BACKGROUNDS: Anti-MDA5 antibody-positive dermatomyositis (DM) is characterized by the presence of anti-MDA5 antibodies. We aimed to assess longitudinal changes of anti-MDA5 antibody levels and long-term outcomes in 6-month survivors of anti-MDA5 + DM with interstitial lung disease (ILD).
METHODS: Anti-MDA5 antibody titers were measured using enzyme-linked immunosorbent assay upon newly diagnosed anti-MDA5 + DM-ILD patients. Patients with follow-up beyond 6 months and at least two anti-MDA5 antibody measurements were subsequently subjected to group-based trajectory analysis by utilizing latent

A wild, male American crow (Corvus brachyrhynchos) nestling found dead in a pond in San Diego, CA, USA, was submitted for postmortem examination. Autopsy revealed that the chick was markedly underweight for the presumed age with no grossly appreciable fat stores. The primary histologic finding was lymphocytic vasculitis throughout the cerebrum and optic tectum, with lymphohistiocytic meningoencephalitis. The areas of inflammation were consistently associated with myriad, argyrophilic, curved bacilli with a characteristic "seagull" morphology. Additional significant findings included severe uni

BACKGROUND: Myositis-associated interstitial lung disease (MAILD) is one of the most severe complications of idiopathic inflammatory myopathy, characterised by rapidly progressive pulmonary fibrosis and high mortality. Treatment options are limited, and mechanisms driving epithelial-mesenchymal transition (EMT) in MAILD are incompletely understood. Anlotinib, a multitarget tyrosine kinase inhibitor, shows potential in fibrotic diseases; however, its role and mechanism in MAILD need clarification.
METHODS: To assess anlotinib's therapeutic effects, we established a MAILD mouse model and a neut

BACKGROUND Melanoma differentiation-associated gene 5 (MDA5) is an intracellular sensor that detects double-stranded RNA viruses and triggers type I interferon pathways. Anti-MDA5 dermatomyositis (MDA5-DM) is an uncommon autoimmune subtype, often linked to rapidly progressive interstitial lung disease (RP-ILD) and sometimes presenting without obvious muscle weakness. It can appear in pulmonary, cutaneous-articular, or vascular forms, with RP-ILD carrying the worst prognosis. We describe 3 Emirati patients with MDA5-DM, each with a different clinical picture, organ involvement, and outcome. CAS

Autoimmune diseases are characterized by systemic inflammation that can affect multiple tissues. In idiopathic inflammatory myopathies (IIM), skeletal muscle is primarily involved; however, subclinical cardiac dysfunction may also occur. While left ventricular ejection fraction (EF) is commonly used to assess cardiac function, global longitudinal strain (GLS) has proven more sensitive in detecting early myocardial impairment. This study aimed to evaluate left ventricular GLS (LV GLS) in patients with IIM and no known cardiovascular disease, assessing both the prevalence of reduced GLS values a

Dermatomyositis (DM) is a systemic inflammatory disease predominantly affecting the skin and muscles, with potential involvement of internal organs. Despite multiple therapeutic options, treatment response may be unsatisfactory. Type-I interferon (IFN) signaling pathways have been proposed as key drivers of disease pathogenesis. Consequently, treatment with anifrolumab, a monoclonal antibody targeting IFN receptor-1, may offer therapeutic benefit. To date, only a few cases of refractory DM successfully treated with anifrolumab have been reported. We present two cases of refractory DM treated w

INTRODUCTION: This is a unique case of fulminant respiratory failure secondary to a rare cause of rapidly progressing ILD; antisynthetase syndrome (ASS). Failure to deliver timely multi-modal treatment in these cases can lead to increased morbidity and mortality.
CASE PRESENTATION: A previously healthy 27-year-old male presented to his local hospital with a 1-week history of malaise, shortness of breath and cough. Initial work up including bloods and imaging were suggestive of community acquired multi lobar pneumonia, for which he received treatment as per local guidelines. Unfortunately, des

BACKGROUND: Patients with anti-melanoma differentiation-associated gene 5 (MDA5) antibody-positive dermatomyositis (DM) frequently develop rapidly progressive interstitial lung disease and may also exhibit hepatic dysfunction, yet the mechanisms of hepatic injury remain poorly defined. We investigated the roles of M2-like macrophages and complement activation in hepatic injury associated with anti-MDA5 antibody-positive DM.
METHODS: Liver specimens from five autopsy cases of anti-MDA5 antibody-positive DM were examined for the presence of CD80-positive M1-like and CD206-positive M2-like macro

OBJECTIVE: Dysregulated type I interferon (IFN) signaling contributes to autoimmune myositis pathogenesis. We investigated the therapeutic effects of JAK inhibitors in two mouse models. We also examined how type I IFNs affect muscle vasculature.
METHODS: Myositis was induced in major histocompatibility complex class I double transgenic ([TRE-H-2Kb (H mice)/MCK-tTA (T mice)] (HT)) female mice at day 21. Mice were randomized into four groups (n = 10-11/group): baricitinib (10 mg/kg), tofacitinib (20 mg/kg), vehicle, and healthy controls. Outcomes included survival, weight, muscle strength, hist

