Obturator neuropathy from SLE lymphadenopathy: an interdisciplinary diagnostic challenge.

OBJECTIVE: To describe an unusual cause of thigh myopathy and nerve compression in a patient with systemic lupus erythematosus (SLE), illustrating the importance of interdisciplinary collaboration. CASE PRESENTATION: A 26-year-old woman with a 10-year history of SLE presented with new-onset right thigh pain and restricted hip movement. Knee pathology was excluded, and symptoms were initially attributed to hydroxychloroquine treatment. Despite conservative management, pain worsened. MRI revealed multifocal myositis in the gluteal and adductor regions with signal changes in the sciatic nerve an

Dysregulated IL-6 pathway activity in idiopathic inflammatory myopathies.

BACKGROUND: The objective of this study was to characterise interleukin-6 (IL-6) pathway activity in idiopathic inflammatory myopathies (IIM) and investigate the contribution of circulating IL-6, soluble IL-6 receptor (sIL-6R) and anti-IL-6 autoantibodies to systemic IL-6 signalling. METHODS: IL-6 pathway activity was assessed in skeletal muscle transcriptomic data from patients with IIM and healthy controls. Serum samples from 51 patients with IIM and 32 controls were analysed for IL-6, sIL-6R and anti-IL-6 autoantibodies by ELISA. The capacity of serum to modulate IL-6 signalling in serum w

Nomenclature and abbreviation heterogeneity in immune-mediated necrotizing myopathy: A call for standardization.

BACKGROUND: Immune-mediated necrotizing myopathy (IMNM) is a distinct subtype of idiopathic inflammatory myopathies (IIMs). Despite its increasing recognition, considerable variability persists in its nomenclature, posing challenges for research comparability, clinical trial design, and patient care. METHODS: We conducted a scoping review of English-language publications from November 1974 to December 2023 to assess the diversity and trends in IMNM nomenclature and abbreviations. Articles were screened for IMNM terminology and abbreviations, and categorized by publication year, region, and st

Dermatomyositis masquerading as angioedema: a crucial differential not to overlook.

Dermatomyositis is a rare autoimmune disease that typically affects the skin, muscle and respiratory system. Patients with dermatomyositis usually require prompt treatment with intense immunosuppression to reduce disease progression and multiorgan damage. A previously well male in his 40s initially presented to an emergency department with severe neck swelling. He was reviewed initially by ear, nose and throat surgeons for a probable deep neck space infection. Having been cleared of this as a cause, he was referred to clinical immunology for investigation and treatment of suspected idiopathic

An interpretable machine learning model integrating clinical and CT radiomic features for the detection of interstitial lung disease in idiopathic inflammatory myopathies.

BACKGROUND: Interstitial lung disease (ILD) is a leading cause of morbidity and mortality in patients with idiopathic inflammatory myopathy (IIM). However, early and accurate identification remains a clinical challenge. We aimed to develop and validate an interpretable machine learning framework to predict ILD presence in IIM patients. METHODS: A retrospective cohort of 115 IIM patients (n = 115, including 70 with ILD) was divided into training (n = 81) and test (n = 34) sets. 1,316 radiomic features were extracted from automated lung segmentations of high-resolution CT (HRCT) scans. To preven

Statin-Induced Necrotizing Autoimmune Myositis Presenting With Progressive Limb-Girdle Weakness Following Low-Dose Atorvastatin Use.

Statins are a mainstay in the prevention of cardiovascular disease and are associated with rare adverse effects, including statin-induced necrotizing autoimmune myositis (SINAM). This condition is characterized by progressive, symmetric, proximal muscle weakness and elevated creatine kinase (CK), persisting despite discontinuation of the statin, and is confirmed by the presence of anti-HMG-CoA reductase (HMGCR) antibodies. We present a case of a 72-year-old male who presented with three months of progressive limb-girdle weakness with chronic use of low-dose atorvastatin (10 mg). Our review of

Clinical characteristics and risk factors associated with objective flares in patients with dermatomyositis.

BACKGROUND: Flares in dermatomyositis (DM) are associated with increased disability, health care utilization, and reduced quality of life. We aim to determine the characteristics and risk factors of flares in patients with DM. METHODS: Patients were included if seen at The Johns Hopkins Myositis Center and fulfilled the 2017 EULAR/ACR Classification Criteria for DM. Two authors reviewed all notes and recorded patient symptoms, exams, labs, imaging findings, and medication changes. Objective flare was defined as active disease on exam or ancillary testing that resulted in an increase in immuno

Immune-mediated necrotising myopathy following semaglutide treatment: a contributing factor?

This report describes a female patient in her 60s with an immune-mediated necrotising myopathy (IMNM) occurring after semaglutide use. She was found to have progressive dysphagia, dysphonia, proximal muscle weakness and creatine kinase elevation over a 4-month course. Her symptoms were preceded by a single dose of semaglutide. Her work-up revealed features consistent with necrotising myopathy with inflammatory infiltrate on biopsy, and she responded to immunosuppression with corticosteroids and intravenous immunoglobulin (IVIG). She gradually improved and successfully returned to her baseline

Standardized Interoperable Data Collection for Myositis Research: Developing Expert Consensus on Common Data Elements for Myositis Outcome Measures.

OBJECTIVE: Recent progress has been made in developing validated myositis outcome measures. However, critical deficiencies remain for data standardization across myositis registries. Although the National Institutes of Health (NIH) Common Data Elements (CDE) Repository has been developed to facilitate standardized data collection and sharing, few myositis-specific CDEs currently exist. We developed CDEs for myositis outcome measures using novel data science strategies. METHODS: Data dictionaries of myositis registries were examined to understand how outcome measures are currently captured. We

Whole-body magnetic resonance imaging in rheumatology: advancements, key applications and future perspectives.

Whole-body magnetic resonance imaging (WB-MRI) is widely used in rheumatology to assess peripheral and axial joints and entheses throughout the body. Despite some limitations, it has potential in determining the overall inflammatory burden, tracking disease progression, and evaluating treatment response. It is used in the evaluation of idiopathic inflammatory myositis (IIM) or in the pediatric population where it is becoming the gold standard for the diagnosis and monitoring of conditions, such as juvenile idiopathic arthritis (JIA) and chronic recurrent multifocal osteomyelitis (CRMO). In add

Anti-MDA5 Dermatomyositis Presenting With Lupus-Like Mucocutaneous Features and Inflammatory Arthritis: A Case Report.

Understanding diagnosis and management of specific autoimmune diseases is based on existing classifications or diagnostic criteria. However, patients frequently present with broad, multisystem features that mimic multiple autoimmune diseases. Recognizing clinical mimicry is crucial because overclassifying these features as distinct coexisting diseases often presents a diagnostic challenge. We report the case of a 36-year-old man who presented with progressive weakness. The patient initially complained of facial rash and skin lesions, which then developed into tetraparesis, arthralgia, and diff
