COVID-19 vaccination and systemic autoimmune rheumatic diseases: No evidence of disproportionately increased reporting in VAERS.

OBJECTIVES: To investigate whether systemic autoimmune rheumatic diseases (SARDs) were disproportionately reported as adverse events following COVID-19 vaccination compared with other vaccines, using data from the Vaccine Adverse Event Reporting System (VAERS). METHODS: We conducted a retrospective disproportionality analysis of VAERS reports collected between December 2020 and December 2024. Reports were identified using a structured query based on MedDRA terms for specific SARDs, including polymyalgia rheumatica, giant cell arteritis, systemic lupus erythematosus, systemic sclerosis, Sjögre

The Acid sphingomyelinase/ceramide System in Idiopathic Inflammatory Myopathies: a Potential Treatment Target.

Acid sphingomyelinase (ASM) is essential for the hydrolysis of sphingomyelin, leading to the production of ceramide (CE), a compound that plays a significant role in numerous physiological and pathological processes. Extensive researches have revealed the significant interactions and impacts of the ASM/CE system on numerous signaling pathways and diseases. Idiopathic inflammatory myopathies (IIM) are a distinctive group of autoimmune disorders characterized primarily by ongoing inflammation of the muscles and muscle weakness. A deeper exploration of the ASM/CE system and the mechanisms behind

Spatial protein expression patterns across pathologically-associated fibers revealed molecular specialization in inclusion body myositis.

OBJECTIVES: In Inclusion Body Myositis (IBM), myofibers undergo structural and functional changes, including increased regeneration, atrophy, and fibrosis. The molecular mechanisms driving pathologically -associated fibers (PAF) remain poorly understood. METHODS: We developed a myofiber-level proteomic workflow to identify protein signatures of three PAF subtypes. Laser-capture microdissection mass spectrometry of immunolabeled cryosections was performed, complemented by immunofluorescence and electron microscopy validation. RESULTS: Regenerating fibers expressing embryonic myosin heavy chain

A case of immune-mediated myocarditis after durvalumab/tremelimumab treatment for unresectable hepatocellular carcinoma.

Immune checkpoint inhibitors (ICIs) are important for the treatment of unresectable hepatocellular carcinoma (HCC). Furthermore, severe immune-mediated adverse events (imAEs) can be life threatening. We herein report a case of myocarditis and myositis after ICI treatment. A male patient with unresectable HCC was treated with durvalumab and tremelimumab. Three weeks later, the patient presented to the emergency department with syncope. Electrocardiography revealed a complete atrioventricular block. Therefore, immune-mediated myocarditis was suspected. High-dose corticosteroid therapy was initia

Macrophagic myofasciitis associated with transferrin receptor 2 and solute carrier family 40 member 1 variants and iron dysregulation: a case report.

BACKGROUND: Macrophagic myofasciitis (MMF) is a rare inflammatory muscle disease linked to the long-term persistence of aluminum-based adjuvants in macrophages. Its clinical spectrum includes fatigue, myalgia, and cognitive impairment, and genetic susceptibility has been suggested. To our knowledge, this is the first reported case suggesting an association between MMF, mild yet clinically relevant iron dysregulation, and variants in key iron regulatory genes, including transferrin receptor 2 (TFR2) and solute carrier family 40 member 1 (SLC40A1), also known as ferroportin. CASE PRESENTATION:

Clinical characteristics and therapies of multi-organ immune related adverse events.

Immune checkpoint inhibitors (ICIs) activate the immune system by blocking PD-1, CTLA-4, and PD-L1, thereby inducing autoimmune-mediated adverse reactions known as immune-related adverse events (irAEs). When two or more organs are involved, this condition is defined as multi-organ immune-related adverse events (multi-organ irAEs). Patients with multi-organ irAEs account for approximately 20%-30% of all irAE cases; however, clinical research focusing on this subset remains limited. The purpose of this study is to analyze the clinical characteristics, optimal therapeutic approaches, and mortalit

Idiopathic inflammatory myopathies: one year in review 2025.

Idiopathic inflammatory myopathies (IIMs) are a heterogeneous group of autoimmune diseases characterised by skeletal muscle inflammation and frequently by involvement of other organs, in particular lung and skin, but also joints, heart and gastrointestinal tract. Although they are rare diseases, the literature on IIMs has been growing rapidly and many studies have been published in order to clarify pathogenesis and to better define diagnosis, clinical manifestations (muscular and extramuscular) and treatment. The purpose of this review article is to summarise the most relevant contributions pub

Shaping the landscape of interstitial lung disease in idiopathic inflammatory myopathies: state of the art, evidence gaps and a need for clinical trials.