This case describes anti-nuclear matrix protein-2 (NXP-2) antibody-positive dermatomyositis sine dermatitis (DMSD) with an acute onset and marked creatine kinase (CK) elevation, closely mimicking drug-induced rhabdomyolysis. The patient lacked a skin rash and initially improved with hydration, but later developed respiratory failure, leading to a diagnosis via a muscle biopsy and antibody testing. Notably, the imaging findings also resembled rhabdomyolysis, further complicating differentiation. This case highlights that when anti-NXP-2-positive myositis presents as DMSD with acute, transient C

BACKGROUND AND AIMS: Inflammatory myopathies (IMs) are a group of autoimmune diseases characterized by progressive symmetric muscle weakness and various extramuscular manifestations. Cardiac involvement in IM has been associated with worse outcomes, but evidence to support specific screening and management algorithms for cardiac comorbidities in IM is still limited.
METHODS: For this observational study, 37 adult IM patients recruited from the rheumatology outpatient clinic at Ludwig-Maximilians-University Hospital between August 2023 and January 2025 completed a questionnaire on IM character

Autoimmune-mediated connective tissue diseases such as antisynthetase syndrome (ASyS) and systemic sclerosis (SSc) have a high unmet medical need. Here we report on treatment under compassionate use with the CD19×CD3 T cell engager (TCE) blinatumomab and the BCMA×CD3 TCE teclistamab in five patients with treatment-refractory ASyS and in five patients with treatment-refractory SSc, respectively. Induction therapy with blinatumomab or teclistamab reduced target cells in affected muscle and skin, respectively, and decreased autoantibody titers. Blinatumomab induced rapid clinical, serological and

BACKGROUND: Antisynthetase syndrome (ASyS) is an idiopathic inflammatory myopathy characterized by interstitial lung disease (ILD), mechanic’s hands, fever, arthritis, and Raynaud’s phenomenon. Although several domestic and international reports have described the clinical manifestations of ASyS, regional differences in clinical characteristics, including skin involvement, remain poorly understood. This study aimed to compare the regional differences in clinical features of ASyS by comparing data of Japanese patients with ASyS with those of previously published cohorts. METHODS: We conducted a

BACKGROUND: Idiopathic inflammatory myopathies (IIMs) are a group of systemic disorders characterized by chronic autoimmune dysregulation; their hallmark is symmetric proximal muscle weakness, often with characteristic cutaneous eruptions in dermatomyositis subtypes. Anti-nuclear matrix protein-2 (NXP-2) antibodies are classified as myositis-specific autoantibodies, yet their prevalence remains persistently low. Adult dermatomyositis associated with NXP-2 positivity exhibits a distinctive phenotype in terms of clinical manifestations, complications, and prognosis-particularly an elevated malig

Antisynthetase syndrome is a rare autoimmune disorder characterized by a range of clinical features, most commonly associated with autoantibodies targeting aminoacyl-tRNA synthetases. The condition typically presents with a combination of symptoms, including myositis, arthritis, Raynaud's phenomenon, "mechanic's hands," fever, and interstitial lung involvement. Pulmonary manifestations, particularly interstitial lung disease (ILD), are significant prognostic determinants. Management often involves a combination of immunosuppressive agents to address both muscular and respiratory complications.

Duchenne muscular dystrophy (DMD) is an X-linked neuromuscular disorder most commonly diagnosed between four and five years of age, while diagnosis during infancy remains uncommon due to initially normal neurological examinations and delayed onset of motor weakness. Although dystrophin deficiency affects cardiac muscle from birth, clinically apparent cardiac involvement is generally considered a later manifestation, rendering echocardiographically evident structural cardiac abnormalities during infancy rare. We report a male infant with normal early motor development who initially presented at

OBJECTIVES: Autoantibodies targeting melanoma differentiation associated protein 5 (MDA5) are strongly associated with dermatomyositis (DM) and may contribute to its pathogenesis. Here we aimed to investigate MDA5+ B cells, their phenotype and generate MDA5 monoclonal antibodies to assess their epitope specificity.
METHODS: MDA5-reactive B cells were captured from peripheral blood of patients with anti-MDA5+ DM (n = 3) using an MDA5-fluorescent probe. B cell receptor (BCR) sequences were analysed from single-sorted B cells (n = 240). Selected clones were re-expressed as IgG1 monoclonal antibo

OBJECTIVE: Depression and anxiety are prevalent comorbidities in patients with inflammatory-rheumatic diseases (IRD). The presence of a mental health condition usually contributes to an increased disease burden and poorer clinical outcomes. Despite their high relevance, symptoms of depression and anxiety often remain unrecognized by physicians and therefore untreated. The aim of our study was to estimate the frequency of depressive and anxiety symptoms and the gap between self-reported symptoms and previously diagnosed psychiatric conditions.
METHODS: Between February and July 2024, 1000 pati
