that could have explained their clinical presentation. Because diagnostic criteria were not met, we performed VUS resolution. For patient 1, we conducted tissue analysis and RNA-based Sanger sequencing. For patients 2 and 3, RNA-based Sanger sequencing wa

Myocardial involvement in inflammatory myopathies: from subclinical disease to life-threatening myocarditis.

INTRODUCTION: Inflammatory myopathies (IM) are a heterogeneous group of systemic disorders characterized by chronic muscle inflammation. Cardiac involvement has historically been underrecognized, partially because of its subclinical presentation, and is associated with a poor prognosis, underscoring the urgent need for effective diagnostic strategies and therapeutic approaches. Cardiac magnetic resonance and its new mapping techniques appear as one of the most valuable instruments to detect myocardial involvement, thereby limiting the need for an endomyocardial biopsy when the underlying cause

Atypical Interplay of Dermatomyositis, Metastatic Caecal Carcinoma, and Connective Tissue Disease.

This report presents the case of a 71-year-old Chinese female with atypical symptoms leading to the diagnosis of metastatic caecal adenocarcinoma with overlapping connective tissue disease, initially identified as possible paraneoplastic dermatomyositis. The patient, with no significant medical history, was referred for elevated serum creatine kinase (CK) levels and exhibited progressive lethargy, upper limb weakness, and notable weight loss over three months. Clinical examination revealed violaceous rashes and significant muscle weakness, prompting a provisional diagnosis of dermatomyositis.