Interstitial lung disease (ILD) affects a significant proportion of adults and children with idiopathic inflammatory myopathies (IIM-ILD and JIIM-ILD). Despite its major impact on mortality and therapeutic decision‑making, robust studies and clinical trials to inform evidence‑based practice are strikingly scarce. Heterogeneous clinical manifestations, variable testing practices, and lack of widely accepted nomenclature and standardised endpoint definitions magnify the challenges to design clinical trials for this novel disease subtype. Thus, the Myositis Clinical Trials Consortium (MCTC) develo

Recurrent and new primary cancer risk in adult patients with idiopathic inflammatory myositis and a history of malignancy.

OBJECTIVES: In adult patients with idiopathic inflammatory myopathies (IIM) and a history of cancer, the risk of subsequent cancer is unclear. We describe our centre's experience with recurrent and new primary cancer development after IIM symptom onset. METHODS: A retrospective cohort study was conducted at The Johns Hopkins Myositis Center, analysing adult IIM patients enrolled from 2003 to 2024 with a documented cancer history. Patients were categorised into three groups: 1. remained cancer-free, 2. had cancer recurrence, or 3. developed a new primary cancer. RESULTS: Among 2,476 IIM patie

One mutation, divergent journeys: expanding the clinical spectrum of homozygous SAMHD1 deficiency in childhood.

OBJECTIVES: Homozygous loss-of-function mutations in SAMHD1 classically cause Aicardi-Goutières syndrome type 5 (AGS5), characterized by neuroinflammation and intracranial calcifications. Increasing evidence suggests a broader clinical spectrum. We aimed to describe the phenotypic heterogeneity associated with a single homozygous SAMHD1 variant in paediatric patients and to highlight diagnostic and therapeutic implications. METHODS: We retrospectively reviewed three paediatric patients evaluated at a tertiary centre who carried the same homozygous SAMHD1 missense variant (c.625G>A; p. Gly209S

AI-based chest CT quantification of interstitial lung disease in idiopathic inflammatory myopathies: agreement with expert visual assessment in 107 patients.

BACKGROUND: Interstitial lung disease (ILD) is a determinant of morbidity and mortality in idiopathic inflammatory myopathies (IIM), but chest HRCT evaluation remains observer-dependent. Artificial intelligence (AI) may provide reproducible quantitative assessment. We compared AI-based quantification of ILD with expert visual scoring in IIM. METHODS: In this monocentric retrospective study, 107 patients with IIM-associated ILD from a national myositis registry were included. One representative chest HRCT per patient was evaluated by a thoracic radiologist using a semi-quantitative lobar score

Clinical Value of 18F-FDG PET in Paraneoplastic and Pembrolizumab-Associated Myositis.

Fluorine-18 fluorodeoxyglucose positron emission tomography (18F-FDG PET) is widely used to evaluate patients with cancer and to detect various inflammatory processes. Here, we report a rare case of a 53-year-old woman with breast cancer who developed generalized pain, weakness, rash, and edema over the trunk after the first cycle of neoadjuvant pembrolizumab and cytotoxic chemotherapeutic agents. 18F-FDG PET revealed diffusely increased uptake in the muscles without skeletal or visceral metastasis, aiding in the diagnosis of paraneoplastic and pembrolizumab-associated myositis and subsequent

Association between anti-synthetase syndrome and malignancy: a systematic review and meta-analysis.

OBJECTIVES: Anti-synthetase syndrome (ASyS) is a rare autoimmune disorder defined by anti-synthetase antibodies. While malignancy is established in DM, its association with ASyS remains unclear. We aimed to estimate malignancy prevalence in ASyS and assess whether autoantibody subtype, age, sex or clinical features influence malignancy risk. METHODS: We performed a systematic review and meta-analysis following PRISMA guidelines and a PROSPERO protocol (CRD42024540200). PubMed, Embase, Web of Science Core Collection, Scopus and CENTRAL were searched for studies reporting malignancy in adult AS

A pharmacovigilance data-driven approach to reveal high fatal adverse events following checkpoint immunotherapy.

Immune checkpoint inhibitors (ICIs) have significantly improved the outcome of cancer treatment, but they also expose most patients to a variety of treatment-related adverse events (AEs). This study introduces a modified pharmacovigilance approach to identify "ICI-related high-mortality AEs", a subset of treatment-related complications that are disproportionately reported with elevated fatality during ICI therapy. Utilizing large-scale pharmacovigilance data from 148,972 ICI-treated cases in the FDA AEs Reporting System and 142,645 ICI-treated cases in the WHO global VigiBase, we found 63 type

Lead Toxicity Masquerading as Autoimmune Haemolytic Anaemia: A Diagnostic Pitfall in Unexplained Anaemia.

Lead toxicity is an uncommon but clinically important cause of unexplained anaemia. Its heterogeneous haematological manifestations can mimic autoimmune haemolytic anaemia, resulting in diagnostic uncertainty and delayed recognition.  A 49-year-old woman presented with a three-week history of worsening abdominal pain and lethargy. She had a background of inclusion body myositis treated with long-term intravenous immunoglobulin and recent initiation of mycophenolate mofetil. Initial investigations demonstrated severe anaemia with reticulocytosis but minimal biochemical evidence of haemolysis. A
